ABSTRACT: To explore the clinical characteristics and early effective treatment for pulmonary infection after acute cervical spinal cord injury.
A total of 215 inpatients with acute cervical spinal cord injury were retrospectively analyzed. Their chest radiological films and blood profiles at discharge were analyzed. The fourth generation cephalosporin was used to treat pulmonary infection as soon as admission and the antibiotics switched according to the results of sputum culture and drug sensitive test. Incision of trachea was performed and breath supported by breath machine according to respiratory condition and blood gas analysis. All patients were turned over and slapped on the back in order to excrete phlegm in time. Sometimes bronchial lavage was used to excrete phlegm. The chest radiological examinations and sputum culture were performed twice one week. Once fungal infection was definite, specific antibiotic was used to treat infection. Three-liter bas and nasal feeding were used to improve the nutrition condition. Incision of trachea was closed as soon as possible.
Pulmonary infection of 214 patients was finally cured. Among them, 43 suffered from pulmonary closure. One patient died from severe infection of Klebsiella pneumoniae. Pulmonary infection appeared upon admission and was mostly accompanied with hyperpyrexia. The result of sputum culture revealed baumanii and the pathogen of hemoculture was Pseudomonas aeruginosa. At 3 - 4 weeks later, mycotic infection appeared. And 17 patients suffered from Klebsiella pneumoniae and one died.
Pulmonary infection after acute cervical spinal cord injury is severe and occurs early. Effective antibiotics according to the result of sputum culture, turnover & back-slapping for excreting phlegm in time, expectoration training and strengthening overall nutrition are effective therapeutic measures.

ABSTRACT: It is necessary to provide seamless multimedia services to mobile users. However, due to the effects of host mobility, it is difficult to achieve seamless QoS guarantee. Several solutions have been proposed to couple QoS with mobility management. In this paper, a QoS-aware HMIPv6 solution using QoS NSLP is proposed. The cross-layer solution takes use of L2 and L3 triggers to execute the resource reservation in new path and CRN(crossover node) discovery before MN moves into new domain. Also the proposed scheme can avoid the tunnel traverse in HMIPv6 as the advance resource reservation and CRN discovery procedure is done before MN registers with nAR.. The proposed approach can significantly reduce the re-reservation delay after the handover.

ABSTRACT: Multi-radio access selection (MRAS) is a key issue to obtain performance improvement in heterogeneous wireless networks. MRAS can affect the users' QoS directly, as well as the resource allocation and load balance for networks. In this paper, a universal MRAS decision model was set up by multi-objective optimization methods. Parameters from upper layer and lower layer related with network selection were classified systematically, and a new multi-attributes decision making algorithm combined genetic algorithm and Tchebycheff method was proposed. The simulation results show that the parameters are identified effectively, and the network selection mechanism we proposed based on the multi-objective optimization modeling can select the appropriate network for users.

ABSTRACT: In this paper, we consider an OFDMA-based two-hop relay network which consists of a single base station, dedicated fixed relay stations and subscriber stations. We investigate the optimal location of the relay stations aiming to maximize system throughput. We also focus on the impact of different relay strategies (i.e., decode-and-forward vs. amplify-and-forward) for each source-destination pair on the network performance. For a wireless network operating in a multiuser frequency-selective slow-fading environment, it is an important task for the network operator to decide where to place the relay stations and how to relay. Through a system lever simulation, we obtain the network performance of our proposed approach in an OFDMA based cooperative environment for different relay locations and different relay strategies. In the end, we give the optimal location of RS and present the impact of RS number on the system capacity under our assumptions. Our approach will be heuristic in practice for the deployment of relay stations in the networks.

ABSTRACT: In order to avoid the problem caused by single point of failure of the root AAA (authentication, authorization, and accounting) server in hierarchical AAA mobility management (HAMM), a novel robust fault tolerant scheme for hierarchical AAA (RFT-HA) is proposed in Mobile IPv6. In this scheme, the period of detecting the root AAA server's (RAAAS) failure is decreased by adopting ICMPv6 and Diameter protocol. Furthermore, the procedure of recovering from the RAAAS's failure state is accelerated by using backup binding information. The performance of RFT-HA has been theoretically analyzed. As the consequence, the simulation results show that its performance is better than any other manners in terms of RAAAS's reliability, packet loss rate, and the period of discovery and recovery of RAAAS's failure in hierarchical AAA.

ABSTRACT: Facing the different wireless access networks, this paper has proposed several novel interworking AAA architectures among the 3GPP, WiMAX and hotspot Wireless LANs. In these architectures, a new entity named standby 3GPP AAA server is defined to cope with the single point failure. Moreover, based on the proposed interworking AAA architectures, a hierarchy AAA scheme is proposed. As a result, the simulation results show that the performance of proposed scheme is better than the existed ones, and the total cost of the authentication process is reduce drastically.
