Introduction
I have published as Monika Arzanauskaite before. I am a Consultant Cardiothoracic Radiologist Liverpool Heart and Chest NHS Foundation Trust. Along with clinical imaging, I teach and do research in cardiovascular and thoracic imaging. I had the honour to be the Radiology TSTL/College Tutor at my Trust for over two terms. I speak and write in English, Spanish, Portuguese and Lithuanian and still have not forgotten Italian.

Aim To explore barriers to academic activities that cardiovascular radiology-oriented radiology trainees face worldwide. Materials and methods An international call for participation in an online survey was distributed via social media and radiological societies to radiology trainees. Questions covered barriers and involvement in academic activi...

Background: Cardiothoracic ratio (CTR) in chest radiographs is still widely used to estimate cardiac size despite the advent of newer imaging techniques. We hypothesise that a universal CTR cut-off value of 50% is a poor indicator of cardiac enlargement. Our aim was to compare CTR with volumetric and functional parameters derived from cardiac magn...

Congenital heart disease is the most common group of congenital pathology. Over the past few decades, advances in surgical treatment have resulted in a rising population of adult patients with repaired complex congenital heart disease. Although the quality of life has greatly improved, a significant proportion of morbidities encountered in clinical...

Objectives: We performed a comprehensive assessment of the effect of myocardial ischemia duration on cardiac structural and functional parameters by serial cardiac magnetic resonance (CMR) and characterized the evolving scar. Background: CMR follow-up on the cardiac impact of time of ischemia in a closed-chest animal model of myocardial infarcti...

Optimal management of adult congenital heart disease (ACHD) requires a multidisciplinary team (MDT) approach, fostering a collaborative culture over an individualistic approach. Within this framework, subspecialty-trained radiologists provide crucial imaging expertise, supporting cardiologists, surgeons, and interventional cardiologists in diagnose...

Pre-procedural imaging is critical for transcatheter mitral valve repair planning in patients with mitral valve disease. As differences among various measurement techniques for valve evaluation are still poorly understood, we sought to assess the intra- and interobserver agreement of complex measurements derived from a prototype mitral evaluation t...

Aims Recurrent acute myocardial infarction (RE-AMI) is a frequent complication after STEMI, and its association with stent thrombosis can be life-threatening. Intravenous atorvastatin (IV-atorva) administration during AMI has been shown to limit infarct size and adverse cardiac remodeling. We determined by cardiac magnetic resonance (CMR) whether t...

There is a wide and ever-increasing range of imaging indications for thoracic computed tomography (CT). Identifying and appropriately triaging cardiovascular findings is often challenging, especially in non-gated or unenhanced studies. The authors provide a pictorial review of clinically relevant abnormalities of the main intrathoracic vessels (aor...

Background Non-atherosclerotic causes of chest pain are challenging to diagnose, particularly in younger patients where coronary artery atherosclerosis is uncommon. Cardiac computed tomography (CT) has emerged as a valuable non-invasive imaging modality for assessing vascular wall and tissue characteristics. Objective Discuss the utility of cardia...

Background and Aims The ecto–nucleoside triphosphate diphosphohydrolases of the CD39 family degrade ATP and ADP into AMP, which is converted into adenosine by the extracellular CD73/ecto-5-nucleotidase. This pathway has been explored in antithrombotic treatments but little in myocardial protection. We have investigated whether the administration of...

Coronary artery disease (CAD) diagnosis requires a precise assessment of patient profile and disease extension. While non-invasive imaging modalities offer an in-depth evaluation of CAD through differential approaches, this is based primarily on detecting coronary plaques or inducible myocardial ischaemia, thus each offering only a partial outlook...

Purpose To determine the reliability of the cardiothoracic ratio (CTR) as a simple method to assess the cardiac size and function in patients with non-ischemic cardiomyopathy (NICM). Methods In a sample of 91 patients (66 patients with diagnosed non-ischemic cardiomyopathy and 25 controls) we calculated the CTR on a posteroanterior chest radiograp...

Background Mitigating myocardial tissue injury (MTI) remains an unmet clinical need in the reperfusion era. Stimulating adenosine synthesis through the breakdown of adenosine triphosphate (ATP) by apyrase (CD39), an endothelial ADPase, has emerged as a potential new avenue in the reduction of MTI. Purpose In this study we examined whether the intr...

Background Preclinical and pilot human studies suggest platelet-independent cardioprotective effects of ticagrelor most likely due to its ability to block cellular uptake of adenosine. Adenosine is a potent endogenous protective molecule, concentrations of which locally rise as a response to ischemia because of the breakdown of extracellular ATP by...

The mitral valve apparatus is one of the key structures of the heart with complex anatomy and function. The prevalence of mitral valve disease is associated with age: elderly people are affected more frequently. There are multiple inherited and acquired conditions; tumors may also involve the mitral valve. Stenosis and regurgitation are the main ma...

Objective HDL (high-density lipoprotein) role in atherosclerosis is controversial. Clinical trials with CETP-Inhibitors have not provided benefit. We have shown that HDL remodeling in hypercholesterolemia reduces HDL cardioprotective potential. We aimed to assess whether hypercholesterolemia affects HDL-induced atherosclerotic plaque regression. A...

Platelets are endowed with a repertoire of surface receptors that enable them to adhere, activate and aggregate upon vascular injury. Platelet adhesion is governed by the interaction between vascular collagen and GPIb-IX-V and GPVI-FcRγ complexes. Platelet kinases downstream 14–3-3ζ-bound GPIb and the FcRγ ITAM domain enable the activation of PLC-γ...

Background The DANISH trial emphasised that the selection of patients with dilated cardiomyopathy (DCM) for implantable cardioverter defibrillators (ICD) needs to be improved. Registries demonstrate that the major burden of sudden cardiac death (SCD) occurs in those with a left ventricular ejection fraction (LVEF) >35%. Those at high-risk of SCD wi...

Background: Current guidelines only recommend the use of an implantable cardioverter defibrillator in patients with dilated cardiomyopathy for the primary prevention of sudden cardiac death (SCD) in those with a left ventricular ejection fraction (LVEF) <35%. However, registries of out-of-hospital cardiac arrests demonstrate that 70% to 80% of suc...
