Detection of peanut antigens in breast milk of lactating women

Most individuals who react to peanuts do so on their first known exposure. A potential but unproven route of occult exposure resulting in sensitization to peanut is via breast milk during lactation.
To investigate the ability of maternal dietary peanut protein to pass into breast milk during lactation.
Clinical investigation conducted at 2 North American hospitals from March 1999 to October 2000.
Twenty-three healthy, lactating women aged 21 to 35 years.
Each woman consumed 50 g of dry roasted peanuts, after which breast milk samples were collected at hourly intervals.
Presence in breast milk of total peanut protein, analyzed by a sandwich enzyme-linked immunosorbent assay, and 2 major peanut allergens, Ara h 1 and Ara h 2, detected by immunoblot analysis.
Peanut protein was detected in 11 of 23 subjects. It was detected in 10 subjects within 2 hours of ingestion and in 1 subject within 6 hours. The median peak peanut protein concentration in breast milk was 200 ng/mL (mean, 222 ng/mL; range, 120-430 ng/mL). Both major peanut allergens Ara h 1 and Ara h 2 were detected.
Peanut protein is secreted into breast milk of lactating women following maternal dietary ingestion. Exposure to peanut protein during breastfeeding is a route of occult exposure that may result in sensitization of at-risk infants.

    ABSTRACT: The carry-over of certain feed components into animal products can be of concern for human health. The safety assessment of chemical contaminants including natural toxins, agrochemicals, veterinary drugs, and environmental pollutants is a key element of the “farm-to-fork” (“One Health”) approach. The transmissibility of proteinaceous feed constituents such as enzymes, proteins from genetically engineered crops, and infectious prions in animal meal has also become of interestbut the transfer of proteins with allergic potential is little studied.In the present study, an exploratory zebrafish feeding trial using feed containing 20 percentof processed larvae of the marine fish parasite Anisakis simplex was performed as a proof-of-principle experiment. After a two-week exposure period, anisakid peptides were detected in zebrafish tissueby high-resolution liquid-chromatography Orbitrap mass spectrometry and immunostaining using specific polyclonal antibodies or sera from patients with confirmed allergy to A. simplex. Since fishmeal produced from marine pelagic fish is an important feed component in the culture of Atlantic salmon and in the poultry industry, it should be considered as a source of potentially allergenic peptides in the final products. Furthermore, the substitution of fishmeal with plant proteins would not eliminate the potential health risk by allergencarry-oversince crops of high nutritional value such as legumes also contain important food allergens. If our preliminary results from the present zebrafish feeding trial should be confirmed in necessary follow-up experiments, the question of labeling information on fish and animal food products raised on feed containing potentially allergenic ingredients could arise in order to minimize the exposure risk of allergic consumers.

    ABSTRACT: The increase in the incidence of food allergy is a growing problem for the western world. This review will focus on the findings from several macromolecular epithelial transport experiments and drug permeability studies to provide a recent comprehension of food allergen intestinal epithelial cell transport and the allergen-epithelial relationship. Specifically, this review will aim to answer whether allergens can permeate the intestinal barrier directly via intestinal epithelial cells, and whether this mode of transport affects downstream immune reactions. By improving our understanding of the interactions which take place during exposure of food allergens with the intestinal epithelium, we can begin to understand whether the epithelial barrier plays a major role in the allergic sensitization process rather than simply restricting the entry of allergens to the underlying lamina propria.

    ABSTRACT: Neonatal immune system faces considerable challenges as it must cope with antigenic stimulus following gut and skin bacterial colonization, and exposure to numerous new environmental antigens while organs are developing requiring low inflammation to allow harmonious growth. In addition to its impact on child growth and prevention of infectious disease, breast-milk may help to educate the immune system towards acceptance of non “dangerous” antigens. Indeed, epidemiological studies report that breastfeeding protects from immune-mediated diseases such as allergy, celiac disease and Type-1 Diabetes. However, data are controversial and mechanisms unclear. Here we will present and discuss experimental data indicating that breastfeeding-induced protection may rely on immune tolerance induction in the breastfed child upon antigen transfer through maternal milk. The tolerogenic potential of breast milk depends on maternal exposure to common environmental and dietary antigens and the efficiency of antigen transfer across mammary epithelium. Induction of tolerance upon breast-milk mediated antigen transfer will also depend on the presence of immunomodulatory factors in breast-milk and of its impact on neonatal gut and immune system maturation. The better understanding of maternal influence on tolerance induction by antigen transfer through breast milk should allow the development of new strategies for prevention of immune mediated diseases.
