  • Article: Valvopatia aórtica crônica grave sintomática: estudo comparado da ressonância magnética cardíaca e ecocardiografia

    ABSTRACT: OBJETIVO: Mostrar o real valor da ressonância magnética cardíaca (RMC) na avaliação dos portadores de valvopatia aórtica crônica sintomática. MÉTODOS: Foram estudados 70 pacientes, 35 com estenose aórtica (EAo) e 35 com insuficiência aórtica (IAo), com indicação cirúrgica, que realizaram ecocardiograma (ECO) e RMC pré-operatórios para avaliação da função ventricular, volumes e índice de massa ventricular esquerda por meio da cinerressonância. RESULTADOS: Quando comparadas as variáveis do ECO e da RMC, nos grupos da EAo e da IAo não houve diferença estatística entre os dois métodos. Quando comparadas com o tipo de sintoma, as variáveis pelo ECO e pela RMC apresentam o mesmo comportamento. CONCLUSÃO: A RMC apresentou concordância com o ECO na avaliação do volume ventricular esquerdo e fração de ejeção e com a clínica dos pacientes com valvopatia aórtica crônica.

  • Article: Coronary heart disease clinical manifestation and risk factors in Japanese immigrants and their descendents in the city of São Paulo

    ABSTRACT: OBJECTIVE: To assess whether a difference exists in coronary heart disease clinical manifestations and the prevalence of risk factors between Japanese immigrants and their descendents in the city of São Paulo. METHODS: Retrospective analysis of coronary artery disease clinical manifestations and the prevalence of risk factors, comparing 128 Japanese immigrants (Japanese group) with 304 Japanese descendents (Nisei group). RESULTS: The initial manifestation of the disease was earlier in the Nisei group (mean = 53 years), a difference of 12 years when compared with that in the Japanese group (mean = 65 years) (P<0.001). Myocardial infarction was the first manifestation in both groups (P = 0.83). The following parameters were independently associated with early coronary events: smoking (OR = 2.25; 95% CI = 1.35-3.77; P<0.002); Nisei group (OR = 10.22; 95% CI = 5.64-18.5; P<0.001); and female sex (OR = 5.04; 95% CI = 2.66-9.52; P<0.001). CONCLUSION: The clinical presentation of coronary heart disease in the Japanese and their descendents in the city of São Paulo was similar, but coronary heart disease onset occurred approximately 12 years earlier in the Nisei group than in the Japanese group.

  • Article: Noninvasive diagnosis of allograft vascular disease after heart transplantation

    ABSTRACT: OBJECTIVE: To determine the predictive values of noninvasive tests for the detection of allograft vascular disease. METHODS: We studied 39 patients with mean ages of 48±13 years and a follow-up period of 86±13 months. The diagnosis of allograft vascular disease was made by cine-coronary arteriography, and it was considered as positive if lesions existed that caused > or = 50% obstruction of the lumen. Patients underwent 24h Holter monitoring, thallium scintigraphy, a treadmill stress test, and dobutamine stress echocardiography. Sensitivity, specificity, and positive and negative predictive values were determined in percentages for each method, as compared with the cine-coronary arteriography results. RESULTS: Allograft vascular disease was found in 15 (38%) patients. The Holter test showed 15.4% sensitivity, 95.5% specificity. For the treadmill stress test, sensitivity was 10%, specificity was 100%. When thallium scintigraphy was used, sensitivity was 40%, specificity 95.8%. On echocardiography with dobutamine, we found a 63.6% sensitivity, 91.3% specificity. When the dobutamine echocardiogram was associated with scintigraphy, sensitivity was 71.4%, specificity was 87%. CONCLUSION: In this group of patients, the combination of two noninvasive methods (dobutamine echocardiography and thallium scintigraphy) may be a good alternative for the detection of allograft vascular disease in asymptomatic patients with normal ventricular function.

  • Article: Trends in death from circulatory diseases in Brazil between 1979 and 1996

    ABSTRACT: OBJECTIVE: To analyze the trends in mortality due to circulatory diseases in men and women aged > or = 30 years in Brazil from 1979 to 1996. METHODS: We analyzed population count data obtained from the IBGE Foundation and mortality data obtained from the System of Information on Mortality of the DATASUS of the Ministry of Health. RESULTS: Circulatory diseases, ischemic heart disease, and cerebrovascular disease were the major causes of death in men and women in Brazil. The standardized age coefficient for circulatory disease in men aged > or = 30 years ranged from 620 to 506 deaths/100,000 inhabitants and in women from 483 to 383 deaths/100,000 inhabitants for the years 1979 and 1996, respectively. In men, the mean coefficient for the period was 586.25 deaths with a significant trend towards a decrease (P<0.001) and a decline of 8.25 deaths/year. In women, the mean coefficient for the period was 439.58 deaths, a significant trend towards a decrease (P<0.001) and a rate of decline of 7.53 deaths/year. The same significant trend towards a decrease in death (P<0.001) was observed for ischemic heart disease and cerebrovascular disease. Risk of death from these causes was always higher for men of any age group (P<0.001). Cerebrovascular disease was the primary cause of death in women. CONCLUSION: Although circulatory diseases have been the major cause of mortality in men and women in the Brazilian population, with a greater participation by cerebrovascular diseases, a trend towards a decrease in the risk of death from these causes is being observed.

  • Article: Treatment of persistent rejection with methotrexate in stable patients submitted to heart transplantation

    ABSTRACT: OBJECTIVE: To evaluate the use of methotrexate for the treatment of recurrent rejection in heart transplant recipients. METHODS: We studied 6 patients submitted to heart transplantation that showed rejection grade > or = 3A (ISHLT) in two consecutives endomyocardial biopsy specimens. The dose was 11.26±3.75mg/week. The evaluated data were: ventricular function, endomyocardial biopsy, white cell count and number of rejection episodes before and after methotrexate administration. RESULTS: There was a reduction in the number of rejection episodes (5.17±1.47 before methotrexate; 2.33±1.75 after 6 months and 3.17±2.99 after 12 months of treatment, p=0.0193). The ventricular function was normal with ejection fraction of 76.5±4.80 before and 75.6±4.59 after methotrexate (p=0.4859). One patient did not finish the treatment because he showed signs of rejection associated with severe pericardial effusion. Five patients had a reduction in the white cell count (8,108±23.72 before and 5650±1350 after methotrexate, p=0.0961). One pulmonary infection with complete resolution after antibiotic treatment was observed. CONCLUSION: Methotrexate in low doses is an effective adjunct therapy in the treatment of recurrent rejection after heart transplantation.

  • Article: Effects of carvedilol in heart failure due to dilated cardiomyopathy. Results of a double-blind randomized placebo-controlled study (CARIBE study)

    ABSTRACT: OBJECTIVE: To assess the effects of carvedilol in patients with idiopathic dilated cardiomyopathy. METHODS: In a double-blind randomized placebo-controlled study, 30 patients (7 women) with functional class II and III heart failure were assessed. Their ages ranged from 28 to 66 years (mean of 43±9 years), and their left ventricular ejection fraction varied from 8% to 35%. Carvedilol was added to the usual therapy of 20 patients; placebo was added to the usual therapy of 10 patients. The initial dose of carvedilol was 12.5 mg, which was increased weekly until it reached 75 mg/day, according to the patient's tolerance. Clinical assessment, electrocardiogram, echocardiogram, and radionuclide ventriculography were performed in the pretreatment phase, being repeated after 2 and 6 months of medication use. RESULTS: A reduction in heart rate (p=0.016) as well as an increase in left ventricular shortening fraction (p=0.02) and in left ventricular ejection fraction (p=0.017) occurred in the group using carvedilol as compared with that using placebo. CONCLUSION: Carvedilol added to the usual therapy for heart failure resulted in better heart function.

  • Article: Peripheral arterial embolism. Report of hospitalized cases

    ABSTRACT: OBJECTIVE: We analyzed the frequency of peripheral embolisms, the underlying heart disease,triggering factors, the sites of the emboli, and evolution of the patients. METHODS: We analyzed 29 cases of peripheral arterial embolism out of a total of 20,211 hospitalizations in a cardiology center in the city of São Paulo. The age was 51.89±18.66 years, and 15 were males. RESULTS: Embolism in the right lower limb occurred in 18 patients (62.0%),in the left lower 11(37.9%) and right upper 3 (10.3%) limbs, and in the left arm (1). Four patients had embolism in two limbs. The heart disease, mitral valvar heart disease (9 patients - 31.0%); infective endocarditis (7- 24.1%); dilated cardiomyopathy (6 - 20.6%); ischemic coronary heart disease (6 patients - 20.6%); and one patient with cor pulmonale. Atrial fibrillation was observed in 20 patients (68.9%), chronic in 12 patients (41.3% ) and acute in 8 (27.5%). All patients with mitral valvar heart disease had atrial fibrillation, chronic in 8 patients (88.8%); patients with cardiomyopathy and coronary heart disease, 4 in each group had atrial fibrillation, acute in 60% of the patients.Patients with infective endocarditis, 3 had staphylococcus and 2 Gram-negative bacteria. In the follow-up, 2 patients (6.8%) required limbs amputation, and 5 (17.2%) died due to embolism. CONCLUSION: Most of the time, embolism does not cause permanent complications. Our data highlight the importance of anticoagulation for patients acute atrial fibrillation in myocardial dysfunction and for patients with chronic atrial fibrillation in cases of mitral valvar heart disease to prevent peripheral embolism.

  • Article: Analysis of the risk factors for allograft vasculopathy in asymptomatic patients after cardiac transplantation

    ABSTRACT: OBJECTIVE: To study the influence of immune and nonimmune risk factors on the development of allograft vasculopathy after cardiac transplantation. METHODS: We studied 39 patients with a mean age of 46±12 years. The following variables were analyzed: weight (kg), body mass index (kg/m²), donor's age and sex, rejection episodes in the first and second years after transplantation, systolic and diastolic blood pressures (mmHg), total cholesterol and fractions (mg/dL), triglycerides (mg/dL), diabetes, and cytomegalovirus infection. The presence of allograft vasculopathy was established through coronary angiography. RESULTS: Allograft vasculopathy was observed in 15 (38%) patients. No statistically significant difference was observed between the two groups in regard to hypertension, cytomegalovirus infection, diabetes, donor's sex and age, rejection episodes in the first and second years after transplantation, and cholesterol levels. We observed a tendency toward higher levels of triglycerides in the group with disease. Univariate and multivariate analyses showed statistically significant differences between the two groups when we analyzed the body mass index (24.53±4.3 versus 28.11±4.6; p=0.019). CONCLUSION: Body mass index was an important marker of allograft vasculopathy in the population studied.

  • Article: Efeitos do carvedilol (bloqueador b1,b2,a1) na insuficiência cardíaca refratária

    ABSTRACT: OBJETIVO: Os efeitos dos beta-bloqueadores na insuficiência cardíaca (IC) refratária não têm sido adequadamente estudados. Investigamos os efeitos do carvedilol (bloqueador b1,b2,a1 ) nos sintomas e na função ventricular de portadores de IC refratária. MÉTODOS: Foram estudados 21 pacientes, idade média de 56±10 anos, 9 em classe funcional (CF) IV, e 12 em CF III intermitente com IV. A dose inicial de carvedilol foi de 6,25mg e, se tolerada, aumentada progressivamente. A dose média final foi 42±11mg. Os pacientes foram submetidos a avaliações clínicas e eletrocardiográficas seriadas. Realizaram-se, antes e com 196±60 dias de evolução, ecocardiograma e ventriculografia radioisotópica. RESULTADOS: O medicamento foi tolerado em 16 (76%) pacientes. Um paciente está em fase de titulação em CF II. Com 196±60 dias de evolução observaram-se 8 pacientes em CF I e 7 em II; redução da freqüência cardíaca de 96±15 para 67±10bpm (p<0,0001); redução do diâmetro diastólico final de ventrículo esquerdo (VE) de 73±13 para 66±12mm (ecocardiograma) (p<0,009); e aumento da fração de ejeção de VE de 0,21±0,06 para 0,34±0,12 (p<0,0003). CONCLUSÃO: O carvedilol devido aos seus efeitos benéficos na função ventricular, remodelamento e CF é, se tolerado, uma potencial alternativa terapêutica no tratamento medicamentoso da IC refratária. Entretanto, estudos adicionais são necessários para definição do efeito a longo prazo neste específico subgrupo de pacientes.

    Article: Evolução dos níveis de colesterol na população adulta de São José do Rio Preto (1991-1997)

    ABSTRACT: OBJETIVO: Realizar levantamento de fatores de risco para aterosclerose na população > ou = 20 anos de São José do Rio Preto, e comparar os dados obtidos com os coletados, em levantamento similar, em 1991. MÉDOTOS: Pesquisa quantitativa com amostragem estratificada por sexo e idade. Os entrevistados (646 indivíduos, 303 homens) foram abordados em Postos de Saúde e outros pontos de afluxo da população, distribuídos por zona geográfica e por classe social dos bairros. A margem de erro foi de 4%, e o intervalo de confiança (IC) de 95%. RESULTADOS: A) Níveis médios de HDL-colesterol (apenas em 1997): homens 43,7±15mg/dL, mulheres 49,6±13,5mg/dL (p<0,001, 95% IC 3,7 a 8,1); B) níveis médios de colesterol total em 1991 vs 1997: população global 192,5±48,9mg/dL vs 190,5±42,5mg/dL (p=NS); homens 187,6±53,3 vs 190,5±42,5mg/dL (p=NS); mulheres 196,8±40mg/dL vs 187,6±37,8mg/dL (p=0,008, 95% IC 2,4 a 15,9). C) Em análise de regressão, as variáveis que se correlacionaram significativamente com níveis de colesterol, em ambas as pesquisas, foram: idade (p<0,001), pressão arterial sistólica (p<0,001) e diastólica (p<0,001), dieta (p<0,001). Sexo feminino correlacionou-se apenas em 1991 (p=0,011) e sedentarismo apenas em 1997 (p=0,014). CONCLUSÃO: Os níveis de colesterol na população adulta de São José do Rio Preto são bastante aceitáveis, sendo que no sexo feminino diminuíram, de maneira significativa, na pesquisa atual, em relação à realizada em 1991.
