Question
I'm writing a systematic review and been asked by the editor to furnish a graphical representation of the results. Studies considered show associations of two ordinal parameters with overall survivals through KM analysis in various studies, but it is not a meta-analysis (so I did not consider forest or funnel plots). Could you help?
Question
I have a group of postoperative oncologic patients at 5 years follow-up which I divided into four subgroups according to the alive status (dead or alive) and recurrence (disease-free, recurrent). What are the subgroups when calculating the disease-free survival at 5 years of follow-up? Any theoretical literature background on how the calculation is performed is appreciated. Thanks.
Question
The ROC curves measures the accuracy of a diagnostic test for the entire range of values and allows the identification of the cut-off that best discriminate between false positives and true positives. This statistical tool provides numerous sensitivities and specificites for each cut-off value and the researcher decides which one is ideal through a balance of the two. Different from other fields of medicine in oncologic research greater sensitivities are preferable at the expense of lower specificities due to the nature of the diseases treated. Does anybody has experience on what are the acceptable sensitivity values in oncology and especially can you provide a useful reference to cite?
Question
H-index is now overcoming the total impact factor for assessing the quality of a researcher. I think adding the h-index is not difficult from google scholar or other softwares on the internet and could an additional info about the researcher and its citation metrics.
Question
I wrote two articles (a meta-analysis and a systematic review) on two different topics regarding the surgical technique of open appendicectomy. I'm finding it difficult to publish them because the Editors of various journals claim that nowadays open appendicectomy is a procedure almost abandoned in "developed" countries which favour laparoscopic appendicectomy. What do you think about this statement? Can you also suggest any journal that could possibly be interested?
Question
Do you think the presence of drains influences the occurrence of wound infections, intrabdominal abscesses or postoperative ileus? Or you just wash abundantly after the appendicectomy and leave no drains?
Question
I had a case of testicular pain and enlarged testicle presenting as an emergency that required scrotal exploration and orchidectomy for the suspicion of testicular torsion. Intraoperative appearance was of a spermatic cord lesion encroaching the vessels with normal testicle and epididymis. Final histology showed multiple large vessels set in inflamed fibrous tissue.
Question
Numerous studies and one recent meta-analysis seem to favour the elective neurectomy during open inguinal repair at the expense of an increased numbness of the area. What is the current practice of the audience regarding the three nerves in terms of preservation or neurectomy (ilioinguinal, iliohypogastric and genital branches of genitofemoral)? If division is performed, when (always or only pragmatically if the nerves are in the operative field) and how (simply cut, ligation, diathermy)? Where is performed (how proximal / distal to internal inguinal ring)? What are the outcomes in terms of inguinodynia and orchalgia? Thanks.
