Influence of exercise training with resveratrol supplementation on skeletal muscle mitochondrial capacity

Physical inactivity reduces, and exercise training increases, mitochondrial capacity. In rodents, exercise training effects can be augmented by large doses of resveratrol supplementation but whether this can occur in humans with a smaller dose is unclear. This study sought to determine the effects of resveratrol supplementation in combination with exercise training on skeletal muscle mitochondrial capacity. Sixteen healthy young adults were randomly assigned in a double-blind fashion to consume either placebo or 500 mg of resveratrol plus 10 mg of piperine, a bioenhancer to increase bioavailibilty and bioefficacy of resveratrol. Participants ingested the pills daily for 4 weeks and completed 3 sessions per week of submaximal endurance training of the wrist flexor muscles of the nondominant arm. The contralateral arm served as an untrained control. Skeletal muscle mitochondrial capacity was measured using near-infrared spectroscopy. Changes in mitochondrial capacity from baseline to post-testing indicated significant differences between the resveratrol+piperine-trained arm and the placebo-trained arm (p = 0.02), with the resveratrol+piperine group increasing about 40% from baseline (Δk = 0.58), while the placebo group increased about 10% from baseline (Δk = 0.13). Neither the placebo group nor the resveratrol+piperine group exhibited changes in mitochondrial capacity in the untrained arm. In conclusion, low-intensity exercise training can increase forearm skeletal muscle mitochondrial capacity when combined with resveratrol and piperine supplementation.

Abstract

Introduction

Dietary supplements that target health and performance are a large global commercial
37
market, with billions of dollars spent per year (Hardy 2000). One type of dietary supplement is
38
polyphenols, which are the most abundant antioxidants in the diet and have recently gained
39
attention due to their potential beneficial effects on metabolic health and aging (Scalbert et al.
40
2005; Schrauwen and Timmers 2014). A popular supplement known as resveratrol has been the
41
focus of much research due to its reported anti-inflammatory, anti-oxidant, and anti-tumorigenic
42
properties (Smoliga et al. 2011).

Resveratrol is a polyphenol antioxidant found in grapes, red wine, peanuts, cranberries,
44
and other plant species. Resveratrol has become of particular interest in the past decade in part
45
because of its potential ability to stimulate the expression of the SIRT1-AMPK-PGC1α pathway
46
in skeletal muscle, leading to enhanced mitochondrial capacity (Lagouge et al. 2006; Timmers et
47
al. 2011; Ungvari et al. 2011; Menzies et al. 2013; Goh et al. 2014; Kulkarni and Canto 2014).

Exercise training also activates this pathway and results in enhanced mitochondrial capacity
49
(Winder et al. 2000; Jager et al. 2007; Hood 2009).

Several experiments in rodents have reported an augmented effect on skeletal muscle
51
mitochondrial capacity of exercise training when it was combined with resveratrol
52
supplementation (Murase et al. 2009; Dolinsky et al. 2012; Menzies et al. 2013). In humans,
53
there have been few studies assessing resveratrol’s effect on mitochondrial capacity and the
54
research to date has produced contradictory findings. Some studies show upregulation of
55
mitochondria and the pathways that lead to increased mitochondrial capacity (Timmers et al.
56
2011; Goh et al. 2014), while others show minimal(Voduc et al. 2014) or attenuated training
57
adaptations (Gliemann et al. 2013; Olesen et al. 2014; Scribbans et al. 2014; Voduc et al. 2014) .

These inconsistent findings in human studies could be due to multiple factors including the
59
health of the participants, variations in the muscles trained, the training stimulus, and the dose
60
and bioavailability of the resveratrol.

A concern in human studies is the low bioavailability of resveratrol (Amri et al. 2012).
62
Maximum plasma resveratrol concentrations (Cmax) typically are around 75 ng/mL or less with a
63
single dose of 500mg of resveratrol (Boocock et al. 2007). This is much lower than the in vitro
64
concentrations shown to have pharmacological benefits (>1000 ng/mL), which has been
65
achieved in plasma of rodents treated with large doses of resveratrol (de la Lastra and Villegas
66
2005). This suggests that a higher dose, or enhanced bioavailability, is potentially needed to
67
produce beneficial effects in humans. Piperine has been used in combination with
68
neutraceuticals such as coenzyme Q10, beta-carotene, and resveratrol to enhance their
69
bioavailability and bioefficacy (Badmaev et al. 1999; Badmaev et al. 2000; Johnson et al. 2011;
70
Wightman et al. 2014). Therefore, it seems plausible that co-supplementing resveratrol with
71
piperine could lead to enhanced bioavailability and bioefficacy of resveratrol and improve the
72
ability to produce physiological benefits in humans yet minimize the potential risks associated
73
with high doses (Mennen et al. 2005).

The purpose of this study was to evaluate the influence of co-ingesting 500 mg of
75
resveratrol with 10 mg of piperine on changes in muscle mitochondrial capacity with
76
submaximal endurance training in healthy subjects compared to a placebo group. It was
77
hypothesized that 4 weeks of supplementation of resveratrol+piperine in combination with
78
submaximal endurance training would produce a greater increase in muscle mitochondrial
79
capacity than exercise training or resveratrol+piperine alone.

Methods

Participants

A total of 16 healthy young adults (9 males, 7 females) volunteered for the study.
84
Participants were excluded if they were currently taking medications other than oral
85
contraceptives or any vitamin supplements. Participants were also excluded from the study if
86
there exercise regimen involved excessive use of the forearm muscle, such as rowing, rock
87
climbing, or Crossfit. Participants were instructed to maintain exercise and dietary habits and
88
abstain from vigorous forearm activity during the study. Participants were instructed to consume
89
their study pills each morning upon waking. All experimental procedures were approved by the
90
Human Subjects Institutional Review Board at the University of Georgia. Verbal and written
91
explanations of the experimental protocol and associated risks were provided to all participants
92
prior to obtaining written informed consent.

Exercise Training Procedures

This was a longitudinal training study in which participants performed 30 minutes of
96
supervised forearm wrist flexor exercises of the non-dominant arm 3 times per week over 4
97
weeks. The wrist flexor muscles were chosen as the muscle of interest because they are
98
relatively detrained, non-weight bearing muscles compared to muscles of the lower limb. The
99
dominant arm was not trained and used as the untrained control arm for each subject.

The exercise training regimen used in this study was a submaximal endurance training
101
protocol similar to previous endurance training programs (Ryan et al. 2013a). During baseline
102
testing, participants performed a maximal voluntary isometric contraction (MVIC) to determine
103
the appropriate weight for exercise training and testing. MVIC was determined using a JAMAR
104

® handgrip dynamometer (Sammons Preston Rolyan, Bolingbrook, IL). Participants were given
105
the hand dynamometer and instructed to perform three MVIC’s on each hand. Participants were
106
instructed to rest 20 seconds between each contraction and the highest value of the three
107
contractions was recorded. Participants trained with dumbbell weights adjusted to 12-15% of
108
their MVIC and performed the exercise on a flat surface, such as the arm of a chair, with the
109
elbow at 90 degrees of flexion. Training was performed during the morning hours of the day
110
before 11am so that the time of training coincided with the time resveratrol reaches peak plasma
111
concentrations after ingestion of pills (Boocock et al. 2007). Participants began training with a
112
contraction frequency of 1 contraction every 3.5 seconds (514 contractions per session) during
113
week one. Starting with week two the frequency was increased to 1 contraction every 2.5
114
seconds (720 contractions per session) and remained at this frequency for the remaining 2 weeks
115
of the study.

Supplementation Procedures

Participants were randomly assigned in a double blind fashion to receive one pill
119
containing 500 mg of resveratrol and one pill containing10 mg of piperine (n=8); or two
120
identically appearing placebo pills containing flour (n=8). Participants were asked to consume
121
the pills upon waking each morning. Participants were randomized in a double-blind fashion
122
and given pills by a researcher who was not a member of the investigational team and did not
123
participate in either the training or testing of participants. Randomization was done using
124
randomizer.org. The resveratrol supplement contained 99% Pure Trans Resveratrol
125
(MegaResveratrol, Danbury, USA) in size “0” vegetable capsules and the piperine supplement
126
(BioPerine®) was provided by Sabinsa and contained 95% Piperine prepared in to size “3”
127

vegetable capsules. All pills were prepared by the lead researcher and coded by a third party
128
researcher who did not participate in either the training or testing of participants. No member of
129
the investigative team was aware of the contents of the capsules until all training and
130
mitochondrial capacity measurements were completed and analyzed.

Mitochondrial Capacity Procedures and Measurements

Mitochondrial capacity measurements took 45 minutes and were made at weeks 0, 2, 3,
134
and 4. Mitochondrial capacity, measured using near infrared spectroscopy (NIRS) (Ryan et al.
135
2012; Ryan et al. 2013a), yields results highly correlated to gold standard measures of
136
mitochondrial function including magnetic resonance spectroscopy (Ryan et al. 2013b) and
137
biochemical analysis of muscle biopsies (Ryan et al. 2014). The NIRS protocol was performed
138
on both the non-dominant (training arm) and dominant arm (non-training arm). The participant
139
was placed supine on a padded table with the test arm extended 90 degrees from the body. The
140
NIRS probe was placed over the superficial wrist flexor muscles (flexor carpi radialis, palmaris
141
longus, and flexor carpi ulnaris) approximately 2-3 cm distal to the medial epicondyle of the
142
humerus. A blood pressure cuff (Hokanson SC5, Bellevue, WA) was placed above the elbow
143
joint and was attached to a rapid cuff-inflation system (Hokanson E20 cuff inflator) powered by
144
a 30-gallon commercial air compressor (Husky VT6315, Kenosha, WI).

NIRS signals were obtained using a continuous wave NIRS device (Oxymon MK III;
146
Artinis Medical Systems, The Netherlands). Adipose tissue thickness (ATT) was measured at
147
the site of the NIRS probe using B-mode ultrasound (LOGIQe; GE HealthCare, USA). The
148
NIRS probe had two source detector separation distances that were set based on the amount of
149
adipose tissue thickness (ATT) on top of the muscle of interest. The transmitter and receiver on
150

the NIRS probe were set to a distance at least twice the ATT depth in order to assure NIRS
151
penetration depth was adequate to reach the muscle. NIRS data were collected at 10 Hz. NIRS
152
signals that represent oxygenated (O2Hb) and deoxygenated (HHb) hemoglobin/myoglobin were
153
corrected for changes in blood volume as previously described (Ryan et al. 2012).

Mitochondrial capacity was measured using a short bout of voluntary exercise to increase
155
metabolic rate, and the rate of recovery of metabolic rate after the exercise was measured
156
(McCully et al. 2009; Ryan et al. 2013a). The testing protocol consisted of a 5 10 second bout
157
of voluntary exercise followed by a series of 17-20 short duration arterial occlusions (5-10
158
seconds) (Figure 1A). The exercise/occlusion protocol was performed twice. The rate of
159
recovery of oxygen consumption measured by the difference signal (Hbdifference= O2Hb HHB)
160
was calculated as the slope of the change during arterial occlusion using linear regression. The
161
repeated measures were fit to a monoexponential curve and a mitochondrial rate constant (k) was
162
determined for each fit (Figure 1B). Prior to mitochondrial capacity tests, a five minute
163
ischemic calibration was used to normalize NIRS signal, completely deoxygenating the muscle
164
tissue under the probe. The cuff was then released to obtain a peak hyperemic response which
165
was used to indicate 100% oxygenation. This calibration was also used to monitor depletion of
166
oxygen saturation during the short bout of exercise in the recovery protocol. Oxygen depletion
167
was kept between 30-50% of the 100% hyperemic response to ischemia to reduce the likelihood
168
that rate of oxygen utilization during the ischemic cuff periods were limited by oxygen delivery.

Statistical Analysis

The differences between baseline testing and time points 2, 3 and 4 were first calculated
172
and a two-way repeated measures ANOVA was used to compare the effect of group
173

(resveratrol+piperine vs. placebo) and time (difference between weeks compared to baseline) for
174
mitochondrial capacity data in the trained and untrained arms. Examination of individual data
175
indicated considerable variability in initial values of mitochondrial capacity among study
176
participants. ANCOVA was therefore used to control for individual variation in baseline
177
mitochondrial capacity that plausibly could influence responsiveness to the intervention.

Because of equipment problems, missing data occurred at week 3 for one individual in the
179
placebo trained arm group and a second individual in the placebo untrained arm. These missing
180
values were replaced with the last value carried forward. Fishers LSD was used for the post-hoc
181
pair-wise comparisons. A two-way repeated measures ANOVA was applied to strength
182
measurement data with a within subjects factor of time (pre- and post-) and a between subjects
183
factor of group. All statistical analysis were performed using SPSS 19.0 (IBM®, Armonk, NY).

Statistical significance was accepted at p < 0.05 and all data are presented as means ± SD.

Results

All participants completed the supplementation, exercise training, and testing without any
188
adverse events. The physical characteristics of the participants in this study are shown in Table
189
1. Compliance was monitored via daily text messages and determined using a post-screening
190
questionnaire which was administered after completion of the study (Wei et al. 2011). Based on
191
the questionnaire, there was a 99.1% adherence to supplementation and no participant missed
192
more than 2 out of 28 days of capsules. All training sessions were completed by all participants
193
(12/12) resulting in 100% adherence to exercise training.

The statistical analysis of the strength measurements revealed that the interaction was not
195
significant in either the trained arms (p=0.55) or the untrained arms (p=0.43). Baseline strength
196

measurements were not different between the resveratrol group and the placebo group, and did
197
not change during the course of the intervention.

Average mitochondrial rate constants between groups are displayed in Figures 2A and
199
2B. Examination of individual baseline data indicated substantial heterogeneity among subjects
200
within each group (Figure 3A, 3B). When using baseline mitochondrial capacity values as a
201
covariate in the ANCOVA model, the analysis identified a significant group × time interaction
202
effect in the trained arm (p=0.002). In the resveratrol+piperine group only, pairwise
203
comparisons indicated significant differences in the change in mitochondrial rate constants from
204
baseline to post testing compared to changes at weeks 2 (Δk= 0.03 vs Δk= 0.58, p=0.001) and
205
weeks 3 (Δk= 0.13 vs Δk= 0.58, p = 0.005) (Figure 4). There were no significant differences
206
among time points in the placebo group. When comparing the resveratrol+piperine trained arm
207
to the placebo trained arm, the change from baseline to post testing indicated significant
208
differences between the two groups (p=0.02), the resveratrol+piperine group increasing about
209
40% from baseline (Δk= 0.58), where the placebo group increased about 10% from baseline
210
(Δk= 0.13) . A two-way ANCOVA did not identify a significant interaction effect in the
211
untrained arms between the 2 variables (group × time) (p=0.66), meaning that the untrained arms
212
NIRS rate constants did not differ significantly between groups or over time.

Discussion

The primary finding in this study was that 4 weeks of resveratrol and piperine
216
supplementation combined with a submaximal endurance training stimulus significantly
217
increased mitochondrial oxidative capacity, compared to exercise training with a placebo.
218
Previous studies have shown administration of resveratrol enhances AMPK and SIRT1, leading
219

to activation of PGC-1α (Lagouge et al. 2006; Timmers et al. 2011; Ungvari et al. 2011; Menzies
220
et al. 2013) which is the primary pathway associated with endurance exercise adaptations leading
221
to mitochondrial biogenesis (Winder et al. 2000; Jager et al. 2007; Hood 2009). Therefore,
222
resveratrol along with exercise training, could enhance this pathway to a greater extent than
223
exercise alone. This is supported by studies in mice and rats that have shown resveratrol
224
enhances physiological adaptations to exercise training (Dolinsky et al. 2012; Hart et al. 2013;
225
Menzies et al. 2013), which are consistent with the findings in this study demonstrating that
226
mitochondrial capacity was increased after 4-weeks of arm training in the resveratrol+piperine
227
group, but not the placebo group.

Recent studies investigating the interactive effects of training and resveratrol
229
supplementation in humans have produced mixed results. Studies performed in obese and
230
diabetic populations have been consistent with rodent models, indicating increases in skeletal
231
muscle SIRT1 and AMPK protein levels with resveratrol supplementation (Timmers et al. 2011;
232
Goh et al. 2014). However, studies combining exercise and resveratrol supplementation in
233
healthy adults have reported attenuated training induced adaptations with supplementation of
234
resveratrol. A study examining the effects of 4 weeks of high-intensity interval training (HIIT)
235
of the legs found that mitochondrial biogenesis-related gene expression in skeletal muscle was
236
unchanged from baseline with resveratrol supplementation. Specifically, PGC- and SIRT1
237
gene expression were increased in the placebo group, and unchanged in the resveratrol group
238
after the training period, indicating resveratrol supplementation may attenuate mitochondrial
239
adaptations to leg exercise training (Scribbans et al. 2014). Another study examining the effects
240
of 8 weeks of a daily dosage of 250 mg of resveratrol in combination with high intensity exercise
241
training also found no effects of resveratrol on SIRT1, AMPK, and PGC-. However, exercise
242

training alone did increase these mitochondrial markers (Olesen et al. 2014). Other studies have
243
reported resveratrol has no effect on physiological adaptations to training, such as whole body
244
maximal oxygen uptake (Gliemann et al. 2013; Voduc et al. 2014; Macedo et al. 2015). A
245
potential difference between these studies and the current study may be due to the muscle group
246
trained, the intensity of the exercise training regimens or the dosage amounts. For example,
247
Scribbans et al. (Scribbans et al. 2014) ,Gliemann et al. (Gliemann et al. 2013) and Olesen et al.
248
(Olesen et al. 2014) performed leg HIIT protocols 3 times per week. Based on our data, it seems
249
possible that resveratrol+piperine could enhance adaptations to lower intensity training stimuli in
250
relatively untrained muscle, controlling for baseline differences in mitochondrial capacity.
251
Future studies are needed to systematically examine whether resveratrol differentially modifies
252
adaptations to low vs. high intensity exercise training, or arm vs. leg training.

An additional consideration is the amount of resveratrol necessary to produce
254
physiological changes. Scribbans et al. (Scribbans et al. 2014),Gliemann et al. (Gliemann et al.
255
2013), and Oleson et al. (Olesen et al. 2014) supplemented with 250mg/day or less of resveratrol.
256
Increasing the dosage of resveratrol given in this study to 500mg instead of the 250mg given in
257
the previous studies may have been important in inducing beneficial adaptations and increasing
258
mitochondrial capacity. Significant enhancement of mitochondrial capacity did not occur until
259
the last measurement in the resveratrol group. This suggests that supplementation of resveratrol
260
may need a preloading period to significantly increase blood concentrations to produce
261
physiological effects (Almeida et al. 2009; Brown et al. 2010). Another concern is the
262
bioavailability and bioefficacy of resveratrol. In this study, resveratrol was combined with
263
piperine in hopes to enhance the bioavailability and bioefficacy of resveratrol. Resveratrol is
264
rapidly metabolized and results in the formation of various resveratrol metabolites and
265

conjugates. These conjugates could act differently than the parent compound, possibly inducing
266
different physiological effects, however further research needs to be done to better understand
267
the biological activity of resveratrol and its conjugates (Baur and Sinclair 2006; Brown et al.
268
2010). The use of piperine in the current study could have been a key component due to its
269
reported ability to inhibit glucuronidation of polyphenol antioxidants (Shoba et al. 1998;
270
Lambert et al. 2004; Srinivasan 2007), therefore possibly preventing extensive metabolism of
271
resveratrol and the formation of resveratrol conjugates, allowing a greater amount of
272
untransformed resveratrol to be absorbed. Combining resveratrol with piperine has been shown
273
to increase the absorption and maximum serum levels of resveratrol concentration by ~1500% in
274
mice given 100 mg/kg resveratrol and 10 mg/kg piperine (Johnson et al. 2011). Along these
275
lines, it has also been reported that the bioefficacy of resveratrol is also enhanced when co-
276
supplementing with piperine in healthy human subjects. This study also confirmed that
277
resveratrol metabolites and conjugate serum levels were lower with the addition of piperine,
278
indicating piperine’s inhibition of resveratrol glucuronidation (Wightman et al. 2014). Therefore,
279
combining piperine with resveratrol could have been a contributing factor to the changes
280
observed in this study.

This study is also different than previous studies in our use of a low intensity exercise
282
stimulus instead of a high-intensity exercise stimulus combined with resveratrol
283
supplementation. The low intensity stimulus was chosen because previous literature has only
284
assessed resveratrol supplementation in combination with high intensity exercise training and
285
was chosen to mimic an intensity that is feasible for all populations, including patient
286
populations who may be unable to exercise at high intensities. It is interesting that the
287
submaximal exercise stimulus in this study was not sufficient to increase mitochondrial capacity
288

within the placebo group, only 3 of 8 individuals within the placebo group exhibited training-
289
induced increases in mitochondrial rate constants from baseline. Conversely, in the
290
resveratrol+piperine group, supplementation and training increased mitochondrial rate constants
291
in 7 of 8 participants. Moreover, no changes in mitochondrial capacity were observed in the
292
untrained arm of the resveratrol+piperine group. Taken together, these findings indicate that
293
combination of exercise and resveratrol is needed for eliciting muscle mitochondrial adaptations
294
to low-intensity training programs such as that used in the present study. We speculate that
295
small changes at the molecular level with this submaximal stimulus could have occurred, but the
296
signals were not enough to elicit training adaptations at the physiological level in the placebo
297
trained arm. This conclusion is supported by Timmers et al. who detected resveratrol-induced
298
increases in muscle AMPK, PGC-1α, and citrate synthase expression- all biomarkers of
299
mitochondrial biogenesis- but did not find a change in mitochondrial function in vivo when
300
measuring phosphocreatine recovery rate after acute exercise (Timmers et al. 2011).

Some of the limitations of this study were that plasma levels of resveratrol in the blood
302
were not assessed, muscle tissue samples were not taken to determine activation of mitochondrial
303
biogenesis- related signaling pathways, and the effect of piperine on mitochondrial capacity was
304
not assessed independent of co-supplementation with resveratrol. Measuring resveratrol
305
concentrations in the blood could have led to a better understanding of the bioavailability and
306
dosage amounts needed to induce physiological effects, and how repeated dosing at the amount
307

served as another indicator of participant compliance. Diet was also not controlled or accounted

for in this study, which may be a confounding variable. However, dietary intake of resveratrol is

estimated to be small relative to the doses administered in this study. Muscle tissue samples

could have provided more insight in to the effects of training and resveratrol+piperine on

mitochondrial capacity, as well as the mechanistic pathways resveratrol activates. Muscle biopsy
314
analysis would have also made this study easier to compare to others indicating activation of
315
proteins such as SIRT1 and AMPK. Finally, future studies may determine the independent and
316
combined effects of piperine and resveratrol on mitochondrial adaptations to training (Srinivasan

2007).

Conclusions

This is the first study to show that the combination of resveratrol+piperine
321
supplementation and low-intensity, submaximal exercise training enhances mitochondrial
322
capacity in humans. Using piperine as a bioenhancer may be a key component when
323
supplementing with resveratrol, although further studies are needed to determine whether
324
piperine definitively augments resveratrol-mediated improvements in mitochondrial training
325
adaptations. More studies are also needed to determine the cellular mechanisms by which
326
resveratrol increases mitochondrial capacity and the optimal dosage of resveratrol required to
327
safely elicit these physiological adaptations.

Resveratrol is a natural polyphenol utilized in Chinese traditional medicine and thought to be one of the determinants of the “French Paradox”. More recently, some groups evidenced its properties as a calorie-restriction mimetic, suggesting that its action passes through the modulation of skeletal muscle metabolism. Accordingly, the number of studies reporting the beneficial effects of resveratrol on skeletal muscle form and function, in both experimental models and humans, is steadily increasing. Although studies on animal models confer to resveratrol a good potential to ameliorate skeletal muscle structure, function and performance, clinical trials still do not provide clear-cut information. Here, we first summarize the effects of resveratrol on the distinct components of the skeletal muscle, such as myofibers, the neuromuscular junction, tendons, connective sheaths and the capillary bed. Second, we review clinical trials focused on the analysis of skeletal muscle parameters. We suggest that the heterogeneity in the response to resveratrol in humans could depend on sample characteristics, treatment modalities and parameters analyzed; as well, this heterogeneity could possibly reside in the complexity of skeletal muscle physiology. A systematic programming of treatment protocols and analyses could be helpful to obtain consistent results in clinical trials involving resveratrol administration.

Cranberries are rich in polyphenols, have a high antioxidant capacity, and may protect against exercise-induced free radical production. Mitochondria are known producers of free radical in skeletal muscle, and preventing overproduction of radicals may be a viable approach to improve muscle health. This study aimed to investigate the effect of a polyphenol-rich cranberry extract (CE) on muscle oxidative capacity and oxygenation metrics in healthy active adults. 17 participants (9 males and 8 females) were tested at: (i) baseline, (ii) 2 h following an acute CE dose (0.7 g/kg of body mass), and (iii) after 4 weeks of daily supplement consumption (0.3 g/kg of body mass). At each time point, muscle oxidative capacity was determined using near-infrared spectroscopy to measure the recovery kinetics of muscle oxygen consumption following a 15–20 s contraction of the vastus lateralis. Cranberry supplementation over 28 days significantly improved muscle oxidative capacity (k-constant, 2.8 ± 1.8 vs. 3.9 ± 2.2; p = 0.02). This was supported by a greater rate of oxygen depletion during a sustained cuff occlusion (−0.04 ± 0.02 vs. −0.07 ± 0.03; p = 0.02). Resting muscle oxygen consumption was not affected by cranberry consumption. Our results suggest that cranberry supplementation may play a role in improving mitochondrial health, which could lead to better muscle oxidative capacity in healthy active adult populations.

Resveratrol (RSV), as a functional food component extracted from natural plants, has been widely studied and recognized in preventing and treating various diseases, with major mechanisms including executing anti-inflammation and anti-oxidation functions, and improving mitochondrial quality. Chronic diseases as non-communicable diseases are mainly caused by multiple factors, such as physiological decline and dysfunction in the body, and have become a significant challenge on public health worldwide. It is worth noting that chronic diseases such as Alzheimer’s disease (AD), Parkinson’s disease (PD), muscle atrophy, cardiovascular disease, obesity, and cancer are accompanied by abnormal mitochondrial function. Therefore, targeted regulation of mitochondria may be a meaningful way to prevent and treat chronic diseases. Increasing evidence has confirmed that RSV is actively involved in regulating mitochondria, and it has become an essential consideration to prevent and treat chronic diseases through targeting mitochondria and improving corresponding functions. In this article, current studies on RSV to optimize mitochondrial quality for preventing and alleviating chronic disease are systematically summarized, which can provide a theoretical reference for the development of functional foods or drugs to combat chronic diseases.

Background/objectives: To investigate the effect and regulatory mechanism of resveratrol supplementation on the mitochondrial energy metabolism of rats with exercise-induced fatigue. Materials/methods: Forty-eight Sprague-Dawley male rats were divided randomly into a blank control group (C), resveratrol group (R), exercise group (E), and exercise and resveratrol group (ER), with 12 rats in each group. Group ER and group E performed 6-wk swimming training with 5% wt-bearing, 60 min each time, 6 days a wk. Group ER was given resveratrol 50 mg/kg by gavage one hour after exercise; group R was only given resveratrol 50 mg/kg by gavage; group C and group E were fed normally. The same volume of solvent was given by gavage every day. Results: Resveratrol supplementation could reduce the plasma blood urea nitrogen content, creatine kinase activity, and malondialdehyde content in the skeletal muscle, increase the total superoxide dismutase activity in the skeletal muscle, and improve the fatigue state. Resveratrol supplementation could improve the activities of Ca2+-Mg2+-ATPase, Na+-K+-ATPase, succinate dehydrogenase, and citrate synthase in the skeletal muscle. Furthermore, resveratrol supplementation could up-regulate the sirtuin 1 (SIRT1)-proliferator-activated receptor gamma coactivator-1α (PGC-1α)-nuclear respiratory factor 1 pathway. Background: Delivery of oxygen to the mitochondria is essential for oxidative phosphorylation and the production of ATP during rest and during muscle contraction. Despite widespread application of near infrared spectroscopy (NIRS), to our knowledge it has not been used to evaluate whether oxygen delivery to the mitochondria, as assessed by post-ischemic reoxygenation parameters, is related to the oxidative capacity of the muscle. Purpose: To determine the relation between microvascular reactivity and mitochondrial oxidative capacity in the knee joint musculature using NIRS. Methods: Bilateral measurements of the vastus lateralis (VL) and medial hamstring (MH) muscles were taken in 14 moderate to highly active healthy adults (7M/7F, age: 22 ± 3 yrs, BMI: 21.3 ± 3.5 kg/m2 , physical activity levels: 4190 ± 1324 MET minutes/week). Two NIRS probes (Portamon, Artinis) were secured over the respective muscle bellies, which collected data during a 30 second bout of surface electrical stimulation that caused an increase in metabolism followed by brief arterial occlusions to determine the metabolic rate of the muscle over time. A final resting metabolism measurement was taken 5 minutes after the arterial occlusions to obtain the time to half recovery of metabolic rate (NIRS k), which is a valid estimate of mitochondrial oxidative capacity. Microvascular reactivity was measured as 1) the 10-second reoxygenation slope of the oxygen saturation signal (StO2) and 2) the half time to peak oxygenation following 5 minutes of arterial occlusion in each muscle. Results: There was a significant positive association between NIRS kand the reoxygenation slope in both the VL (r = 0.64, p = 0.025) and the MH (r = 0.53, p = 0.049) muscles. NIRS kvalues were also positively associated with the time to half peak oxygenation in the VL (r = 0.65, p = 0.021) and the MH (r = 0.60, p = 0.026) muscles. There were no differences between the VL and MH muscles for NIRS kvalues (Mean ± SD: 1.78 ± 0.24 vs. 1.78 ± 0.33, p = 0.99), 10-second reoxygenation slope (2.16 ± 0.84 vs. 1.99 ± 0.96, p = 0.65), and half time to peak oxygenation (8.4s ± 1.6 vs. 9.1s ± 2.2, p = 0.38). Conclusion: Our results suggest that microvascular reactivity is positively associated with the mitochondrial capacity of the knee joint musculature in this sample of healthy adults. Whether these associations exist in different clinical populations warrants future investigation.

The skeletal muscle (SkM) is the largest organ, which plays a vital role in controlling musculature, locomotion, body heat regulation, physical strength, and metabolism of the body. A sedentary lifestyle, aging, cachexia, denervation, immobilization, etc. Can lead to an imbalance between protein synthesis and degradation, which is further responsible for SkM atrophy (SmA). To date, the understanding of the mechanism of SkM mass loss is limited which also restricted the number of drugs to treat SmA. Thus, there is an urgent need to develop novel approaches to regulate muscle homeostasis. Presently, some natural products attained immense attraction to regulate SkM homeostasis. The natural products, i.e., polyphenols (resveratrol, curcumin), terpenoids (ursolic acid, tanshinone IIA, celastrol), flavonoids, alkaloids (tomatidine, magnoflorine), vitamin D, etc. exhibit strong potential against SmA. Some of these natural products have been reported to have equivalent potential to standard treatments to prevent body lean mass loss. Indeed, owing to the large complexity, diversity, and slow absorption rate of bioactive compounds made their usage quite challenging. Moreover, the use of natural products is controversial due to their partially known or elusive mechanism of action. Therefore, the present review summarizes various experimental and clinical evidence of some important bioactive compounds that shall help in the development of novel strategies to counteract SmA elicited by various causes.

Background To describe for the first time a patient with ankylosing spondylitis (AS) successfully treated with cycling, and marked improvement of his physical performance after nutraceuticals. Case report A 63-year-old male with a diagnosis of AS. He received several kinds of drugs. He practices physical activity since he was a teenager, and cycling 10 years ago reaching 30km/week. He had excellent disease control with cycling, and all medications were stopped. His physical examination demonstrates limitations of the lumbar and cervical spine movements. X-ray confirmed AS. He was treated with a nutraceutical supplementation and probiotics. He felt a progressive improvement in his physical performance and marked improvement of his mild residual AS clinical symptoms. After 1 year of follow up, he was doing 200km/week by cycling. Conclusion This study illustrates a patient with AS under control, who could get a higher performance cycling after nutraceuticals supplementation.

A decline in function and loss of mass, a condition known as sarcopenia, is observed in the skeletal muscles with aging. Sarcopenia has a negative effect on the quality of life of elderly. Individuals with sarcopenia are at particular risk for adverse outcomes, such as reduced mobility, fall-related injuries, and type 2 diabetes mellitus. Although the pathogenesis of sarcopenia is multifaceted, mitochondrial dysfunction is regarded as a major contributor for muscle aging. Hence, the development of preventive and therapeutic strategies to improve mitochondrial function during aging is imperative for sarcopenia treatment. However, effective and specific drugs that can be used for the treatment are not yet approved. Instead studies on the relationship between food intake and muscle aging have suggested that nutritional intake or dietary control could be an alternative approach for the amelioration of muscle aging. This narrative review approaches various nutritional components and diets as a treatment for sarcopenia by modulating mitochondrial homeostasis and improving mitochondria. Age-related changes in mitochondrial function and the molecular mechanisms that help improve mitochondrial homeostasis are discussed, and the nutritional components and diet that modulate these molecular mechanisms are addressed.

With rising technological advancements, several factors influence the lifestyle of people and stimulate chronic inflammation that severely affects the human body. Chronic inflammation leads to a broad range of physical and pathophysiological distress. For many years non-steroidal drugs and corticosteroids were most frequently used in treating inflammation and related ailments. However, long-term usage of these drugs aggravates the conditions of chronic diseases and is presented with morbid side effects, especially in old age. Hence, the quest for safe and less toxic anti-inflammatory compounds of high therapeutic potential with least adverse side effects has shifted researchers' attention to ancient medicinal system. Resveratrol (RSV) - 3,4,5' trihydroxystilbene is one such naturally available polyphenolic stilbene derivative obtained from various plant sources. For over 2000 years, these plants have been used in Asian medicinal system for curing inflammation-associated disorders. There is a wealth of in vitro, in vivo and clinical evidence that shows RSV could induce anti-aging health benefits including, anti-cancer, anti-inflammatory, anti-oxidant, phytoesterogenic, and cardio protective properties. However, the issue of rapid elimination of RSV through the metabolic system and its low bio-availability is of paramount importance which is being studied extensively. Therefore, in the present article, we scientifically reviewed the molecular targets, biological activities, beneficial and contradicting effects of RSV as evinced by clinical studies for the prevention and treatment of inflammation-mediated chronic disorders.

Based on the usefulness and importance among the spices, black pepper (Piper nigrum L.) is commonly referred to as “The King of Spices”. It is valued for its flavor, aroma, nutritional, and medicinal uses. The value of pepper is owed to its pungency and flavor, which is attributed to the presence of a naturally occurring alkaloid known as piperine. The amount of piperine varies in plants belonging to the Piperaceae family; it constitutes 2% to 7.4% of both black pepper and white pepper. It has been confirmed from the scientific studies that piperine has many bioactive effects, such as antimicrobial action, as well as many physiological effects that can contribute to general human health, including immunomodulatory, hepatoprotective, antioxidant, antitumor, and many other activities. Clinical studies demonstrated remarkable antioxidant, antitumor, and drug availability-enhancing characteristics of this compound, together with immunomodulatory potential. All these properties showed the therapeutic potentials of piperine and the need to incorporate this compound into general health-enhancing medical formulations, as well as into those that would be used as adjunctive therapy in order to enhance the bioavailability of various therapeutic drugs. The present chapter accounts for an overview of the therapeutic properties of piperine, the bioactive constituent of pepper. It also focuses on methods used to extract piperine from pepper along with recent approaches for the enhancement of the bioavailability of piperine.

The present study examined the effect of concurrent exercise training and daily resveratrol (RSV) supplementation (150 mg) on training-induced adaptations following low-dose high-intensity interval training (HIIT). Sixteen recreationally active (∼22 years, ∼51 mL·kg⁻¹·min⁻¹) men were randomly assigned in a double-blind fashion to either the RSV or placebo group with both groups performing 4 weeks of HIIT 3 days per week. Before and after training, participants had a resting muscle biopsy taken, completed a peak oxygen uptake test, a Wingate test, and a submaximal exercise test. A main effect of training (p < 0.05) and interaction effect (p < 0.05) on peak aerobic power was observed; post hoc pairwise comparisons revealed that a significant (p < 0.05) increase occurred in the placebo group only. Main effects of training (p < 0.05) were observed for both peak oxygen uptake (placebo – pretraining: 51.3 ± 1.8, post-training: 54.5 ± 1.5 mL·kg⁻¹·min⁻¹, effect size (ES) = 0.93; RSV – pretraining: 49.6 ± 2.2, post-training: 52.3 ± 2.5 mL·kg⁻¹·min⁻¹, ES = 0.50) and Wingate peak power (placebo: pretraining: 747 ± 39, post-training: 809 ± 31 W, ES = 0.84; RSV – pretraining: 679 ± 39, post-training: 691 ± 43 W, ES = 0.12). Fibre-type distribution was unchanged, while a main effect of training (p < 0.05) was observed for succinate dehydrogenase activity and glycogen content, but not α-glycerophosphate dehydrogenase activity or intramuscular lipids in type I and IIA fibres. The fold change in PGC-1α, SIRT1, and SOD2 gene expression following training was significantly (p < 0.05) lower in the RSV group than placebo. These results suggest that concurrent exercise training and RSV supplementation may alter the normal training response induced by low-volume HIIT.

To assess the effect on exercise capacity and tolerability of resveratrol, 13 healthy, sedentary adult volunteers were enrolled in a randomized crossover study comparing resveratrol and placebo over two 4-week periods, with a 2-week time between periods when subjects received no treatment. No significant changes in exercise duration or aerobic capacity (peak oxygen uptake) were observed. Gastrointestinal side effects were more common during resveratrol treatment (77% vs. 15%, p = 0.0048). A small reduction in fasting glucose and small but statistically significant increases in liver enzymes, total cholesterol, and triglycerides were observed, although mean results remained within normal limits. There was no change in complete blood count, inflammatory markers, renal function, or other measures of liver function.

Previous research has shown that resveratrol can increase cerebral blood flow (CBF) in the absence of improved cognitive performance in healthy, young human subjects during the performance of cognitively demanding tasks. This lack of cognitive effects may be due to low bioavailability and, in turn, reduced bioefficacy of resveratrol in vivo. Piperine can alter polyphenol pharmacokinetics, but previous studies have not investigated whether this affects the efficacy of the target compound. Therefore, the objective of the present study was to ascertain whether co-supplementation of piperine with resveratrol affects the bioavailability and efficacy of resveratrol with regard to cognition and CBF. The present study utilised a randomised, double-blind, placebo-controlled, within-subjects design, where twenty-three adults were given placebo, trans-resveratrol (250 mg) and trans-resveratrol with 20 mg piperine on separate days at least a week apart. After a 40 min rest/absorption period, the participants performed a selection of cognitive tasks and CBF was assessed throughout the period, in the frontal cortex, using near-IR spectroscopy. The presence of resveratrol and its conjugates in the plasma was confirmed by liquid chromatography-MS analysis carried out following the administration of the same doses in a separate cohort (n 6). The results indicated that when co-supplemented, piperine and resveratrol significantly augmented CBF during task performance in comparison with placebo and resveratrol alone. Cognitive function, mood and blood pressure were not affected. The plasma concentrations of resveratrol and its metabolites were not significantly different between the treatments, which indicates that co-supplementation of piperine with resveratrol enhances the bioefficacy of resveratrol with regard to CBF effects, but not cognitive performance, and does this without altering bioavailability.

(-)-Epigallocatechin-3-gallate (EGCG), from green tea (Camellia sinensis), has demonstrated chemopreventive activity in animal models of carcinogenesis. Previously, we reported the bioavailability of EGCG in rats (1.6%) and mice (26.5%). Here, we report that cotreatment with a second dietary component, piperine (from black pepper), enhanced the bioavailability of EGCG in mice. Intragastric coadministration of 163.8 mumol/kg EGCG and 70.2 mumol/kg piperine to male CF-1 mice increased the plasma C,a and area under the curve (AUC) by 1.3-fold compared to mice treated with EGCG only. Piperine appeared to increase EGCG bioavailability by inhibiting glucuronidation and gastrointestinal transit. Piperine (100 mumol/L) inhibited EGCG glucuronidation in mouse small intestine (by 40%) but not in hepatic microsomes. Piperine (20 mumol/L) also inhibited production of EGCG-3"-glucuronide in human HT-29 colon adenocarcinoma cells. Small intestinal EGCG levels in CF-1 mice following treatment with EGCG alone had a C-max = 37.50 +/- 22.50 nmol/g at 60 min that then decreased to 5.14 +/- 1.65 nmol/g at 90 min; however, cotreatment with piperine resulted in a C-max = 31.60 +/- 15.08 nmol/g at 90 min, and levels were maintained above 20 nmol/g until 180 min. This resulted in a significant increase in the small intestine EGCG AUC (4621.80 +/- 1958.72 vs. 1686.50 +/- 757.07 (nmol/g(.)min)). EGCG appearance in the colon and the feces of piperine-cotreated mice was slower than in mice treated with EGCG alone. The present study demonstrates the modulation of the EGCG bioavailablity by a second dietary component and illustrates a mechanism for interactions between dietary chemicals.

Resveratrol has emerged in recent years as a compound conferring strong protection against metabolic, cardiovascular and other age-related complications, including neurodegeneration and cancer. This has generated the notion that resveratrol treatment acts as a calorie-restriction mimetic, based on the many overlapping health benefits observed upon both interventions in diverse organisms, including yeast, worms, flies and rodents. Though studied for over a decade, the molecular mechanisms governing the therapeutic properties of resveratrol still remain elusive. Elucidating how resveratrol exerts its effects would not only provide new insights in its fundamental biological actions but also new avenues for the design and development of more potent drugs to efficiently manage metabolic disorders. In this review we will cover the most recent advances in the field, with special focus on the metabolic actions of resveratrol and the potential role of SIRT1 and AMPK. AIM: The present study aimed to compare in vivo measurements of skeletal muscle mitochondrial respiratory capacity made using near infrared spectroscopy (NIRS) with the current gold standard in situ measurements of high-resolution respirometry performed in permeabilized muscle fibers prepared from muscle biopsies. METHODS: Mitochondrial respiratory capacity was determined in 21 healthy adults in vivo using NIRS to measure the recovery kinetics of muscle oxygen consumption following a ~15 second isometric contraction of the vastus lateralis muscle. Maximal ADP-stimulated (State 3) respiration was measured in permeabilized muscle fibers using high-resolution respirometry with sequential titrations of saturating concentrations of metabolic substrates. RESULTS: Overall, the in vivo and in situ measurements were strongly correlated (Pearson’s r = 0.61 – 0.74, all p < 0.01). Bland-Altman plots also showed good agreement with no indication of bias. CONCLUSION: The results indicate that in vivo near infrared spectroscopy corresponds well with the current gold standard, in situ high-resolution respirometry, for assessing mitochondrial respiratory capacity.

Objectives: The primary aims of the study were to examine the effect of resveratrol on skeletal muscle SIRT1 expression and energy expenditure in subjects with Type 2 diabetes mellitus (T2DM). Background: Animal and in vivo studies indicate that resveratrol increases SIRT1 expression that stimulates PGC1α activity. Subsequent upregulation of AMPK and GLUT4 expression are associated with improved insulin sensitivity in peripheral tissues. Methods: Ten subjects with T2DM were randomized in a double-blind fashion to receive 3g resveratrol or placebo daily for 12 weeks. Secondary outcomes include measures of AMPK, p-AMPK and GLUT4 expression levels, energy expenditure, physical activity levels, distribution of abdominal adipose tissue and skeletal muscle fiber type composition, body weight, HbA1c, plasma lipid subfraction, adiponectin levels, and insulin sensitivity. Results: There was a significant increase in both SIRT1 expression (2.01 vs. 0.86 arbitrary units [AU], p = .016) and p-AMPK to AMPK expression ratio (2.04 vs. 0.79 AU, p = .032) in the resveratrol group compared with the placebo group. Although the percentage of absolute change (8.6 vs. -13.9%, p = .033) and percentage of predicted resting metabolic rate (RMR; 7.8 vs. -13.9%, p = .013) were increased following resveratrol, there was a significant reduction in average daily activity (-38 vs. 43.2%, p = .028) and step counts (-39.5 vs. 11.8%, p = .047) when compared with placebo. Conclusions: In patients with T2DM, treatment with resveratrol regulates energy expenditure through increased skeletal muscle SIRT1 and AMPK expression. These findings indicate that resveratrol may have beneficial exercise-mimetic effects in patients with T2DM.

Key points Ageing is associated with lifestyle‐related metabolic diseases, and exercise training has been suggested to counteract such metabolic deteriorations. The natural antioxidant resveratrol has been reported to exert ‘exercise‐like’ health beneficial metabolic and anti‐inflammatory effects in rodents, but little is known about the metabolic effects of resveratrol supplementation alone and in combination with exercise training in humans. The present findings showed that exercise training markedly improved muscle endurance, increased content and activity of oxidative proteins in skeletal muscle and reduced markers of oxidative stress and inflammation in skeletal muscle of aged men. Resveratrol alone did not elicit metabolic effects in healthy aged subjects, but even impaired the exercise training‐induced improvements in markers of oxidative stress and inflammation in skeletal muscle. Abstract The aim was to investigate the metabolic and anti‐inflammatory effects of resveratrol alone and when combined with exercise training in skeletal muscle of aged human subjects. Healthy, physically inactive men (60–72 years old) were randomized to either 8 weeks of daily intake of 250 mg resveratrol or placebo or to 8 weeks of high‐intensity exercise training with 250 mg resveratrol or placebo. Before and after the interventions, resting blood samples and muscle biopsies were obtained and a one‐legged knee‐extensor endurance exercise test was performed. Exercise training increased skeletal muscle peroxisome proliferator‐activated receptor‐γ co‐activator‐1α mRNA ∼1.5‐fold, cytochrome c protein ∼1.3‐fold, cytochrome c oxidase I protein ∼1.5‐fold, citrate synthase activity ∼1.3‐fold, 3‐hydroxyacyl‐CoA dehydrogenase activity ∼1.3‐fold, inhibitor of κB‐α and inhibitor of κB‐β protein content ∼1.3‐fold and time to exhaustion in the one‐legged knee‐extensor endurance exercise test by ∼1.2‐fold, with no significant additive or adverse effects of resveratrol on these parameters. Despite an overall ∼25% reduction in total acetylation level in skeletal muscle with resveratrol, no exclusive resveratrol‐mediated metabolic effects were observed on the investigated parameters. Notably, however, resveratrol blunted an exercise training‐induced decrease (∼20%) in protein carbonylation and decrease (∼40%) in tumour necrosis factor α mRNA content in skeletal muscle. In conclusion, resveratrol did not elicit metabolic improvements in healthy aged subjects; in fact, resveratrol even impaired the observed exercise training‐induced improvements in markers of oxidative stress and inflammation in skeletal muscle. Collectively, this highlights the metabolic efficacy of exercise training in aged subjects and does not support the contention that resveratrol is a potential exercise mimetic in healthy aged subjects.
