Pharmaco-Therapeutic Profiles of Chandraprabhavati- An Ayurvedic Herbo-Mineral Formulation

Abstract

The knowledge about indigenous therapeutics was deposited in the ayurvedic classics like Charaka Samhita, Sushruta Samhita, Ashtanga hrudaya etc. These works of ancient period mainly incorporated herbal formulation and a few herbo-mineral compounds. During medieval period the works like Gadanigraha, Chakradatta and Saranagadhara Samhita included certain herbo-mineral and mineral preparations (containing mercury and certain heavy metal like Gold, Lead etc.). Among the herbo-mineral preparations ChandraprabhaVati was formulated with broad spectrum activity. It is a safe formulation containing Loha (Iron)and shilajita (Bitumen) without any heavy metals.Certain researches had been carried out related to its hypoglycaemic, hypotensive and bacteriostatic activities.Pharmaco-therapeutic profile clearly indicate that Chandraprabha Vati is useful in 31 disease conditions like Prameha (Urinary disease including diabetes), Ashmari (Urolithiasis), Katishoola (lumbago), Shoola (Pain in abdomen), Kamala (Jaundice), Pleeharoga (Splenomegaly), Arsha (Piles), Bhagandhara (Fistula in ano), shukradosha (Defective spermatogenesis), Artavaruja (Dysmenorrhea)etc. The details are furnished in this paper.

INTRODUCTION
AcharyaCharaka identified three important organs
i.e. Shira (Head/Brain), Hridaya (Heart) and Vasti
(Urinary bladder and related organs of urine
formation) under the heading of Trimarmas
(Three vital parts of the body) [1]

. Keeping in view
of the importance of these organs Charaka had
allocated a separate chapter which details the
management of different diseases originating or
involving these organs. Ayurvedic claims
enumerated for certain diseases related to Vasti
such as Mutrakruchhra (Dysuria), Mutraghata
(Anuria), Prameha (It includes diabetes mellitus
and D. insipidus), Asthila (Benign prostate
hypertrophy) and Ashmari (Urolithiasis), Sotha
(Edematous condition due to involvement of
Heart, Liver, Kidney), Chhardi (Vomiting
including condition like Uraemia), Hikka
(Hiccup), Adhogata raktapitta (Haematuria)have
also been described with certain formulations
containing Punarnava, Gokshura, Dashamoola
etc. ChandraprabhaVati appears to be one of the
potent herbo-mineral formulation having wider
therapeutic application in such conditions.

Sarvameha
Various Ayurvedic formulations with the name
of ChandraprabhaVati:
Various formulations quoted in ayurvedic texts
with the name of ChandraprabhaVati are
analysed with a main emphasis on the marketed
formulations currently being prescribed in general
practice. The published data on the studies carried
out with ChandraprabhaVati have also been
reviewed.
In total 8 formulations with the name of
ChandraprabhaVati were recorded in various
compendia of Ayurveda like Harita Samhita,
Gada nigraha, Vangasena, Sharangadhara
samhita, Bhavaprakasha samhita, Yogaratnakara,
Bhaishajya ratnavali, Rasa Rajasundara, Rasa
ratnasamuchhaya, Rasachandashu, Brihat
yogatarangaini, Rasakamadhenu,
Rasasarasamgraha (Table 1).
Ingredient of ChandraprabhaVati:
The analysis of data clearly indicates that, out of 8
formulations four formulations have mercury and
sulphur combination. Though Chandraprabha
Vati mainly indicated in treatment of Prameha,
Mutravaha srotaja vikara but the other
formulations with the same name quoted in
various texts have incorporated the diseases like
Kushtha, Atisara, Jwara, Udararoga, Netraroga,
Dantaroga, Gudagata Vyadhi in the list of
indication.
ChandraprabhaVatifromSharanagadharasamhita
was included in Ayurvedic formulary of India [3]
Most of the pharmacies like Baidyanath, Dabur,
AVN, Kottakkal Arya Vaidyashala, Sri Sri
Pharmacy, Divya pharmacy are preparing the
Chandraprabhavatiaccording to formulation
mentioned by
Sharangadharasamhita.(http://ayurmedinfo.com/)
The drugs having action on major system like
urinary system, reproductive system are
incorporated in this formulation. It is a herbo-
mineral compound preparation. Its ingredients are
listed below
.

It appearsthat ChadraprabhaVati mostly acts on
Tridoshas; Rakta, Mamsa, Meda, Shukra Dhatu;
Purisha, Mutra malas; Rasavaha, Raktavaha,
Mamsavaha, Medovaha, Shukravaha,
Purishavaha andMutravaha srotasa.
Research studies:
1. An experimental study was carried out to
observe the effect of ChandraprabhaVati
on alloxan induced diabetes in rabbits and
significant regression of fasting blood
sugar was noted. The improvement in
hyperglycaemia suggests that either the
drug helps in regeneration of the islets of
Langerhans or it stimulates the secretion of
undamaged ones.
2. In a clinical experiment the single dose of
ChandraprabhaVati on blood sugar in
diabetes has been estimated. And the effect
of ChandraprabhaVati on blood sugar
level in usual subjects has been estimated.
In both these experiments the fasting blood
sugar has been recorded. The peak of the
action has been noted in 2 hours and
hypoglycaemic effect had a reducing trend
after 3 hrs.
In another study ChandraprabhaVati has
been administered in the dose of 3gm/day
in 3 divided doses before meals, for the
period of 3 months in 20 cases of maturity
onset DM. The patients were kept on

recording and it was analysed statistically.
The difference was classified as good, fair
and poor control following the criteria of
Joslin’s clinics. On this basis, the inference
has been drawn that ChandraprabhaVati is
a dependable oral hypoglycaemic agent
suitable for the use from mild to moderate
cases of maturity onset diabetes.

3. An experimental study had been carried
out to evaluate anti-inflammatory activity
of two ayurvedic formulations containing
Guggului.e.ChandraprabhaVati (CPV)
and Mahayogarajaguggulu (MYG) by
using ethanol extract of CPV and MYG in
carrageenan induced rat paw edema. This

4. Polycystic Ovarian Syndrome (PCOS) can
be correlated with the condition known as
AartavaKshayadescribed in Ayurvedic
literature. It is common endocrinopathy in
women of reproductive age. Therefore a
study was carried out to evaluate clinical
efficacy of Ayurveda treatment regimen on
Subfertility with poly cystic ovarian
syndrome (PCOS). Total 40 patients were
selected by using purposive sampling
method. According to the theories of
Ayurveda of Shodhana, Shamana and
Tarpana, the treatment was conducted in 3
stages for the duration of 6 months. The
response to the treatment was recorded and
therapeutic effects were evaluated by
symptomatic relief and through Trans
vaginal scan and LH, FSH hormone levels.
The results revealed that, subfertility due
to PCOS can be cured successfully by
using this Ayurvedic treatment regimen
.

5. Mutraghata’ is a group of obstructive
uropathic disorders, as explained in
SushrutaSamhita, is of 12 types &
Vatashtila’ is one among them, which is
closely similar to Benign Prostatic
Hyperplasia (BPH). Usually men around
60 years, suffer from Mild, Moderate &
Severe grade of BPH. In classics
ChandraprabhaVati(BhaishajyaRatnavali)
&Mustadikalpa (Sushruta Samhita) are
indicated in Mutraghata&Mutrakruccha.
So the study was carried out as single
blind method of diagnosed cases of B.P.H,

Chandraprabhavati 1-tid &Mustadikalpa
1-tsp bid given for 30 days & follow-up
was carried upto of 60 days.Changes in
subjective criteria (by IPSS Index) and
objective criteria (by USG) were recorded
before and after the treatment and relief
from the symptoms were

DISCUSSION
The drugs of ChandraprabhaVatilike Yavakshara
and Svarjikakshara- alkaline substances decrease
the acidity of urine and gives relief in burning
micturition. The drugs like Pippalimoola [12] and
Vacha[13,14]act as sedative and relieve anxiety
which help in the management of psycho-somatic
diseases like hypertension, diabetes and obesity.
The drugs namely Trikatu, Chavya, Musta,
Elavaluka, Lavanas, Ksharas help to relieve the
state of Mandagni which is identified as a root
cause for all disease conditions.
ChandraprabhaVati successfully prevents
Diabetic neuropathy, Diabetic retinopathy and
mayprevents the renin formation in Kidney which
is a known cause for essential hypertension. The
drugs like Musta[15], Triphala, Shilajita and
Guggulu[16,17]
AcharyaVagbhata suggests Shilajita as the drug of
choice for the management of Vastigatavyadhi
(Urinary disease)
help to treat dyslipidaemias and
prevent ischaemic condition in all the organs in
the body including Kidney, heart, liver and brain.
[18]. The diseases related to
Mutra (Urine) like Mutraghata (Anuria),
Mutrakrichhra (Dysuria),Ashmari (Urolisthesis)
are indicated for therapeutic application of
Chandraprabhavati. Prameha includes the disease
conditions like Diabetes mellitus and research
studies proved anti-diabetic activity of
ChandraprabhaVati. The drug namely Guggulu is
included in higher quantity which is a proven anti-
inflammatory drug [19]. Inflammatory conditions
in entire genito-urinary tract are amenable with
administration of ChandraprabhaVati. The
conditions like cystitis which presents as dysuria;
the disease like BPH, Epididymitis, Salpingo-
oopheritis are addressed with ChandraprabhaVati.
The formulation acts as anti-spasmodic and
analgesic agent to manage Kashthartava
(Dysmenorrhoea) and Katishoola (Lumbago). It
relieves the infection and acts as spermatopoietic
agent. In a nut shell this is employed in male as
well as female infertility. The drugs like Triphala

conditions of autoimmune and immune-deficient
state. It protects the liver and act as hematopoietic
agent. Though it contains salts and sugar, the
formulation can be suggested for hypertension and
diabetes as each tablet of 250mg or 500mg
contains least quantity (in milligram) of salt and
sugar.
CONCLUSION
ChandraprabhaVaticontains 37 ingredients of
plant and mineral origin, used in the treatment of
diabetes, diseases of urinary tract, and many other
disease conditions having involvement of all the
three Doshasi.e. Vata, Pitta, Kapha..It has broad
spectrum activity for the management of life style
disorders. It is widely used in Ayurvedic practice,
across the country.

When menopausal syndrome incorporates vasomotor, genito-urinary, psychological, and general indicators of natural ageing, it can be disturbing. Hormone Replacement Therapy (HRT) was assumed to be the only therapeutic approach for menopause. HRT and selective oestrogen receptor modulators (SERMs) are only advantageous for short-term usage since they are associated with a risk of developing breast cancer, ovarian cancer, heart attacks, strokes, gall bladder disease, thromboembolism, and urine incontinence. Single drugs and compound formulas described in several Ayurvedic scriptures and validated by scientific findings open new paths in the treatment of menopausal syndrome which is reported in this paper. Diagnosis and outcome- The onset of dyslipidemia, osteoarthritis, and metabolic disorders with the onset of menopause confirmed the diagnosis of menopausal syndrome. The patient became asymptomatic at the end of three months of treatment with improved liver, lipid, and diabetic profile. Conclusion- The findings of this case paper can be useful in the prevention of harmful effects of prolonged HRT and a holistic treatment protocol can opt for the management of similar cases.

Diabetes mellitus Type 2 (DM Type 2) is a major health problem in the present era. It is characterized by increased blood sugar level (BSL) with or without some classical features such as polydipsia, polyphagia, and polyuria. In Ayurveda, diabetes comes under a Vatic subtype of Prameha -called Madhumeha (~DM). The present study deals with a newly diagnosed case of Type 2 DM since February 22, 2019, with no family history of diabetes and oral hypoglycemic agent. His BSLs were high with associated symptoms such as pain in multiple joints, burning micturition, and pindikoudvesatna (~cramps while walking/calf muscle pain). Ayurveda-based pathogenesis suggested that the patient is of Kapha-Pittavrit-VataDosha (~a biohumor) is the chief vitiating factor and also vitiation of Meda, Kleda, Vasa, Ambu , and Ojas with Dhatwagnimandya are the co-factor. By considering the Nidana (~etiological factors), Dosha (~a biohumor), and Dusya (~tissue elements), a judicial combination of Ayurveda drugs such as Gokshuradi Guggulu, Gudmar Patra, Jamun Bija, Sudarshan, Nagarmotha Churna , and Chandraprabha Vati was advised to take orally in morning and evening with lukewarm water and proper diet and lifestyle modification was also prescribed. At the time of consultation, his fasting blood sugar (FBS) and postprandial blood sugar (PPBS) were 377 mg/dl and over 450 mg/dl, respectively, along with glycated hemoglobin (HbA1c) as 16.7%. After taking the prescribed formulation for 4 months, his FBS and PPBS levels came down to the normal range, but HbA1c level declined at 6 months to 6.4%. The prescribed antidiabetic Ayurvedic medicines showed good response by maintaining normal BSLs in a Type 2 DM case.

In the present era, there is a heightened awareness about the safety of drugs rather than the efficacy, which is largely due to the side effects as well as their consequences. This case study highlights the benefits of Ayurvedic medicine, which has yielded positive results by reducing hypo-pigmented patches of vitiligo, though the treatment was intended to treat Necrozoospermia. Vitiligo is an autoimmune cutaneous disorder of pigmentation which is correlated with Shwitra in Ayurveda. Main line of treatment for Shwitra in Ayurveda is Snehana (medicated fats) Shodhana (Purificatory therapies) followed by anointing body and exposure to sunlight. However, in this present case astounding effects were seen when the patient was treated only with internal medication for Necrozoospermia without considering Shwitra. Fifty-six years old man approached with chief complaint of secondary infertility and was diagnosed as Necrozoospermia (Shukra Dosha). There were hypo-pigmented patches of Vitiligo on his forehead. The patient was given Koshtha shuddhi (purgation) with Eranda Taila (medicated castor oil) to treat shukra dosha followed by medicines like Arogyavardhini Vati, Chandraprabha Vati, Shilapravanga and Phala Ghrita (medicated ghee). At the end of 3½ month of treatment, marked improvement in sperm count and increase in the sperm motility was observed, along with that vitiligo patches on the forehead were reduced remarkably without any external application. The present case study shows the effective management of vitiligo with Ayurvedic formulations without use of external applications highlighting the basic Dhatu Siddhanta (theory of tissue formation) of Ayurveda.

ChandraprabhaVati is a very commonly used herbo-mineral preparation in dayto-day practice being efficient in various disorders mostly genitor-urinary ones. Chandraprabha Vati is available in various ayurvedic texts with different preparations and different ingredients. The preparation, described by Sarangdhar Samhita and accepted by AFI, is being taken for a critical review of its properties, actions and its clinical efficacies in different diseases. It is made up of 37 ingredients with Shilajit and Guggulu present in maximum quantity 32 parts each. It is katu (73.38%) tikta (69.78%) in rasa, laghu (76.98%) ruksha (74.10%) in guna, ushna virya (70.50%), katu vipaka (69.06%) and tridosha shamak predominantly Vatakapha shamak (91.37% and 84.89% respectively). This preparation is indicated in Prameha, Mutravahasrotaja vyadhi, Anaha, Shoola, Shwasa, Kasa, Shukradosha, Pandu, Kamala, GudagataVyadhi, Netraroga, Dantaroga, Kushtha and various other diseases as mentioned in different texts. Clinical studies have also proved its efficacy in various diseases. It is antidiabetic, anti-inflammatory, anti-tumour, renal protective, hepato-protective, effective in jaundice and genitor-urinary diseases like UTI, dysmenorrhoea, PCOD, female infertility, etc. and many diseases are still left to be explored. Chandraprabha Vati is a multifaced drug and can be used in a variety of ailments successfully.

A healthy woman is a potential of a sound family. Due to alteration in lifestyle, faulty food habits, use of junk foods, inability to follow the guidelines of Dincharya (daily routine), rules and regulation onset and during menstruation, and Sutikaparicharya (post-natal period routine explained in the literature for the wellbeing of women's health. Because of such reasons, she is prone to cause various vaginal disorders. Yonigat Shewta-Picchil strava, Yonikandu, Yonigata Alpa vedana, which are the features of Kaphaja yonivyapada, and most of the women neglect such minor sign and symptoms which may lead to significant diseases. Kaphaja yonivyapada is very common nowadays in women due to faulty diet regimens. The kaphaja yonivyapada was managed with Basti (vasti), a panchakarma procedure, and some internal medications. Case: A 29 years old female patient was presented with chief complaints for 5-6 years, i.e., itching in the vaginal region, headache, white discharge before menses, smell during micturition, and yellowish discoloration body parts. Result & Observations: External procedures, Panchakarma treatment, and some internal medications were given to the patient for seven days, 15 days, and for one month, respectively. The patient had 50% relief in 15 days of treatment and, over one month, relived all the complaints. Conclusion: In Kaphaja Yonivyapada, Varti administration, cow's urine for Basti, and using Katu Kashaya, Ushna, Rruksha drugs plays a substantial role in treating a disease thoroughly.

Background: Male Infertility is one of the burning issues now a day’s nevertheless disregarded reproductive health problems in India. Incidences of this issue expands day by day because of the disturbing lifestyle pattern. Almost 30-40-% of infertility cases found to be related to male factor. Asthenozoospermia is the most common identifiable anomaly related to male infertility found in semen analysis having reduced motility of sperm. Aim and Objectives: To assess the efficacy of Ayurvedic management (Shodhana and Shamana Chikitsa) in the management of Ksheena Shukra Vikara w.s.r. to Asthenozoospermia. Methods: It is a single case study. A 33-year-old male patient who was already diagnosed with Asthenozoospermia for three years approached to Pancharkarma OPD. Sperm motility was only 12%. The patient was treated with Shodhana Chikitsa (Vamana and Virechana with Mahatiktaka Ghritapana and Dashmooladi Niruha Vasti and Uttarvasti with Vidaryadi Ghrita followed by Shamana Chikitsa (Tab Neo Charak Pharmacy, Tab Addyzoa, Chandraprabha Vati, Paripathadi Kashaya, Ashwagandhadi Yamaka, Avipattikar Churna) approximately 3 months. After 3 months, patient-reported improvement. Results: Assessment of the patient with clinical symptoms and sperm analysis report was done following 3 months. Sperm motility increased up to 40% with increment in sperm count. Conclusion: This case report provides us a guideline that infertility associated with Asthenozoospermia can be treated successfully by adopting basic Ayurveda Siddhanta's.
