The authors’ aim is to provide breast surgeons’ perspectives on the relative importance of the criteria for choosing an appropriate surgery method between mastectomy (total removal of a breast) and breast-conserving surgery (removal of a breast cancer tumor and some of the normal tissue that surrounds it) for women with breast cancer by using the m...

... and BCS are evaluated for the surgical treatment of breast cancer based on three main criteria "tumor-related, patient-related, and post-operative process-related factors" and nineteen sub-criteria "tumor/ breast ratio, tumor localization, presence of tumor multifocality and/or multicentricity, age of the patient, breast size, skin type, breast density, ethnicity, body mass index (BMI), presence of collagen tissue diseases, presence of additional diseases (hypertension, diabetes), radiotherapy history, presence of breast asymmetry, radiotherapy acceptance level, demand for esthetic appearance, quality-of-life (QoL) level, recurrence risk level, reoperation risk level." The proposed hierarchy model is illustrated in Figure 1. ...

... 8 The choice of surgical modality is influenced not only by the disease itself but also by social and economic factors, the patient's knowledge of the disease, and medical technology. 9 As a result of ongoing clinical research and technological advancements, endoscopic surgery techniques have developed rapidly. 10 Endoscopic surgery has a number of advantages, including being less invasive, hidden incisions, and allowing rapid recovery. ...
Objective
Endoscopic surgery is an effective technique for preserving the nipple and areola, as well as for sentinel lymph node biopsy and breast implant reconstruction. However, the technical challenges associated with endoscopic surgery have limited its widespread adoption.
Methods
In the normal single-port endoscopic surgery, the ultrasonic knife was accessed through the retractor. In our modified procedure, a tiny 5 mm incision was made at the lateral margin underneath the breast, serving as the second entry port for the ultrasonic scalpel, which was referred to as the “Haigui-1 hole”. Preoperative and postoperative indicators such as blood loss, operative time, and postoperative drainage volume were collected. Differences between parameters were compared using Student’s t test.
Results
Endoscopic surgery with the assistance of the “Haigui-1 hole” led to preserved breast aesthetics with minimal scarring. Moreover, “Haigui-1 hole” surgery significantly reduced the operation time, intraoperative bleeding, and postoperative drainage volume compared to normal single-port endoscopic surgery.
Conclusion
The “Haigui-1 hole” procedure, which involves the addition of a second entrance to improve the maneuverability of the ultrasonic knife, is worthy of further promotion.
... However, in the study of Al-Gaithy the positive HER2 and triple-negative receptor status were also significantly correlated with mastectomy. Moreover, Koksalmis et al. 35 showed that the most important factors affecting the surgeons' choice were post-operative process-related factors, especially the demand for esthetic appearance and conversely, the least significant criteria were tumor-related factors. According to the result of a nationwide study done by Machuca et al. 36 number of affected lymph nodes, tumor size, and location were determinants of BCT, while the histological type was not significant. ...
Background: Survival after breast conserving surgery (BCS) vs. modified radical mastectomy (MRM) is a controversial issue. In this study, we want to compare the disease-free survival (DFS) of women who underwent BCS with those treated by MRM. Method: In this historical cohort study, a total of 1097 women who were diagnosed with breast cancer between 2001 and 2007 and received modified MRM or BCS were entered into the study and followed up to March 2017. Kaplan-Meier estimator and extended cox model, and Cox proportional hazards model with propensity score weighting were implemented to compare overall survival between two groups. Results: A total of 283 women with a maximum follow-up of 11.1 years and age 47.17 ± 11.278 were met the inclusion criteria. The results of the extended cox model did not show any difference between the survival of two groups (P = 0.35). After implementing the Cox model with propensity score weighting, the inferences remained unchanged (P = 0.67). Conclusion: The patients treated with BCS tend to have the same DFS rate as those who underwent a mastectomy in a randomized controlled trial-like setting using propensity score weighting.
... The TOPSIS and VIKOR methods were used to choose the best surgical option between mastectomy (complete removal of the breast) and breast-conserving surgery (removal of for breast cancer tumor and some normal surgery). The model considered 19 sub-criteria related to tumor-related, patient-related, and postoperative course [50]. The ordinal relationship analysis method and TOPSIS were combined to rank four drug regimens and determine the best drug regimen for patients with chronic cancer to avoid possible side effects from increased doses or potent drug use [51]. ...
The “Song of Life (SOL)” is a kind of music therapy in palliative care for addressing emotional and existential needs in terminally ill patients nearing the end of life. Few previous studies focus on objective data analysis methods to validate the effectiveness of psychotherapy therapy for patients’ overall state. This article combines the entropy weighting method (EWM) and the technique for order preference by similarity to the ideal solution (TOPSIS) method to evaluate the effectiveness of SOL music therapy and the treatment satisfaction of the patients and family members. Firstly, the collaborative filtering algorithm (CFA) machine learning algorithm is used to predict the missing ratings a patient might have given to a variable. Secondly, the EWM determines the weights of quality of life, spiritual well-being, ego-integrity, overall quality of life, and momentary distress. Thirdly, the EWM method is applied for the TOPSIS evaluation model to evaluate the patient’s state pre- and post-intervention. Finally, we obtain the state change in patients and recognition based on the feedback questionnaire. The multiple criteria decision making (MCDM) comprehensive evaluation method objectively validated the overall effectiveness of SOL music therapy. Based on MCDM method, we provide a new approach for judging the overall effect of psychological intervention and accurately recommend psychotherapy that fits the symptoms of psychological disorders.
