Rosacea: Treatment with topical vitamin K

Rosácea is a dermopathy selectively affecting the face, characterized by the successive or simultaneous manifestation of erythema, telangiectasia, papules and pustules: erythema being substratum of rosacea and vascular dilation, a stage at its onset. A therapeutic assay is carried out with vitamin K1 in the form of a mask and cream for daily use showing a good response in reducing this dermopathy.

Vitamin K – Structure and Function of Vitamin K Epoxide Reductase

Vitamin K epoxide reductase (VKOR) is an integral membrane protein that catalyzes the reduction of vitamin K 2,3-epoxide and vitamin K to vitamin K hydroquinone, a cofactor required for the gamma-glutamyl carboxylation reaction. [Show full abstract] vitamin K, which reduces the rate of vitamin K-dependent carboxylation and leads to under-carboxylated, inactive vitamin K-dependent proteins. It is proposed that an active site disulfide needs to be reduced for the enzyme to be active. VKOR uses two sulfhydryl groups for the catalytic reaction and these two sulfhydryl groups are oxidized back to a disulfide bond during each catalytic cycle. The recent identification of the gene encoding VKOR allows us to study its structure and function relationship at the molecular level. The membrane topology model shows that VKOR spans the endoplasmic reticulum membrane three times with its amino-terminus residing in the lumen and the carboxyl-terminus residing in the cytoplasm. Both the active site (cysteines 132 and 135) and the proposed warfarin binding site (tyrosine 139) reside in the third transmembrane helix. VKOR is made at high levels in insect cells and is relatively easily purified. This should allow the determination of its three-dimensional structure. A detailed mechanism has been published and the purified enzyme should allow the testing of this mechanism. A major unanswered question is the physiological reductant of VKOR.

Methods: We included eight PXE patients with typical clinical symptoms (skin, retina, and vascular calcification) and two ABCC6 causative mutations; 13 clinically unaffected first-degree patients’ relatives (9 carrying one ABCC6 mutation and 4 non-carriers). We assessed urinary vitamin K1 metabolites and serum Glu- and Gla-OC, Gas6 and undercaboxylated prothrombin (PIVKA-II), at baseline and after 1 and 6 weeks after a single intramuscular injection of 10 mg vitamin K1. Results: Comparison of PXE patients, heterozygous, and non-carriers revealed differences in baseline levels of serum MK-4 and of urinary vitamin K metabolites. The response to phytomenadione administration on vitamin K-dependent proteins was similar in all groups. Conclusion: The physiological axis between vitamin K1 and vitamin K-dependent proteins is preserved; however, differences in the concentration of vitamin K metabolites and of MK-4 suggest that vitamin K1 metabolism/catabolism could be altered in PXE patients.

[Show full abstract] equal groups, TA group received TA (10 mg/kg) and vitamin K1 (20 mg) as a bolus injection and hypotension group, patients were maintained on hypotensive anaesthesia [mean arterial pressure (MAP) decreased to 30% of its baseline level] using nitroglycerine 1-2 μg /kg/min. Baseline haemoglobin was determined. Other laboratory investigations included platelet count, activated partial thromboplastin time (aPTT), antithrombin III (AT-III) and fibrinogen concentration preoperatively (T1), one hour after skin incision (T2) and after skin closure (T3). The surgeon graded the dryness of surgical field on a 10-point scale. Blood loss and amount of blood transfusion required were recorded. Results: There was progressive significant decrease in platelet count, AT-III and fibrinogen accompanied by significant increase of aPTT in TA group at T2 and T3 compared with T1 and the hypotension group. Surgeon's satisfaction score was higher in TA group (P < 0.001). Blood loss was higher in the induced hypotension group in association with increased consumption of allogenic blood compared to TA group. There was significant prolongation of the duration of surgery in hypotension group. There was a negative significant correlation between the amount of blood loss and surgeon's satisfaction rate in both groups (r = -0.6033 and -0.9353, respectively). Conclusion: Intraoperative co-administration of TA and vitamin K1 could reduce blood loss down to a satisfactory level that reduced need for blood transfusion through their effects on patients' coagulation profile.

Vitamin D3 and Vitamin K1 Levels in Infant Formula Produced in the United States

Seventy milk- and soy-based infant formulas from four U.S. manufacturers were assayed from October 1985 through September 1987 for vitamin D3 and vitamin K1. Mean vitamin levels per 100 kcal were 71 IU for vitamin D3 and 18 μg for vitamin K1. Vitamin D3 levels (IU/100 kcal) were similar (α = 0.05) in soy-based (72) and milk-based (70) formulas. Significant differences were found in vitamin D3 and vitamin K1 levels in formulas produced by different manufacturers. Mean levels of the various manufacturers products ranged from 66 to 78 IU/100 kcal for vitamin D3 and 15 to 24 μg/100 kcal for vitamin K1. Levels of vitamin D3 ranged from 41 to 99 IU/100 kcal. For vitamin K, the microgram per 100 kcal levels ranged from 8 to 39. All formulas met minimum levels of the 1980 Infant Formula Act for both vitamin D3 and vitamin K1. For vitamin D3, none of the formulas exceeded the maximum of 100 IU/100 kcal specified by the Act, and the mean level was 177% of the minimum requirement of 40 IU/100 kcal. On the basis of the Act requirement for vitamin K1 of 4 μg/ 100 kcal, milk-based and soy-based formulas contained 368 and 555%, respectively, of the minimum required level.
