L’osservazione clinica dell’esistenza di un certo numero di pazienti deceduti per tumore polmonare nonostante avessero smesso di fumare molti anni prima costituisce una fascia “grigia” di pazienti morti per una patologia, quella tumorale, apparentemente non causale rispetto al tabagismo pregresso. Ciò rappresenta pertanto un interessante spunto di ricerca sul perché ciò succeda. Nell’ottica speculativa di capire perché questi ex-fumatori, che hanno smesso anche da molti anni in assenza di un insulto sicuramente cancerogeno come il fumo, e al netto di altri fattori di rischio come amianto e radon, possano poi sviluppare tumore polmonare, è nato questostudio di ricerca “Polonio in vivo”. I due capisaldi etiologici e fisiopatologici su cui poggia questo studio sono: a. la BPCO è un fattore di rischio indipendente per lo sviluppo di tumore polmonare ed è associato con il rischio di tumore polmonare da due a sei volte rispetto ai fumatori senza BPCO. La riduzione del FEV1 è associata ad un aumento del rischio di incidenza di tumore polmonare anche indipendentemente dalla storia di fumo. Circa il 50% dei pazienti con neoplasia polmonare presenta BPCO, in linea con vari studi che mostrano che il tumore polmonare è una delle principali cause di morte in pazienti con BPCO [42]. Questo rischio sembra più elevato in pazienti con media- moderata BPCO che in quelli con più severa malattia. Inoltre, il rischio solitamente persiste per almeno 20 anni dopo la cessazione del fumo. b. Il rilievo della presenza di Polonio-210 (Po-210), emivita 138,38 giorni, e del suo precursore il Piombo-210 (Pb- 210) che ha un’emivita 22 anni, nel fumo di tabacco. La combinazione di questi due elementi (BPCO & Po- 210/Pb-210) potrebbe rappresentare l’anello mancante per spiegare l’incidenza di tumori polmonari in ex forti fumatori che, a causa di un sempre più progressivo rallentamento della Clearance Muco-Ciliare tendono ad un accumulo di carica radioattiva. Obiettivi (End Points) dello studio: a. Valutare la presenza di radioattività da Po210 nell’apparato broncopolmonare di soggetti fumatori, ex fumatori e mai fumatori con diagnosi di tumore polmonare (End Point Primario). b. Valutare in tali pazienti presenza e grado di BPCO, secondo le Linee Guida GOLD (1° End Point Secondario). c. Determinare in tutti i pazienti il tasso di radioattività nelle urine (2° End Point Secondario). d. valutare lo status dell’oncosoppressore p16 (INK4a) (3° End Point Secondario).
