Identification of Haemophilus influenzae clones associated with invasive disease in Italy by Multilocus Sequence Typing.

The introduction of Haemophilus influenzae serotype b (Hib) conjugate vaccines has changed the epidemiology of invasive H. influenzae disease with a shift in the predominant serotype from Hib to nonencapsulated (ncHi). The objective of this study was to identify the genotypes/clones associated with invasive H. influenzae disease in Italy. Eighty-seven H. influenzae strains isolated in the years 2009-2011 within the National Surveillance of Invasive Bacterial Disease were analyzed. Strains were characterized by serotyping, antimicrobial susceptibility testing and multilocus sequence typing (MLST). Genetic polymorphisms in the blaTEM gene promoter region as well as the occurrence of both adhesin genes (hmwA and hia) and IgA1 protease-encoding gene (igaB) was also investigated.

    ABSTRACT: Introduction: Globally, the three main pathogens causing serious infections are Haemophilus influenzae type b, Streptococcus pneumoniae and Neisseria meningitidis. Over the last 5 years, new vaccines protecting against these bacteria have been developed and introduced in various countries. Areas covered: This review describes the recently licensed glycoconjugates being used to protect against these encapsulated bacteria. Immunogenicity and safety data that led to licensure or licensure expansion of these glycoconjugates are discussed in addition to the resultant impact on the disease burden. Expert opinion: The maintenance of robust immunisation programmes with high uptake rates is important in maintaining low rates of disease. Epidemiological surveillance systems are essential in monitoring any changes in infectious disease trends and in identifying emerging infections such as from non-typeable H. influenzae, pneumococcal serotype replacement disease and changes in the epidemiology of meningococcal serogroups. This is important to guide future vaccine development. Accessibility of these glycoconjugate vaccines in resource poor regions, which bear the highest disease burden from these pathogens, remains challenging largely due to high vaccine pricing. Recent aids from public and private funding, tiered vaccine pricing and the transfer of vaccine technology have helped in introducing these vaccines where they are most needed.

    ABSTRACT: In Japan, publicly subsidized Haemophilus influenzae serotype b vaccines became available in 2011; consequently, the incidence of invasive H. influenzae infection in pediatric patients less than 5 years of age decreased dramatically. In 2013, the first case of H. influenzae serotype f (Hif) meningitis in a Japanese infant was reported, and another case of Hif meningitis in a Japanese infant was observed in 2013. We experienced a fatal pediatric case of Hif bacteremia in 2004; therefore, we conducted an analysis of three Hif strains isolated from these three Japanese children with invasive Hif infections. All three strains were β-lactamase-nonproducing ampicillin-sensitive strains, with MICs of 1 µg ml-1 or less. However, one of the three strains showed slightly elevated MICs for ampicillin (1 µg ml-1), cefotaxime (0.25 µg ml-1), and meropenem (0.13 µg ml-1). A molecular analysis by multilocus sequence typing identified all three strains as sequence type (ST) 124, which is a predominant invasive Hif strain in many countries. SmaI-digested pulsed-field gel electrophoresis showed variable DNA fragmentation patterns among the strains, suggesting that some highly virulent strains originated from a single ST 124 clone and caused invasive Hif infections in Japan. Additional studies are needed to determine the factors that led to the clonal expansion of virulent ST 124 strains.
