In a journey that begins with the birth of Anatomy in the Alexandria of the third century BC, and covers up to the first half of the twentieth century, we shall explore the evolution of the Morphological Sciences and the most relevant discoveries made in the urinary system and the male genital tract. We will analyse the progression and see how anat...

Introduction & Objectives At the turn of the 20th century, male prostate growth and prostatic diseases were recognized as a significant problem, and studied by clinicians, anatomists and embryologists, who recorded changes during intrauterine development, and in the next stages of life. Then, a body of knowledge, hypothesis on growth causes, and to...

We present a historic video of a whole organ transplant of the testicle in man with the full surgical technique performed by Prof. JM Gil-Vernet in 1978. A cadaveric testis was transplanted but was rejected three weeks later. Now, the great advances in immunosuppressant agents will deliver improved results in selected cases.

Salvador Gil Vernet (1892-1987) was aware that only a urologist with solid anatomical principles would be able to ensure effective, minimally invasive surgery, which he described as: “elevating surgical technique to the level of true science.” As a result of this great effort, he explained with high precision the architecture of the vesico-prostato...

A detailed topographic anatomical study of the male EUS has been presented. The male EUS was easily detectable by TRUS as a hypoechoic circular shaped structure surrounding the membranous urethra, which remained unchanged before and after the EEP procedure. The patient was discharged 24 hours postoperation without complications. The bladder cathete...

One might think that, now that we are in the twenty-first century, urogenital anatomy and, specifically, the anatomy of the perineum would have been thoroughly characterized, leaving no room for doubts or contradictions. If we look back to the sixteenth century, we can see that most surgery was performed through the perineum and that a multitude...

To assess the incidence of genitourinary infections associated with transrectal ultrasound-guided prostate biopsy (TRUS-BX) using endorectal povidone-iodine gel as a bactericidal agent. We prospectively studied a total of 530 patients who were given 30g of 10% povidone-iodine intrarectally before TRUS-BX. Each patient received antibiotic prophylaxi...

The male perineal topographic and microscopic anatomy is presented with an insight into the errors that have been historically perpetuated, for example, the myth of the urogenital diaphragm and the existence of the musculus transversus perinei superficialis and musculus transversus perinei profundus, muscles that do not exist really . Also, we expl...

OBJECTIVES METHODS First reports on intrarectal iodine-povidone solution antisepsis were published in 1980 and 1984 with good results in small group of patients. Rectal lavage with 10% iodine povidone solution guaze before prostate biopsy in 31 patients, without antibiotic prophylaxis, yielded a 17% bacteriemia incidence compared with 76% in a cont...

We describe a new type of perineum based scrotal flap with biaxial vascularization supplied by both superficial perineal arteries. Flap length of up to 20 cm. may be attained for urethral reconstruction. A total of 37 men with complex urethral stenosis of different etiologies underwent surgery using 1 of 3 urethroplasty techniques based on this new...

To describe a simple method of depilation of scrotal skin and the advantages of utilizing scrotal skin flaps for urethroplasty in the postpubertal male. Depilation of the scrotal flap is achieved by electrocoagulating the dermal papillae with a needle specifically designed for scrotal hair. The frequency and the number of depilations are determined...

Today, as in the past, there are two therapeutic strategies for CaP; one aims to cure the localized process, the other to control the disseminated disease. The therapeutic option for curing the localized disease is still radical prostatectomy, although radiotherapy is also used in some patients with localized CaP.

The urogenital diaphragm raising maneuver, a procedure to facilitate membranous urethral surgery, is described. The technique shortens operating time by permitting easy identification and exposure of the severed urethral ends, thus, allowing for precise re-establishment of urinary tract continuity. Selective membranous urethral surgery can be perfo...
