Web-based tailored intervention programs show considerable promise in effecting health-promoting behaviors and improving health outcomes across a variety of medical conditions and patient populations. This meta-analysis compares the effects of tailored versus nontailored web-based interventions on health behaviors and explores the influence of key moderators on treatment outcomes. Forty experimental and quasi-experimental studies (N = 20,180) met criteria for inclusion and were analyzed using meta-analytic procedures. The findings indicated that web-based tailored interventions effected significantly greater improvement in health outcomes as compared with control conditions both at posttesting, d = .139 (95% CI = .111, .166, p <.001, k = 40) and at follow-up, d = .158 (95% CI = .124, .192, p < .001, k = 21). The authors found no evidence of publication bias. These results provided further support for the differential benefits of tailored web-based interventions over nontailored approaches. Analysis of participant/descriptive, intervention, and methodological moderators shed some light on factors that may be important to the success of tailored interventions. Implications of these findings and directions for future research are discussed.

Understanding users’ acceptance of smoking cessation interventions features is a precursor to mobile cessation apps’ uptake and use. We gauged perceptions of three features of smoking cessation mobile interventions (self-monitoring, tailored feedback and support, educational content) and their design in two smoking cessation apps, Quit Journey and QuitGuide, among young adults with low socioeconomic status (SES) who smoke. A convenience sample of 38 current cigarette smokers 18-29-years-old who wanted to quit and were non-college-educated nor currently enrolled in a four-year college participated in 12 semi-structured virtual focus group discussions on GoTo Meeting. Discussions were audio recorded, transcribed verbatim, and coded using the second Unified Theory of Acceptance and Use of Technology (UTAUT2) constructs (i.e., performance and effort expectancies, hedonic motivation, facilitating conditions, social influence), sentiment (i.e., positive, neutral, negative), and app features following a deductive thematic analysis approach. Participants (52.63% female, 42.10% non-Hispanic White) expressed positive sentiment toward self-monitoring (73.02%), tailored feedback and support (70.53%) and educational content (64.58%). Across both apps, performance expectancy was the dominant theme discussed in relation to feature acceptance (47.43%). Features’ perceived usefulness centered on the reliability of apps in tracking smoking triggers over time, accommodating within- and between-person differences, and availability of on-demand cessation-related information. Skepticism about features’ usefulness included the possibility of unintended consequences of self-monitoring, burden associated with user-input and effectiveness of tailored support given the unpredictable timing of cravings, and repetitiveness of cessation information. All features were perceived as easy to use. Other technology acceptance themes (e.g., social influence) were minimally discussed. Acceptance of features common to smoking cessation mobile applications among low socioeconomic young adult smokers was owed primarily to their perceived usefulness and ease of use. To increase user acceptance, developers should maximize integration within app features and across other apps and mobile devices.

Background Although personalization and tailoring have been identified as alternatives to a “one-size-fits-all” approach for eHealth technologies, there is no common understanding of these two concepts and how they should be applied. Objective This study aims to describe (1) how tailoring and personalization are defined in the literature and by eHealth experts, and what the differences and similarities are; (2) what type of variables can be used to segment eHealth users into more homogeneous groups or at the individual level; (3) what elements of eHealth technologies are adapted to these segments; and (4) how the segments are matched with eHealth adaptations. Methods We used a multimethod qualitative study design. To gain insights into the definitions of personalization and tailoring, definitions were collected from the literature and through interviews with eHealth experts. In addition, the interviews included questions about how users can be segmented and how eHealth can be adapted accordingly, and responses to 3 vignettes of examples of eHealth technologies, varying in personalization and tailoring strategies to elicit responses about views from stakeholders on how the two components were applied and matched in different contexts. Results A total of 28 unique definitions of tailoring and 16 unique definitions of personalization were collected from the literature and interviews. The definitions of tailoring and personalization varied in their components, namely adaptation, individuals, user groups, preferences, symptoms, characteristics, context, behavior, content, identification, feedback, channel, design, computerization, and outcomes. During the interviews, participants mentioned 9 types of variables that can be used to segment eHealth users, namely demographics, preferences, health variables, psychological variables, behavioral variables, individual determinants, environmental information, intervention interaction, and technology variables. In total, 5 elements were mentioned that can be adapted to those segments, namely channeling, content, graphical, functionalities, and behavior change strategy. Participants mentioned substantiation methods and variable levels as two components for matching the segmentations with adaptations. Conclusions Tailoring and personalization are multidimensional concepts, and variability and technology affordances seem to determine whether and how personalization and tailoring should be applied to eHealth technologies. On the basis of our findings, tailoring and personalization can be differentiated by the way that segmentations and adaptations are matched. Tailoring matches segmentations and adaptations based on general group characteristics using if-then algorithms, whereas personalization involves the direct insertion of user information (such as name) or adaptations based on individual-level inferences. We argue that future research should focus on how inferences can be made at the individual level to further develop the field of personalized eHealth.

Background Since December 2019, COVID-19 led to a pandemic causing many hospitalizations and deaths. Vaccinations were developed and introduced to control viral transmission. In the Dutch context, the decision to accept vaccination is not mandatory. An informed decision is based on sufficient and reliable information, in line with one’s attitudes and values, and with consideration of pros and cons. To support people in informed decision-making, we developed an online COVID-19 vaccination decision aid (DA). Objective This article aims to describe the development, dissemination, and use of the DA. Methods Building on a previously developed DA, the COVID-19 vaccination DA was developed in 3 phases following a user-centered design approach: (1) definition phase, (2) concept testing, and (3) prototype testing. End users, individuals with low literacy, and experts (with relevant expertise on medical, behavioral, and low literacy aspects) were involved in the iterative development, design, and testing, with their feedback forming the basis for adaptations to the DA. Results The DA was developed within 14 weeks. The DA consists of 3 modules, namely, Provide Information, Support Decision-Making, and Facilitate Actions Following a Decision. These modules are translated into various information tiles and diverse functionalities such as a knowledge test, a value clarification tool using a decisional balance, and a communication tool. The DA was disseminated for use in May 2021. Users varied greatly regarding age, gender, and location in the Netherlands. Conclusions This paper elaborates on the development of the COVID-19 vaccination DA in a brief period and its dissemination for use among Dutch adults in the Netherlands. The evaluation of use showed that we were able to reach a large proportion and variety of people throughout the Netherlands.

Background Losing a loved one, through death or separation, counts among the most stressful life events and is detrimental to health and well-being. About 15% of people show clinically significant difficulties coping with such an event. Web-based interventions (WBIs) are effective for a variety of mental health disorders, including prolonged grief. However, no validated WBI is available in French for treating prolonged grief symptoms. Objective This study aimed to compare the efficacy and adherence rates of 2 WBIs for prolonged grief symptoms following the loss of a loved one through death or romantic separation. Methods LIVIA 2.0 was developed relying on theoretical and empirical findings on bereavement processes and WBIs, and is compared with LIVIA 1, which has already demonstrated its efficacy. We conducted a randomized controlled trial and provided on-demand guidance to participants. Outcomes were assessed through web-based questionnaires before the intervention, after the intervention (12 weeks later), and at follow-up (24 weeks later). Primary outcomes were grief symptoms, depressive symptoms, and well-being. Secondary outcomes were anxiety symptoms, grief coping strategies, aspects related to self-identity, and program satisfaction. Results In total, 62 participants were randomized (intent-to-treat [ITT] sample), 29 (47%) in LIVIA 2.0 (active arm) and 33 (53%) in LIVIA 1 (control arm). The dropout rate was 40% (37/62), and 10 participants were removed due to exclusion criteria, leading to a final per-protocol sample of 27 (44%) completers who differed from noncompleters only based on reporting fewer anxiety symptoms (t60=3.03; P=.004). Participants who are separated reported more grief symptoms (t60=2.22; P=.03) and attachment anxiety (t60=2.26; P=.03), compared to participants who are bereaved. There were pre-post within-group differences for both programs in the ITT sample, with significant reductions in grief (Cohen d=−0.90), depressive symptoms (Cohen d=−0.31), and centrality of the loss (Cohen d=−0.45). The same pattern was observed in the per-protocol sample, with the exception that anxiety symptoms also significantly diminished (Cohen d=−0.45). No difference was found in efficacy between the 2 programs (all P>.33). Participants (ITT sample) reported overall high levels of program satisfaction (mean 3.18, SD 0.54; over a maximum of 4). Effect stability was confirmed at the 6-month follow-up for all outcomes, with an improvement in self-concept clarity. Conclusions The 2 grief-related WBIs were effective in reducing grief, depressive and anxiety symptoms for participants who are bereaved or separated. The analyses did not reveal any pre-post between-group differences, suggesting that the innovations brought to LIVIA 2.0 did not significantly affect the outcome. However, caution is warranted with the interpretation of the results given the limited power of the sample, which only allows the detection of medium effect sizes. In the transitioning era towards the COVID-19 endemic, there is still a sizable population that has never been vaccinated against COVID-19 in the Netherlands. This study employs Bayesian spatio-temporal modelling to assess the relative chances of COVID-19 vaccination uptake – first, second, and booster doses – both at the municipal and regional (public health services) levels. Incorporating ecological regression modelling to consider socio-demographic factors, our study unveils a diverse spatio-temporal distribution of vaccination uptake. Notably, the areas located in or around the Dutch main urban area (Randstad) and regions that are more religiously conservative exhibit a below-average likelihood of vaccination. Analysis at the municipal level within public health service regions indicates internal heterogeneity. Additionally, areas with a higher proportion of non-Western migrants consistently show lower chances of vaccination across vaccination dose scenarios. These findings highlight the need for tailored national and local vaccination strategies. Particularly, more regional efforts are essential to address vaccination disparities, especially in regions with elevated proportions of marginalized populations. This insight informs ongoing COVID-19 campaigns, emphasizing the importance of targeted interventions for optimizing health outcomes during the second booster phase, especially in regions with a relatively higher proportion of marginalized populations.

Background Migrants and refugees with low language proficiency (LLP) have a higher risk of experiencing certain mental health disorders compared to non-migrant populations. They are also more likely to experience a lack of access to mental healthcare due to language- and culture-related barriers. As part of the MentalHealth4All project, a digital multilingual communication and information platform was developed to promote access to mental healthcare for LLP migrants and refugees across Europe. This paper describes the study protocol for evaluating the platform in practice, among both health and/or social care providers (HSCPs) and LLP migrants and refugees. Methods We will conduct a pretest-posttest cross-national survey study to perform the platform’s effect-evaluation (primary objective) and process-evaluation (secondary objective). The primary outcomes (measured at T0, T2 and T3) are four dimensions of access: availability, approachability, acceptability, and appropriateness of mental healthcare. Secondary outcomes (measured at T2) are: actual usage of the platform (i.e. tracking data), ease of use, usefulness of content, comprehensibility of information, attractiveness of content, and emotional support. Participants will be recruited from nine European countries: Belgium, Germany, Italy, Lithuania, the Netherlands, Poland, Slovakia, Spain, and the United Kingdom. Using convenience sampling through professional networks/organisations and key figures, we aim to include at least 52 HSCPs (i.e. 6-10 per country), and 260 LLP migrants (i.e. 30-35 per country). After completing a pretest questionnaire (T0), participants will be requested to use the platform and HSCPs will participate in an additional personalised training (T1). Next, participants will fill out a posttest questionnaire (T2), and will be requested to participate in a second posttest questionnaire (T3, about 6-8 weeks after T2) to answer additional questions on their experiences through a brief phone interview (T3 is optional for migrants/refugees). Discussion The findings of this prospective pretest-posttest cross-national study will deepen our understanding of how a multilingual platform may promote access to mental healthcare services for LLP migrants and refugees. If successful, this intervention could be used to improve access to mental healthcare services, as well as HSCPs’ competencies in delivering such services, for any LLP migrants and refugees across Europe (and beyond).

This chapter provides an overview of the literature on tailored health communication messages, an innovative area of health communication research that has broad applicability to other areas of communication. Unlike traditional message targeting practices, which operate at the group level, tailored interventions involve the development of persuasive health communications designed for (and based upon an assessment of) the individual. We introduce tailoring and discuss the historical context of message tailoring research. We then provide a broad review of seminal and more recent message tailoring studies. Next, we discuss the mechanisms through which tailored messages may exert their effects, reviewing theoretical perspectives as well as empirical data. Finally, we consider future directions for research on tailored health communication.

Issue addressed: Potentially modifiable risk factors such as high body mass, physical inactivity and poor nutrition are, when combined, the largest contributors to the preventable burden of disease in Australia. Computer-tailoring has shown promise in modifying lifestyle risk factors. This paper describes a narrative systematic review examining the evidence for 'second' generation computer-tailored primary prevention weight reduction interventions. Method: Studies published from January 1996-2008 were identified through electronic databases and searches of reference lists of relevant articles. Only randomised controlled trials or quasi-experimental designs with pretest and post-test weight reduction outcome data were included. Results: Six articles were identified, three of which reported significant positive effects. The intervention intensity and duration, method of tailoring and theory used differed. The internal validity was good but external validity poor for the majority of studies. Conclusions: The evidence of effectiveness for computer-tailored primary prevention interventions for weight reduction is limited to a small number of heterogeneous studies. There is uncertainty whether reported effects are generalisable and sustained. The relative success of different components of efficacious interventions and the optimal intervention intensity and tailoring methods are unclear.

In more and more medical settings, physicians have less and less ti me to be effective communicators. To be effective, they need accurate, current information about their patients. Tailored health communications can facilitate positive patient-provider communications and foster behavioral changes conducive to health. Tailored communications (TCs) are produced for an individual based on information about that person. The focus of this report is on tailored print communications (TPCs). TPCs also enhance the process of evaluation, because they require a database and the collection of patient-specific information. We present a Tailoring Model for Primary Care that describes the steps involved in creating TPCs. We also provide examples from three ongoing studies in which TPCs are being used in order to illustrate the kinds of variables used for tailoring the products that are developed and how evaluation is conducted. TPCs offer opportunities to expand the reach of health professionals and to give personalized, individualized massages in an era of shrinking professional contact time.

American children continue to be less physically active than they were a decade ago. Web-based programs (e-Learning), requiring minimal teacher training and expertise, could contribute to improvements in children's health-related knowledge, attitudes and behaviors. The purpose of this study was to evaluate the impact of the e-Learning module Healthy Hearts 4 Kids (HH) on the physical activity knowledge, attitudes and behaviors of 233 fifth grade children. A 2×3 repeated measures design was employed and significant changes in knowledge and attitude were found upon completion of the module and also six weeks after the post-test was completed. Results of this study suggest HH could be an effective component of a coordinated school health program (CSHP).
