Abstract

Road traffic accidents (RTA) are a global problem resulting in deaths, physical injuries, psychological problems and financial losses. These financial damages have immediate consequences and long term consequences on the victims and their families. Different countries have different impact of road traffic accidents and therefore spend dissimilar amounts in their budgets to prevent the road traffic accidents. If the financial losses due to road traffic accidents are calculated and highlighted by the researchers, the respective governments will be willing to spend higher amount in their budgets to prevent such accidents; as governments will be able to directly see the benefits to their countries, of spending higher budget amounts. Various countries are acting differently to reduce this menace of road traffic accidents and World Health Organization (WHO) is celebrating “Decade for road safety” to reduce the accidents and thus the financial loses to the society.

Introduction
There are about 1.25 million deaths due to road traffic accidents in the world and 20-50 million cases of non-fatal
injuries every year. Pedestrians, cycle users and motorcyclists are more vulnerable to accidents (1). Not only the
victims but simultaneously their friends and family members are affected by the consequences of these road
traffic accidents. Consequences of RTA, causes financial burden on the victims and their families, friends,
employers, insurance companies and the governments. It is very difficult to measure exactly the financial loss for
sacrifices and human sufferings (2).
Different countries have different problems and factors responsible for road traffic accidents as depicted by
different studies in various parts of the world. Highest rate of deaths due to Road traffic Injuries (RTI) is in the
Dominican Republic where there are 41.7 deaths per 100,000 population (3). In a study done in 2013 Nigeria has
the 2

highest rate of accidents in a list of 198 countries and recommended that condition of the vehicle,
pavements, roads, environment and that of driver should be taken care of to prevent the accidents (4). In UK 65%
of the accidents are due to driver error or reaction; injudicious action in 31%; due to inexperience or behavior in
28% and impairment or distraction in 19.6% of cases of fatal road traffic accidents (5). In USA alcohol was
responsible for 82% of cases(6).
Financial loss to the victims or compensation can be calculated by different parameters. Financial compensation
can be for pain and sufferings from the accident, financial losses of the future in the form of reduction in
expectancy of life and future lost income. Past lost income on medical expenses, repair of vehicle, hire vehicle and
travel expenses has also to be included for total financial loss. Insurance company outlay and interest on the
compensation claim and legal fees are also part of financial losses and may be claimed in a case of motor vehicle
accident claim(7). Financial losses due to accidents can be measured by adding costs of emergency services costs,
medical costs, legal and court cost, lost productivity, property damages, workplaces loss, insurance administrations
costs (6)(8). Wage and productivity loss, motor vehicle damage, employer’s uninsured cost, fire loss and motor
vehicle duplication costs also adds to the financial losses (8) .
There are some indirect losses due to RTA. Persons suffering from PTSD after accidents had more problems to
return to work as compared to persons without PTSD because of their need of financial stability (9). There are also

job losses due to long leaves. Number of persons living with spinal injuries and traumatic brain injuries is increasing
due to better medical treatment (10) and cost is increasing for maintaining such persons.
Calculation of financial losses is not easy. There is under reporting and misclassification of injuries and it can be
improved by synchronizing hospital records and police records and guidelines should be developed for determining
the economic losses Various quality of life scales should be used; various surveys should be used & mild traumatic
brain injuries and their socioeconomic costs should be included. Persons with low socioeconomic status also suffer
from more traffic injuries (10).
Calculation of financial losses for RTA can also be done by Disability Adjusted Life Years (DALY) and Gross Domestic
Product (GDP) per capita and utilizing this it was estimated that in 2005 there was loss of 167,752.4 million US $ in
the world. Majority of the losses (96%) were in low and middle income countries (11).
There are other ways of calculating these losses by utilizing data of years of life lost (YLL). In a study done in Kashan
region of Iran in 2012-2013, 3754 years in males (18.4 per 1000) and 949 years for females (4.8 per 1000) (12)
were lost and multiplying by average loss/year financial loss can be calculated.
Cost of treatment varies in different countries and in Brazil where men and young people are mostly involved in
road traffic accidents and each hospitalization costs R$ 1300 on an average (13). In Bangla Desh economic cost of
the accidents involve 1-2% of GDP (14). Most of the victims do not have health insurance. In one study done in
India in 2015, 74.8% of the victims had to spend more than Rs. 5000 and 73.2% of victims had to spend 10-11
times of their monthly income whereas only 3.5% of victims had health insurance cover(15). In India annual loss is
about $20 billion(16).
In USA each inhabitant has to bear a burden of $ 784 for accidents equivalent to 1.6% of GDP. If quality of life
values are added losses are of the magnitude of $836 in 2010 in USA (6). In USA seat belt use prevented a loss of
$50 billion and not using it caused a loss of $10 billion. Distraction of the driver caused a loss of $40 billion (6).
In European Union there are 1.3 million accidents and 40,000 deaths annually which means 200 billion loss
equivalent to 2% of GDP (17) and it has decreased by 45% between 2004 and 2014. In 2014, 25,000 people lost
their lives (18).
In 2000 AD financial loss was to the tune of 1% of GDP (19). In 2004 it was estimated 1% of GDP for low income
countries, 1.5% for middle income countries and 2% high income countries and global cost is estimated US $ 18
billion in a year. Out of this low and middle income countries lose 6.5 billion (2). It has increased gradually in 2015
in the low and middle income countries financial losses by deaths and road traffic accidents are to the tune of 5%
of GDP where as in the world 3% of GDP in lost in fatal road traffic accidents (20). In a report of WHO it has been
reported that 90% of the fatalities are in low and middle income countries though they have half of the numbers of
the vehicles in the world (21).
In the last three years 17 different countries have made rules and regulations for enforcing speed limits, seat belts,
prevent drunken driving, using of child restraint seat and wearing of helmets by two wheeler users and WHO is
trying to reduce the number of these deaths by 50% thus reducing the financial losses in the world due to RTI (22).
To reduce these losses in future, Google suggested autonomous driverless cars will reduce accidents by 90% (23)
and thus reduce the burden by 90% due to accidents. Automated system will help to reduce the number of
accidents and increase safety (24) and save economical loses.
Discussion
Financial losses are due to injuries, deaths and property damages. To calculate the financial losses for human
sufferings will always have ethical dilemmas. But due to increase in compensation cases in the courts all over the
world some criteria definitely needs to be evolved to make it objective rather than subjective compensation. There
are financial losses occurring in different continents of the world as shown by the various studies but all these

losses do impact their economies to the different extent. This impact is more felt in poor countries as compared to
the rich countries (11)(20). Many families suffer due to poverty when their bread earners die or disabled (2). If
more money is spent in budgets to take care of the factors responsible for accidents it will definitely reduce the
incidence of road traffic accidents this will help in preventing the financial losses. Automated vehicle industry may
greatly reduce the number of accidents thus preventing the financial losses to the victims and respective
governments(23) but this will take a long time to develop and to be accepted by the people (24). If nothing is done
it is estimated that by 2030 road traffic injuries will become the 7
th
leading cause of death and those countries who
have made systematic efforts have reduced these losses (21).
Conclusion
Financial losses due to road traffic accidents are a great burden on the victims, their families, different countries
and the world as a whole and especially to the low and middle income countries that bear the major burden due to
RTA. These financial losses are preventable by reducing the number of accidents by taking care of the factors
responsible for these accidents. Money saved by these measures can be better utilized for developing the new and
better roads.

Traffic rules and implementing road regulations. Road traffic accidents are rapidly becoming a danger to public health and to regional economic development in many developed countries. Road traffic accidents add to the misery by causing death, sickness, disability, grief, loss of income, and property damage. In this report, some of the main findings of the analysis were presented in order to evaluate the causes and economic impacts of road accidents in Mogadishu- Somalia. The research methodology of this study includes a literature review, site observations, personal interviews as well as questionnaire surveys, and survey analysis. The sampling method was used, in order to determine the most representative size of a population. In the process of selecting the sample from the target population, Random sampling technique as a probability sampling was used. Purposive sampling was used as non-probability sampling to categorize the respondents into various categories. The study found that the level of accidents in Mogadishu is very high, while these accidents are causing great loss of lives and destruction of valuable properties. The key factors contributing to road accidents in Mogadishu are night driving, narrow roads, and poor construction and bad planning of roads. Vehicle repair costs as well as medical costs are listed as having the highest economic impact due to road traffic accidents in Mogadishu. There is a long history and growing evidence base that the use of drugs, such as anabolic-androgenic steroids, to enhance human performance is common amongst armed forces, including in Australia. We should not be surprised that this might have occurred for it has long been predicted by observers. It is a commonplace of many recent discussion of the future of warfare and future military technology to proclaim the imminent arrival of Super Soldiers, whose capacities are modified via drugs, digital technology and genetic engineering, in ways that increase their performance exponentially. This is what some observers have referred to as the “Gladiator Model” wherein the aim is to create soldiers able to perform feats of which ordinary citizens are not capable. One key aspect of this “gladiator project” is the use of illicit drugs to enhance performance. Could we use drugs, such as steroids or amphetamines, to enhance performance? Should we use such drugs? In this paper we explore the ethics of creating Super Soldiers, and raise issues of consent, coercion and the extent to which such use is permitted or condemned by just war theory. We conclude that much will depend on the extent to which such use is harmful to the soldiers themselves and this is still an open question.

Pekanbaru City is a metropolitan city with a very dense traffic volume with a low level of discipline for road users. Violations of traffic signs often occur and a sense of care to comply with traffic regulations is still low, accidents often occur. As a result of undisciplined road users, it creates anxiety and worries about accidents. For this reason, it is necessary to conduct traffic safety education. Traffic counseling should be taught from an early age, where children are able to provide an understanding of good behavior in order to form a disciplined child character. The teaching method is adjusted to the age where the Shalsabila PAUD children are aged 2.5 - 5 years which is included in the category of Toddler and preschool age. The method used is made fun by telling stories, singing and playing. So they are very enthusiastic and happy. Most preschoolers begin to understand and can distinguish what can be done and what can't be done.

Background: Road traffic accidents are not a new phenomenon, rather it’s a disturbing occurrence which shows no sign of abating very soon. Rather, it remains one of those public health issues which even the most educated and civilized populations tend to ignore, primarily for the thrill of it. This study aims to identify the outcome associated with vehicular accidents and its association with socio-demographic factors. Method: This is a retrospective, record-based study of victims of road traffic accidents admitted in a tertiary care hospital in Nashik, Maharashtra. The data would include records from the 2018 to 2019. The variables will include the socio-demographic factors, site of injuries and its severity. A descriptive analysis would be done by SPSS software to find out the prevalence of vehicular accidents, association of site of injury with age and severity of the trauma. Ethical approval would be taken before the initiation of the study. Result: A total of 486 victims of RTAs were included from the medical records of the casualty of a tertiary care hospital, out of which 330 were from the year 2018 and 156 from 2019. A look at the sociodemographic profiles of the RTA victims showed that females comprised only 19.3% (2018) and 18.6% (2019) of the total victims in road traffic accidents, while majority, 80.6% (2018) and 81.4% (2019) were males during the same period. Overall, we can also observe that both in 2018 (38.5%) and 2019 (50.6%) most of the road traffic accidents among the victims were of moderate grade. Conclusion: The need of the hour is to bring about a change from within through self-reflection of lawmakers, strict implementation of traffic rules and guidelines with hefty fines, lockup, and criminal punishment to habitual wrongdoers.

As SAE level 2 semi-autonomous vehicles are now available in the UK vehicle displays are required to communicate more complex information to the driver. It is clear from examining interfaces from different manufacturers that there is lack of consistency in these displays with different vehicles showing icons of a variety of shapes and colours to convey the same message. When driving a semi-autonomous vehicle it is essential the driver knows whether they are in an automated or manual driving mode. However the lack of standardisation in icon design can lead to mode confusion. To understand the impact on users a focus group was held where participants were asked to interpret the meaning of icons from a variety of different driver interfaces. As a result it is recommended that new ISO standard icons are introduced in order to support drivers in SAE level 2 and 3 automated vehicles.

This study aimed to evaluate the effectiveness of blood alcohol testing in decreasing the prevalence of current one-month drinkers among road traffic crash patients within emergency departments at one, two, and three months after a crash. A cluster quasi-experimental study was conducted on 600 crash patients who visited one of the emergency departments at 21 hospitals in Udon Thani province, Thailand. The hospitals were categorised into a (i) high-adherence hospital group (≥70% of all patients) and (ii) low-adherence hospital group (<70% of all patients) according to the compliance of blood alcohol testing in their emergency departments. The data were collected by a trained nurse using a structured questionnaire. The primary outcome was the prevalence of one-month current drinkers. We included 600 patients: 291 from six hospitals in the high-adherence group and 309 from 15 hospitals in the low-adherence group. The prevalence of one-month current drinkers significantly decreased in both the high-adherence and low-adherence groups. However, the prevalence of current drinkers at two and three months after a crash was not statistically significant compared to that one month prior to a crash (48.0% to 19.3% and 31.7% to 13.8% in the high- and low-adherence hospital groups, respectively; p < 0.05 from McNemar’s test). The effectiveness of blood alcohol testing in decreasing the prevalence of one-month current drinkers among traffic crash patients within emergency departments was observed to be statistically significant only at one month after a crash, and not at two and three months.

The main aim of the present study was to investigate the spatiotemporal trends of urban traffic accident hotspots during the COVID-19 pandemic. The severity index was used to determine high-risk areas, and the kernel density estimation method was used to identify the risk of traffic accident hotspots. Accident data for the time period of April 2018 to November 2020 were obtained from the traffic police of Tabriz (Iran) and analyzed using GIS spatial and network analysis procedures. To evaluate the impacts of COVID-19, we used the seasonal variation in car accidents to analyze the change in the total number of urban traffic accidents. Eventually, the sustainability of urban transport was analyzed based on the demographic and land use data to identify the areas with a high number of accidents and their respective impacts on the local residences. Based on the results, the lockdown measures in response to the pandemic have led to significant reductions in road traffic accidents. From the perspective of urban planning, the spatiotemporal urban traffic accident analysis indicated that areas with high numbers of elderly people and children were most affected by car accidents. As we identified the hotspots of urban traffic accidents and evaluated their spatiotemporal correlation with land use and demography characteristics, we conclude that the results of this study can be used by urban managers and support decision-making to improve the situation, so that fewer accidents will happen in the future.

Background: Childhood road traffic injuries (RTIs) are a major public health problem worldwide. Reliable and valid information on childhood RTIs is essential to reduce the number of deaths. Aims: To determine the burden of deaths from RTIs in children aged 0-14 years from 2006 to 2019 in Turkey. Methods: This descriptive study examined the change in road traffic fatalities in children according to age, gender, road user type, and place. The necessary data for this study were obtained from the Turkish Statistical Institute. We used Microsoft Excel to analyse data from 4614 children who died from RTIs in 2006-2019 in Turkey. Results: The fatality rate from RTIs per 100 000 children aged 0-14 years increased from 1.41 in 2006 to 2.13 in 2019. The fatality rate for boys aged 0-9 and 10-14 years was higher than that for girls of the same age. The fatality rate for girls aged 0-9 years was higher than that for girls aged 10-14 years. The fatality rate for boys aged 10-14 years was higher than that for boys aged 0-9 years. Among the children who died from RTIs, 6.65% were drivers, 41.31% pedestrians and 52.04% passengers. Children lost their lives mostly as pedestrians on urban roads and as passengers on rural roads. Conclusion: The death of children due to RTIs is a significant health burden in Turkey.

Road traffic injuries (RTIs) are a major public health problem, particularly in developing countries. High quality data on the burden of this problem can help to provide the preventive strategies. The aim of this study was to determine of mortality and years of life lost (YLL) due to RTI between March 2012 and March 2013 in Kashan Region, Iran. The global burden of disease (GBD) methodology 2010 was used to compute years of life lost due to premature mortality (YLL). The results showed that a total of 101 persons [83 male (82.2%), male/female sex ratio of almost 5:1] were died during the study period. The mortality rate of RTI was 25.3 per 100,000 populations and men had a greater mortality rate. No significant differences were found between the fatality rate and different seasons of the year. The YLL during our study were 3754 years for males (18.4 per 1000), 949 years for females (4.8 per 1000) and 4703 years for both sexes (12.1 per 1000). The most YLL due to premature death in males were accounted for the age group of 30-44 yr (22.1 per 1000). It can be concluded that the RTIs' fatality rate is high in our region and should be considered as a major public health problem, notable for authorities, policymakers and strategic planners.

Introduction: WHO states that the proportion of deaths and casualty registrations of RTA cases represents only the tip of the iceberg. A similar tip of the iceberg phenomenon exists between the visible expenses incurred due to the accidents and the much larger invisible losses are there that imperceptibly disable the family budget. Objective: (1) To estimate the total expenditure on treatment of RTA cases and its proportion to their monthly family income on treatment.(2) To estimate the loss of wages of RTA cases and project the other losses.(3) To enquire reimbursement of losses of RTA cases. Materials and methods: A hospital based cross sectional study, conducted at ED and Hospital of S.N. Medical College, Agra. Data were collected by using semi structured questionnaire from RTA cases or their attendants. Results: Out of 425 admitted RTA cases, 318 (74.8%) had spent Rs > 5,000 on the treatment and 304(71.5%) had lost their wages Rs >1, 000. 73.2% had incurred hospital expenses from 1to10 times or even more of their monthly family income. About 1/4th cases recovered completely and 64.01% recovered with temporary physical deficit. 3.5% had health insurance. 75.8% cases had remained hospitalized for more than1 week or up to a month duration and a strong positive correlation (r = 0.83) was found between the length of stay in the hospital and the amount spent on the treatment. Conclusion: The financial assault suffered by the road traffic accident cases and their families even if they are treated in Government Hospital are so huge that cannot be ignored. © 2015, Indian Association of Preventive and Social Medicine. The road traffic accidents are a worldwide problem that has led several countries and organizations to gather efforts to reverse the current scenario. Annually, it makes millions of deaths, leaving many other people with physical or psychological disturbances. The understanding of scenarios related to non-fatal victims of traffic provides managers improved means to plan and implement preventive actions. Based on indicators and absolute numbers, this work presents the Brazilian scenario of non-fatal victims of traffic accidents. The data indicate that men and young people are the biggest victims, and that each hospitalization costs (on average) to the public treasury more than R$ 1,300.00. The Northeast region as the highest cause for concern, requires immediate action to mitigate damages caused by traffic in their society.

To estimate the economic loss due to road traffic injuries (RTIs) of the World Health Organization (WHO) member countries and to explore the relationship between the economic loss and relevant health system factors. Data from the World Bank and the WHO were applied to set up the databases. Disability-adjusted life year (DALY) and gross domestic product per capita were used to estimate the economic loss relating to RTIs. Regression analysis was used. Data were analyzed by IBM SPSS Statistics, Versions 20.0. In 2005, the total economic loss of RTIs was estimated to be 167,752.4 million United States Dollars. High income countries (HIC) showed the greatest economic losses. The majority (96%) of the top 25 countries with the greatest DALY losses are low and middle income countries while 48% of the top 25 countries with the highest economic losses are HIC. The linear regression model indicates an inverse relationship between nurse density in the health system and economic loss due to RTI. RTIs cause enormous death and DALYs loss in low-middle income countries and enormous economic loss in HIC. More road traffic prevention programs should be promoted in these areas to reduce both incidence and economic burden of RTIs.

Work potential in adult survivors of road accidents with and without post-traumatic stress disorder (PTSD) was examined at a mean of 8.6 months (SD = 3.77) post-accident. All participants were working prior to their accident. Results showed that survivors with PTSD had significantly less work potential post-accident than survivors without PTSD. Specific barriers to employability for survivors with PTSD identified by this study included high levels of depression, reduced time-management ability, and an over-concern or anxiety with physical injuries. Respondents with PTSD, however, reported significantly greater extrinsic motivation to work than those without PTSD. Early intervention and referral to occupational rehabilitation programs that: (1) help address these barriers to employability and stimulate the existing motivation to return to work, and (2) work alongside clinical treatment programs, may assist in the reduction of poor work outcomes that people with PTSD following road accidents often experience.
