We have applied a therapy to patients suffering different disorders (Herrera-Rivero M, Aranda-Abreu GE. Therapeutics of Alzheimer's disease. Advanced Understanding of Neurodegenerative Diseases. InTech. 2011) and this has been able to considerably improve behavioural alterations in most patients, it is based in environmental enrichment, patient's sleep and nutrition and a combination of supplements; originally designed for neurodegenerative disorders, some people decided to follow it for their patients with stroke, vascular dementia, psychosis and others...Drug treatment should be the last option, when nothing else is giving any satisfactory results or the behavioural alterations are too severe.

At the present, for indoor environments provided with HVAC systems the only reliable method you can use is the PMV index proposed by Fanger in 70's and actually forming the base of ISO 7730 Standard. This index allows the assessment of comfort conditions indoor. In case of heat or cold stress you can use WBGT index, PHS model and, finally IREQ index (cold). . Concerning the assessment of comfort condition indoors you can take into account the adaptive approach by ASHRAE 55 Standard. I am hoping to find a good reference that argues for an adequate retention rate in psychotherapy controlled trials. I am completing a meta-analysis and want to provide justification for a cut-off. The cut-off will be used to help determine study quality (e.g., studies with retention rates of at least 70% receive a point toward their quality score). Also, I am interested in standard quality assessment forms that might be used in these cases (i.e., to rate studies for reviews and MAs).

If you are in a searcher posture, you have to consider at first that possession is a belief, not an objective reality! The belief in possession is determined by individual, social and cultural factors. Hallucinations may be the result either of a psychopathological situation or/and believes of the belonging group of the subject; in some micro-groups, the belief in possession works as a social norm for their members because all of them share the same belief. But you can't make an objective, rational relationship between a supposed action of a supernatural entity and the psychiatric symptoms

I suscribe with many of the ideas posed by Jeremy. The idea that the assessment process and actual intervention are two distinct moments is illusory, as our mere presence and questions can provoke profund changes. I also suscribe the idea that the therapeutic process is an open ended process of exploration into the patient's inner world, so the assessment should be about how much do I need to know (enough and sufficient) to start working with the patient. I also agreed with the idea that assessment should be to see if we can help and in which way, and not about if the patient suits our particular technique or needs. In my case it can take 2 session (I try to have at least two separate encounters with a patient) or more. I don't pre-establish a duration, but it shouldn't go on forever. If I'm not concluding, I try to understand why it's happening, by myself or with the aid of others (supervision, peers). I think we all do assessment, there may be differences in terms of the framework (diagnose, inner dynamic, relational, problem focus, experience focus, etc), but at the start of our work we all try to understand what's the problem before actually deciding to do something. I also think, as András mentions, that considering transference - countertransference dynamic is essential part of psychotherapeutic work, but I would differentiate between observing and managing transference - countertransference and overtly exploring and interpreting it. Some type of patients react with a lot of anxiety when confronted with some aspects of reality, e.g. the emotional bond with others, that they have been denying or warding off, so confronting them too soon is actually like showing them the door out.

"Measuring effectiveness of eye movement desensitization and reprocessing (EMDR) in non-clinical anxiety: A multi-subject, yoked-control design." Journal of Behavior Therapy and Experimental Psychiatry 27(3): 231-239. Abstract: Twenty-eight subjects from a university's subject pool were paired on sex, age, severity, and type of stressful or traumatic incident. One subject in each pair was selected to receive EMDR; the experimental partner spent the same amount of time receiving a visual (non-movement) placebo. Subjective units of discomfort (SUD) scores and physiological measurements were taken prior to and following treatment. Analysis of physiological measurements and self-reported levels of stress were performed within and between each group. While the EMDR group showed significant reductions of stress, EMDR was no better than a placebo. This suggests EMDR's specific intervention involving eye movement may not be a necessary component of the treatment protocol.

This was the approach I used for my dissertation and it produced a theoretical outcome that was truly grounded in the research. Arthur Mary, I agree that countertransference is involved in the psychiatrist -patient interaction, or lack there of. From a relational perspective countertransference is necessarily involved, more or less, so that we could say transference is an ongoing process of mutual and reciprocal influence. Generally, in the case of the biological psychiatrist the countertransference would be based on the mind set of the "illness" as resulting in meaningless symptoms and the patient as not really a centre of experience. I also believe that the personality of the psychiatrist may involve a tendency to rely on control and doing to rather than with the patient. Regarding intersubjectivity as the theory, I would note that it is a psychoanalytic theory but the emphasis now is on the expansion of consciousness, drawing from the past present and fantasy of the future. Rather than traditional insight as the emphasis, I understand relational models as considering the self as part of a fluid process influenced by the past and present context necessarily. Stolorow talks about expanding one's experiential horizons by developing new organizing principles (rather than fixed self and object configurations) through the relational experience itself with the therapist and awareness of these principles and the associated affects. He views the unconscious as three realms including the traditional dynamic unconscious, the prereflective unconscious(organizing principles) and the unvalidated unconscious( or the experiences that were never validated but not dissociated actively from consciousness.

Thanks Melissa. I think the SRS represents one good way at looking at alliance. There are some merits in being upfront and interactive but I also like the option we have of allowing the client to make ratings away from the scrutiny of the therapist. This means that the conversation about alliance (and other factors) may be delayed until the following session but may have advantages in capturing the whole session whereas the SRS system as I understand it requires a further intervention that is not then rated. It is in the nature of these discussions that we focus on on one facet at a time of something that is multifaceted so, for example, it is a problem when something about the therapy leads to disengagement but less effective therapy may lead to continuing engagement but without good results for the client. I think that alliance is important but not everything. There can be a very good alliance and poor outcome so we need to go wider than alliance (as indeed the ORS/SRS system does). I am incidentally on the heroicagencies weblist so have some familiarities with the issues around ORS and SRS.
