Acetabular retroversion is a form of dysplasia where the acetabulum is tilted excessively in the anterior plane with a loss of posterior coverage. Excessive anterior coverage may cause impingement, and reduced acetabular coverage may increase cartilage contact pressures in the posteroinferior region, thereby causing hip osteoarthritis (OA). Treatment of retroversion is controversial: patients receive debridement of the anterior acetabular rim or posteriorly directed reorientation of the acetabulum via peri-acetabular osteotomy (PAO). Improved methods to quantify pre- and post-operative cartilage mechanics could be used to standardize treatment.

Injury to the Growth Plate After Pemberton Osteotomy*

The animals were killed, and the acetabula were studied with use of radiography, computed tomography, and histological analysis for evidence of physeal injury. Plain anteroposterior radiographs of the pelvis did not clearly demonstrate the formation of osseous bars. However, Bucholz radiographs, made with the acetabulum placed directly on the cassette, showed osseous bars in three of the four specimens that were studied in this manner. Histological sections of the eight specimens of triradiate cartilage demonstrated five osseous bars in the iliopubic limb and four in the ilioischial limb. In two specimens, there was disruption of the cartilage without osseous bridging. Only two of the eight specimens had normal histological findings in both the iliopubic and the ilioischial limb of the triradiate cartilage.

[Show full abstract] control of correction, and excellent stability, sufficient for immediate full weight bearing. It can be used in any type of dysplasia in adults and also in children, as it avoids the triradiate cartilage. We use this osteotomy for both correction of acetabular dysplasia and correction of over-coverage in cases of acetabular retroversion and protrusio acetabula (the so-called reverse peri-acetabular osteotomy) and to recapture, contain and mould the head in Perthes’ disease. Pre-operative planning is simple: in most cases the goal is to achieve a horizontal sourcil and neutral roof edge angle. The surgical technique is relatively simple, as all the cuts are made under direct vision. Only simple instruments and implants are required. We present up to 20-year results of 116 Birmingham osteotomies in 100 patients. The results are similar to comparable osteotomies, with younger patients doing better. 89 % of patients under 20 years of age and 67 % of patients over 30 retained their natural hip at 20 years post-operation.

The Fate of DDH Hips Showing Cartilaginous or Fibrous Tissue-filled Joint Spaces Following Primary R...

Because the use of magnetic resonance imaging is still not universal for the patients with developmental dysplasia of the hip patients, orthopaedists do not generally distinguish widened joint spaces which are "empty" after primary treatment (and therefore still reducible), from those which are filled and much more difficult to treat. To date no studies have focused on the latter hips. We treated ... [Show full abstract] and observed the outcomes for 19 hips which showed filled joint spaces after primary treatment. We retrospectively reviewed 19 cases of developmental dysplasia of the hip: (1) who showed a widened joint space on radiographs after primary treatment; and (2) whose magnetic resonance imaging showed that the widened joint space was accompanied by acetabular cartilage hypertrophy and/or was filled with fibrous tissues. All patients were over 1 year old at the time of primary reduction (reduction was closed in 4 patients, open in 6, and open with pelvic osteotomy in 9). Thirteen patients received at least 1 secondary treatment. Final results were classified using a modified Severin classification. Final outcomes were satisfactory in 10 (52.6%) and unsatisfactory in 9 (47.4%). The widened joint spaces gradually filled with bone, resulting in a shallow acetabulum in the patients with unsatisfactory results. Of 9 patients who underwent combined pelvic osteotomy at the time of primary reduction, results were satisfactory in 6 (66.7%), whereas all patients who had only closed or open primary reduction had unsatisfactory results. Combined pelvic osteotomy at the time of primary reduction is advisable in hips with widened joint spaces. However, hips with filled joint spaces after primary treatment often have unsatisfactory results even after additional pelvic and/or femoral osteotomy. Level IV-prognostic study.

Abstract The Pemberton periacetabular pelvic osteotomy is a procedure commonly used in the treatment of the developmental dysplasia of the hip, that modifi es the shape and volume of the acetabulum descending its anterolateral portion pivoting in the triradiate cartilage. This change in the acetabular confi guration theoretically causes a wrinkle in the articular cartilage surface modifying the hip biomechanics. This study is a experimental model that describes the reparative process of the afore mentioned wrinkle in the cartilage in 20 New Zealand rabbits 3 months-old who were taken to a Pemberton pelvic osteotomy. We found the wrinkle in the cartilage surface after the surgery, wich remodels partially in 8 weeks postoperatively. Besides we observed during the healing process initially an infl ammatory response, after that a decrease in the number of condrocytes and the cartilage height, both recovering partially in 8 weeks.

Methods: We reviewed 31 hips with early stage ... [Show full abstract] osteoarthritis in 31 patients (mean age, 39.6 years), including three men and 28 women who underwent RAO for hip dysplasia. Clinical evaluations including Japanese Orthopedic Association (JOA) score and Japanese Orthopedic Association Hip Disease Evaluation Questionnaire (JHEQ), and radiographical evaluations on X-ray were performed. Longitudinal qualitative assessment of articular cartilage was also performed using 3.0-T MRI with T2 mapping technique preoperatively, 6 months, and at 1 and 2 years postoperatively. Results: There was no case with progression of osteoarthritis. The mean JOA score improved from 70.1 to 93.4 points, the mean postoperative JHEQ score was 68.8 points, and radiographical data also improved postoperatively. We found that the T2 values of the cartilage at both femoral head and acetabulum increased at 6 months on coronal and sagittal views. However, they significantly decreased 1 and 2 years postoperatively. The T2 values of the center to anterolateral region of acetabulum negatively correlated with postoperative JHEQ score, particularly in pain score. Conclusions: This study suggests that biomechanical and anatomical changes could apparently cause decreased T2 values 1-2 years postoperatively compared with those preoperatively. Furthermore, preoperative T2 values of the acetabulum can be prognostic factors for the clinical results of RAO.

Grade 3 hips show severe epiphyseal flattening (more than one third), the epiphyseal growth cartilage is horizontal, and the epiphyseal subluxation persists even in the abduction test. In our opinion, the advantage of this classification, which is not dependent on a determination of the extent of the necrosis, is that it is less subjective, and can be applied more easily and immediately, as compared with Catterall's classification. The results in sixty cases of intertrochanteric centralising osteotomy were constantly satisfactory in grades 1 and 2, but unsatisfactory in grade 3. The writers consider that surgical intervention is unnecessary in patients with hips in grade 1, while femoral osteotomy is contra-indicated in grade 3 cases. In the latter event, surgical intervention, if considered necessary, should be a pelvic osteotomy or cheiloplasty of the femoral epiphysis. Centralising osteotomy is the operation of choice in patients of grade 2. The results were better in patients who were less than six years of age at the time of operation.

Despite this low risk of injury to the triradiate cartilage after a Gill or Pemberton acetabuloplasty, long-term follow-up is recommended to observe acetabular development. Premature closure is most likely to occur if the bone graft used to maintain fragment displacement crosses the triradiate cartilage.

After 24 weeks, the Mankin grading score in the operated group was significantly lower than that in the non-operated group. The latter hips showed an irregular surface of the cartilage, exfoliation and proliferation of synovial tissue. In those undergoing osteotomy, primary cloning of chondrocytes or hypercellularity was seen and at 24 weeks after operation and metaplasia of the cartilage in the fibrous tissue was observed in the boundary between the medial area of the acetabulum and the acetabular fossa.

In addition, we describe supplementary steps that could be used to avoid these problems. Results: The causes of instability may be divided into three main groups: the first includes causes directly related to SCFE (acetabular labral damage, severe abrasion of the acetabular cartilage, flattening of the acetabular roof and a bell-shaped deformity of the epiphysis); the second, causes not related to the SCFE (acetabular orientation and poor quality of the soft tissues); the third, causes directly related to the surgery (capsulotomy, division of the ligamentum teres, shortening of the femoral neck, pelvi-trochanteric impingement, previous proximal femoral osteotomy and post-operative positioning of the leg). Conclusion: We present examples drawn from our clinical practice, as well as possible ways of reducing the risks of these complications, and of correcting them if they happen. Particularly when viewed on axial CT images, this should not be mistaken for comminuted acetabular fracture (Figure 97.4). The bony acetabulum fuses around 11 to 14 years of age, achieving its adult appearance slightly earlier in girls than boys [1]. Around puberty, three secondary ossification centers develop around the acetabulum, including the os acetabuli (epiphysis of os pubis along the anterior wall of the acetabulum), epiphysis of the ilium (forms the superior wall of the acetabulum), and a small epiphysis of the ischium (Figures 97.5 and 97.6) [2]. These contribute to the depth of the acetabulum but may be confused with avulsion injuries (Figure 97.7). The os acetabuli may persist into adulthood as a separate, well-corticated ossicle (Figure 97.8). The body and alae of the sacrum develop from several separate primary ossification centers (Figure 97.1), which typically fuse between one and seven years of age [3]. Cartilage bordering the articular surfaces of the sacroiliac joints in young children makes them appear wider on radiographs than would be normal for an adult (Figure 97.1). Small triangular secondary ossification centers appear around puberty along the anterior sacroiliac joint spaces at the levels of S1 and S3 (Figure 97.6) [4]. These begin to fuse to the lateral os sacrum around 18 years of age.
