A personalised Healthcare Information Delivery System: pushing customised healthcare information over the WWW

Easier and focused access to healthcare information can empower individuals to make 'informed' choices and judgements about personal health maintenance. To achieve 'optimum' patient empowerment, we need to re-evaluate and potentially re-design the processes of healthcare information delivery. Our suggestion is that healthcare information should be personalised according to each individual's healthcare needs and it should be pro-actively delivered, i.e. pushed towards the individual. We present an intelligent Personalised Healthcare Information Delivery Systems that aims to enhance patient empowerment by pro-actively pushing customised, based on one's Electronic Medical Record, health maintenance information via the WWW.

Lay Summary Patient portals provide patients access to health records, lab results, medication refills, educational materials, secure messaging, appointment scheduling, and telehealth visits, allowing patients to take a more active role in their health care decisions and management. This article systematically reviewed all literature regarding patient education through patient portals to determine its effectiveness in improving patient engagement and ability to manage their health conditions. The results found that efforts are being made to raise awareness of educational resources in patient portals, that patients are increasingly utilizing these resources, that patients are finding them useful, and that they are improving health outcomes. It seems that patient portals are becoming a powerful tool for patient engagement and show promise as a means of improving patients’ health. Continued efforts need to be made to raise awareness of patient educational resources in patient portals so that patients utilize these resources to increase their knowledge, skills, and confidence for managing their own health and health care.

Construction rebar workers face postural ergonomic hazards that can lead to work-related Lower Back Disorders (LBDs), primarily due to their prolonged awkward working postures required by the job. In a previous study, Wearable Inertial Measurement Units (WIMUs)-based Personal Protective Equipment (PPE) was developed to alert workers when their trunk inclination holding time exceeded acceptable thresholds as defined in ISO standard 11226:2000. However, subsequent field testing identified PPE was ineffective for some workers because the adopted ISO thresholds were not personalized and did not consider differences in individual’s response to postural ergonomic hazards. To address this problem, this paper introduces a worker-centric method to assist in the self-management of work-related ergonomic hazards, based on data-driven personalized healthcare intervention. Firstly, personalized information is gathered by providing each rebar ironworker a WIMU-based personalized mobile health (mHealth) system to capture their trunk inclination angle and holding time data. Then, the captured individual trunk inclination holding times are analyzed by a Gaussian-like probability density function, where abnormal holding time thresholds can be generated and updated in response to incoming trunk inclination records of an individual during work time. These abnormal holding time thresholds are then adapted to be used as personalized trunk inclination holding time recommendations for an individual worker to self-manage their working postures, based on their own trunk inclination records. The proposed worker-centric method to assist in the self-management of ergonomic postural hazards leading to LBDs was field tested on a construction site over a three-month duration. The results of the paired t-tests indicate that posture scores evaluated by the Ovako Working Posture Analysis System (OWAS) significantly decrease when the personalized recommendation is applied, while increase again when the personalized recommendation is removed. Based on data-driven personalized healthcare intervention, the results demonstrate the significant potential of the proposed worker-centric self-management method for rebar workers in preventing and controlling postural ergonomic hazards during construction rebar ironwork.

Over the last decade genomics and other molecular biosciences have enabled new capabilities that, according to many, have the potential to revolutionize medicine and healthcare. These developments have been associated with a range of terminologies, including 'precision', 'personalized', 'individualized' and 'stratified' medicine. In this article, based on a literature review, we examine how the terms have arisen and their various meanings and definitions. We discuss the impact of the new technologies on disease classification, prevention and management. We suggest that although genomics and molecular biosciences will undoubtedly greatly enhance the power of medicine, they will not lead to a conceptually new paradigm of medical care. What is new is the portfolio of modern tools that medicine and healthcare can use for better targeted approaches to health and disease management, and the sociopolitical contexts within which these tools are applied.

The featured Internet-based healthcare information dissemination system was designed based on the premise that focussed accessibility is a central component in the empowerment of individuals so as to able to exercise sound judgement with respect personal health- maintenance. This requirement necessitates a re- evaluation and re-design of the information dissemination process so as to emphasise: (1) customisation based on individual-specific environmental and healthcare requirements, and (2) proactive just-in-time delivery combining both push (system-initiated) and pull (user- initiated) oriented mechanisms. Our customisation model features Electronic Medical Record (EMR) based profiling in conjunction with personalised information composition drawing from a generic document base covering a wide spectrum of healthcare information of interest to the general population. The resultant Personalised Healthcare Information (PHI) package— covering both long-term and immediate health- maintenance requirements—would subsequently be periodically refreshed based on: (1) additional EMR- related information, (2) healthcare information updates of potential interest as ascertained from individual profiles, and (3) Web-mediated online consultation sessions; the last of which concludes with users being provided a dynamically generated and up-to-date information dosage. PHI delivery can also be executed via email, thereby ensuring the timely availability of high-quality information without over-dependence on user interactivity with the system.

Introduction: Human papillomavirus (HPV) is the most common viral infection of the reproductive tract. Three prophylactic HPV vaccines are available for the prevention of HPV-related disease. Despite clinical success, immunisation rates remain sub-optimal. The purpose of this systematic review is to synthesise qualitative literature to achieve an understanding of the drivers and barriers to HPV vaccine acceptability and to determine targets for an intervention to improve vaccine uptake. Methods: The seven-step model of meta-ethnography described by Noblit and Hare was used. The quality of the studies was assessed using the CASP (Critical Appraisal Skills Programme) for qualitative research. The ENTREQ (Enhancing transparency in reporting the synthesis of qualitative research) statement was used to guide reporting of results. Results: Thirty-three studies were included in the final analysis, compiling the opinions of 1280 parents/guardians from 14 countries. Five key concepts that reflected the principal findings of studies were determined: is prevention better than cure; the fear of the unknown; limited knowledge and understanding; complex vaccination decisions and; parental responsibility. Third-order interpretations were developed and linked using a 'line of argument' to develop a conceptual model. Conclusion: The majority of parents are motivated to protect their children and prevent disease. The link to sexual intercourse associated with the HPV vaccine often complicates the vaccination decision. Vaccine manufacturers, national healthcare systems and healthcare providers can reinforce the importance of HPV immunisation and reiterate the rationale behind vaccination recommendations, by providing unambiguous information in a timely manner, transparently addressing parental concerns regarding vaccine safety and efficacy, whilst taking account of cultural and religious sensitivities and varying health literacy levels. In recent years, there has been a reduction in HPV vaccine uptake worldwide. Currently, there is a paucity of published qualitative studies addressing these new vaccine concerns. Therefore, such research is required to guide intervention development, to improve HPV vaccine uptake.

The paper presents a new cross-layer bridge protocol between IEEE 802.15.4, IEEE 11073 and IEEE 802.11 for wearable-enabled health-data aggregation formations. The protocol addresses the new reality as imposed by the recent evolution of sensors and networking in situations where backbone infrastructure is experiencing congestion. We discuss real-life application scenarios of nimble-network formations. The main novelty of the proposed algorithm is that it defines presence as a new way of routing.

Context-awareness computing is a research field which often refers to healthcare as an interesting and rich area of application. Context aware computing attains environments monitoring by means of sensors to provide relevant information or services according to the identified context. In particular, wireless ad hoc sensor networks for medical purposes are playing an increasing role within healthcare. Body Sensor Networks (BSN) are being designed for prophylactic and follow-up monitoring of patients in e.g. their homes, during hospitalization, and in emergencies. This work presents an integrated environment aimed at providing personalized healthcare services which appropriately meet the user's context. Deploying the semantics embedded in web services and context models is a mandatory step in the automation of service discovery, invocation and composition. Nevertheless, in a context aware domain purely logic-based reasoning on respectively context and services may not be enough. The main idea of this work is related to enrich with qualitative representation of context underling data by means of Fuzzy Logic in order to automatically recognize the context and to consequently find the right set of healthcare services among the available ones. Semantic formalisms (e.g., OWL, OWL-S, etc.) enable the context and services modeling in terms of domain ontology concepts. On the other hand, soft computing techniques support activity of unsupervised context analysis and healthcare semantic service discovery. Goal is to define context-aware system whose quality of retrieved services relies on the acquisition of user context by means of a robust theoretical approach. Moreover, this work defines hybrid architecture which attains a synergy between the agent-based paradigm and the fuzzy modeling. Specifically, the system exploits some task oriented agents in order to achieve context recognition, services matchmaking and brokerage activities.

It first defines e-health and compiles a list of personal health management applications that range from those that provide access to electronic health records and health information, to community interaction and social media environments, and finally to decision support tools for disease management. It presents examples of e-health services that have adopted customization/ personalization techniques as the means to provide better service, such as those from the National Health System (NHS) in the UK, and online portals such as Healthspace, DiasNet, and others. This chapter concludes with the presentation of a framework for acceptance of personalized e-health services by patients and citizens.

The practice of medicine, with its wide range of environmental conditions and complex dependencies, has long been used as a test bed for various advanced technologies. Telemedicine, as conceptualised within the Multimedia Super Corridor (MSC) context, is seen as the application of several relatively mature technologiesartificial intelligence (AI), multimedia communication and information systems (IS) amongst othersso as to benefit a large cross-section of the Malaysian population. We will discuss in general terms the Malaysian vision on the comprehensive MSC telemedicine solution, its functionality and associated operational conditions. In particular, this paper focuses on the conceptualisation of one key telemedical component i.e. the Lifetime Health Plan (LHP) system, which is eventually intended to be a distributed multi-module application for the periodic monitoring and generation of health-care advisories for upwards of 20 million Malaysians.

The Malaysian Telemedicine initiative advocates a paradigm shift in healthcare delivery patterns by way of implementing a person-centred and wellness-focused healthcare system. This paper introduces the Malaysian Telemedicine vision, its functionality and associated operational conditions. In particular, we focus on the conceptualisation of one key Telemedicine component i.e. the Lifetime Health Plan (LHP) system--a distributed multimodule application for the periodic monitoring and generation of health-care advisories for all Malaysians. In line with the LHP project, we present an innovative healthcare delivery info-structure--LifePlan--that aims to provide life-long, pro-active, personalised, wellness-oriented healthcare services to assist individuals to manage and interpret their health needs. Functionally, LifePlan based healthcare services are delivered over the WWW, packaged as Personalised Lifetime Health Plans that allow individuals to both monitor their health status and to guide them in healthcare planning.

The chronic disease usually can not be cured well but controlling by the daily care activity, regular diagnosis and dosage under the physician's instructions. In this research, we propose a community medical care monitoring model for monitoring and managing the chronic patients of community through the WWW (World Wide Web) service network and/or telephone voice service network. It can collect more patients' physiological data measured at home or community health agent. Therefore, the chronic disease patients will be getting better cure from his doctor's diagnosis due to the patient's effort.

Individuals must increasingly take control of managing their own health affairs. This requires access to quality information that is not easily obtained from traditional social institutions. NetWellness is an electronic consumer health information service that provides a model for reaching the goal of enhancing personal health and quality of life. [1] We present a vision of consumer health information delivery in the 21st century, and a model for reaching that vision. Our experience to date, progressing through a five-phase model, is aimed at providing the best health information possible to the widest population possible.

The periodic examination and health promotion center (PEHPC) in Seoul National University Hospital has developed and adopted an information system supported by computers which use an electronic medical record to improve the quality of patient care, advance the science of medicine, lower health care costs, and enhance the education of health care professionals. This information system adopted the concept of incomplete and evolutionary systems to conduct the pursuit of practicalness and efficacy. It has increased efficiency to save costs and to enhance the quality of the medical service. It has also activated clinical research due to ease of managing data. We are also preparing for telecare. Telecare and WWW-using information system is postponed because the protection of a patients privacy is not established.

In the frame of an European telemedicine project, we have designed and implemented an Andrology server that provides the user (e.g. physicians, students) with a set of anonymized andrology cases. Within this context we faced the problem of re-structuring existing clinical information sources and gained some experience on how much of the problem might be solved by the software engineer. This paper presents a method that partially automates the maintenance of the andrology case collection. Moreover, the potential benefit of XML for information representation will be discussed.
