Introduction
Out-of-hospital cardiac arrest (OHCA) has a high global incidence and mortality rate, with early defibrillation significantly improving survival. Our aim was to assess the feasibility of autonomous drone delivery of automated external defibrillators (AED) in a non-urban area with physical barriers and compare the time to defibrillate (...
Introduction: High altitude regions are characterized by harsh conditions (environmental, rough terrain, natural hazards, and limited hygiene and health care), which all may contribute to the risk of accidents/emergencies when trekking or climbing. Exposure to hypoxia, cold, wind, and solar radiation are typical features of the high altitude enviro...
Introduction: Response to medical incidents in mountainous areas is delayed due to the remote and challenging terrain. Drones could assist in a quicker search for patients and can facilitate earlier treatment through delivery of medical equipment. We aim to assess the effects of drone deployment in search and rescue (SAR) operations in challenging...
Background
Helicopter emergency medical services personnel operating in mountainous terrain are frequently exposed to rapid ascents and provide cardiopulmonary resuscitation (CPR) in the field. The aim of the present trial was to investigate the quality of chest compression only (CCO)‐CPR after acute exposure to altitude under repeatable and standa...
Hypothermia in trauma patients is a common condition. It is aggravated by traumatic hemorrhage, which leads to hypovolemic shock. This hypovolemic shock results in a lethal triad of hypothermia, coagulopathy, and acidosis, leading to ongoing bleeding. Additionally, hypothermia in trauma patients can deepen through environmental exposure on the scen...
Dear Editor,
With great interest we read the correspondence related to our research demonstrating a reduced time to defibrillate (TTD) in out-of-hospital cardiac arrest (OHCA) in remote non-urban areas. (1) In our prospective simulation trial, an automated external defibrillator (AED) delivered by drone led to a significantly reduced TTD versus p...
The Lake Louise Score (LLS) is a common tool for diagnosing acute mountain sickness (AMS) after a recent gain in altitude. Required symptoms (headache, dizziness, fatigue or gastrointestinal symptoms) are unspecific, subjective and not detectable by physiological parameters. (Talks et al., 2022; Boos et al., 2018) This study aimed to enhance presen...
Accurate assessment of vital parameters is essential for diagnosis and triage of critically ill patients, but not always feasible in out-of-hospital settings due to the lack of suitable devices. We performed an extensive validation of a novel prototype in-ear device, which was proposed for the non-invasive, combined measurement of core body tempera...
Importance
Emergency medical services (EMS) providers transiently ascend to high altitude for primary missions and secondary transports in mountainous areas in helicopters that are unpressurised and do not have facilities for oxygen supplementation. The decrease in cerebral oxygen saturation can lead to impairment in attention and reaction time as...
We read with interest the recent study by Niederer etal. about the influence of altitude (3454 m) on physicalexhaustion during cardiopulmonary resuscitation (CPR)[1]. The authors analyze a secondary objective of a previ-ous study that showed a reduction of chest compression(CC) depth at altitude [ 2] and show that heart rates aresignificantly highe...
The use of geospatial infrastructure supported by novel sensors, autonomous and piloted aerial platforms, and computational tools are impacting civil applications such as risk management, particularly interesting in mountain rescue operations. Here, we propose a semi-automatic geospatial system able to identify the coordinates of a distress call, m...
To the editor, We read with interest great interest the commentary by ter Avest et al. We applaud their proposition to promote a green Helicopter Emergency Medical Services (HEMS) approach [1]. The use of helicopters and HEMS missions is widespread in mountain and remote areas where mission times are longer than in urban areas [2] Therefore, some a...
van Veelen, Michiel J., Giulia Roveri, Ivo B. Regli, Tomas Dal Cappello, Anna Vögele, Michela Masè, Marika Falla, and Giacomo Strapazzon. Personal protective equipment protocols lead to a delayed initiation of patient assessment in mountain rescue operations. High Alt Med Biol 00:000-000, 2023. Introduction: Mountain rescue operations can be challe...
Objective
To evaluate, under replicable, blinded and standardised conditions, the effect of acute exposure to hypobaric hypoxia (HH) (equivalent to 200 or 3000 or 5000 m above sea level (asl)) on selected cognitive domains and physiological parameters in personnel of helicopter emergency medical service (HEMS).
Methods
We conducted a randomized cl...
Introduction
Space travel is expected to grow in the near future, which could lead to a higher burden of sudden cardiac arrest (SCA) in astronauts. Current methods to perform cardiopulmonary resuscitation in microgravity perform below earth-based standards in terms of depth achieved and the ability to sustain chest compressions (CC). We hypothesise...
Mitigating the spread of COVID-19, an airborneinfection, can lead to delays in the prehospital response and impair the performance of search and rescue (SAR)services in mountainous and remote areas. We provide an overview of the developing epidemiological situationrelated to the COVID-19 pandemic in mountainous areas and review current protocols to...
Tourism in mountain regions has increased during the last decades. Tourists are attracted to the Dolomites, a mountain range located in northeastern Italy, mainly for outdoor activities. The GEOPARC Bletterbach, is one of the Dolomites UNESCO parks receives 60.000 tourist visits every year, that are attracted by the geological formations
and spect...
Cardiopulmonary resuscitation (CPR) is considered an aerosol-generating procedure. Consequently, COVID-19 resuscitation guidelines recommend the use of personal protective equipment (PPE) during resuscitation. In this simulation of randomised crossover trials, we investigated the influence of PPE on the quality of chest compressions (CCs). Thirty-f...
Scquizzato and colleagues have recently described the fear of contracting SARS-CoV-2 as an obstacle to perform bystander cardiopulmonary resuscitation (CPR).1 A case similar to the one reported has also occurred in our country.2 The authors describe the current ERC guidelines that advise continuous chest compressions (CCC)-CPR and early defibrillat...
Background
Heatwaves have been linked to increased risk of mortality and morbidity and are projected to increase in frequency and intensity due to climate change. The current study uses emergency department (ED) data from Australia, Botswana, Netherlands, Pakistan, and the United States of America to evaluate the impact of heatwaves on ED attendanc...

Many acutely ill or injured adults and children in Africa seek care every day. Frontline providers manage patients with acute problems like injuries, infections, stroke, asthma and complications of pregnancy. These acute presentations unfortunately contribute to a high mortality and morbidity. Healthcare provision on the continent has historically...
The association between heat and ED demand differs between regions. A limitation of the
current study is that it does not consider delayed effects or influences of other environmental factors. Given the association between heat and ED use, hospitals and governmental authorities should recognise the demands that heat can place on local health care s...
BACKGROUND:
In 2013 a General Practitioner Cooperative (GPC) was introduced at the Emergency Department (ED) of our hospital. One of the aims of this co-located GPC was to improve throughput of the remaining patients at the ED. To determine the change in patient flow, we assessed the number of self-referrals, redirection of self-referrals to the GP...
In 2013 a General Practitioner Cooperative (GPC) was introduced at the Emergency Department (ED) of our hospital with the aim of relieving workload. To determine the effect we measured the amount of self-referrals, redirection of self-referrals to the GPC and back to the ED as well as characteristics of the remaining ED population regarding level o...
In 2012 we introduced the AWLS course in The Netherlands. Our results showed that a diverse group of medical specialists attended the courses and gave high feedback scores ( 4.32/5). We conclude therefore that the AWLS is a useful addition to the acute care courses line up.
Case
A 20-year-old woman was referred to the emergency department with rapid acceleration of complaints of palpitations, fever, diarrhea, and agitation that had been present for several weeks. During physical examination, the patient was uncomfortable and restless with a tachycardia of 170/minute, and a fever of 38.5 °C. Palpation of the neck revea...
A 51-year-old African male presented to the emergency department with a sore throat, a productive cough and a striking pinched voice. Investigation revealed severe hypocalcaemia (0.97 mmol/l) with a prolonged QT interval on ECG. The authors administered intravenous calcium in the emergency department, which instantly improved the quality of the pat...
