What would be best primary survey questionnaire for measuring socio-economic impact of self help groups?

I got a question when estimating sample size. After getting the theoretical sample size, it needs to consider the response rate and invalid response rate when collecting data in the study based on an online survey. So, how to determine the invalid response rate before collecting data?

What software would you recommend to design a self-completion postal questionnaire? It's nothing too technical, series of demographic variables to begin with and then a number of items using likert scales. This is not for an online survey, so really only concerned with something looking good when printed out. I have done this in WORD but feel it could look a lot better if I tried something else?
