    ABSTRACT: Wir berichten über 2 Fälle eines traumatischen Einrisses des Synoviaüberzugs über dem vorderen Kreuzband nach Kniegelenkverdrehtrauma. Der femorale Anteil der Synovia retrahierte sich und klemmte zwischen dem femoralen Ansatz des vorderen Kreuzbands und der Femurkondyle im Sinne eines Impingements mit schmerzbedingter Streckhemmung ein. Klinisch imponierte der Befund wie eine akute Meniskuseinklemmung. Arthroskopisch wurden eine Meniskusläsion ausgeschlossen und die eingeschlagenen Weichteile entfernt. Bei sofort postoperativ wiedererlangter schmerzfreier Beweglichkeit blieben die Patienten beschwerdefrei. We report on two cases of acute knee trauma with rupture of the synovial tissue of the anterior cruciate ligament. The femoral part of the synovia had retracted and was locked between the femoral attachment of the anterior cruciate ligament and the femur condyle. Both patients showed clinical signs of an acute lesion of the medial meniscus with loss of extension and tenderness on palpation over the medial joint cavity. After complete arthroscopic resection of the interfered soft tissue, both patients had full range of motion and no signs of any knee discomfort.

    ABSTRACT: Eine angeborene Hiatushernie stellt einen Risikofaktor für die Ausbildung einer Refluxösophagitis und damit einer Barrett-Schleimhaut dar, die auch nach früher operativer Korrektur dieses Defekts fortbestehen kann. Auf diesem Boden entwickelte sich bei unserem Patienten ein gering differenziertes Adenokarzinom, das wir im Rahmen einer Ösophagusresektion mit Magenschlauchbildung und Lymphadenektomie erfolgreich behandeln konnten. A congenital hiatal hernia is a risk factor for reflux esophagitis and therefore Barrett’s esophagus which may persist even after early surgical correction of the defect. On this basis our patient developed a poorly differentiated adenocarcinoma which could be successfully treated by an esophagus resection with gastric tube formation and lymphadenectomy.     ABSTRACT: Die posttraumatische Peritonitis ist die häufigste und schwerste Komplikation nach einem abdominellen Trauma. Das so induzierte septische Multiorganversagen bedingt eine langwierige intensivmedizinische Behandlung, ist schwer zu therapieren und mit einer hohen Letalität behaftet. In der nachfolgenden Übersicht werden die ätiologischen Faktoren und die möglichen therapeutischen Konzepte vorgestellt und diskutiert, die dazu beitragen können, die immer noch unbefriedigenden Behandlungsergebnisse zu verbessern. Posttraumatic peritonitis and abdominal sepsis are frequent and serious complications following severe abdominal injuries. When accompanied by multiorgan failure, consequences are prolonged stay in ICU, difficult treatment, and high mortality. The following review presents the pathophysiology of posttraumatic abdominal sepsis and discusses the therapeutic options which may lead to an improvement of treatment outcome.   • Article: MALT-Lymphom des Larynx bei bekanntem Sjögren-Syndrom

    ABSTRACT: Hämatopoetische Neubildungen machen nur etwa 1% aller Larynxneubildungen aus. MALT-Lymphome („mucosa-associated lymphatic tissue“, MALT) leiten sich vom mukosaassozierten lymphatischen Gewebe ab und entstehen im Rahmen chronisch-inflammatorischer Prozesse. Patienten mit einem Sjögren-Syndrom haben ein erhöhtes Risiko für das Entstehen eines Non-Hodgkin-Lymphoms im Sinne eines MALT-Lymphoms. Bisher sind nur Fälle von MALT-Lymphomen der Speicheldrüsen, des Thymus und des Magens bei bekanntem Sjögren-Syndrom publiziert. Wir präsentieren den Fall eines MALT-Lymphoms des Larynx bei bekanntem Sjögren-Syndrom. Radiatio und Chemotherapie werden in der Literatur als Therapie der Wahl beschrieben. Hematopoietic neoplasms represent about 1% of all laryngeal neoplasms. MALT lymphoma arises from mucosa-associated lymphoid tissue and is associated with chronic inflammatory disease. Patients with Sjögren syndrome have a higher risk for development of non-Hodgkin lymphoma (MALT lymphoma). To date, only cases of MALT lymphoma of the salivary glands, thymus and stomach associated with Sjögren syndrome have been published. We present the case of a MALT lymphoma of the larynx associated with Sjögren syndrome. Radiation and chemotherapy are the first line of therapy as published in the literature.   • Article: Stellenwert von Endoskopie und Endosonographie bei chronischer Pankreatitis und benignen Pankreastumoren

    ABSTRACT: Die endoskopische retrograde Pankreatographie (ERP) und der endoskopische Ultraschall (EUS) sind den Schnittbildverfahren beim Nachweis der geringgradigen Pankreatitis überlegen, erfassen aber Gangveränderungen bei mäßig- und schwergradiger chronischer Pankreatitis zuverlässiger als bei geringgradiger. EUS erlaubt über die Beurteilung von Veränderungen des Pankreasgangs hinaus die Erfassung von Parenchymveränderungen. Aufgrund des Risikos einer Post-ERP-Pankreatitis ist die ERP aus der Diagnostik weitgehend verdrängt worden. Dagegen ermöglicht die endoskopische retrograde Cholangiopankreatographie (ERCP) einen konkurrenzlosen Zugang zur interventionellen Therapie entzündlicher Veränderungen des biliopankreatischen Gangsystems, bei retrograd nicht kanülierbarer Papille auch im Rendezvousverfahren mit der EUS-gesteuerten Sondierung des Ductus Wirsungianus. Trotz des technischen Erfolgs der endoskopischen Verfahren hat sich im prospektiven Vergleich die chirurgische Gangdekompression zur Befreiung von pankreatitischen Schmerzen als überlegen erwiesen. Auch ein biliärer Drainageerfolg ist zumindest bei kalzifizierender Pankreatitis dauerhaft mit chirurgischen Verfahren eher als durch wiederholtes Multistenting zu erreichen. Die perorale transgastrale transmurale Therapie postpankreatitischer Nekrosen eröffnet eine weitere Option über die chirurgische Nekroseausräumung hinaus. Endoscopic retrograde pancreatography (ERP) and endoscopic ultrasound (EUS) are superior to cross-sectional imaging procedures for detection of low-grade pancreatitis, but detection of duct alterations is more reliable by middle and high-grade chronic pancreatitis than by low-grade. In addition to assessment of alterations in the pancreatic duct, EUS also allows detection of parenchymatous alterations. Because of the risk of post-ERP pancreatitis, ERP has been mostly eliminated from diagnostic procedures. In contrast, endoscopic retrograde cholangiopancreatography (ERCP) allows an unrivalled access to interventional treatment of inflammatory alterations of the biliopancreatic duct system, by retrograde, non-penetrable papillae even in the rendezvous procedure with EUS-assisted probing of the Ductus Wirsungianus. Despite the technical success of endoscopic procedures, surgical duct decompression has proven to be superior for relief from pancreatitic pain. Biliary drainage is also more likely to be successful on a permanent basis using surgical procedures than by repeat multi-stenting, at least by calcifying pancreatitis. Peroral transgastral transmural therapy of postpancreatitic necroses opens up further options over surgical removal of necroses.

  • Article: Das seltene Sialadenoma papilliferum am harten Gaumen

    ABSTRACT: Das Sialadenoma papilliferum stellt einen seltenen benignen Tumor der Speicheldrüsen dar, der typischerweise in der Mundhöhle lokalisiert ist. Als Hauptlokalisationen werden, entsprechend der Verteilung der kleinen Speicheldrüsen in der Mundschleimhaut, der harte und weiche Gaumen genannt. In der Literatur finden sich wenige Kasuistiken zu dieser Tumorentität. Es wird der Fall eines 63-jährigen Patienten beschrieben, bei dem das Sialadenoma papilliferum im Rahmen einer hals-, nasen-, ohrenärztlichen Untersuchung zur Abklärung einer progredienten Dysphagie zufällig diagnostiziert wurde. Bei dem Patienten bestand ein Zustand nach Ösophagektomie bei infrabifurkalem Ösophaguskarzinom ein Jahr zuvor. Die histologische Sicherung erfolgte durch eine Biopsie am Übergang vom harten zum weichen Gaumen rechts. Der Fall wird mit 39 weiteren Kasuistiken aus der Literatur verglichen. Die Therapie der Wahl stellt die chirurgische Resektion des Tumors dar. Hierbei sollte eine In-sano-Resektion angestrebt werden, da in der Literatur eine hohe Rezidivrate bei unvollständiger Entfernung beschrieben wird. Sialadenoma papilliferum is a rare tumour of the salivary glands that usually occurs in the oral cavity. Following the course of the salivary glands of the oral mucosa, the hard and soft palate are the most common locations of this tumor. In the international literature, only a few case reports about this entity can be found. We present a 63-year-old patient with a sialadenoma papilliferum accidentally found during diagnostic procedures for progredient dysphagia. Suffering from infrabifurcal cancer of the oesophagus, the patient had undergone partial oesophagectomy 1year before. Histological verification was performed with a tumor biopsy at the border of hard and soft palate, on the right side. This case is compared with 39 further cases reported in the literature. The therapy of choice is resection of the tumor. Because of the high frequency of recurrence, in sano resection should be planned.

    ABSTRACT: We introduce the space BLD(Ω) consisting of all fields u:W® \mathbbRn u:\Omega \to {\mathbb{R}^n} defined on a domain W Ì \mathbbRn \Omega \subset {\mathbb{R}^n} , whose symmetric gradient satisfies òW | e(u) |ln( 1 + | e(u) | )dx < ¥ . \int\limits_\Omega {\left| {\varepsilon (u)} \right|\ell n\left( {1 + \left| {\varepsilon (u)} \right|} \right)dx < \infty } . These fields of bounded logarithmic deformation form a proper subspace of the class BD(Ω) consisting of all functions having bounded deformation. Using the Reshetnyak representation formulas, we prove that BLD(Ω) is compactly embedded in Lp( W; \mathbbRn ) {L^p}\left( {\Omega; {\mathbb{R}^n}} \right) even for p = n/(n − 1). The space BLD(Ω) plays an important role in the theory of plasticity with logarithmic hardening, as well as in the modelling of Prandtl–Eyring fluids. Bibliography: 30 titles.

    ABSTRACT: Uniqueness and regularity results for local vector-valued generalized minimizers and for local stress tensors associated to variational problems with linear growth conditions are established. If the energy density f has structure f(Z) = h(|Z|), only very weak ellipticity assumptions are required.   • Article: Stimmstörungen bei Kindern und Jugendlichen

    ABSTRACT: Die Heiserkeit ist das Leitsymptom von Stimmstörungen bei Kindern und Jugendlichen. Sie treten international mit einer Häufigkeit von 6–25% auf. Übersichtsartig werden die angeborenen und erworbenen organischen sowie die funktionellen Ursachen einschließlich der sekundär-organischen Veränderungen der Stimmlippen, ihre Pathomechanismen und die in dieser Altersgruppe typische Symptomatik dargestellt. In der klinischen Routine sollte bewusst auf Symptome einer kindlichen Stimmstörung geachtet und jede längerfristige Heiserkeit auch in dieser Altersgruppe fachärztlich durch die alters- und entwicklungsentsprechenden Methoden abgeklärt werden. Liegt eine Indikation vor, sollten alle heute verfügbaren therapeutischen Optionen diskutiert werden. Im vorliegenden Beitrag werden vorbereitende Informationen für die Eltern über die bevorstehende fachärztliche Abklärung erläutert. Hoarseness is the leading symptom of dysphonia among children and adolescents. The incidence is evaluated internationally to be 6-25%. In an overview the hereditary and acquired organic and functional causes including secondary organic lesions of the vocal folds, the pathomechanisms and the symptoms typical for this age group are presented. In clinical routine, symptoms of dysphonia in children and adolescents should be consciously looked for and any long-term hoarseness should be examined by specialized physicians, even in this age group, using methods suitable to the age group and the developmental stage. When the appropriate indications are present all therapeutic options currently available should be taken into consideration. Furthermore, this paper provides information for the parents concerning the imminent phoniatric examination.

    ABSTRACT: Bei lang anhaltender, ausgeprägter Stuhlinkontinenz muss immer auch an einen Tumor gedacht werden. Auch wenn beispielsweise rund 90% der Malignome des Analtrakts Karzinome sind, sollte weiter diagnostisch differenziert werden, da die therapeutischen Konsequenzen von Fall zu Fall erheblich voneinander abweichen können. In unserem Fall handelt es sich um ein in dieser anatomischen Region sehr seltenes, spezielles Non-Hodgkin-Lymphom, das prognostisch als infaust einzustufen ist. Symptoms of long-lasting extreme fecal incontinence should always give rise to the suspicion of a tumor. Even when approximately 90% of malignant growths of the anal tract are carcinomas, a differential diagnosis should still be pursued because the therapeutical consequences can be substantially different and case-dependent. The case described here concerns a special form of non-Hodgkin lymphoma which is rare for this anatomic region and the prognosis is more likely to be classified as incurable.

    ABSTRACT: Die wissenschaftliche Recherche in der Medizin ist derzeit durch die systematische Suche in Datenbanken nach Artikeln in Print-Medien geprägt.Zwischen der Erstveröffentlichung aktueller Ergebnisse als Vortrag und dem Erscheinen in den Fachzeitschriften ergibt sich oft eine erhebliche zeitliche Latenz. Durch die Entwicklung neuer Komprimierungsalgorithmen für die audiovisuelle Digitalisierungssoftware ist seit 1999 die zeitnahe Übertragung und Archivierung multimedialer Beiträge im Internet in vertretbarer Bild- und Tonqualität möglich. Die Sektion Telemedizin der Interdisziplinären Arbeitsgruppe bildgestützte chirurgische Navigation der Universität Leipzig entwickelte weltweit erstmalig eine medizinisch-wissenschaftliche Telemedizin-Datenbank, die über eine kostenlose Homepage frei zugänglich ist.Alle Beiträge werden vor der Aufnahme in die “medicstream©-Datenbank” durch ein autorisiertes Redaktionskollegium überprüft.Derzeit können 392 Beiträge mit dem RealPlayer® oder dem Windows Media Player® angesehen werden.Die Qualität der Bild- und Tonübertragung ist von der empfängerseitigen Datenübertragungsrate abhängig. In einem Auswertungszeitraum von 242 Tagen registrierten wir 44.199 Zugriffe und 4.488 Besuche. Searching the medical literature is mainly characterized by the use of databases.The time for reviewing and printing a paper causes a delay between the first presentation of scientific results in a lecture and the appearance of the appropriate journal publication. Moreover, the possibilities for the integration of multimedia data are limited.Because of the development of new compression algorithms for audiovisual digitization software, the near simultaneous transmission of scientific presentations, both visually and aurally, has been possible since 1999. The section telemedicine of the interdisciplinary study group on image guided surgical navigation from the University of Leipzig has developed “medicstream” as a unique, audiovisual, scientific internetdatabase. It allows the documentation of presentations at congresses and courses, including the discussion,over a freely accessible, widely available homepage, using a home developed streaming technology.All presentations are examined by an authorized editorial committee consisting of representatives with different medical specializations for content before the admission to the data base.A total of 392 presentations from seven scientific meetings can be selected by an integrated search machine and viewed with Windows Media Player or Real Player using a conventional or faster internet connection to the homepage www.medicstream.de.The quality of the audiovisual transmission depends on the receiver-lateral data transmission rate,whereby the minimum variant of 56 kB/s is characterized by good detectability of the contents. In an analysis period of 242 days,we registered 44,199 accesses and 4,488 attendances. The telemedicine data base “medicstream” can optimize quality and extend medical education by live-streaming as well as by archiving scientific presentations as audiovisual data.

    Article: Photoproduction of mesons on nuclei near the production threshold

    ABSTRACT: The photoproduction of w \omega mesons on LH2 , C and Nb has been measured for incident photon energies from 900 to 1300MeV using the CB/TAPS detector at ELSA. The w \omega lineshape does not show any significant difference between the LH2 and the Nb targets. The experiment was motivated by transport calculations that predicted a sensitivity of the w \omega lineshape to in-medium modifications near the production threshold on a free nucleon of Eglab=1109\ensuremath{E_{\gamma}^{lab}=1109} MeV. A comparison with recent calculations is given.

    ABSTRACT: This paper gives an overview of the development of a novel biped walking machine. The robot is designed as an experimental system for studying biped locomotion based on torque controlled joints. As an underlying drive technology, the torque controlled joint units of the DLR-KUKA-Lightweight-Robot are employed. The relevant design choices for using this technology in a biped robot with integrated joint torque sensors are highlighted and some first experimental results using a conventional ZMP based control scheme are discussed.

    Conference Proceeding: Learning cellular texture features in microscopic cancer cell images for automated cell-detection

    ABSTRACT: In this paper we present a new approach for automated cell detection in single frames of 2D microscopic phase contrast images of cancer cells which is based on learning cellular texture features. The main challenge addressed in this paper is to deal with clusters of cells where each cell has a rather complex appearance composed of sub-regions with different texture features. Our approach works on two different levels of abstraction. First, we apply statistical learning to learn 6 different types of different local cellular texture features, classify each pixel according to them and we obtain an image partition composed of 6 different pixel categories. Based on this partitioned image we decide in a second step if pre-selected seeds belong to the same cell or not. Experimental results show the high accuracy of the proposed method and especially average precision above 95%.

  • Article: The CMA-ES on Riemannian Manifolds to Reconstruct Shapes in 3-D Voxel Images

    ABSTRACT: The covariance matrix adaptation evolution strategy (CMA-ES) has been successfully used to minimize functionals on vector spaces. We generalize the concept of the CMA-ES to Riemannian manifolds and evaluate its performance in two experiments. First, we minimize synthetic functionals on the 2-D sphere. Second, we consider the reconstruction of shapes in 3-D voxel data. A novel formulation of this problem leads to the minimization of edge and region-based segmentation functionals on the Riemannian manifold of parametric 3-D medial axis representation. We compare the results to gradient-based methods on manifolds and particle swarm optimization on tangent spaces and differential evolution.

  • Article: Characterization of core/shell nanoparticle thin films for gas analytical applications

    ABSTRACT: Gas analytical microsystems such as the Karlsruhe micro nose (KAMINA) can be improved toward higher sensitivity when using SnO2 nanoparticle thin films instead of well-established sputtered layers. For achieving long-term stability, such nanoparticles must be prevented from growing and agglomerating. A very promising approach is to coat SnO2 nanoparticles with an open-pored SiO2 shell, simply acting as a spacer, while preserving electrical contact between adjacent core particles. Such nanoparticulate thin films can easily be realized by the gas-phase Karlsruhe microwave plasma process (KMPP). This provides an all-in-one approach to synthesize particles with diameters less than 10 nm, to coat them in situ with a protective ultrathin SiO2 shell in a downstream step and to finally deposit the core/shell particles solvent-free onto prefabricated micro devices.The present study focuses on the surface analytical characterization of 200 nm thin films consisting of SnO2 core/SiO2 shell nanoparticles with various shell designs. For this purpose, the SiO2 shell thickness was systematically increased while keeping the SnO2 core size constant. X-ray photoelectron spectroscopy (XPS) provides information concerning chemical binding states and shell thickness in a nondestructive manner. Angle-resolved XPS together with transmission electron microscopy (TEM) validate the achieved core/shell structure. In the case of the desired ultrathin SiO2 shells, low-energy ion scattering (LEIS) is solely the suitable means to prove that Sn and Si are the constituents of the outermost monolayer of the spherical particles and it demonstrates the attainability of open-pored coatings.     ABSTRACT: We consider the variational inequality that describes the torsion problem for a long elasto-plastic bar. Using duality methods of the variational calculus, we derive a posteriori estimates of functional type that provide computable and guaranteed upper bounds of the energy norm of the difference between the exact solution and any function from the corresponding energy space that satisfies the Dirichlet boundary condition.

    ABSTRACT: Mobile platforms equipped with several steering wheels are known to be omnidirectional, i.e., able to independently translate and rotate on the plane. As an improvement to this design, the Justin mobile platform also possesses the ability to vary its footprint over time by extending/retracting the wheel legs during motion. In this paper, we discuss the kinematic modeling and control issues for such a platform. The goal is to obtain a tracking controller which is able to realize an arbitrary linear/angular platform motion while, at the same time, independently expanding/retracting each leg. Experimental results support the proposed approach.

  • Conference Proceeding: Rollin' Justin - Design considerations and realization of a mobile platform for a humanoid upper body

    ABSTRACT: Research on humanoid robots for use in servicing tasks, e.g. fetching and delivery, attracts steadily more interest. With Rollin' Justin a mobile robotic system and research platform is presented that allows the implementation and demonstration of sophisticated control algorithms and dexterous manipulation. Important problems of service robotics such as mobile manipulation and strategies for using the increased workspace and redundancy in manipulation task can be studied in detail. This paper gives an overview of the design considerations for a mobile platform and their realizations to transform the formerly table-mounted humanoid upper body system Justin into Rollin' Justin, a fully self-sustaining mobile research platform.
