The Effects of Aromatherapy on Sleep Improvement: A Systematic Literature Review and Meta-Analysis

Abstract and Figures

Objectives: To evaluate the existing data on aromatherapy interventions for improvement of sleep quality. Design: Systematic literature review and meta-analysis on the effects of aromatherapy. Study Sources: Electronic databases, including the Korea Education and Research Information Service (KERIS), Korean studies Information Service System (KISS), National Assembly Library, and eight academies within the Korean Society of Nursing Science, were searched to identify studies published between 2000 and August 2013. Study selection: Randomized controlled and quasi-experimental trials that included aromatherapy for the improvement of sleep quality. Of the 245 publications identified, 13 studies met the inclusion and exclusion criteria, and 12 studies were used in the meta-analysis. Results: Meta-analysis of the 12 studies using a random-effects model revealed that the use of aromatherapy was effective in improving sleep quality (95% confidence interval [CI], 0.540-1.745; Z=3.716). Subgroup analysis revealed that inhalation aromatherapy (95% CI, 0.792-1.541; Z=6.107) was more effective than massage therapy (95% CI, 0.128-2.166; Z=2.205) in unhealthy (95% CI, 0.248-1.100; Z=3.100) and healthy (95% CI, 0.393-5.104; Z=2.287) participants, respectively. Conclusion: Readily available aromatherapy treatments appear to be effective and promote sleep. Thus, it is essential to develop specific guidelines for the efficient use of aromatherapy.

Purpose This meta-analysis was to critically evaluate the effects of aromatherapy on the symptoms of anxiety and depression in cancer patients. Methods Eight Chinese and English databases (CNKI, Wanfang, VIP, CBM, Cochrane Library, PubMed, Embase, and PsycINFO) were systematically searched from the inception of databases to October 2021 for randomized controlled trials (RCTs). According to Cochrane Collaboration criteria, two reviewers independently assessed the risk of bias and extract data from included studies. All analyses were performed with Review Manager 5.4. Results Eleven qualified studies were included in the meta-analysis, ten of which reported the effect of aromatherapy on anxiety in cancer patients, including 1724 patients; five of the studies reported the effect on depression, including 1039 patients. The quality of the included studies was low, and most studies compared aromatherapy to usual care. This meta-analysis indicated that aromatherapy appeared to be effective for anxiety [SMD = -0.51, 95%CI (−0.83, −0.19), P = 0.002] and depression [SMD = -0.44, 95%CI (−0.76, −0.12), P = 0.008] symptoms in cancer patients. Inhalation aromatherapy may be more effective than massage. Aromatherapy seemed to improve the anxiety symptoms in cancer patients in a short time, especially in perioperative patients, but had no effect for patients treated with radiation, chemotherapy or palliative therapy. Placebo also appeared to be effective in some studies. Conclusions Aromatherapy, especially inhalation aromatherapy, may help relieve symptoms of anxiety and depression in cancer patients, but more and higher-quality studies are needed. The literature does not yet support clinical implementation.

Objective To investigate the effect of aromatherapy on sleep quality in cancer patients. Methods Published literature on the effect of aromatherapy in cancer patients with sleep disorders in the form of randomized controlled trials (RCTs) were systematically retrieved and screened from PubMed, Cochrane Library, Embase, CBM, CNKI, VIP, and Wanfang databases from inception to November 2021. The methodological quality of the included studies was critically and independently evaluated by two reviewers using the Cochrane Risk of Bias Assessment Tool for RCTs. The correlated data were extracted using the pre-designed form, and all analyses were performed using Reviewer Manager version 5.4. Due to the difference in sleep quality instruments, the data extracted in this study were in the form of standard mean difference (SMD). Results Ten RCTs included 933 patients (experimental group: 474, control group: 459), and the risk of bias in the included studies was moderate. Aromatherapy could significantly improve the sleep quality of cancer patients [SMD = − 0.79, 95% CI (− 0.93, − 0.66), p < 0.01], especially those with breast cancer [SMD = − 0.98, 95% CI (− 1.57, − 0.40), p < 0.01]. Aromatherapy with single essential oil had a better effect on sleep quality [SMD = -0.94, 95%CI (− 1.25, − 0.62), p < 0.01], of which lavender essential oil had the best effect [SMD = -1.06,95%CI (− 1.49, − 0.63), p < 0.01] while compound essential oils had no effect on sleep quality improvement in cancer patients [SMD = -0.21, 95%CI (− 0.57, 0.14), p = 0.23]. Four of the ten RCTs reported the occurrence of adverse events, of which only one RCT indicated that patients had headache and sneezing while the remaining six did not. Conclusions This meta-analysis of 10 RCTs reveals that aromatherapy with single essential oil had a substantial effect on the sleep quality of cancer patients and should be recommended as a beneficial complementary therapy to promote sleep quality in cancer patients.

As a valuable medicinal herb and spice, agarwood is widely used in the fields of daily chemistry, traditional medicine, religion and literary collection. It mainly contains sesquiterpenes and 2-(2-phenylethyl)chromones, which are often used to soothe the body and mind, relieve anxiety, act as an antidepressant and treat insomnia and other mental disorders, presenting a good calming effect. This paper reviews the chemical composition of the essential oils of different sources of agarwood, as well as the progress of research on the sedative and tranquilizing pharmacological activity and mechanism of action of agarwood essential oil (AEO), and then analyzes the current problems of AEO research and its application prospects in the treatment of mental diseases.

College students, particularly female students, often suffer from severe stress and poor sleep. Aromatherapy yoga has become a popular exercise in recent years and may help reduce stress and improve sleep quality, although empirical evidence is limited. We investigated the effectiveness of aromatherapy yoga intervention in reducing stress and improving sleep quality among Chinese female college students. A total of 89 participants—44 in the experimental group (aromatherapy yoga) and 45 in the control group (yoga)—with an average age of 19.88 ± 1.13 years, were enrolled in this quasi-experimental study. Interventions were performed in the respective groups once a week for 12 weeks, for 90 min each time. The Wilcoxon signed-rank test and Mann–Whitney U test (including effect size) were used to test the differences in stress reduction and sleep quality before and after the intervention. From the findings of the current research, both therapies did not help female students’ stress levels or sleep hygiene significantly. However, aromatherapy yoga has the potential to improve sleep disturbances experienced by female college students. Additionally, there was no difference in the amount of stress or the quality of sleep between the aromatherapy yoga groups and the regular yoga groups before and after therapy.

Bedtime procrastination refers to delaying bedtime without external reasons. It has become serious among the younger generation due to smartphone popularity, leading to several physical and mental disorders. Odor cue has the potential to reduce bedtime procrastination, as prior research has demonstrated its effectiveness in conveying information and triggering desired emotion and behaviors. However, limited research investigates the effect of odor on bedtime regulation outside of clinical settings. We designed and implemented SleepyCloud, a device aiming to deliver odor cue one hour before people’s scheduled bedtime. Our study examines the effectiveness of odor cue on reducing students’ bedtime procrastination using SleepyCloud. Our results suggested that the odor cue was helpful on reducing bedtime procrastination for those who procrastinated due to leisure activities, and that the odor cue also helped regulate sleep. We provide implications for future investigation on this topic.

Given the high pressures of life in modern society, promoting relaxation can improve the quality of daily life. This study proposes a relaxation system that uses monaural beats with ultralow-frequency inaudible sounds to promote relaxation. To effectively promote relaxation, monaural beats in the δ band, θ band, and α band were used and evaluated. To avoid directly influencing people’s activities, the frequencies of the stimulation sounds, which were used to generate the target beats, were out of the frequency range of people’s hearing sense. To examine the performance of the proposed approaches, a subjective testing procedure was designed, and paired-sample t tests were performed. The p values for the experimental relaxation results with monaural beats in the δ band, θ band, and α band were 0.0001, 0.1195, and 0.0065, respectively. Furthermore, the experimental results revealed specific effects of the proposed relaxation system; for example, the δ band had the greatest effect regardless of whether the average, variance, or p value was assessed. On the basis of our experimental results, the proposed relaxation system model can help promote relaxation.

Current literature has identified relationships among sleep, nutrition, and diet-related chronic diseases; however, knowledge about how sleep influences diet-related diseases is lacking in dietetics practice. This narrative review briefly explains sleep physiology and outlines the relationships between sleep duration and quality and common nutrition-related diseases, including obesity, diabetes, cardiovascular disease, obstructive sleep apnea, and cancer. Additionally, the review discusses how sleep influences wound healing and pregnancy outcomes and why hospitalized patients are likely to experience sleep problems. Plausible mechanisms explaining the relationships between sleep and disease are presented. Finally, commonly used sleep assessment tools and interventions are reviewed. Given the importance of sleep to health, dietitians should not only be aware of the role sleep plays in disease development and prevention but also assess sleep when feasible and refer patients and clients who are at high risk for sleep problems to a sleep clinic or community program that can address sleep issues.Teaching points:• Sleep duration and quality influence risk and outcomes of common nutrition-related diseases.• Sleep health evaluation is a missing piece in dietetic practice.• There are easy-to-use, validated tools that dietitians can use to screen for sleep problems in order to refer patients and clients to sleep experts.

Sleep disorders are one of the most prevalent psychiatric diseases. Insomnia is the most common sleep disorder amongst the general population. It is also one of the most frequent complaints in primary healthcare centers. Lavender is called “the broom of the brain” in different oriental traditional medicines. It is one of the most used plants for patients with sleep disorders. This study reviews what is currently known about the use of lavender for sleep disorders in patients with different diseases, from cancers and end‐stage renal disease to neurological‐psychiatric diseases (e.g., depression, dementia, and autism), respiratory, cardiac, and metabolic diseases. Additionally, its most used administration route is the inhalation of its essential oil (i.e., aromatherapy) alone or in combination by massage. Some limitations of the reviewed literature were discussed briefly. Overall, this critical review provides promising evidence of the lavender efficacy for sleep disorders in a wide variety of populations and diseases. However, further clinical studies with robust design and longer durations of intervention are necessary for more evidence‐based judgment on its effect on sleep problems and to investigate its mechanism of action.

Purpose: The purpose of this study is to find out the effects of the aroma massage on shoulder pain, depression, and sleep disturbance in hemiparesis patients. Method: The research design was a nonequivalent control group nonsynchronized design. The data was collected from July 1 to August 31, 2003 at D medical Center in Busan. The subjects were 37 patients that they were assigned to two groups, nineteen subjects in the experimental group and seventeen subjects in the control group. The oil made from a blend of lavendar, clarysage, bergamot at a ratio 3:1:2 used for relieving shoulder pain, depression, sleep disturbance. The instruments used for this study were Visual Analogue Scale(VAS) for shoulder pain, Zung' depression scale, 4 score's scale for sleep disturbance. The data were analyzed using SPSS and hypotheses were examined with ANCOVA. Results: Shoulder pain, depression, and sleep disturbance were significantly lower in the experimental group compared than the control group. Conclusion: The aroma massage using selected essential oils is an effective intervention for relieving shoulder pain, depression, sleep disturbance in hemiparesis patients. However differences between aroma massage and massage were not clear, further study needs to be done.

Purpose: This experimental research explored the effects of aroma inhalation on blood pressure, pulse, sleep, stress, and anxiety in patients with essential hypertension who are taking antihypertensive medication. Methods: A pilot survey, pre-test and post-test were conducted. The data were collected from August to October, 2009. The participants were 44 patients randomly assigned to an experimental group (22 people) and a control group (22 people). The experimental group received individual instruction for aroma inhalation, which lasted for 3 minutes. They inhaled blended oils of Lavender, Majoram and Ylang Ylang (The mixed ratio was 4:3:3), three times a day, for two weeks. Data from 36 patients were used for the final analysis (20 from the experimental group, 16 from the control group). Data were analyzed using SPSS version 13.0. Results: Sleep disturbance decreased in the experimental group (t=-2.258, p=.030). Blood pressure, pulse, stress, and anxiety were not significantly different between the two groups (p >.05). Additional analysis of the experimental group showed that physical stress and psychological stress were significantly lowered. Conclusion: Aroma inhalation is an effective paramedical intervention for patients with essential hypertension.

Nowadays, sleep disorder is one of diseases many people have suffered. Many researchers have studied on the features or the method to overcome sleep disorder. In this paper, we have reported papers and patents relevant to sleep disorder to find current trends in the researches. All retrieved papers were 316 articles, patents, all 8 cases. The scale of research on sleep disorder including insomnia has been increasing in recent years. About a half of the analyzed researches were conducted by medical communities, and also various researches were performed based on nursing and Korea medicine. But, we found the application of patents on sleep disorder is less active than the research area. It will be conducted to research to consider with various developing research methodology and planing commercialization through health-related networks of collaborators.

Purpose: To determine the type and degree of effect that a hot footbath has on sleep quality and fatigue level in older Korean adults. Methods: A non-equivalent control group quasi-experimental design was used. Fifty participants from a long-term care facility in Kwangju, South Korea, were randomly selected and assigned to two groups: experimental group (27 participants) and control group (23 participants). The participants in the experimental group received hot footbaths in a temperature-controlled water tub of . They soaked their feet up to 20cm above the ankles for 30 minutes before going to bed for 3 nights. Actigraphy was used to measure their sleep patterns. The data were analyzed using the SAS program. Results: After the intervention, the total sleep satisfaction rate of the participants increased, while sleep latency and fatigue decreased significantly in the experimental group compared to the control group. Conclusion: The study results suggest that hot footbaths are beneficial for older Korean adults in enhancing sleep quality as well as reducing fatigue. Therefore, hot footbaths are recommended as a nursing intervention to improve sleep quality and to reduce fatigue in older Korean adults.

Purpose: This study was done to examine the effects of aroma hand massage on stress response and sleep in hospitalized elderly patients. Methods: A non-equivalent pre-and post-test design was used. Sixty-six hospitalized patients were recruited using a convenience sampling method and assigned to the experimental (n=34) or the control group (n=32). For the experiment, bergamot, lavender, and roman camomile oil were mixed in 2:1:1 ratio to provide aroma hand massage to both hands of each patient in the experimental group for 5 minutes per hand, for 3 days. Post-test data collection was conducted three days after completion of the hand massage. Results: The experimental group had significantly lower stress score (t=-2.648, p=.010, and anxiety scores (t=-4.166, p=.000), and higher sleep measurement scores (t=2.328, p=.023) than those in the control group. Conclusion: These results indicate that short-term application of aroma hand massage for hospitalized elderly patients has a positive effect on hospitalization stress, anxiety, and sleep. Therefore, aroma hand massage should be applied as a nursing intervention to reduce stress response and enhance sleep in elderly hospitalized patients.

Purpose: This study aimed to identify major trends and characteristics of studies on aromatherapy in Korea. Methods: Literature on the studies of aromatherapy in Korea was retrieved using RISS, KISS, DBpia, and Pubmed. The studies were analyzed based on publication year, study design, intervention method, and measuring variables. The level of applied intervention was assessed by target group, the types of aroma, and their effects. Results: A total of 111 studies was included in this study. Most of studies on aromatherapy were conducted using a experimental design to test the effects of aromatherapy as a nursing interventions. Conclusion: This study showed that methods of mixing oils, sites of massage, and target groups vary among studies on aromatherapy in Korea. It is suggestive that standardized methods to mix oils and to apply aroma is in need.
