Development and reliability of a system to classify gross motor function in children with Cerebral Palsy

Abstract

To address the need for a standardized system to classify the gross motor function of children with cerebral palsy, the authors developed a five-level classification system analogous to the staging and grading systems used in medicine. Nominal group process and Delphi survey consensus methods were used to examine content validity and revise the classification system until consensus among 48 experts (physical therapists, occupational therapists, and developmental pediatricians with expertise in cerebral palsy) was achieved. Interrater reliability (kappa) was 0.55 for children less than 2 years of age and 0.75 for children 2 to 12 years of age. The classification system has application for clinical practice, research, teaching, and administration.

Introduction & User Instructions
The Gross Motor Function Classification System for cerebral palsy is based on self-initiated movement with
particular emphasis on sitting (truncal control) and walking. When defining a 5 level Classification System,
our primary criterion was that the distinctions in motor function between levels must be clinically
meaningful. Distinctions between levels of motor function are based on functional limitations, the need for
assistive technology, including mobility devices (such as walkers, crutches, and canes) and wheeled mobility,
and to much lesser extent quality of movement. Level I includes children with neuromotor impairments
whose functional limitations are less than what is typically associated with cerebral palsy, and children who
have traditionally been diagnosed as having “minimal brain dysfunction” or “cerebral palsy of minimal
severity”. The distinctions between Levels I and II therefore are not as pronounced as the distinctions
between the other Levels, particularly for infants less than 2 years of age.
The focus is on determining which level best represents the child’s present abilities and limitations in motor
function. Emphasis is on the child’s usual performance in home, school, and community settings. It is
therefore important to classify on ordinary performance (not best capacity), and not to include judgments
about prognosis. Remember the purpose is to classify a child’s present gross motor function, not to judge
quality of movement or potential for improvement.
The descriptions of the 5 levels are broad and are not intended to describe all aspects of the function of
individual children. For example, an infant with hemiplegia who is unable to crawl on hands and knees, but
otherwise fits the description of Level I, would be classified in Level I. The scale is ordinal, with no intent
that the distances between levels be considered equal or that children with cerebral palsy are equally
distributed among the 5 levels. A summary of the distinctions between each pair of levels is provided to
assist in determining the level that most closely resembles a child’s current gross motor function.
The title for each level represents the highest level of mobility that a child is expected to achieve between 6-
12 years of age. We recognize that classification of motor function is dependent on age, especially during
infancy and early childhood. For each level, therefore, separate descriptions are provided for children in
several age bands. The functional abilities and limitations for each age interval are intended to serve as
guidelines, are not comprehensive, and are not norms. Children below age 2 should be considered at their
corrected age if they were premature.
An effort has been made to emphasize children’s function rather than their limitations. Thus as a general
principle, the gross motor function of children who are able to perform the functions described in any
particular level will probably be classified at or above that level; in contrast the gross motor functions of
children who cannot perform the functions of a particular level will likely be classified below that level.

Before 2nd Birthday
Level I Infants move in and out of sitting and floor sit with both hands free to manipulate objects. Infants
crawl on hands and knees, pull to stand and take steps holding on to furniture. Infants walk between
18 months and 2 years of age without the need for any assistive mobility device.
Level II Infants maintain floor sitting but may need to use their hands for support to maintain balance. Infants
creep on their stomach or crawl on hands and knees. Infants may pull to stand and take steps
holding on to furniture.
Level III Infants maintain floor sitting when the low back is supported. Infants roll and creep forward on their
stomachs.
Level IV Infants have head control but trunk support is required for floor sitting. Infants can roll to supine and
may roll to prone.
Level V Physical impairments limit voluntary control of movement. Infants are unable to maintain antigravity
head and trunk postures in prone and sitting. Infants require adult assistance to roll.
Between 2nd and 4th Birthday
Level I Children floor sit with both hands free to manipulate objects. Movements in and out of floor sitting
and standing are performed without adult assistance. Children walk as the preferred method of
mobility without the need for any assistive mobility device.
Level II Children floor sit but may have difficulty with balance when both hands are free to manipulate
objects. Movements in and out of sitting are performed without adult assistance. Children pull to
stand on a stable surface. Children crawl on hands and knees with a reciprocal pattern, cruise
holding onto furniture and walk using an assistive mobility device as preferred methods of mobility.
Level III Children maintain floor sitting often by "W-sitting" (sitting between flexed and internally rotated hips
and knees) and may require adult assistance to assume sitting. Children creep on their stomach or
crawl on hands and knees (often without reciprocal leg movements) as their primary methods of self-
mobility. Children may pull to stand on a stable surface and cruise short distances. Children may
walk short distances indoors using an assistive mobility device and adult assistance for steering and
turning.
Level IV Children floor sit when placed, but are unable to maintain alignment and balance without use of their
hands for support. Children frequently require adaptive equipment for sitting and standing. Self-
mobility for short distances (within a room) is achieved through rolling, creeping on stomach, or
crawling on hands and knees without reciprocal leg movement.
Level V Physical impairments restrict voluntary control of movement and the ability to maintain antigravity
head and trunk postures. All areas of motor function are limited. Functional limitations in sitting and
standing are not fully compensated for through the use of adaptive equipment and assistive
technology. At Level V, children have no means of independent mobility and are transported. Some
children achieve self-mobility using a power wheelchair with extensive adaptations.
Between 4th and 6th Birthday
Level I Children get into and out of, and sit in, a chair without the need for hand support. Children move
from the floor and from chair sitting to standing without the need for objects for support. Children
walk indoors and outdoors, and climb stairs. Emerging ability to run and jump.
Level II Children sit in a chair with both hands free to manipulate objects. Children move from the floor to
standing and from chair sitting to standing but often require a stable surface to push or pull up on
with their arms. Children walk without the need for any assistive mobility device indoors and for
short distances on level surfaces outdoors. Children climb stairs holding onto a railing but are unable
to run or jump.
Level III Children sit on a regular chair but may require pelvic or trunk support to maximize hand function.
Children move in and out of chair sitting using a stable surface to push on or pull up with their arms.
Children walk with an assistive mobility device on level surfaces and climb stairs with assistance from
an adult. Children frequently are transported when travelling for long distances or outdoors on
uneven terrain.
Level IV Children sit on a chair but need adaptive seating for trunk control and to maximize hand function.
Children move in and out of chair sitting with assistance from an adult or a stable surface to push or
pull up on with their arms. Children may at best walk short distances with a walker and adult
supervision but have difficulty turning and maintaining balance on uneven surfaces. Children are
transported in the community. Children may achieve self-mobility using a power wheelchair.
Level V Physical impairments restrict voluntary control of movement and the ability to maintain antigravity
head and trunk postures. All areas of motor function are limited. Functional limitations in sitting and
standing are not fully compensated for through the use of adaptive equipment and assistive
technology. At Level V, children have no means of independent mobility and are transported. Some
children achieve self-mobility using a power wheelchair with extensive adaptations.
Between 6th and 12th Birthday
Level I Children walk indoors and outdoors, and climb stairs without limitations. Children perform gross
motor skills including running and jumping but speed, balance, and coordination are reduced.
Level II Children walk indoors and outdoors, and climb stairs holding onto a railing but experience limitations
walking on uneven surfaces and inclines, and walking in crowds or confined spaces. Children have
at best only minimal ability to perform gross motor skills such as running and jumping.
Level III Children walk indoors or outdoors on a level surface with an assistive mobility device. Children may
climb stairs holding onto a railing. Depending on upper limb function, children propel a wheelchair
manually or are transported when travelling for long distances or outdoors on uneven terrain.
Level IV Children may maintain levels of function achieved before age 6 or rely more on wheeled mobility at
home, school, and in the community. Children may achieve self-mobility using a power wheelchair.
Level V Physical impairments restrict voluntary control of movement and the ability to maintain antigravity
head and trunk postures. All areas of motor function are limited. Functional limitations in sitting and
standing are not fully compensated for through the use of adaptive equipment and assistive
technology. At level V, children have no means of independent mobility and are transported. Some
children achieve self-mobility using a power wheelchair with extensive adaptations.
Distinctions Between Levels I and II
Compared with children in Level I, children in Level II have limitations in the ease of performing movement transitions; walking outdoors
and in the community; the need for assistive mobility devices when beginning to walk; quality of movement; and the ability to perform gross
motor skills such as running and jumping.
Distinctions Between Levels II and III
Differences are seen in the degree of achievement of functional mobility. Children in Level III need assistive mobility devices and frequently
orthoses to walk, while children in Level II do not require assistive mobility devices after age 4.
Distinctions Between Level III and IV
Differences in sitting ability and mobility exist, even allowing for extensive use of assistive technology. Children in Level III sit
independently, have independent floor mobility, and walk with assistive mobility devices. Children in Level IV function in sitting (usually
supported) but independent mobility is very limited. Children in Level IV are more likely to be transported or use power mobility.
Distinctions Between Levels IV and V
Children in Level V lack independence even in basic antigravity postural control. Self-mobility is achieved only if the child can learn how to
operate an electrically powered wheelchair.

The aim of this study was to develop a Spanish Version of the Trunk Measurement Scale (TCMS-S) to analyze its validity and reliability and determine the Standard Error of Measurement (SEM) and Minimal Detectable Change (MDC) in children with Cerebral Palsy (CP). Participants were assessed twice 7–15 days apart with the TCMS-S and once with the Gross Motor Function Measurement-88 (GMFM-88), Pediatric Disability Inventory-Computer Adaptive Test (PEDI-CAT), Cerebral Palsy Quality of Life (CPQoL), and Gross Motor Classification System (GMFCS). Internal consistency was evaluated using Cronbach’s alpha, and the intraclass correlation (ICC) and kappa coefficients were used to investigate the agreement between the assessments. Finally, 96 participants with CP were included. The TCMS-S showed excellent internal consistency (Cronbach’s alpha = 0.95 [0.93 to 0.96]); was highly correlated with the GMFM-88 (rho = 0.816) and the “mobility” subscale of the PEDI-CAT (rho = 0.760); showed a moderate correlation with the “feeling about functioning” CPQoL subscale (rho = 0.576); and differentiated between the GMFCS levels. Excellent test–retest agreement was found for the total and subscale scores (ICC ≥ 0.94 [0.89 to 0.97). For the total TCMS-S score, an SEM of 1.86 and an MDC of 5.15 were found. The TCMS-S is a valid and reliable tool for assessing trunk control in children with CP.

Our investigation aimed to assess the reliability of the femoral head shape classification system devised by Rutz et al. and observe its application in patients with cerebral palsy (CP) at different skeletal maturity levels. Four independent observers assessed anteroposterior radiographs of the hips of 60 patients with hip dysplasia associated with non-ambulatory CP (Gross Motor Function Classification System levels IV and V) and recorded the femoral head shape radiological grading system as described by Rutz et al. Radiographs were obtained from 20 patients in each of three age groups: under 8 years, between 8 and 12 years and above 12 years old, respectively. Inter-observer reliability was assessed by comparing the measurements of four different observers. To determine the intra-observer reliability, radiographs were reassessed after a 4-week interval. Accuracy was checked by comparing these measurements with the assessment of expert consensus. Validity was checked indirectly by observing the relationship between the Rutz grade and the migration percentage. The Rutz classification system's evaluation of femoral head shape showed moderate to substantial intra- and inter-observer reliability (mean κ = 0.64 for intraobserver and mean κ = 0.5 for interobserver). Specialist assessors had slightly higher intra-observer reliability than trainee assessors. The grade of femoral head shape was significantly associated with increasing migration percentage. Rutz's classification was shown to be reliable. Once the clinical utility of this classification can be established, it has the potential for broad application for prognostication and surgical decision-making and as an essential radiographic variable in studies involving the outcomes of hip displacement in CP. A home program is implemented as an evidence-based mode of delivering services for physiotherapy and rehabilitation. Telerehabilitation is a method applied in physiotherapy modalities for children. This study aims to determine the effectiveness of usual care plus a Telerehabilitation-Based Structured Home Program on preschool children with cerebral palsy (CP) compared to usual care. Forty-three children aged 3–6 years (mean age 4.66 ± 1.08 years) with CP were randomly assigned to the Telerehabilitation-Based Structured Home Program and usual care groups. Their motor function was assessed with the Gross Motor Function Measure (GMFM); performance and satisfaction were evaluated with the Canadian Occupational Performance Measure (COPM); goal achievement was assessed with the Goal Attainment Scale (GAS); and activity and participation were evaluated with Pediatric Evaluation of Disability Inventory (PEDI). Participants were evaluated at baseline, immediately post-intervention (12 weeks) and at follow-up (24 weeks). There was a statistically significant difference between pre-and post-test GMFM, COPM, GAS and PEDI scores in the intervention and control groups (p < 0.001). The Telerehabilitation-Based Structured Home Program showed statistically significant changes in activity, participation and goal achievement after 12 weeks of intervention (p < 0.001). However, significant results were not obtained in the usual care group. The Telerehabilitation-Based Structured Home Program may be an effective method for preschool children with CP. Context Adults with cerebral palsy (CP) display a higher prevalence of cardiometabolic disease compared with the general population. Studies examining cardiometabolic disease risk in children with CP are limited. Objective The purpose of this study was to determine if children with CP exhibit higher cardiometabolic risk than typically developing children, and to examine its relationship with visceral adiposity and physical activity. Methods Thirty ambulatory children with CP and 30 age-, sex-, and race-matched typically developing control children were tested for blood lipids, glucose, and the homeostatic model assessment of insulin resistance (HOMA-IR). Visceral fat was assessed using dual-energy x-ray absorptiometry. Physical activity was assessed using accelerometer-based monitors. Results Children with CP had higher total cholesterol, low-density lipoprotein cholesterol, and non-high-density lipoprotein cholesterol (non-HDL-C), glucose, prevalence of dyslipidemia, prevalence of prediabetes, and visceral fat mass index (VFMI) and lower physical activity than controls (all P < .05). In the groups combined, non-HDL-C and glucose were positively related to VFMI (r = 0.337 and 0.313, respectively, P < .05), and non-HDL-C and HOMA-IR were negatively related to physical activity (r = −0.411 and −0.368, respectively, P < .05). HOMA-IR was positively related to VFMI in children with CP (r = 0.698, P < .05), but not in controls. Glucose was not related to physical activity in children with CP, but it was negatively related in controls (r = −0.454, P < .05). Conclusion Children with CP demonstrate early signs of cardiometabolic disease, which are more closely related to increased visceral adiposity than decreased physical activity.

Introducción: La Parálisis Cerebral (PC) es la principal causa de defi­ciencia motriz en la población infanto-juvenil. Su rehabilitación requiere de sistemas de medición no invasivos, costo-efectivos y de fácil aplicación. La variabilidad del ritmo cardiaco (VRC) cumple estos requisitos, pero aún faltan parámetros de referencia en menores de 18 años con PC. El objetivo es escribir la VRC en menores de 18 años con PC y su relación con la motricidad gruesa, según el Gross Motor Function Classification System (GMCFS). Materiales (o pacientes) y Métodos: Revisión sistemática. Se buscó en 11 bases de datos y referencias de estudios elegibles que incluyeron diseños observacionales, experimentales y cuasi-experimentales, población menor de 18 años con PC y clasificados con GMFCS. La VRC se evaluó en reposo utilizando análisis de dominio de tiempo y frecuencia. El riesgo de sesgo se evaluó con Robins-I. Resultados: Se identificaron 264 artículos. Siete cumplieron con los criterios de inclusión, con un total de 242 pacientes con edades entre 4 y 14 años. Los informes de VRC se realizaron a través de los dominios de tiempo (SDNN y rMSSD) y/o frecuencia (HF, LF, HF y LF/HF). Conclusiones: Existe evidencia limitada sobre el análisis de los diferentes dominios de la VRC en menores de 18 años con CP. Los hallazgos son contradictorios con respecto a la relación entre VRC y GMFCS. Aún se requiere un mayor número de estudios, mayo­res tamaños muestrales y un mejor control de las variables de confusión para generar relaciones y parámetros de referencia según GMFCS.

We sought to prospectively characterize the nutritional status of adults ≥ 19 years (n=22, 27% males) and children (n=38, 61% male) with genetically-confirmed primary mitochondrial disease (PMD) to guide development of precision nutritional support strategies to be tested in future clinical trials. We excluded subjects who were exclusively tube-fed. Daily caloric requirements were estimated using World Health Organization (WHO) equations to predict resting energy expenditure (REE) multiplied by an activity factor (AF) based on individual activity levels. We developed a M itoch o ndrial Disease Ac tiv ity F a c tor s (MOTIVATOR) score to encompass fatigue typical of PMD. PMD cohort daily diet intake was estimated to be 1,143 ± 104.1 Kcal in adults (mean ± SEM, 76.2% of WHO-MOTIVATOR predicted requirement), and 1,114 ± 62.3 Kcal in children (86.4% predicted). A total of 11/22 (54.5%) adults and 18/38 (47.4%) children with PMD consumed ≤75% predicted daily Kcal needs. Malnutrition was identified in 16/60 (26.7%) PMD subjects. Increased protein and fat intake correlated with improved muscle strength in those with insufficient daily Kcal intake (£75% predicted); higher protein and fat intake correlated with decreased muscle fatigue; and higher protein, fat and CHO intake correlated with improved quality of life (QoL). These data demonstrate the frequent occurrence of malnutrition in PMD and emphasize the critical need to devise nutritional interventions to optimize clinical outcomes.

Introduction: Patients with cerebral palsy (CP) may receive intrathecal baclofen (ITB) to reduce muscle spasticity and dystonia. It can be challenging to identify the proper dose of ITB, and anecdotally these dosing needs may change after spinal fusion surgery. This study aimed to evaluate the need for changes in ITB dosing following a spinal fusion in pediatric neuromuscular scoliosis (NMS) patients and identify predisposing factors for those changes. Methods: This was a retrospective case-control study of NMS patients with an ITB pump who later received a spinal fusion surgery. Dosing changes and the indications for the changes were postoperatively noted. Demographics, preoperative factors, and surgical factors were evaluated for correlation with dosing changes. Results: A total of 49 patients were included in this study. Most had no change in ITB dose (71.4%), and others required a change that averaged about 10%. Male patients, those with larger pumps, and those that had a longer hospital stay were more likely to require a decrease in dose. Complications were similar between groups. Three catheters were revised during surgery: two continued on the same dose and one required an increase in dose after surgery. Conclusions: Spinal fusion after ITB pump placement is feasible and safe. Most patients did not require dosing changes after spine fusion; however, careful evaluation postoperatively remains prudent.

Background: In newborns with hypoxic-ischemic encephalopathy (HIE), the correlation between neonatal neuroimaging and the degree of neurodevelopmental impairment (NDI) is unclear. Methods: Infants with HIE enrolled in a randomized controlled trial underwent neonatal MRI/MR spectroscopy (MRS) using a harmonized protocol at 4-6 days of age. The severity of brain injury was measured with a validated scoring system. Using proportional odds regression, we calculated adjusted odds ratios (aOR) for the associations between MRI/MRS measures of injury and primary ordinal outcome (i.e., normal, mild NDI, moderate NDI, severe NDI, or death) at age 2 years. Results: Of 451 infants with MRI/MRS at a median age of 5 days (IQR 4.5-5.8), outcomes were normal (51%); mild (12%), moderate (14%), severe NDI (13%); or death (9%). MRI injury score (aOR 1.06, 95% CI 1.05, 1.07), severe brain injury (aOR 39.6, 95% CI 16.4, 95.6), and MRS lactate/n-acetylaspartate (NAA) ratio (aOR 1.6, 95% CI 1.4,1.8) were associated with worse primary outcomes. Infants with mild/moderate MRI brain injury had similar BSID-III cognitive, language, and motor scores as infants with no injury. Conclusion: In the absence of severe injury, brain MRI/MRS does not accurately discriminate the degree of NDI. Given diagnostic uncertainty, families need to be counseled regarding a range of possible neurodevelopmental outcomes. Impact: Half of all infants with hypoxic-ischemic encephalopathy (HIE) enrolled in a large clinical trial either died or had neurodevelopmental impairment at age 2 years despite receiving therapeutic hypothermia. Severe brain injury and a global pattern of brain injury on MRI were both strongly associated with death or neurodevelopmental impairment. Infants with mild or moderate brain injury had similar mean BSID-III cognitive, language, and motor scores as infants with no brain injury on MRI. Given the prognostic uncertainty of brain MRI among infants with less severe degrees of brain injury, families should be counseled regarding a range of possible neurodevelopmental outcomes.

This paper explains changes in Chinese and German temporal understanding as a cultural construct due to internet-mediated communication as one oft the key mediating factors aside others. It integrates the anthropological construct of polychronicity, pace of life and temporal horizon into a broader framework, which goes beyond Western biased constructs through the theory driven incorporation of Confucian notions. A central question is whether and how many of the sub dimensions of temporal understanding could be changed through internet-mediated communication. Current media theoretical assumptions are evaluated. Mediatization and recent acceleration theory are taken into consideration. Prospective research is advized to take a closer look at this issue through an interdisciplinary transnational approach.

This paper reports the results of a study to validate a measure of gross motor function in detecting change in the motor function of disabled children. Physiotherapists used this instrument to assess 111 patients with cerebral palsy, 25 with head injury and 34 non-disabled preschool children on two occasions, the second after an interval of four to six months. Parents and therapists independently rated the children's function within two weeks of each assessment, and a sample of paired assessments was videotaped for 'blind' evaluation by therapists. Correlations between scores for change on this measure and the judgments of change by parents, therapists and 'blind' evaluators supported the hypothesis that the instrument would be responsive to both negative and positive changes.

Health status, although commonly defined in terms of the presence or absence of disease, can also be defined in terms of performance or functional status. Loss of performance results in disablement. The authors summarize the conceptual framework and categorization in the World Health Organization's International Classification of Impairments, Disabilities and Handicaps (ICIDH) (1980) and the Model of Occupational Performance in the Guidelines for the Client-Centred Practice of Occupational Therapy (Health and Welfare Canada and the Canadian Association of Occupational Therapists, 1983; 1986). Complementary elements of the ICIDH and the Model of Occupational Performance appear to be recognition that performance is an element of holistic health influenced by the social, physical and cultural environment. Also, the ICIDH categories of disablement appear to be conceptually mirrored in the model's categories of occupational performance. The authors use client and service data to illustrate three uses of the ICIDH in occupational therapy: communication with other health professionals; program planning and management; and, clinical evaluation and research.

This chapter presents analytic methods for matched studies with multiple risk factors of interest. We consider matched sample designs of two types, prospective (cohort or randomized) and retrospective (case-control) studies. We discuss direct and indirect parametric modeling of matched sample data and then focus on conditional logistic regression in matched case-control studies. Next, we describe the general case for matched samples including polytomous outcomes. An illustration of matched sample case-control analysis is presented. A problem solving section appears at the end of the chapter.

Cerebral palsy may be defined as a condition characterized by paralysis, paresis, incoordination, dyskinesia, or any aberration of motor function that is due to involvement of the motor control centers of the brain. It should be differentiated from spinal palsies, such as poliomyelitis or transection of the cord, and from peripheral nerve palsies, such as peripheral neuritis due to lead poisoning or diphtheria. Since the brain has many functions in addition to control of motor ability, it is not surprising that in the cerebral palsied there may be many associated defects. Defects in intelligence, personality, and sensation or epileptic diatheses may coexist. In addition, there may be many sequelae of cerebral palsy, such as physical deformities due to contractures and personality defects incident to the lack of opportunity for normal social development. Prognosis of locomotion in children with cerebral palsy (CP) is complicated by the variety of motor and other neurologic deficits in each case. Children presenting with spastic diplegic (SD) CP related to prematurity generally demonstrate more uniform and comparable neuromotor abnormalities. In this study, longitudinal data were collected on the early postural-locomotor development of children with spastic diplegic cerebral palsy (SDCP). The data were then correlated with the level of ambulatory function attained. Fifty spastic pediatric patients, aged 5 to 19 years, were included in the study completed in 1982. The duration of follow-up ranged from 4 to 16 years. The study is based upon a method of assessing preambulatory milestones, a classification of ambulatory function into four levels of ability, and correlates these two sets of data. The results indicate that the motor control of sitting and crawling at 1 1/2 to 2 1/2 years was predictive of the eventual level of ambulatory function the child was able to attain and maintain.

The neurodevelopmental course of 219 Neonatal Intensive Care Unit graduates followed prospectively over 2 years was determined on retrospective chart review. Mild neuromotor abnormalities during the first year were identified in 50%, three-quarters of which proved transient with normal development at 2 years of age. Moderate abnormalities were identified in 7%, half of which proved transient. Severe neuromotor abnormalities were identified in 20%, two-thirds of whom had cerebral palsy at 2 years. Persistent noncerebral palsy neuromotor abnormalities remained in one-third of those in the severe abnormality group, one-half of those in the moderate abnormality group, and one-quarter of those in the mild abnormality group. As birthweight decreased, the incidence of always normal development and transient neuromotor abnormalities decreased while the incidence of persistent neuromotor abnormalities increased. Small for gestational age infants demonstrated a slightly higher incidence of persistent abnormalities than their appropriate for gestational age counterparts. No significant sex differences were observed.
