Distinctive features of systolic function of the left ventricle in patients with chronic obstructive lung disease in extremely cold climate conditions

Abstract

The purpose of this study was to identify the characteristics of systolic dysfunction and the formation of chronic pulmonary heart disease in patients with COPD living in the central zone of the Republic of Sakha (Yakutia). This article describes the echocardiographic indices of systolic dysfunction of the heart in 229 patients with chronic obstructive pulmonary disease of the Republic of Sakha (Yakutia). According to our data, severe decompensation of chronic pulmonary heart disease with symptoms of heart failure of right ventricular type in most cases combined with severe left ventricle systolic dysfunction, not related to the primary pathology of the left heart and leading to a further decline in the quality of life of patients and increase in the risk of fatal diseases.

Aim. To study effect of course treatment with isosorbide-5-mononitrate (50 mg o.d.) on parameters of systolic and diastolic cardiac function and pulmonary artery blood pressure in patients with chronic cor pulmonale. Method and material. Doppler echocardiography was used for assessment of hemodynamic parameters and estimation of pulmonary artery pressure in 30 patients with decompensated chronic cor pulmonale. Results. Treatment with isosorbide-5mononitrate was associated with signs of effective unloading of pulmonary circulation, significant lowering of systolic, diastolic and mean pulmonary artery pressure (p<0,05), decreases of cardiac output (p<0,05), right ventricular enddiastolic dimension (p<0,01), left ventricular end-diastolic (p<0,001) and end-systolic (p<0,01) volumes, augmentation of ejection fraction (p<0,01) and left ventricular ΔS% (p<0,05). Significant improvement of main parameters of right and left ventricular diastolic function was also observed. Conclusion. Long-acting preparation of isosorbide-5-mononitrate can be used in combination therapy of patients with chronic cor pulmonale.
