Provides an overview of the special psychotherapy issues involved in the treatment of neurobehavioral syndromes or composite "organic" patients, and makes recommendations for evaluation, therapeutic strategy and referral. Neuropsychological assessment, besides localizing lesions or quantifying deficits, helps to develop a profile of individual cognitive strengths and weaknesses for planning and predicting the course of treatment. Special consideration is given to the psychological profile and attending difficulties in treating the "organic personality." Two major principles in psychotherapeutic treatment of neurobehavioral patients include individual assessment of limitations in function and related emotional problems and multiple treatment efforts. Chronic pain is a multifaceted syndrome that spans the artificial dichotomy of organic vs psychodynamic. Evaluation of the chronic pain patient must take into account cognitive, emotional, personality, family, legal, and social factors. Psychological approaches to the treatment of chronic pain include operant, cognitive behavioral, and psychodynamically oriented psychotherapeutic modalities. It is increasingly recognized that the complex nature of chronic pain requires a multifaceted treatment approach, and suggestions for implementing such measures are offered.
