• [Saliva as an alternative diagnostic material in determination of hormone--advantages and limitations].

  Saliva can be an excellent alternative diagnostic material in determination of hormone concentration substituting the routinely used serum, plasma and urine. Particularly useful is the measurement ofSaliva can be an excellent alternative diagnostic material in determination of hormone concentration substituting the routinely used serum, plasma and urine. Particularly useful is the measurement of nonconjugated steroid hormones and cyclic amines (melatonin), the concentration of which is not dependent on saliva production, and correlates well with hormone concentration in blood. In routine laboratory diagnostic saliva is mainly used for multiple measurement of hormone concentration (establishing of diurnal cycle, monitoring the function of endocrine system with dynamic tests ex. Synacthen and dexamethasone), monitoring the concentration and metabolism of hormones used as a drugs (hormone replacement therapy) or when the determination of free (bioactive) fraction of hormone is required. Noninvasive saliva collection at patient's home, which does not need medical staff assistance, may reduce the costs of hormonal diagnostics in specialty clinics and hospitals.

  Introduction: Increased bone turnover markers (BTM) level is consider as independent risk factor of bone fracture. However, it was not used in 10-year probability of bone fracture method (FRAX)Introduction: Increased bone turnover markers (BTM) level is consider as independent risk factor of bone fracture. However, it was not used in 10-year probability of bone fracture method (FRAX) proposed by WHO, which helps in qualification of patients for pharmacological treatment of osteoporosis. The aim of the study was to evaluate the usefulness of BTM in qualification for pharmacological treatment of osteoporosis. Material and methods: The study was performed in 152 subjects (20 men and 132 women) referred to Krajowe Centrum Osteoporozy. One-hundred thirty two of them were qualified for pharmacological treatment and 20 for prophylaxis on the basis of qualitative method. The following BTM were examined in all patients: of bone formation - N-terminal propeptide of procolagen type I (PINP) and N-mid osteocalcin (OC) and of bone resorption - C-terminal cross-linked telopeptide of collagen type I (CTx). Results: The values over that considered as independent fracture risk (in women only, no data for men) were found in 39 women with PINP, 39 with OC and 41 with CTx. Part of women had decreased serum BTM (10 women 3 BTM and 35 with 1 at least). There were not significant differences in serum BTM depending on the presence of clinical fracture risk factors: osteoporotic fracture in past, osteoporotic hip fracture in parents, chronic treatment with glucocorticosteroids and qualification for pharmacological treatment on the basis of qualitative and FRAX method. There was no significant difference in the presence of fracture risk factors depending on increased or decreased serum BTM. Conclusions: Results of the study did not show the practical use of BTM in qualification for pharmacological treatment of osteoporosis.

  BACKGROUND: Iodine prophylaxis in Poland started in 1935 however it has been interrupted during Second World War, and also in 1980. In January 1997 a new approach of preventing iodine deficiency wasBACKGROUND: Iodine prophylaxis in Poland started in 1935 however it has been interrupted during Second World War, and also in 1980. In January 1997 a new approach of preventing iodine deficiency was introduced as obligatory iodination model of household salt to be extent of 30 mg KI/kg. The aim of the study was to determine iodine intake after introduction of mandatory salt iodization in group of persons being on low salt diet because of hypertension. Investigated group and methods: The study was performed on 227 persons (94 males and 133 females), aged 18-89 years (average age 56 +/- 15,2 years). Each person answered questions concerning salt consumption and other products being additional source of iodine (fishes, pharmaceutical preparations containing iodine). Subjects were divided into two groups: according to the amount of salt consumption (119 persons restricted to household salt and 108 persons used kitchen salt) and according to the value of blood pressure (170 persons was hypertensive and 57 persons was normotensive). The urine iodine concentration (UIC) was determined in morning urine samples by the method of Sandell and Kolthoff. RESULTS: The average UIC in the whole studied group was 156,1 +/- 124,9 microg/l. Woman showed lower UIC than man (132,9 microg/l vs 177,5 microg/l). In 8% of patients iodine concentration was below 50 microg/l and in 64% of patients it was 100 microg/l or more. In the case of persons consuming limited, low amount of salt, the average urine iodine concentration was 150,3 microg/l, whereas those who add some salt to a dish was 162,4 microg/l. In hypertension patients ioduria was lower than in persons with normal blood pressure (153,1 microg/l vs 164,9 microg/l). CONCLUSIONS: The model of iodine prophylaxis has used in Poland is effective. It also concerns persons consuming limited amount of salt in the our studies group. The amount of consumed salt should be determined by measuring of Na+ concentration in 24-hour urine collection. Additional monitoring of iodine supply is necessary because of possible consumption of iodine from other sources than salt.

  Numerous studies have been performed to determine diagnostic or prognostic utility of tumor markers in patients with lung cancer. The aim of the study was to evaluate the diagnostic usefulness of theNumerous studies have been performed to determine diagnostic or prognostic utility of tumor markers in patients with lung cancer. The aim of the study was to evaluate the diagnostic usefulness of the tumor markers CA 125, CEA and CYFRA 21-1 in bronchoalveolar lavage fluid (BALF) in patients with non-small cell lung cancer (NSCLC). BAL was performed in 13 patients with NSCLC during diagnostic bronchofibroscopy. The control group consisted of 12 patients with sarcoidosis and 13 healthy volunteers. Tumor markers were determined in BALF supernatants using electrochemiluminescence technique (Elecsys 1010, Roche). To determine optimal cut-off values of tumor markers in BALF ROC curve was used. CEA and CA 125 concentration in BALF were significantly higher in NSCLC patients than in healthy volunteers and patients with sarcoidosis. CYFRA 21-1 in BALF was higher in NSCLC patients than in healthy volunteers, but no significant difference was found between NSCLC and sarcoidosis patients. The cut-off values of BALF concentration of CA 125, CEA and CYFRA 21-1 were 95 IU/mL, 3 ng/ml and 3 ng/ml, respectively. The sensitivity and specificity of CEA and CA 125 in BALF were 100%, 84% and 92%, 80%, respectively. In conclusion, we suggest that among the chosen markers, determination of CEA in BALF is the most useful in diagnosis of NSCLC. It may be a complementary method in diagnosing of patients in whom tumor cannot be visualized by bronchofibroscopy. These results need confirmation in larger groups of patients.

  Three years after introduction of obligatory salt iodization in Poland we evaluated iodine supply in children from Mazovia region. Urinary iodine concentration (UIC) was estimated in morning samplesThree years after introduction of obligatory salt iodization in Poland we evaluated iodine supply in children from Mazovia region. Urinary iodine concentration (UIC) was estimated in morning samples from 779 schoolchildren aged 6-14 years. Previous investigation 7 years ago revealed mild or moderate iodine deficiency in this area. UIC was lower than 100 micrograms/l in 80% of samples and mean UIC was 56.9 +/- 42.3 micrograms/l. According to WHO and ICCIDD recommendations median value of UIC at least 100 micrograms/l in the representative sample of population indicates satisfactory iodine supply. Recent data revealed that UIC was much higher than in the first phase of study, 7 years ago--mean 115.5 +/- 76.6 micrograms/l and median 100.69 micrograms/l. UIC lower than 100 micrograms/l occurred in 48% of samples, more frequently in rural regions. Iodine supply in the region was diversified--we revealed rural cohort where mean and median values of UIC (corresponding 78.96 +/- 47 micrograms/l and 67.8 micrograms/l) were unexpectedly low. Our results confirm the improvement of iodine supply in Poland after 1997. Nevertheless, taking under consideration that daily iodine supply in children and adolescents should reach 120-150 micrograms, the situation is not fully satisfactory--almost in half of schoolchildren iodine consumption hardly reaches recommended value for the whole population. We stress the necessity of continuous monitoring of the adequacy of iodine supplementation and its results.

The goal of the present study was to determine whether the mandatory iodization of kitchen salt (30 mg KI/kg) has had any impact on ioduria. MATERIAL/METHODS: The study was performed on 29 healthy volunteers, aged 22-29 (average age 23.93 +/- 1.14), examined in 1996 and retested after 36 months of iodine supplementation in 1999. Each person underwent a physical examination and thyroid ultrasonography. The iodine level was determined using morning urine samples. RESULTS: For the duration of the study thyroid volume remained unchanged, ranging from 11.48 I3.89 ml in 1996 to 12.11 +/- 3.85 ml in 1998. The echostructure was normal in all subjects both in 1996 and in 1999. In 1996 the iodine concentration in urine (ICU) in the study group averaged 100.4 +/- 41.5 mg/L (range from 43.3 to 175.6 microg/L). Seventeen persons had an iodine concentration below 100 microg/L. In 1999 the average ICU was 140.7 +/- 78.87 microg/L. Three subjects had a lower ICU in 1999 (53.93 +/- 24.65 microg/L) than in 1996 (104.1 +/- 24.6 microg/L). These persons claimed to have completely eliminated the use of added salt in their diet. CONCLUSIONS: We conclude that mandatory iodine supplementation method is sufficiently effective when salt is used in the diet. In the case of restrictions on salt consumption the use of preparations containing iodine should be recommended.
