Transaortic replacement of the mitral valve at the time of aortic valve or root replacement is a rarely used technique that offers many possible advantages in the setting of multivalve replacement. Reports in the literature are few and dated. The purpose of this brief communication is to describe technical and procedural details specific to mitral procedures done through the aortic annulus.

ABSTRACT: We present a case of aortic and mitral valve replacement through aortotomy in a patient with previous operations and unfavorable anatomy to expose the mitral valve. We describe the technique used and reviewed that used by other authors. This uncommon approach to the mitral valve, easily performed in this case, encourages more liberal use of transaortic access in selected patients. (Open acces publication)
