[Show abstract][Hide abstract] ABSTRACT: In spite of the reported probiotic effects, Bifidobacterium bifidum BGN4 (BGN4) showed no β-glucosidase activity and failed to biotransform isoflavone glucosides into the more bioactive alycones during soy milk fermentation. To develop isoflavone biotransforming BGN4, we constructed the recombinant B. bifidum BGN4 strain (B919G) by cloning the structural β-glucosidase gene from B. lactis AD011 (AD011) using the expression vector with constitutively active promoter 919 from BGN4. As a result, B919G highly expressed β-glucosidase and showed higher β-glucosidase activity and heat stability than the source strain of β-glucosidase gene, AD011. The biotransformation of daidzin and genistin compounds using the crude enzyme extract from B919G was completed within 4 h, and the bioconversion of daidzin and genistin in soy milk during fermentation with B919G also occurred within 6 h, which was much faster and higher than AD011. The incorporation of β-glucosidase producing Bifidobacterium strain in soy milk could lead to a production of fermented soy milk with elevated amount of bioavailable forms of isoflavones as well as the indigenous probiotic effects of the Bifidobacterium strain.

[Show abstract][Hide abstract] ABSTRACT: This paper introduces a simple, inexpensive, and robust quantitative proteomic method for quantifying N-linked glycoproteins based on isotope-coded carbamidomethylation (iCCM) incorporated into an on-line microbore hollow fiber enzyme reactor and nanoflow liquid chromatography-tandem mass spectrometry (mHFER-nLC-MS/MS). The iCCM quantitation uses carbamidomethylation (CM; a routine protection of thiol groups before proteolysis) of the Cys residue of proteins with iodoacetamide (IAA) or its isotope (IAA-13C2,D2: 4 Da difference). CM-/iCCM-labeled proteome samples are mixed for proteolysis, then on-line enrichment of N-glycopeptides using lectin affinity is carried out in an mHFER before nLC-MS/MS for quantification using multiple reaction monitoring (MRM). Initial evaluation of the iCCM method varying the mixing ratio of CM-/iCCM-labeled bovine serum albumin (BSA) standards yielded successful quantification of 18 peptides with less than 2% variation in the calculated ratio of light/heavy-labeled peptides. The iCCM quantitation with mHFER-nLC-MS/MS was evaluated with -1-acid glycoproteins (AGP) and then applied to serum glycoproteins from liver cancer patients and controls, resulting in successful quantification of 73 N-glycopeptides (from 49 N-glycoproteins), among which 19 N-glycopeptides from 14 N-glycoproteins showed more than 2.5-fold aberrant change in liver cancer patients' sera compared with the pooled control. Although iCCM quantitation with mHFER-nLC-MS/MS applies only to glycopeptides with Cys residue, the method can offer several advantages over other labeling methods when applied to targeted glycoproteins: The iCCM method does not require an additional labeling reaction under special conditions nor complicated procedures to purify labeled products using additional columns. Isotope labeling at the protein level can minimize potential uncertainty originating from unequal efficiencies in protein digestion in separate vials and retrieval of each labeled peptide when labeling takes place at the peptide level. In addition, the labeling reagents for the iCCM method are readily obtained at a reasonable cost, which can make protein quantification easily accessible.
[Show abstract][Hide abstract] ABSTRACT: : Acral lentiginous melanoma (ALM) is the most common type of cutaneous melanoma in Asian populations. Traditionally, ALM was believed to have a poorer prognosis than other subtypes of cutaneous melanoma because of its aggressive behavior. However, in Asians, there have been several unusual case reports that have shown only subtle melanocytic proliferation despite clinically malignant manifestations. We performed a retrospective study of 13 patients with ALM. We reviewed the clinical histories, histopathologies, and immunohistochemical staining from these patients. Clinically, the lesions were characterized by a brown to black pigmented patch with irregular borders and variegated pigmentation on the sole, heel, or finger. Histopathologically, most specimens revealed only bland scattered proliferation of atypical melanocytes without marked cellular atypia or dermal invasion. However, some of the lesions were suspected to progress to the invasive stage, into the papillary dermis after a long period of time. Although cytological atypia of the melanocytes is not sufficient to ALM in situ, the melanocytic proliferation pattern, dermal inflammation, and correlation with clinical presentation would suffice in making the correct diagnosis of ALM in situ. In these cases, more biopsies of other areas should be recommended for the precise diagnosis and early complete excision in accordance with ALM in situ.

[Show abstract][Hide abstract] ABSTRACT: Pincer nails are characterized by the transverse curvature increasing from the proximal to the distal aspect along the longitudinal axis. A novel treatment technique using a shape memory alloy device was recently introduced.
To determine the treatment outcomes and safety of a shape memory alloy device.
This was a retrospective analysis of 21 cases of pincer nail of the great toe in 14 patients. Subjective indices (pain, inconvenience, global assessment scores) and objective parameters (transverse angle, width index) on days 0 (D0) and 1 (D1) and weeks 2 (W2) and 12 (W12) were investigated.
All parameters started to show significant improvement on D1. From D0 to D1, pain score fell from 3.6 ± 1.0 to 1.5 ± 0.7, inconvenience score fell from 3.3 ± 1.0 to 1.5 ± 0.7, global assessment score increased from 1.5 ± 0.7 to 2.5 ± 0.7, transverse angle improved from 86.4 ± 27.7° to 114.7 ± 21.6°, and width index improved from 67.6 ± 13.1% to 89.0 ± 9.7%. These effects were consistent during 12 weeks of follow-up. Two cases (9.5%) recurred, but symptoms were less severe than before. No complication was identified.
A shape memory alloy device was an effective and safe way to rapidly correct symptomatic pincer nail deformity.

[Show abstract][Hide abstract] ABSTRACT: Plasminogen activator inhibitor-1 (PAI-1) is important for tumor growth, Invasion, and metastasis. In this study, we investigated the relationship between plasma levels of PAI-1 and colorectal adenomas.
We reviewed the medical records of 3,136 subjects who underwent colonoscopy as a screening exam. The subjects were classified into a case group with adenomas (n=990) and a control group (n=2,146). Plasma PAI-1 levels were categorized into three groups based on tertile.
The plasma levels of PAI-1 were significantly higher in adenoma cases than in controls (p=0.023). The prevalence of colorectal adenomas increased significantly with increasing levels of PAI-1 (p=0.038). In the adenoma group, advanced pathologic features, size, and number of adenomas did not differ among the three groups based on tertiles for plasma PAI-1 levels. Using multivariate analysis, we found that plasma level of PAI-1 was not associated with the risk of colorectal adenomas (p=0.675). Adjusted odds ratios for colorectal adenomas according to increasing plasma levels of PAI-1 were 0.980 (95% confidence interval [CI], 0.768 to 1.251) for the second-highest plasma level and 1.091 (95% CI, 0.898 to 1.326) for the highest level, compared with the lowest levels.
These results suggest that elevated plasma PAI-1 levels are not associated with the risk of colorectal neoplasms.

[Show abstract][Hide abstract] ABSTRACT: In this study, we report the development of a microbore hollow fiber enzyme reactor (mHFER) coupled to nanoflow liquid chromatography-tandem mass spectrometry (nLC-ESI-MS/MS) for the on-line digestion or selective enrichment of glycopeptides and analysis of proteins. With mHFER, enzymatic digestion of protein could be achieved by continuous flow within a very small volume (~ 10 μL) of mHF inserted in a PEEK tube. Digested peptides exited through the pores of the hollow fiber membrane wall to external single or multiplexed trap columns for nLC-ESI-MS/MS analysis. Evaluation of on-line mHFER-nLC-ESI-MS/MS system was made with bovine serum albumin (BSA) by varying the temperature of digestion and the amount of protein injected. We evaluated the ability of the mHFER system to enrich glycopeptides by injecting a mixture of lectin (concanavalin A) and digested peptides from α-1-acid glycoprotein (AGP) into the mHFER, followed by delivery of PNGase F for endoglycosidic digestion. Non-glycosylated peptides unbound to lectins eluted at the first breakthrough run while N-linked glycopeptides eluted after the endoglycosidic digestion. The developed method was applied to urine samples from patients with prostate cancer and controls; 67 N-linked glycopeptides were identified and relative differences in glycopeptide content between patient and control samples were determined.
[Show abstract][Hide abstract] ABSTRACT: The purpose of this study is to analyze the current attitudes toward professionalism, the core values, and the type of professionalism among plastic surgeons in Korea to establish a code of ethics regarding the role of professionalism for plastic and reconstructive surgeons.
From March 9, to July 1, 2012, face-to-face and mail surveys were conducted targeting the 325 participants (256 specialists and 69 residents) who are registered members of the Korean Society of Plastic and Reconstructive Surgeons. The proportion of each response given to an item was obtained through statistical processing through frequency analysis. The Mann-Whitney U test was used to compare the differences in the responses between the resident group and the specialist group.
The survey results on the perception of professionalism in plastic surgery showed that a high proportion (90.5%) of the respondents viewed the future of plastic surgeons as bright. Through evaluation of the importance of the value items,"professional dominance" (4.58 pts), "autonomy" (4.45 pts), "lifestyle" (4.34 pts), and "commercialism" (4.31 pts) were assessed as critical values. "Altruism" (3.84 pts), "interpersonal competence" (3.79 pts), and "social justice" (3.61 pts) were viewed as lesser values. This difference showed the characteristics of an entrepreneurial outlook.
Plastic surgeons should pursue excellence, humanism, accountability, and altruism in order to overcome the crisis of professionalism in plastic surgery. In order to develop the necessary competencies of professionals, vocational education should be arranged by the Korean Society of Plastic Surgeons, and an appropriate code of ethics should be established.

[Show abstract][Hide abstract] ABSTRACT: OBJECTIVE:: The aim of this study was to evaluate the efficacy of preoperative chest computed tomography (CT) and the risk factors for lung metastasis in colon cancer patients without liver metastasis who had negative findings on initial chest X-ray (CXR). BACKGROUND:: Preoperative staging with chest CT is recommended in colon cancer patients. However, there have been only scant data on the clinical efficacy. METHODS:: Three hundred nineteen consecutive colon cancer patients without liver metastasis were retrospectively reviewed and analyzed. The patients had negative findings on preoperative CXR, and they underwent surgery for colon cancer during the period of January 2008 to April 2010. RESULTS:: Lung nodule on chest CT was found in 136 patients (42.6%). Twenty of those were definitely diagnosed with lung metastasis (6.3%) by follow-up chest CT or pathologic confirmation. There was no case of delay in surgery due to findings of lung nodule. Comparing the group with lung metastases to that without lung metastases, postoperative pathologic findings reported more advanced T and N status (P = 0.004, P < 0.001, respectively), and lymphatic invasion was more frequent (P = 0.003) in the group with lung metastasis. By multivariate analysis, CT-predicted lymph node metastases and pathologic lymph node metastases were risk factors for lung metastases. CONCLUSIONS:: Preoperative staging chest CT is not beneficial to colon cancer patients without liver metastasis and lymph node metastasis suggested on abdominal and pelvic CT who had negative finding on initial CXR.
[Show abstract][Hide abstract] ABSTRACT: High temperature (250-350°C) sodium-beta alumina batteries (NBBs) are attractive energy storage devices for renewable energy integration and other grid related applications. Currently, two technologies are commercially available in NBBs, e.g., sodium-sulfur (Na-S) battery and sodium-metal halide (ZEBRA) batteries. In this study, we investigated the combination of these two chemistries with a mixed cathode. In particular, the cathode of the cell consisted of molten NaAlCl4 as a catholyte and a mixture of Ni, NaCl and Na2S as active materials. During cycling, two reversible plateaus were observed in cell voltage profiles, which matched electrochemical reactions for Na-S and Na-NiCl2 redox couples. An irreversible reaction between sulfur species and Ni was identified during initial charge at 280°C, which caused a decrease in cell capacity. The final products on discharge included Na2Sn with 1< n < 3, which differed from Na2S3 found in traditional Na-S battery. Reduction of sulfur in the mixed cathode led to an increase in overall energy density over ZEBRA batteries. Despite of the initial drop in cell capacity, the mixed cathode demonstrated relatively stable cycling with more than 95% of capacity retained over 60 cycles under 10mA/cm2. Optimization of the cathode may lead to further improvements in battery performance.

In particular, we determine conditions on a ring R such that: (1) each ideal of R is a finite intersection of ideals satisfying one of the generalizations of the primary concept; or (2) R is a finite direct sum of rings satisfying one of the generalizations of the primary concept; or (3) R has a generalized triangular matrix representation in which each ring on the main diagonal satisfies one of the generalizations of the primary concept. [Show abstract][Hide abstract] ABSTRACT: Uniform coating of nanometer‐scale BaTiO3–Dy2O3–SiO2 layers on spherical Ni particles are achieved by controlled hydrolysis of tetrabutyl titanate (TBT), hydrothermal reaction with Ba(OH)2, and co‐precipitation of tetraethylorthosilicate (TEOS) and Dy(NO3)3. The composition of the coating layer is similar to rare earth oxide‐silica–doped BaTiO3, which is the main component of dielectric layer for base metal electrode (BME) multilayer ceramic capacitors (MLCCs). After coating, the shrinkage onset temperature of Ni particles is significantly increased. After sintered to pellets, the electrode has good electrical conductivity. This electrode material has good compatibility with rare earth oxide and silica‐doped BaTiO3 dielectric materials, and could serve as promising candidate for application in the next generation BME‐MLCCs.

[Show abstract][Hide abstract] ABSTRACT: BACKGROUND:: Endoscopic resection could be a curative treatment for early colorectal cancer without the possibility of lymph node metastasis. However, if the resection margin is positive, and there is a risk of lymph node metastasis, additional surgery should be performed. OBJECTIVE:: The aim of this study was to investigate the characteristics of patients who underwent additional surgery to determine risk factors associated with residual tumor and lymph node metastasis. DESIGN:: This study is a retrospective analysis. SETTINGS:: This study was conducted at a tertiary academic hospital. PATIENTS:: We evaluated 85 patients who underwent additional surgery with curative intent after endoscopic resection for early colorectal cancer at the Samsung Medical Center, Seoul, South Korea, between January 2001 and April 2010. MAIN OUTCOME MEASURES:: We identified risk factors associated with residual tumor or lymph node metastasis in surgical specimens after noncurative endoscopic resection for early colorectal cancer. RESULTS:: Among 85 patients who underwent additional surgery after noncurative endoscopic resection, 76 (89.4%) had submucosal invasion greater than 1000 μm. Twenty-one (24.7%) and 25 patients (29.4%) had a positive lateral or vertical resection margin, and 11 patients (12.9%) had inadequate lifting sign. After additional surgery, patients were divided into 2 groups according to the presence or absence of residual tumor and/or lymph node metastasis. There was no significant difference between the groups in positive lateral margin, but there was a significant difference in positive vertical margin (p = 0.015 with an OR of 15.02). In patients with inadequate lifting sign, the OR was 13.68 (p = 0.013). LIMITATIONS:: This study was limited by its retrospective nature. CONCLUSION:: There is a greater need for additional surgery in cases with positive vertical resection margin or inadequate lifting sign, because the risk of residual tumor and lymph node metastasis is higher than in other cases.

[Show abstract][Hide abstract] ABSTRACT: The sodium-nickel chloride (ZEBRA) battery is typically operated at relatively high temperature (250~350°C) to achieve adequate electrochemical performance. Reducing the operating temperature in the range of 150 to 200°C can lead to enhanced cycle life by suppressing temperature related degradation mechanisms. The reduced temperature range also allows for lower cost materials of construction such as elastomeric sealants and gaskets. To achieve adequate electrochemical performance at lower operating temperatures requires an overall reduction in ohmic losses associated with temperature. This includes reducing the ohmic resistance of ”-alumina solid electrolyte (BASE) and the incorporation of low melting point molten salt as the secondary electrolyte. In present work, planar-type Na/NiCl2 cells with a thin flat plate BASE (600 m) and low melting point secondary electrolyte were evaluated at reduced temperatures. Molten salt formulation for use as secondary electrolytes were fabricated by the partial replace of NaCl in the standard secondary electrolyte (NaAlCl4) with other lower melting point alkali metal salts such as NaBr, LiCl, and LiBr. Electrochemical characterization of the ternary molten salts demonstrated , improved ionic conductivity, and sufficient electrochemical window at reduced temperatures. Furthermore, Na/NiCl2 cells with 50 mol% NaBr-containing secondary electrolyte exhibited reduced polarizations at 175°C compared to the cell with the standard NaAlCl4 catholyte. The cells also exhibited stable cycling performance even at 150oC.
[Show abstract][Hide abstract] ABSTRACT: Blue rubber bleb nevus syndrome (BRBNS) is a rare systemic vascular disorder characterized by multiple venous malformations involving many organs. BRBNS can occur in various organs, but the most frequently involved organs are the skin and gastrointestinal (GI) tract. GI lesions of BRBNS can cause acute or chronic bleeding, and treatment is challenging. Herein, we report a case of GI BRBNS that was successfully treated with a combination of intraoperative endoscopy and radical resection.
