It seems to me that this TM might perhaps be better designated, "Typus Saturnus", given its saturnine character. Melancholia, per se, does not seem to be its chief feature, as one would see in, for example, a "Depressive Personality Disorder". TM appears to have qualities of exactitude and obsessiveness that one does not expect to see in a DPD, for example. Does not "Obsessive-Compulsive Personality Disorder" better befit individuals with this kind of personality presentation? And does not this presentation conform to the basic pre-requisites of "personality disorder" in general? Certainly, there are many with personality disorders who seem to thrive in one context or another, and at times, may find their personalities more or less accepted in a given environment. For me, that does not deny some level of stand-alone disorder that creates propensity for mood and anxiety disorders, for example, in the way many PDs do. I must admit that it is quite possible that I, even as one without medical training, but psychological training instead, could be over-pathologizing a constellation of personality traits. Thoughts?

Do not, repeat not writeyet another interview. There are standard semi structured interviews. All interviews require training and certification of interviewers. Use either the MINI, the SCAN modules or SCID-IV modules if you want a semi structured interview. These interviews are long. If you have nothing wrong with you, a SCID will take 20 -30 minutes. If you have problems, budget hours. The SCID has been translated into a series of languages. I'm less sure about the SCAN and SCID. Thanks Laura, I know Ann Blake Tracey and SSRI stories well. Those of us who have lost normal, healthy children to these drugs are building a global network to warn other families and hold big pharma, psychiatry and government regulators accountable for their marketing of drugs which have nil or negligible benefit above placebo but double the risk of suicide in those who use them. In the memory of our children, we are committed to ensuring others are not harmed as they were.

This is an interesting debate. Perhaps try Stephen Frosh's 'Politics of Psychoanalysis' for critical discussion about psychoanalytic theory and practice. The clinical literature I have read so far tends to overwhelmingly support psychoanalytic intervention as the most effective way of tackling problems at their root. I think there's not a huge difference between psychoanalysis and psychoanalytic psychotherapy. The latter is usually less frequent and, perhaps, more flexible but both try, amongst other things, to alleviate symptoms and distress by bringing unconcious conflicts to consciousness. Families may find it difficult if their loved one alters previously adhered to narratives about themselves and others but they may find their loved one growing in self esteem and self awareness - I guess to a certain extent it depends on the levels of dysfunctionality in the family as to how they experience the changes they perceive in their family member!

Neurosis once upon a time was a DSM disease, illness etç..Maybe into the future we all will be "cured" or "insane". The " neuroscience" has its own psychoanalysis and does not its neuropsychiatry. Astonishing??

i found very intresting the possible relation between Kepra (levetiracetam) and depression. So i tried to find information about Kepra and looking to the wikipedia i noticed that: Some serious side effects can be depression, hallucinations, suicidal thoughts. A study published in 2005 suggests that the addition of pyridoxine (vitamin B6) may curtail some of the psychiatric symptoms ("Clinical Epilepsy: Pediatrics". It's a real problem because (as to my mind) it’s the only way to find axial phenomenon (in Jasper’s phenomenology understanding) from which all other symptoms appear and thus to create natural taxonomy of mental disorders instead of today's systematization with comorbid clusters and without dynamic features.

In my experience, having worked mostly with people with severe and enduring symptoms of schizophrenia, often including 'lack of insight' the biggest cause of relapse is 'non compliance' with medication or, to be more PC 'non concordance'. Sometimes, also, relapse results from doctors trying to reduce medication to minimise side effects or even themselves stopping a patient's medication because they seem to have been well for a long time. Most of the people I have been working with no longer live in the family home and have little or no contact with family so 'high expressed emotion' is not an issue with this group. It would be interesting to research relapse in those people who are definitely still on regular medication. The only group for whom one can be sure this is the case is those on regular depot medication. But relapse is much rarer in this group.
