Assessing the impact of a fire safety intervention

Abstract

Objectives: Cheshire Fire and Rescue Service (CFRS) has experienced a strategic shift in their community safety work, this has involved the delivery of a fire safety intervention, the Home Safety Assessment (HSA). The primary objective of this study was to explore the impact of this intervention. The second objective was to investigate the role of self-efficacy in the uptake of fire safe behaviours. Design: This research was conducted within a wider mixed methods PhD study into the implementation of the HSAs by CFRS. This phase of the research employed a cross-sectional survey design investigating whether individuals changed their behaviour to be more fire safe in their home. Methods: Participants completed a questionnaire, with questions focussing on the rating of the HSA, behavioural change in response to the advice, and their self-efficacy in continuing with the advice given. 2000 residents from across Cheshire were randomly selected to receive the postal questionnaire, and 311 completed questionnaires were returned. Results: A majority of the participants agreed that they took steps to make their home safer since the HSA (N=183), they still follow the majority of the advice (N=211), and they regularly test their smoke alarm (N=193). Self-efficacy was positively correlated with taking steps to make their homes safer (r=.495, p=.000), and, still following the advice given during the HSA (r=.523, p=.000). Conclusions: The study highlights the usefulness of the HSA in promoting fire safety and suggests that those with higher self-efficacy are more likely to continue with fire safe behaviours after the HSA.

The present chapter describes brief health-specific self-efficacy scales that were developed to examine the relationship between self-efficacy, intentions, and behaviors in the context of large-scale field studies designed to screen diverse populations. The idea was to construct parsimonious measures that can be integrated into more comprehensive questionnaires. The scales were not developed for clinical settings, although it would be worthwhile to study them there. The measures to assess perceived self-efficacy for preventive nutrition, physical exercise, and alcohol resistance were tested in the German versions. Adaptations to other languages have not yet been evaluated.

Convergent evidence from the diverse lines of research reported in the present special issue of this journal attests to the explanatory and predictive generality of self-efficacy theory. This commentary addresses itself to conceptual and empirical issues concerning the nature and function of self-percepts of efficacy.

The chapters, by internationally known experts, cover such concepts as infancy and personal agency, competency through the life span, the role of family, and cross-cultural factors.

This article examines health promotion and disease prevention from the perspective of social cognitive theory. The areas of overlap with some of the most widely applied psychosocial models of health are identified. The models of health promotion and disease prevention have undergone several generational changes. We have shifted from trying to scare people into health, to rewarding them into health, to equipping them with self-regulatory skills to manage their health habits, to shoring up their habit changes with dependable social supports. These transformations have evolved a multifaceted approach that addresses the reciprocal interplay between self-regulatory and environmental determinants of health behavior. Social cognitive theory addresses the socio structural determinants of health as well as the personal determinants. A comprehensive approach to health promotion requires changing the practices of social systems that have widespread detrimental effects on health rather than solely changing the habits of individuals. Further progress in this field requires building new structures for health promotion, new systems for risk reduction and greater emphasis on health policy initiatives. People's beliefs in their collective efficacy to accomplish social change, therefore, play a key role in the policy and public health approach to health promotion and disease prevention.

Background Deaths and injuries related to fires are largely preventable events. In the UK, a plethora of community-based fire safety initiatives have been introduced over the last 25 years, often led by fire and rescue services, to address this issue. This paper focuses on one such initiative-home safety assessments (HSAs). Cheshire Fire and Rescue Service (in England) implemented a uniquely large-scale HSA intervention. This paper assesses its effectiveness.Methods The impact of HSAs was assessed in relation to three outcomes: accidental dwelling fires (ADFs), ADFs contained and injuries arising from ADFs. A two-period comparison in fire-related rates of incidences in Cheshire between 2002 and 2011 was implemented, using Poisson regression and adjusting for the national temporal trend using a control group comprising the 37 other English non-metropolitan fire-services.ResultsSignificant reductions were observed in rates of ADFs [incidence rate ratios (IRR): 0.79, 95% confidence interval (CI): 0.74-0.83, P < 0.001, 2002/03-2007/08 versus 2008/09-2010/11] and associated injuries (IRR: 0.49, 95% CI: 0.39-0.60, P < 0.001, 2002/03-2006/07 versus 2007/08-2010/11), but not in the proportion of fires contained to room of origin.Conclusions There is strong evidence to suggest that the intervention was successful in reducing domestic fires and related injuries. © 2012 © The Author 2012, Published by Oxford University Press on behalf of Faculty of Public Health.
