#NephMadness is an annual evidence-based, non-commercial, medical education game hosted by the American Journal of Kidney Disease blog. It uses the tools and interactivity of SoMe and the structure of a competitive game to teach the latest advances and the most important breakthroughs in nephrology. Players have open access to 32 blog posts written about a specific, topical area in nephrology by experts in the field. These address controversies and developments from eight different topic areas and players complete their 'brackets', selecting hierarchically which topics are the most important when pitted against each other. A panel of experts then determines the actual winners and losers of each round sequentially until an ultimate winner is declared. The results spark conversation and discussion that is played out on SoMe (particularly Twitter), which deepens and cements learning. Background As the organ-shortage crisis continues to worsen, many patients in need of a kidney transplant have turned to social media to find a living donor. The effect of social media on living kidney donation is not known. The goal of this study is to investigate the influence of social media on those interested in donating a kidney. Methods Self-referrals for living kidney donation from December 2016 to March 2019 were retrospectively reviewed. Age, sex, race, and relationship of individuals petitioned through social media (SM) were compared with those petitioned through verbal communication (VC). Data were analyzed using chi-squared tests, with z tests of column proportions, and multivariable logistic regression. Results A total of 7817 individuals (53% SM, 36% VC, and 10% other) were self-referred for living kidney donation. The analysis sample included 6737 adults petitioned through SM ( n =3999) or VC ( n =2738). Half ( n =3933) of the individuals reported an altruistic relationship, and 94% of these respondents were petitioned through SM. Although univariate analyses indicated that SM respondents were younger, more likely female, more likely White, and more likely to have directed altruistic intent than those petitioned through VC (all P <0.05), multivariable logistic regression demonstrated that only decreased age, female sex, and relationship were significantly related to likelihood of SM use (all P <0.001). Conclusions The use of SM to petition living kidney donors is prevalent and accounts for a greater proportion of respondents compared with VC. SM respondents tend to be younger, female, and altruistic compared with VC. Directed altruistic interest in kidney donation is almost exclusively generated through SM.

Journal clubs have existed since William Osler started them in the 19th century, and serve as an important avenue for critical appraisal of medical research in academia. In the past decade, particularly the past 5 years, there has been a proliferation of online journal clubs on Twitter (Twitter Inc, San Francisco, CA). These Twitter-based journal clubs have used the conversational nature of the medium and the focus allowed by specifying a hashtag to bring critical appraisal to the wider community. Currently, there are 27 active Twitter-based journal clubs. NephJC is one such online, Twitter-based, nephrology-focused journal club. NephJC has been active since April 2014. We describe elements of NephJC and the participation over the past 135 sessions. Apart from critical appraisal, these journal clubs allow for archived and searchable postpublication peer-review, and are a crucial element of building an online community. They are one of the myriad ways in which social media is changing medicine and medical education.

Medical online newsletters are an important component of communication between learners and educators. The ease of creation and distribution has broadened the appeal and growth of online formats. This manuscript details the history of the modern newsletter as it relates to the nephrology community. From historical listservs to content-rich emails and blogs, the creative newsletter formats are changing to meet the demands of readers. With continued expansion of online education and access, this communication format can continue to meet the needs of virtually focused world.
