A 43-year-old woman presented with a linear skin eruption following the Blaschko lines on the left flank. Histopathological examination revealed lichenoid inflammation and a perieccrine lymphocytic infiltrate. Based on these clinical and histopathological findings the diagnosis lichen striatus was made.

Granuloma annulare is a benign inflammatory disease which is often asymptomatic and self-limiting and most often characterised by erythematous papules and plaques localized on the acral sites. This case of painful lesions on the palms is a less known variant of granuloma annulare.

We describe a 42-year-old man, who presented with a lichen planus-like eruption clinically and histopathologically, that turned out to be a secondary syphilis. Syphilis should be considered in lichenoid eruptions, especially in patients not responding to treatment and in patients with involvement of the palms and soles.
