The Influence of Mistrust, Racism, Religious Participation, and Access to Care on Patient Satisfaction for African American Men: The North Carolina-Louisiana Prostate Cancer Project

The purpose of this study was to explore whether a particular combination of individual characteristics influences patient satisfaction with the health care system among a sample of African American men in North Carolina with prostate cancer. Patient satisfaction may be relevant for improving African American men's use of regular care, thus improving the early detection of prostate cancer and attenuating racial disparities in prostate cancer outcomes.
This descriptive correlation study examined relationships of individual characteristics that influence patient satisfaction using data from 505 African American men from North Carolina, who prospectively enrolled in the North Carolina-Louisiana Prostate Cancer Project from September 2004 to November 2007. Analyses consisted of univariate statistics, bivariate analysis, and multiple regression analysis.
The variables selected for the final model were: participation in religious activities, mistrust, racism, and perceived access to care. In this study, both cultural variables, mistrust (p=<.0001, F=95.58) and racism (p=<.002, F=5.59), were significantly negatively associated with patient satisfaction and accounted for the majority of the variability represented by individual characteristics.
Mistrust and racism are cultural factors that are extremely important and have been negatively associated with patient satisfaction and decreased desires to utilize health care services for African American men. To overcome barriers in seeking health care services, health care providers need to implement a patient-centered approach by creating a clinical environment that demonstrates cultural competence and eliminating policies, procedures, processes, or personnel that foster mistrust and racism.

    ABSTRACT: Objective; To determine which itidividual characteristics (religious participation, mistrust, racism/ discrimination, spirituality, perceived access to care, and continuity of care) were predictors of patient outcomes (patient satisfaction, physical health, and mental health status) for an Army health care clinic transitioning to the Patient-Centered Medical Home (PCMH). Method: A descriptive, correlational design using stepwise multivariate regression analyses to assess the effect of individual characteristics on patient outcomes for 200 Army Soldiers and family members receiving health care services. Results: Perceived access to care was positively and mistrust was negatively related to patient satisfaction (p < 0.001 for both variables). Participants who reported more support from God and more mistrust also reported poorer physical health status (/; < 0.008 and p < 0.003, respectively). Perceived access to care was the only individual characteristic that showed a significant (p < 0.019) positive association with a better mental health status. Conclusion: This study suggests that better access to care improves patient satisfaction and mental health status; however, those with higher levels of mistrust tend to have lower patient satisfaction and poorer health. Participants with poorer health also tend to rely on more support from God. These important individual characteristics should be the considered when implementing the PCMH. INTRODUCTION In 2010, a course was set for the U.S. Army Medical Com-mand to transition from a "health care system" to a "system of care."' This transition was to be facilitated with the imple-mentation of the Patient-Centered Medical Home (PCMH), including recognition from the National Committee for Qual-ity Assurance (NCQA) as a PCMH. In addition, the NCQA states that the PCMH is a health care setting that facilitates partnerships between individual patients, their personal phy-sicians, and the patients' family to assure patients get the indicated care when and where needed in a culturally and linguistically appropriate manner.^ The military health care system has processes in place to deliver care; however, the challenges of implementing the PCMH with regards to a military population with constraints of existing regulations, laws, and financial budgets are unique.''     ABSTRACT: Longitudinal studies report racial disparities in prostate cancer (PCa) including greater incidence, more aggressive tumor biology, and increased cancer-specific mortality in African American (AA) men. Regret concerning primary treatment selection is underevaluated in patients with PCa. We investigated the relationships between clinicopathologic variables across racial and socioeconomic lines following robotic-assisted laparoscopic prostatectomy. We assessed treatment decisional regret using a validated questionnaire in a total of 484 white and 72 AA patients with PCa who were followed up for a median of 16.6 months post-robotic-assisted laparoscopic prostatectomy. Socioeconomic status (SES) information was aggregated from 2010 US census zip code data. Perioperative clinicopathologic characteristics and functional outcomes were compared between groups. Univariate and multivariate regression analyses were used to evaluate the influence of race, aggregate SES, and other clinical and demographic characteristics on decisional regret. The majority (87.7%) of the population was not regretful of their decision to undergo treatment. However, a greater proportion of AA vs. white patients were regretful (20.6% vs. 11.2%, respectively; P = 0.03). AA and white men were similar on all functional, clinical, and pathologic features with the exception of younger age among AA men (56 vs. 60y, respectively; P<0.001). Although there were significant differences in SES by race (P<0.001), regret did not differ by SES (β =-1.53; P = 0.15). Race, postoperative sexual dysfunction, pad usage, and length of hospital stay, however, were significantly associated with decisional regret. AA men were more regretful than white men, after adjusting for clinicopathologic characteristics and postoperative functional outcomes.

    ABSTRACT: Background: To examine the impact of race on treatment regret among men with recurrent prostate cancer after surgery or radiation. Methods: The prospective Comprehensive, Observational, Multicenter, Prostate Adenocarcinoma (COMPARE) registry was used to study a cohort of 484 men with biochemically recurrent prostate cancer after radical prostatectomy, external beam radiation or brachytherapy. Multivariable logistic regression was used to model the association between race and treatment regret and to determine whether there was an interaction between race and sexual problems after treatment with regards to treatment regret. Results: Black men (N=78) were significantly more likely to have treatment regret when compared with non-black men (N=406; 21.8% versus 12.6%) on univariable analysis (odds ratio (OR) 1.94; 95% confidence interval 1.05-3.56; P=0.03). On multivariable analysis, black race trended towards but was no longer significantly associated with an increase in treatment regret (adjusted OR (AOR) 1.84 (0.95-3.58); P=0.071). There was an interaction between race and sexual problems after treatment (Pinteraction=0.02) such that among those without sexual problems, black men had more treatment regret than non-black men (26.7% versus 8.4%: AOR 4.68 (1.73-12.63); P=0.002), whereas among those with sexual problems, there was no difference in treatment regret between black and non-black men (18.8% versus 17.3%: AOR 1.04 (0.44-2.46); P=0.93). Conclusions: Among men with recurrent prostate cancer after surgery or radiation, black men were nearly twice as likely to experience treatment regret. Treating physicians should ensure that patients are fully apprised of the pros and cons of all treatment options to reduce the risk of subsequent regret.
