A fuzzy expert system design to monitor patient's condition during heart surgery

Abstract

The aim of this study is to design a Fuzzy Expert System to trace vital functions of the patient for directing the courses of the surgery during the Coronary Bypass Surgery (CBS). The designed fuzzy expert system presents and monitors four of the vital functions (Blood pressure, Hemoglobin, Pulse and Beta-blocker) of the patient and also interprets them to indicate current situation of the patient during the operation. The output of the system is indicator for the patient during the surgery in visual and audial forms that have (1) very thick, (2) thick, (3) normal, (4) thin and (5) very thin forms of the signal. The data (Course of the patient during the surgery) obtained from designed fuzzy expert system are compared with the data in the literature.It has been seen that better results are observed with designed system. The system can be viewed as an alternative for existing methods to monitor for directing the course of the coronary surgery.

Patient monitoring systems (PMS) is a method used for rating and controlling of the treatments and disorders. While PMS offers as wearable technologies for adults; it is used for adapting to incubator modules to ensure continuous and instant monitoring for newborn babies at the pediatric level. The purpose of this study is to make a pediatric PMS with the ability to be integrated to all type of classical incubators, and monitoring the conditions of the newborn babies with various sensors. It is designed as an Arduino based system that instantly records the variables such as temperature, humidity, air quality to the server using Wi-Fi connection and instantly providing the monitoring ability for clinicians and nurses. Accelerometer is used to control the baby's awake and asleep status, and patientbased safe identification and assessments are made using RFID. As a result, a system has been designed to integrate all the classical incubators and ensure that a large number of incubators can be monitored from a single center.

The aims of the study are to design a Fuzzy Expert System (FES) for diagnosis of periodontal dental disease by analyzing diagnosis, to define treatment methods and also to develop a computer program to help dentists for investigation, diagnosis and treatment of the disease. The designed FES is expected to help dentists by facilitating their job with the most correct diagnosis and treatment method. In addition, this system increases the intervention speed by reducing time loss. The FES to be used in diagnosing periodontal disease perceives the clinical and radiographic findings and determines the severity of disease as output. Expert system (ES) includes the severity of disease and other risk factors that cannot be fuzzified, and thus determines the type of disease and treatment method as output values. Contribution of this study is to be the first work of fuzzy logic application in the area of periodontal dental disease. It has some advantages comparing with traditional diagnosis and treatment methods.

Osaba, E. Onieva, F. Diaz, R. Carballedo, Volume: 41, Issue: 4, Pages: 1530-1531, Part: 1, March 2014) [4] the Osoba E. et al have discussed our method to solve the Traveling Salesman Problem pointing that we use our developed new algorithm to compare different versions of a classical genetic algorithm, each of one with a different mutation operator and they write that this can generate some controversy. Here we shortly analyze the comment of Osaba E. et al. to show that our comparing method has a chance of existence.

Expert system has been widely used since it was created as early as 1970s. One of the challenges that expert systems faced is to deal with uncertain information. Even though there are many uncertainty management approaches which can deal with problems of different types, the term uncertain information is not clearly defined. This paper reviews some of the uncertainty management in order to highlight, compare and clarify the differences of these approaches in terms of the application area and target area of problem solving. With this, we propose a comparison model which can serve as a guide in selecting a suitable uncertainty management with the consideration of three uncertain information categories: event, evidence and variable. These three categories are different in terms of their area of problem solving.

In the process of protein purification, the amount of proteins isolated with the help of commercial protein purification processes remains uncertain and vague. The present paper proposed a set of fuzzy rule system based on Fuzzy Expert System (FES) which predicts the amount of purified proteins based upon the flow rate of the protein in column, pH and binding capacity of resin to desired protein present in column. The potential benefit of this fuzzy system is to develop a computational model which helps the scientific community to determine the amount of purified protein before starting the process of purification and saving time and related procedural works.

In this study, chronic intestine illness symptoms such as sedimentation and prostate specific antigen are used for the design of fuzzy expert system to determine the drug (salazopyrine) dose. Suitable drug dose for patients is obtained by using data of ten patients. The results of some patients are compared with the doses recommended to them by the physician. As a result, it has been seen that proposed system is helped to shorten the treatment duration and minimize or remove the negative effects of determination of drug dose for helping physicians.

Decision-making is a difficult and quite responsible task for doctors. Some of the computer decision models assisted the doctor with some computer decision models. In this study, neuro-fuzzy network has been designed to determine anemia level of a child. The performance analyses have been obtained by leaving-one-out cross-validation. After statistical measurements, it was found that MPE=−0.0018, MAE=0.2090, MAPE=0.0511, RMSE=0.2743 and R2=0.9957 of this developed system. According to these results, the designed neuro-fuzzy network may be considered as adequate close to traditional decision-making methods and thus the designed network can be used effectively for child anemia prediction.

We introduce a type-2 fuzzy logic system (FLS), which can handle rule uncertainties. The implementation of this type-2 FLS involves the operations of fuzzification, inference, and output processing. We focus on “output processing,” which consists of type reduction and defuzzification. Type-reduction methods are extended versions of type-1 defuzzification methods. Type reduction captures more information about rule uncertainties than does the defuzzified value (a crisp number), however, it is computationally intensive, except for interval type-2 fuzzy sets for which we provide a simple type-reduction computation procedure. We also apply a type-2 FLS to time-varying channel equalization and demonstrate that it provides better performance than a type-1 FLS and nearest neighbor classifier

This chapter summarizes only two popular methods of classification. The first is classification using equivalence relations. This approach makes use of certain special properties of equivalence relations and the concept of defuzzification known as lambda-cuts on the relations. The second method of classification is a very popular method known as fuzzy c-means (FCM), so named because of its close analog in the crisp world, hard c-means (HCM). This method uses concepts in n-dimensional Euclidean space to determine the geometric closeness of data points by assigning them to various clusters or classes and then determining the distance between the clusters. In the case of fuzzy relations, for all fuzzy equivalence relations, their ?-cuts are equivalent ordinary relations. Purpose. To use artificial neural networks for predicting dissolution profiles of matrix-controlled release theophylline pellet preparation, and to evaluate the network performance by comparing the predicted dissolution profiles with those obtained from physical experiments using similarity factor. Methods. The Multi-Layered Perceptron (MLP) neural network was used to predict the dissolution profiles of theophylline pellets containing different ratios of microcrystalline cellulose (MCC) and glyceryl monostearate (GMS). The concepts of leave-one-out as well as a time-point by time-point estimation basis were used to predict the rate of drug release for each matrix ratio. All the data were used for training, except for one set which was selected to compare with the predicted output. The closeness between the predicted and the reference dissolution profiles was investigated using similarity factor (f 2). Results. The f 2 values were all above 60, indicating that the predicted dissolution profiles were closely similar to the dissolution profiles obtained from physical experiments. Conclusion. The MLP network could be used as a model for predicting the dissolution profiles of matrix-controlled release theophylline pellet preparation in product development.

Using process control approaches, pharmacokinetic models are developed. All models presented are based on the one-compartment model. These neural network-based and adaptive versions of linear, nonlinear, and time-dependent models are tested on data sets collected from hemodialysis patients receiving anti-coagulant heparin during treatment. Results show that increased model complexity ensures improved quality of identification, while it decreases the initial quality of estimation

The aim of this paper is to introduce the algorithm proposed in (Fuzzy Sets and System, 2002) and some further modifications. Its applications presented in (ibid; A comparison of five approaches for lithium dose and serum concentration prediction, IFSA-NAFIPS 2001, pp. 104-110) is reviewed as a full collection of its use in phar- macokinetic analysis. First a recently developed fuzzy system modeling algorithm and approximate reasoning tool are introduced along with the modifications. Later the performance of the proposed algorithm is tested in two different data sets and compared with some well-known algorithms from the literature. In the comparison, individualized pharmacokinetic data (i.e., alprazolam data) and population pharmacokinetic data (i.e., lithium data) are used. The comparisons demonstrate that the proposed algorithm can be successfully applied in pharmacokinetic modeling.

From October 1969 through March 1972, 1,276 patients underwent coronary artery bypass at the Texas Heart Institute. Associated conditions included simultaneous valve replacement and resection or repair of ventricular aneurysms or ascending aortic aneurysms. Among the series were 1,105 patients who underwent bypass surgery alone, with or without endarterectomy. Factors affecting outcome were analyzed and revealed adverse influences including concomitant endarterectomy, female sex, age, evidence of left ventricular failure or serious ventricular dysfunction, and previous myocardial infarction within the month before surgery, but more importantly within the week prior to surgery. Functional class III and IV angina was significantly decreased or totally relieved in a high percentage of patients after bypass surgery, and remains the major indication at this time for myocardial revascularization. Progressive reduction in surgical risk and improvement in patient selection allow coronary artery bypass to be performed currently with an early mortality risk of only 3.7%.

In this paper, we investigate the ability of fuzzy to adapt the parameters of a pharmacokinetic and pharmacodynamic model-based controller for the delivery of the muscle relaxant pancuronium. The system uses the model to control the rate of drug delivery and uses feedback from a sensor which measures muscle relaxation level to adapt the model using fuzzy logic. The control strategy administers mini-bolus doses of pancuronium and modulates the magnitude and time interval between the bolus doses to maintain a patient's muscle relaxation within an allowable range specified by the user. Before each new dose is given, the fuzzy logic adaptation scheme uses the error between the predicted patient response and the measured response to adapt the model. The system was tested using computer simulation by varying the parameters of the model by 50% from their nominal values. It was also evaluated in a clinical trial of five patients undergoing surgical procedures lasting 5 h or longer.
