ABSTRACT: MHC associations with IDDM in the Korean population were studied to investigate genetic susceptibility to this disorder. The frequencies of HLA-DR3, -DR4 and -DR9 were significantly higher in diabetic patients. However, the frequency of DR2 was significantly decreased in diabetic patients. DQA1*0301 and DQA1*0501 were positively and DQA1*0102 and DQA1*0201 negatively associated with IDDM. DQB1*0301 and DQB1*0601 were negatively associated with IDDM. Heterodimers DQA1*0301-DQB1*0201, DQA1*0501-DQB1*0201 and DQA1*0501-DQB1*0302 were positively associated with DQA1*0102-DQB1*0601 negatively associated with IDDM. The frequencies of DR3-DQA1*0301-DQB1*0201 and -DQA1*0501-DQB1*0201 were significantly higher in diabetic patients. The frequencies of DR4-DQA1*0301-DQB1*0201 and DR9-DQA1*0301-DQB1*0303 were significantly higher in diabetic patients. The presence of non-aspartic acid at position 57 of the DQ beta-chain was not associated with susceptibility to IDDM. However, the frequency of Arg 52 homozygotes was significantly higher in diabetic patients. These results suggest a role of the MHC molecule and also suggest racial differences in susceptibility to IDDM even within the Asian populations.

ABSTRACT: Vitreous fluid and serum were obtained at the time of vitrectomy from 15 subjects with diabetic proliferative retinopathy and 6 control subjects. The mean serum IGF-I (IGF-I level: 1 nM/l = 7.7 ng/ml) concentrations were 15.40 +/- 4.97 nM/l in controls and 16.24 +/- 4.32 nM/l in diabetic patients. The mean vitreous IGF-I concentration was 1.05 +/- 0.30 nM/l in controls and 0.91 +/- 0.50 nM/l in diabetic patients. There were no significant differences between the two groups, and serum IGF-I concentrations were not correlated with vitreous IGF-I concentrations in the patients with diabetic proliferative retinopathy. These data suggest that IGF-I may not be a risk factor for the development of proliferative diabetic retinpathy.

ABSTRACT: Recent reports on growth hormone (GH) therapy have shown that GH has various beneficial effects in GH deficient adults. In most of these studies, GH was administered daily. Because GH is still expensive and has to be delivered by subcutaneous injection, we studied the 6-month therapeutic effects of thrice weekly GH injection therapy and compared it with daily therapy. Twenty eight adult patients with GH deficiency were randomly assigned into group 1 (ten cases, thrice weekly injections of GH), group 2 (nine cases, daily injections), and group 3 (nine cases, placebo injections). Lean body mass, serum basal GH levels, and insulin-like growth factor 1 levels significantly increased after six months of GH therapy in both groups 1 and 2. According to computed tomography, the mean mid-thigh muscle mass significantly increased in group 1, while the visceral fat mass significantly decreased in group 2. GH levels significantly increased exercise rate-pressure product and hand grip strength only in group 1. These results suggest that thrice weekly injections of GH are as effective as daily injections in GH deficient adults.

ABSTRACT: Maximizing peak bone mass is advocated as a way to prevent osteoporosis. To evaluate the peak bone mass and the affecting factors in Korean women, we analyzed bone stiffness in 116 middle school students, 118 high school students and 115 female college students by using the Achilles densitometer (Lunar Corporation). Peak bone stiffness of Korean women was relatively lower than that of white women (94% of white women) and a rapid rise of bone stiffness was observed in those subjects 3-4 years after menarche. In adolescent females without menstruation, the bone stiffness was lower than that of adolescent girls with menstruation. The factors affecting the peak bone mass was similar to the risk factors of post menopausal osteoporosis: menstruation status, calcium intake and physical activity. The amount of calcium intake in Korean girls at the critical age (3-4 years after menarche) was lower than the RDA (requirement of daily allowance) at this age. To improve any program aimed at maximizing peak bone mass, further intensive study will be required to evaluate some other common factors affecting peak bone mass in Korean.

ABSTRACT: We present a 45-yr-old male with clinical signs and symptoms of mild hyperthyroidism, high serum levels of T3, T4, and FT4 as well as serum TSH concentration. The elevated alpha-subunit level and alpha-subunit/TSH molar ratio were also observed. These findings indicated the presence of hyperthyroidism due to inappropriate secretion of TSH, whose neoplastic origin was documented by nuclear magnetic resonance scan showing a 0. 6 cm pituitary adenoma. Selective pituitary adenomectomy was completely successful; alpha-subunit, TSH, T3, T 4, and FT4 normalized, and euthyroidism was restored. Light microscopic immunohistochemistry showed that the adenoma was composed of TSH-secreting cells.
