PLACE OF THE HEMODYNAMIC FACTOR IN THE UNIFORM HIERARCHIC REACTION OF THE LIVING SYSTEM IN PATIENTS FROM THE NEUROOTOLOGICAL PROFILE

Abstract

The study covers problems of the hybrid living system’s functioning of organism from point of hierarchy of control and inclusion of responses’ system via indication of hydrodynamic, hemodynamic and neurodynamic balance. Various types of localization of hemodynamic disbalance are considered at different diseases, which are accompanied by neurootologic phenomena. The hierarchy of a convulsive seizure development is considered from positions of reactions and self-organizing damages in the living system. It is noted, that the neurootologic approach to diagnostics and control of indicative system of the organism enables to consider the possibility of the integrated diagnostics and the necessity of adequate medical influence on various levels of organization of the living system.

Actuality
An occurrence of new science on the horizon of ancient sciences neurootology is
observed in literature and real scientific-practical activity for the last decade. At this
time neurootology as science about combined injury of the brain and sense organs
(sight, audition, and vestibular apparatus) got its recognition and place in the
practical health protection. But the pathology of the nervous system is nonexclusive
from subsystem of its functioning providing hemodynamic supply on the macro-
and microlevels. It is the level of blood supply for vitally important sense organs
namely for the vestubular apparatus, became the target one in our study.
Investigation of a role of vascular factor in formation and deepening of
neurootological disturbances in the living organism became objectives of our study.
MATERIAL AND METHODS OF THE STUDY
For the last 7 years on the basis of the Scientific and Methodical Center of
USD “Istyna” (Kyiv) an integrated examination of patients with various paroxysmal
reactions of different etiology is taking place. Within the multidisciplinary approach
726 patients aged from 5 to 82 were observed with various diseases of the
psychoneurological profile (from perinatal encephalopathy, apallic syndrome and up
to serious lesions after CCI, neuroinfections etc). All patients had integrated clinical-
instrumental examination with application of EEG- mapping, USD of vessels and
organs (US scanning and dopplerography), in particular USD of brain vessels, ECG,
smart optic capillaroscopy.
DISCUSSION OF RESULTS OBTAINED FRON THE RESEARCH
Human organism like any other living organism must be considered as a
complex dynamic system created by nature. On the modern stage of the science
development the living organism is modeling in particular as cybernetic system with
numerous constant changeable parameters, which characterize its dynamics, take
active part in processes of regulation and control.
Therefore today we must speak not only about diagnostics of structural
changes in the human organism but also about investigation of adaptable
mechanisms of reconstructions in the living organism under one or another external
factor, in particular under pathological processes in the organism. Quite often in
such situation a term “reactivity” is used, that is readiness, ability, adequate
reconstruction of the living system, and also how quick the decision is made by the
system itself and how quick the decision is performed.
According to the hierarchy of the living system functioning we consider the
organism as the uniform system with numerous indicators, which work by the
principle of the feedback and are subject of one center. Only this hierarchy enables
to make a principle of subordination and precise subordination (hierarchy) of various
levels of control.
It is necessary to consider polyvectorly the complex hierarchy of the living
system and virtually it can be presented at least in 3D:
1. Macro- and microlevels of organization of processes in living organism
(macro- and microlevel of blood supply, macro- and microlevels of
functioning of the nervous system ascending and descending conducting
ways and cellular cortical field of the higher cortical functions).
2. Level of preservation (support) of living and providing vital activity of the
living organism:
Minimal for providing living;
Medium (background) – for providing vital activity of the organism;
High (reserve) – for support the organism under overloads.
3. Hierarchic level of intersystem balance:
Hydrodynamic level of the system’s functioning (intracellular and
extracellular edema as the motive force of “circuit” of electric wiring in the
organism);
Thus, dizziness quite often accompanies numerous illnesses. Quite often
dizziness is accompanied by disturbances of memory, irritability. In case of more
thorough investigation one can diagnose the hydrodynamic conflict in the form of
moderately expressed signs of the chronic brain edema (total or local) with
worsening of the level of blood supply in the reservoir of the anterior or/and
posterior cerebral artery. Thus, when treating we must take into account the
hydrodynamic situation, where is the cerebrum, vestibullum, eyeball.
Patients with hypertension in the ACF in the height of the seizure of cephalalgia
had headache, light and loud sounds intolerance, nausea, vomiting, hyperosmia that
are quite often are accompanied by phenomena of sense of heat or cold (signs of the
intracellular hypertension).
Hemodynamic level of blood supply for vital activity of organs and systems;
With disturbances in blood supply in the vertebral-basilar reservoir there are
darkening or blinking in eyes (area of vascularization of the posterior cerebral
artery), sleep disorder, dizziness, loss of consciousness (expressed deficiency of
blood supply in the basilar artery), disorders in movement coordination (reservoir of
cerebellar arteries).
Neurodynamic level of the higher control via nervous fiber and central
nervous system.
A disorder of neurodynamic balance with dystonia of the cortical-subcortical
interrelations, attacks of dizziness, which reflect the initial disorder of the vestibular
state (Н 81 per МКХ-10) on the background of slightly expressed signs of
cardiovascular insufficiency, obtains the first place with psychosomatic disorders.
In sight of neurootology there are such diseases and disorders like posttraumatic
stress disturbances, astenic, diencephalic symptom-complex, cerebrovascular
dysfunctions, neuroinfections, lunacy, climax, hestosis, which must be considered as
combined disorders of hydro-, hemo- and neurodynamic balance etc.
According to the law of Arndt- Schulz (1855) there are various kinds of
organism’s responses depending on the force of the stimuli of the nervous system:
Slight stimuli stimulate vital activity;
Medium stimuli supported by the living organism by the principle of
resonance;
Strong - oppress;
Extra-strong – destroy.
Paradoxical reactions can be an exception, which say about disbalance or failure
in one or another regulation system, and they must be taken into account and
foreseen before beginning of the treatment. Paradoxical reactions of the disbalanced
organism can cause significant worsening of the patients’ condition in comparison
with background ill state.
Neurootological phenomena of the living organism have scientific- practical
interest by researchers for a long time. Simultaneous “inclusion” in the pathological
symptom-complex of various analyzers is not finally realized reaction of the living
organism under one or other lesions of CNS. Thus, in the practice of
neuropathologist there are quite often pathologies of the vestibular, visual analyzers
together with general disbalances of brain functions. Naturally, that a result of
treatment of such reactions is directly connected with realization of the “ideology”
and “motivation” of the organism to react in such a way. Thus, the absence of
pathological paroxysms, decreasing of their frequency and duration on the
background of the harmonic development of the patient’s personality and leveling of
the balance of contra-vector regulation systems must be considered as the final target
of treatment of patients from the neurootological profile.
In the study we tried to describe briefly some aspects of the algorithm of the
hemodynamic reaction formation in the hierarchy of logical reactions of the living
organism to non-adequate stimuli. Within neurootology tactic approaches to
integrated observation of a state and synchronism of functions of various organs and
systems, are possible, that can be explained in an example of convulsive reaction of
organism.
1) All living organisms function by a principle of stable balance of two
interdependent vectors of influencing: arterio-venous balance, harmonic
development – disproportional ontogenesis, excitation-braking and etc.
2) According to laws of mathematical modelling living organisms exist as an
integral system with the precise system of hierarchy. Disturbance of
subordination of those or other systems of the brain causes reorientation of
size and vector of excitation, disturbance of processes of synchronization of
all involved links.
3) The living system functions as a hybrid system with combination and
simultaneous existence of 2 systems: mechanical and electronic control with
their partial mutual submission. The mechanical system is more lasting in
comparison with hybrid.
4) The power supply of the living system also is subject to laws of conservation
of energy and transition of quantity in quality. We must take into account the
phenomenon of “calm before a storm”: both in nature of storms, tornados, and
before the convulsive attack most patients were observed the phenomena of
local irritation of brain cortex various types of auras, other patients had
expressed retardation of brain functions - spontaneous aboulia, disturbance of
synchronization of higher cortical functions, diffused attention, conflict
behaviour, obtrusiveness in communication, acayriya.
5) Disproportional development of brain in children with mosaic over-excitation
of one area and expressed slowing of another causes background
hemodynamic-energetic disbalance.
6) Demonstrative- hysterical type of behaviour, which is formed in children, also
begins at first with local over-excitation, - reactions of loud weeping tearless
with gradual expansion of artistic effect in a movement sphere - are twitching
by extremities, synchronous frequent twitching by extremities up to absence
of contact. Actually, it is the fixed convulsive reaction of reaction on a
negative with impossibility or unwillingness of productive speech contact for
achievement of consent.
7) Memory of the brain – in patients, who in anamnesis had convulsive seizures,
can recommence at presence of unexpected unfavourable factors for the brain
- disturbance of night sleep, death of a relative, morning attacks after sleepless
night.
8) Effect of the bare wiring - with shorting, by the various vector switching and
dispersion of excitation (electric impulse) on a nervous fibre on a background
of the edema of cerebral tissue.
9) Movement uncontrolled excitation as partial or generalized convulsions -
actually basic syndrome, which visually most frightens during the convulsive
attack.
10) Horizontal position of patient’s body during the convulsive attack is of
hemodynamically grounded by the necessity of balancing of the
hemodynamic-liquor balance in the whole organism.
11) Measuring of concentration of anti-convulsive medical remedies in an
ulnar vein can considerably differ from concentration of these preparations in
areas of the brain especially at presence of the phenomenon of the
arteriovenous shunting into the cavernous or other sinuses of the brain.
12) Stress reactions, which are fixed in the brain and not neutralized during
psychotherapy sessions, can be a trigger of frightful dreams with an output in
the night convulsive attack.
13) Today the integrated objectivisation of a state of all systems in the brain
(exactly brain) is urgently necessary under control of modern diagnostic
equipment for adequate and correct tactics of treatment patients with the
convulsive reactions, better from the phase of debut of the disease.
Thus, “switching-off” of patient’s consciousness and convulsive reactions of
the organism must be considered as pathologically sanogenic one - that is attempts
of organism independently to go out from a non-standard situation, or even to inform
about trouble in the living organism.
CONCLUSIONS
Thus, neurootological approach to diagnostics of the state of
psychoneurological diseases enabled to pay attention on the indicator of checking
systems of organism in surrounding environment. Hierarchical approach to reactions
of the living system and disbalance of one or some levels of indication and control
of the state of the organism, will allow understanding more detailed the complex
mechanism of the living system responses. The hybrid living system in its
functioning foresees emergency transition on the system of the hard protective
restarting of the organism without patient’s interference. However it does not mean
that it independently will be able to overcome all present breakages in the organism.
“Do not harm the brain!” but help it to rehabilitate the natural control over an
organism. It is possible that such approach, but not categorical blocking of visual
convulsive reactions with the high doses of anti-convulsive drugs, must accompany
the present tendencies in medical treatment of patients with convulsive attacks. Very
the approach that is in the help of the brain to restore its controlling and regulating
functions gave significant results in our clinical practice.
PERSPECTIVES OF THE FURTHER RESEARCH
Present level of diagnostic equipment and high variableness of clinical signs of
psychoneurological diseases require integrated approach to comprehensive
diagnostics of levels of disturbances in hydrodynamic, hemodynamic and
neurodynamic balances of the organism. We do not exclude the fact that future
diagnostic equipment will approach the computer indication of troubles in the
organism and will help more effectively to treat a patient.

Review: Problems of effective correction and radical solving of cardiovascular disorders are not new. However, the statistic indexes of cardiovascular pathology are growing and they are considerably younger [1-6]; this testifies to lack of effective management strategy and tactics in fight against these diseases [7-13]. Today we can make a real estimation of achieved results: for the last 20-30 years many technologies are created for research of vascular system - USD of vessels, МRТ in angiomode; devices for correction of cardiac disorders are widely used - from numerous technologies for estimation of the heart structure to coronarography and coronarshunting; new fields of radical medicine are successfully implemented, such as cardiac surgery, angiosurgery, phlebology [14-16]. And at the same time, regardless of the most up-to-date technologies for vascular diagnostics and angiosurgery cardiovascular diseases gets top ranking regarding morbidity and death rate [6,9,17,18]. It can be explained by the fact that development and introduction in medical practice of modern vascular technologies are behind a steady tendency to progress and cardiovascular diseases strike younger people. This dissonance testifies to insufficient knowledge and analysis at the application of new technological potential of visualization of the cardiovascular system at structural (in vitro and in vivo) and functional level - in vivo [7,9,10,12,17]. Our 20-year-old experience of research of vascular disorders and successful attempts of their correction [19] has shown that 1) There is a huge layer of unexplored hemodynamic parameters, which are important in the personalized treatment processes. However, long neglecting biomechanics in medicine [20] resulted into excessive emphasis on endothelial dysfunction and cellular metamorphosis beneficial for promoting pharmacological business. As a result, doctors are not ready to think logically with categories of circulation mechanics and applied hydro- and hemodynamics [21]. 2) Physicians and medical staff don't have enough knowledge in angiology, hemodynamics, hydrodynamics, analytical angiocorrection, angiotherapy, treatment management on evidential basis. Without understanding the depth of hemodynamic laws and logics of pathological and sanogenic transformations in blood supply for organs and regional reservoirs as closed system of vascular tubes - vascular blood flow, it is impossible to perform personalized adequate treatment and predict positive outcomes [8,10]. 3) There is an urgent need in development of vascular monitoring technologies based on principles of evidential medicine [2,7,8,10]. Monitoring dynamic changes in any medical process management, doctors may correct on time the decline from sanogenic blood restoration in a particular regional reservoir and logic of blood redistribution in different vascular reservoirs in a patient's body.
