Acoustic Voice Analysis Does Not Identify Presence of Penetration/Aspiration as Confirmed by Videofluoroscopic Swallowing Study

OBJECTIVE: To investigate whether acoustic voice analysis can identify the presence of penetration/aspiration (P/A) as confirmed by videofluoroscopic swallowing study (VFSS). DESIGN: Repeated measures within subjects. SETTING: Rehabilitation department in a tertiary teaching hospital. PARTICIPANTS: Patients (N=44) with swallowing disorders referred for VFSS. INTERVENTIONS: Patients were asked to sustain phonations /a/ for at least 3 seconds before and after swallowing 5mL of liquid barium during a standardized VFSS. The acoustic voice analysis program was used to analyze vocal quality change. MAIN OUTCOME MEASURES: Five acoustic parameters including average fundamental frequency, relative average perturbation, shimmer percentage, noise-to-harmonic ratio, and voice turbulence index were analyzed for each participant before and after swallowing during VFSS. Differences in the pre- and postmeasures were compared between those participants who demonstrated VFSS-confirmed P/A (n=17) and those who did not (n=27). RESULTS: No significant changes were noted in the 5 acoustic parameters in or between P/A and nonpenetration/aspiration groups (P>.05). CONCLUSIONS: As used in this study, acoustic voice analysis does not identify the presence of P/A confirmed by VFSS.

    ABSTRACT: Summary Dysphagia represents a salient concern in many conditions prevalent in older people. There are direct implications for morbidity and mortality. The importance of recognizing and managing dysphagia in hospital and the community also extends to psychosocial impact and quality of life, as well as health, economic and ethical-legal issues. This review outlines reasons for the importance of recognizing and treating dysphagia. It then proceeds to look at recent developments in our understanding of the nature, assessment and management of dysphagia in older people. Whilst there are well-established practices in assessment and management, ongoing work continues to challenge the validity and reliability of many methods. These concerns are covered and directions for future developments highlighted.

    ABSTRACT: Purpose: The purpose of this study was to investigate associations between voice quality and swallowing function in patients treated for oral or oropharyngeal cancer. Methods: Recordings of speech and videofluoroscopy of 51 patients after treatment for oral or oropharyngeal cancer were analysed. Acoustic voice parameters (fundamental frequency (F0), perturbation (jitter %, shimmer %), harmonics-to-noise ratio (HNR), and intensity (extracted from the vowels /a/, /i/, and /u/)) were compared to swallowing function parameters as assessed via videofluoroscopy (oral, oropharyngeal, and total transit time; estimated percent of oral, oropharyngeal, and total residue; oropharyngeal swallowing efficiency (OPSE), and the penetration/aspiration (PA-) scale). Results: Stepwise multivariate regression analyses revealed that jitter, shimmer, and HNR were not associated with swallowing function. Higher voice intensity in all three vowels /a/, /i/, and /u/ was significantly associated with a higher score on the PA-scale (more penetration and aspiration). Higher voice intensity and F0 were significantly associated with lower OPSE. Conclusion: Higher voice intensity was significantly associated with higher amount of penetration/aspiration, and higher voice intensity and fundamental frequency were significantly associated with swallowing inefficiency. Possible explanations may be found in overcompensation by increased laryngeal muscular strength resulting in increased intensity and pitch during phonation. However, the physiology of associated voice and swallowing function is yet unclear, and more research is recommended.

    ABSTRACT: Wet voice is a perceptual vocal quality that is commonly used as an indicator of penetration and/or aspiration in clinical swallowing assessments and bedside screening tests. Our aim was to describe the clinimetric characteristics of this clinical sign using various fluid materials and one solid food in the Parkinson's disease (PD) population. Consecutive PD individuals were submitted for simultaneous fiberoptic endoscopic evaluation of swallowing (FEES) and voice recording. Speech therapists rated the presence or absence of wetness and other voice abnormalities. Two binary endpoints of FEES were selected for comparison with an index test: low penetration (LP) and low penetration and/or aspiration (LP/ASP). The accuracy of wet voice changed according to the testing material in PD patients. Overall, the specificity of this indicator was better than its sensitivity, and the wafer cookie and yogurt drink yielded the best indices. Our data show that wet voice is clearly indicative of LP or LP/ASP in PD patients in case of positive test. However, in the case of a negative result, the wet voice test should be repeated or combined with other clinical tests to include or exclude the risk of LP or LP/ASP.
