Abstract

In a double-blind between-patient study of aspirin and benorylate carried out in 72 outpatients with rheumatoid arthritis, benorylate 4 g twice daily was shown to be an effective analgesic and anti-inflammatory drug, its effects being indistinguishable from those of aspirin 1.2 g four times daily. Compared with the pretreatment values both drugs produced a statistically significant improvement (P < 0.01) in functional grade, overall pain, articular index, and grip strength at the end of the first and second weeks. The overall incidence of side effects was less with benorylate, though this difference was not significant at the 5% level.

Abraham model solute descriptors have been determined for nisoldipine, nizatidine, loratadine, zonisamide, oxaprozin, rebamipide, domperidone, temozolomide, 'florfenicol', florfenicol A, dapsone, chrysin, benorilate, β-lapachone, and Ipriflavone based on published partition coefficients, molar solubilities and gas chromatographic retention indices. The calculated solute descriptors, combined with our previously published Abraham model correlations, are used to predict several important physicochemical and biological properties, such as air-water, air-blood, air-lung, air-fat, air-skin, water-lipid, water-membrane and water-skin partition coefficients, as well as permeation from water through skin.
Solubility Determination and Model Correlation of Benorilate between T = 278.18 and 318.15 K

The objective is to study the solubility of benorilate in 12 pure solvents in the temperature range from 278.15 to 318.15 K by the isothermal saturation method at atmospheric pressure. The solubility increased monotonically with the rising temperature. Moreover, at all evaluated temperatures, the largest solubility value was obtained in N-methyl-2-pyrrolidone, followed by N,N-dimethylformamide > 2-butanone > 1,4-dioxane > acetonitrile > ethyl acetate > 1-butanol > ethanol > methanol > n-propanol > isopropanol > toluene. The solubility values in the reaction solvent (toluene) varied from 1.3 × 10-5 to 1.08 × 10-4 in mole fraction between T = 278.15 and 318.15 K, an increase of 8.31-fold. At all evaluated temperatures, the values in ethanol increased from 1.58 × 10-4 to 9.07 × 10-4 in mole fraction, an increase of 5.74-fold. Hence, the precipitation and the crystallization purification of benorilate could be effectively controlled by changing the temperature. The obtained solubility data were correlated with the modified Apelblat equation and λh equation. The results of relative average deviations indicated that both the λh equation and the modified Apelblat equation are capable of describing the solubility of benorilate in selected pure solvents.

Benorylate, an esterification product of paracetamol and acetylsalicylic acid is indicated in the treatment of rheumatoid and osteo-arthritis, other painful conditions and febrile states. In animals benorylate’ has been shown to be of low toxicity and only has teratogenic activity at high doses. Gastric effects are less severe than occur with aspirin. Analgesic, anti-inflammatory and anti-pyretic effects have been demonstrated in animal models. Benorylate is well absorbed and metabolised to paracetamol and aspirin producing higher steady state levels than that with paracetamol alone. In man the half life is about one hour. Clinically, benorylate has been shown to be comparable with aspirin in the treatment of rheumatoid arthritis, osteo-arthritis and other painful conditions. It shows better gastric tolerance than aspirin and is less likely to produce gastro-intestinal bleeding. A prolonged anti-pyretic effect has been demonstrated.

There is still no general hypothesis to explain convincingly how or why anti-rheumatic drugs work. In particular there is little evidence to suggest any degree of specificity of these drugs (Rooney et al., 1975).
Fenbufen Anti-rheumatic drugs II
Article
Fenbufen is an effective well-tolerated new non-steroidal anti-inflammatory drug with an improved therapeutic ratio. Its potency and long half-life of its active metabolite allow twice daily dosing and should improve patient compliance. Its low incidence of minor gastrointestinal and CNS side effects enables patients to take adequate anti-inflammatory doses. It has a low incidence of serious GI side effects probably because it is a pro-drug and has little effect on bleeding time. Experience to date has not revealed any propensity to seriously damage the bone marrow or liver or cause any other serious side effects. If widespread use confirms these safety features, fenbufen will be a significant analgesic/anti-inflammatory drug for the management of patients with rheumatoid arthritis, osteoarthritis, ankylosing spondylitis, acute gouty arthritis and pain.

Non-steroidal anti-inflammatory drugs (NSAIDs) effectively control the symptoms of many of the rheumatic diseases although they have little effect on the underlying causes. Their effect is mainly on the mediators of the inflammatory process. Unfortunately, these mediators have important physiological roles in the maintenance of health, particularly in the gastrointestinal tract and the kidney, so that their inhibition results in many unwanted reactions of varying severity. The mechanisms underlying these reactions are described. Their occurrence varies, both qualitatively and quantitatively, and an attempt is made to assess these differences, although it may be that they are related directly to differences in dosage and therapeutic efficacy. In addition, immunologically mediated adverse reactions occur. These mechanisms are outlined and related to the clinical picture. There are considerable differences in frequency of reactions between the compounds: in particular there is a wide variation in the rate of dermatological reactions of this type. Agranulocytosis has been particularly assodated with the pyrazolone compounds, although it has been reported with most others. Aplastic anaemia, which may not be an immune-mediated reaction, is also thought of as a pyrazolone reaction, but the incidence with indomethacin approaches a similar level. Although all drugs analysed may cause hepatic reactions, these are rare except with the now withdrawn benoxaprofen. Several types of immunologically mediated renal reactions occur and these rarities are also described. Paracetamol does not have any effect on the inflammatory mediators. Anxieties about this substance relates to the parent compound phenacetin and its necrotic effect on the renal papillae. There is extensive literature on this subject concerning not only paracetamol, but also aspirin and other NSAIDs. This is also assessed and summarised. The danger of paracetamol as a direct hepatic toxin in self-poisoning is discussed. Novel NSAIDs are introduced and others withdrawn with frequent and monotonous regularity. Sometimes the reasons have some medical or scientific plausibility, but often they are over-reactions by registration authorities or pharmaceutical companies in response to uninformed media publicity. The problems of the numerically and scientifically accurate collection and assessment of adverse reaction data are legion and as a result useful agents have been lost. Some of these difficulties are described, and some non-drug ‘adverse reactions’ are described. New publications on this subject continue to appear so that ideas and concepts are continually changing. The perfect NSAID — a powerful anti-inflammatory analgesic without side effects — does not yet exist. A very low dose of a bland compound may be almost without side effects, but is of little comfort to the patient afflicted with severe rheumatoid arthritis. Comparative side effect rates are thus irrelevant unless related to therapeutic efficacy. This article describes side effects, mechanisms and some comparative frequency rates, but because of the efficacy constraint makes no attempt to discuss relative merits of the various compounds.

Reported Influenza in Pregnancy and Subsequent Cancer in the Child

A longitudinal study of 1,959 infants born in the first week of March 1958 to mothers who were reported to have had influenza during pregnancy revealed an incidence of cancer of 4.1 per 1,000 compared with only 0.8 per 1,000 among the 14,791 infants of mothers who had not had influenza.This increase was caused by cases of leukaemia and other neoplasms of lymphatic and haematopoietic tissue (I.C.D. 200-209) (P <0.0001).Data from the reports of the Registrar General for England and Wales were used to estimate the number of infants born in each year from 1955 to 1964 who subsequently died of cancer before 5 years of age. The rates for each year were compared with an estimate of the prevalence of influenza during the preceding winter. After allowing for the overall trend in the cancer death rate, a highly significant correlation was shown with deaths attributed to causes classified as I.C.D. 200-209 (P <0.005), but not with deaths attributed to other cancers. The increase in the risk of developing these neoplasms among children whose mothers had influenza is estimated to be not less than fourfold. Even so the risk remains small (3 to 4 per 1,000).
