Early functional treatment in Class II division 1 subjects with mandibular retrognathia using Fränkel II appliance. A prospective controlled study

This was to clarify dentoalveolar, skeletal and facial profile modifications induced by Fränkel II functional treatment in Class II division 1 subjects in prepubertal stages CS1-2.
Evaluation of 25 caucasian subjects (15 males, 10 females), mean age 8y 11m, in vertebral stages CS1-2. The mean duration of functional treatment was 1y 11m. Study Design: Prospective controlled study. The sample was evaluated at T0, start of treatment, and T1, end of Fränkel II therapy. Untreated subjects in Class II division 1 were the control group. Statistics: T-test for paired data was calculated before and after treatment (P< 0.05).
A statistically significant increase between T0 and T1 was obtained for SNB (p = 0.022), ANB (p <0.001), Mx/Mn differential (p = 0.001), Co-Go-Me (p = 0.014), OJ (p < 0.001), OB (p = 0.042), IMPA (p = 0.021) . OJ (p < 0.001) and OB (p = 0.003) showed a higher significant change in the Fränkel group compared to control group.
The Fränkel II appliance produced, in CS1-2 subiects, significant effects in teeth position, skeletal modifications and soft tissue changes, although the pubertal growth spurt would occur at least two years later.

    ABSTRACT: Aim: The purpose of this study was to compare the effects of a differently designed functional appliance (the R- appliance) with Fränkel-2. Study design: Twenty-seven patients (16 girls and 11 boys) with a mean age of 9.8 (SD 1.6) years were treated with the R-appliance for 15.4 (SD 0.4) months and twentyseven (15 girls and 12 boys) patients with a mean age of 9.1 (SD 1.1) years were treated with a Fränkel-2 appliance for 19 (SD 5.6) months. All patients had Class II division 1 malocclusions due to mandibular deficiency and all of them had prepubertal stages of skeletal development. Lateral cephalograms obtained at the beginning (T1) and at the end (T2) of the study were analysed. Results: Paired t-tests showed that SNB significantly increased in both groups. The incisor mandibular plane angle (IMPA) was reduced in the R- appliance group by 2.2 (SD 4.9) degrees (P<0.03) but increased by 2.2 (SD 2.6) degrees (P<0.001) in the Fränkel-2 group. The SNA in the R-appliance group showed an increase of 0.2 (SD 2) degrees (P<0.6), while it was decreased by 0.4 (SD 0.5) degrees (P<0.6) in the Fränkel-2 group. Conclusions: Both treatment modalities were successful in moving the mandible forward. However, with the R-appliance, this was achieved without proclination of the lower incisors.

    ABSTRACT: The aim of this study was to investigate possible links between competitive swimming during the growth phase and the development of the dentoalveolar arches. The study sample included 100 swimmers and a control group of 100 age-matched non-swimmers who had never practised swimming or related sports. Subjects who had had previous orthodontic treatment were excluded. Overjet, overbite, sagittal and transverse parameters, arch dimension, crowding and oral habits were recorded. In the swimmers, there was a significantly higher frequency of molar symmetry (P = 0.04), together with a greater number of Class I subjects. The overjet in the swimmers was mainly normal, but the arch dimensions were significantly wider (+10% in the upper arch; P < 0.001). Similarly, the swimmers showed significantly less severe crowding (P < 0.001) and significantly reduced oral habits (P < 0.001). Our data and analysis demonstrate that competitive swimming during the growth phase has a favourable effect on dental arch development in the sagittal, vertical and transverse planes.

    ABSTRACT: Aims: The aim of this research was to complete Habets's method on orthopantomogram, in order to measure mandibular symmetry horizontally and diagonally in mixed dentition as the first diagnostic evaluation. Settings and Design: Mixed dentition subjects were consecutively selected according to skeletal maturity. Materials and Methods: Inclusion criteria were: Mixed dentition, cervical vertebral stages 1-2, Class II or Class III malocclusions, with or without unilateral posterior cross-bite. Fourteen subjects with cross-bite (mean age 8 y, 9 m) cross group (CG) and 14 subjects with normal transverse occlusion (mean age 8y, 6m) non-cross group (NCG) were selected. Nine measurements were determined. An asymmetry index was performed for all linear variables. Statistical Analysis Used: NCG patients' data were compared with CG data using parametric t-tests. Probabilities of <0.05 were accepted as significant. Results: In CG, comparing right and left side, single values showed no significant differences up to 6% (twice if compared to the 3% threshold value). T-tests showed statistically significant differences between the groups for ramus + condyle height (index 2.5% vs 4.5%, P = 0.04), condylar height (index 3.7% vs 6.6%, P = 0.02) and mandibular length (index 1.4% vs 2.3%, P = 0.04). Overall CG group was more asymmetrical than NCG. Conclusions: In mixed dentition (CS1-2), a first appraisal of mandibular symmetry may be performed on orthopantomogram utilizing this modified Habets's method. This tracing method provides an early evaluation about mandibular symmetry and is able to show slight asymmetries in mixed dentition thereby reducing the number of cone beam computed tomograms performed.
