However, "urgently" clinical means that there's a real person (N of 1) in disabling pain who can't go back to work so they can earn enough to pay for the lifestyle they'd like to continue so they can go back to their pursuing their career, family, or friends, I propose we ask those providing posts to tell us right up front if their issue is academic or "urgently" clinical, then each one of us can decide if we hang around and debate how many angels can fit on the head of a pin, or come up with an answer that has reliability and validity to support it, and for which there is 95% confidence of getting the Pt back to work and their lifestyle. For the "urgently" clinical issues, I propose functionality (operant behavior that is reinforcing to the N = 1 Pt (and let's call him/her by their first name) and monitor the measure that cuts to the chase - return to work, or in the Marines, "return to full-duty status, in a military rating, without any further need for medical care or medication." In short, all other academic measures aside, treat the pain with the medication(s) that CYP assays and prior response history to the medication have a high likelihood of relieving the pain enough to increase the frequency of relevant (to the Pt not you) functionality, then add a dose of lifestyle self-management skills and turn the Pt loose in their own natural environment. The reinforcing effects of finally not having to act like a "disabled person" 24/7/365 for an average of 7.5 yrs and getting back to earning their living and their family, friends and co-workers shows the highest correlation levels you've ever seen in chronic pain cases. I'll be glad to discuss the experimental set-up to measure this relevantly at the N=1 rather than the population level of all the RCTs.

For instance, what is the UP/Down technique, do you really need monofilament or can string or thread be used, and what is the purpose of the test in language that is not half latin?

I just dont remember what the active ingredient was that was isolated out of the toxin. In remote areas in the Philippines where I come from we use the bark of a banana tree trunk. It immobilizes a fracture and has a cooling effect that works like ice or analgesia.

The literature is abundant as a CV risk factor. Outside of that I think I may be one of the few clinicians using HS-CRP this way. I see the issue as an empirical issue for my clinical work... it guides treatment and the outcomes correlate well with statements by patients of improved quality and quantity of life, decreased VAS levels, and other not so solid measures.
