Encapsulated Papillary Thyroid Carcinoma, Follicular Variant. - Is it a subtype of Papillary Carcinoma? - A non-invasive encapsulated follicular lesions (EnFPLs) of the thyroid showing papillaryEncapsulated Papillary Thyroid Carcinoma, Follicular Variant. - Is it a subtype of Papillary Carcinoma? - A non-invasive encapsulated follicular lesions (EnFPLs) of the thyroid showing papillary carcinoma (PTC) type nuclear features(PTC-N) are classed to malignant (encapsulated follicular variant papillary thyroid carcinoma: EnFVPTC) in current classification scheme, and a total thyroidectomy with neck dissection followed by radioiodine treatment is applied to the patients in most countries. We propose that EnFVPTC and other EnFPLs with equivocal PTC-N should be classified into a borderline malignancy based on their homogeneous excellent outcome (1, 2, 3, 4, 5). Zhu et al proved that follicular variant PTC is characterized by a high prevalence of RAS mutations, a low prevalence of RET/PTC-1 rearrangement and absence of follicular carcinoma-specific PAX8-PPARγ rearrangement (6). This observation strongly suggests that follicular variant PTC is not a subtype of PTC (1, 6). In conclusion, a malignant diagnosis of EnFVPTC should not be used until uncertainty about the nature of this lesion is settled, whether it is benign, precancerous or malignant.
