A Study of the Relational Aspects of the Culture of Academic Medicine

The impact of medical school culture on medical students has been well studied, but little documentation exists regarding how medical faculty experience the culture in which they work. In an ongoing project, the National Initiative on Gender, Culture and Leadership in Medicine, the authors are investigating how the existing culture of academic medical institutions supports all faculty members' ability to function at their highest potential.
The authors conducted a qualitative study of faculty in five disparate U.S. medical schools. Faculty in different career stages and diverse specialties were interviewed regarding their perceptions and experiences in academic medicine. Analysis was inductive and data driven.
Relational aspects of the culture emerged as a central theme for both genders across all career categories. Positive relationships were most evident with patients and learners. Negative relational attributes among faculty and leadership included disconnection, competitive individualism, undervaluing of humanistic qualities, deprecation, disrespect, and the erosion of trust.
The data suggest that serious problems exist in the relational culture and that such problems may affect medical faculty vitality, professionalism, and general productivity and are linked to retention. Efforts to create and support trusting relationships in medical schools might enhance all faculty members' efforts to optimally contribute to the clinical, education, and research missions of academic medicine. Future work will document the outcomes of a five-school collaboration to facilitate change in the culture to support the productivity of all medical faculty.

    ABSTRACT: Many faculty in today's academic medical centers face high levels of stress and low career satisfaction. Understanding faculty vitality is critically important for the health of our academic medical centers, yet the concept is ill-defined and lacking a comprehensive model. Expanding on previous research that examines vital faculty in higher education broadly and in academic medical centers specifically, this study proposes an expanded model of the unique factors that contribute to faculty vitality in academic medicine. We developed an online survey on the basis of a conceptual model (N = 564) and used linear regression to investigate the fit of the model. We examined the relationships of two predictor variables measuring Primary Unit Climate and Leadership and Career and Life Management with an overall Faculty Vitality index comprised of three measures: Professional Engagement, Career Satisfaction, and Productivity. The findings revealed significant predictive relationships between Primary Unit Climate and Leadership, Career and Life Management, and Faculty Vitality. The overall model accounted for 59% of the variance in the overall Faculty Vitality Index. The results provide new insights into the developing model of faculty vitality and inform initiatives to support faculty in academic medical centers. Given the immense challenges faced by faculty, now more than ever do we need reliable evidence regarding what sustains faculty vitality.

    ABSTRACT: By introducing a refractor on the TV camera lens, the image of the measuring point is displaced on the image plane with the corresponding displacements related to the distances between the TV camera and the measuring point. The magnitude of displacement on the image plane is inversely proportional to the distance between the TV camera and measuring point. When the refractor is rotated physically at high speed during the exposure of the TV camera, annular streaks of measuring points appear on an image since the rotational shift is added to the image. Since the size of the annular streak is inversely proportional to the distance of the measuring point from the camera, the three-dimensional information can be calculated by analyzing annular streaks. In this paper, the surface measurement such as swinging water is introduced for one of the applications of our system.

    ABSTRACT: Group C streptococci rarely cause severe disease in humans. The authors report a 15-year-old girl with group C streptococcal pneumonia. No immune deficiencies were detected in our patient and investigation of virulence factors including emmL, sag A, spegg and smeZ genes failed to reveal insight into why the isolated group C streptococcal strain was so virulent. Fourteen other reports of primary group C streptococcal pneumonia were reviewed.
