  Oculoauriculovertebral spectrum (OAVS) is a rare congenital malformation consisting of epibulbar dermoids, lid colobomas, auricular deformities, hypoplasia of the soft and bony tissues of the face,Oculoauriculovertebral spectrum (OAVS) is a rare congenital malformation consisting of epibulbar dermoids, lid colobomas, auricular deformities, hypoplasia of the soft and bony tissues of the face, associated oral deformities and vertebral anomalies. This report presents a child with a choroid of the right eye, coloboma of the upper eyelid, epibulbar dermoid of the left eye, mandibular hypoplasia, facial asymmetry, bilateral complete cleft lip and palate, hypoplasia of the left alar cartilage, appendage of the left nose, butterfly vertebral defects of Th-1, Th-2 and abnormality of rib XI. Widened sulci of frontal and parietal lobes, bilateral white matter density decrease and calcifications of falx cerebelli were noted. Axial hypotony and delayed psycho-motor development were apparent. This rare case presents a range of severe clinical manifestations of oculoauriculovertebral spectrum. Despite a normal cervical vertebral column, tracheostenosis was present. It caused difficulties in tracheal intubation, creating the need for a tracheostomy, and death after a failed attempt at decannulation. This case indicates that in patients with clinical manifestations including cerebral anomalies, a risk of respiratory insufficiency should be always taken under consideration, when planning surgery.

  Temporomandibular joint ankylosis is defined as bony or fibrous adhesion of the anatomic joint components accompanied by a limitation in opening the mouth, causing difficulties with mastication,Temporomandibular joint ankylosis is defined as bony or fibrous adhesion of the anatomic joint components accompanied by a limitation in opening the mouth, causing difficulties with mastication, speaking and oral hygiene as well as inadvertently influencing mandibular growth. Surgical treatment procedures include arthroplasty of the joint cavity with or without a reconstruction and a coronoidectomy, an autogenous costochondral rib graft, distraction osteogenesis and intensive mouth-opening exercise, corrective orthognathic surgery or alloplastic joint prostheses. The authors of this study would like to provide the reader with an evidence-based review of the literature in order to determine the most efficient way to manage TMJ ankylosis and re-ankylosis. The authors have concluded that in order to achieve a satisfactory and durable effective treatment, an individualized approach is necessary in each case.

• Primary correction of nasal septal deformity in unilateral clefts during lip repair-a long-term study.

Materials, Subjects, and Methods: Before primary septal correction was introduced, specimens from the septal cartilage and the anterior nasal spine of 10 deceased newborns and infants were analyzed with the aid of a light microscope to verify the presence of any growth zone. The study group comprised 91 children with unilateral clefts who had undergone primary lip repair with septal correction. The control group comprised 29 children with unilateral clefts operated on without primary septal correction. The appearance of the nasal septum was assessed on extraoral photographs in the second week of life and then 10 to 14 years postoperatively. Results: Morphologic examination revealed fibrous connective tissue with no signs of growth between the cartilaginous septum and bone. At long-term examination, the study group was found to have a nasal septum that was straight in 75 (83%), moderately deviated in 14 (15%), and severely deviated in two (2%) patients, whereas in the control group, the septum was considered straight in four (14%) children, moderately deviated in 11 (38%), and severely deviated in 14 (48%). Conclusions: Primary septal correction may be considered safe because no growth zone exists between the septal cartilage and the anterior nasal spine. Careful primary nasal septal correction improves nose shape in a way that allows normal growth.

• Effect of pathological tooth wear on the content of calcium, magnesium, zinc and phosphate in human dentin.

  To compare the mineral content of the dentin of worn versus unworn teeth. Coronal dentin samples were collected from twenty one premolar teeth extracted for prosthetic or periodontal indications,To compare the mineral content of the dentin of worn versus unworn teeth. Coronal dentin samples were collected from twenty one premolar teeth extracted for prosthetic or periodontal indications, including 11 intact teeth and 10 teeth with a significant occlusal wear. Samples were placed in concentrated nitric acid and diluted 500 times for the analysis of calcium and magnesium content and 11 times for the analysis of zinc. Contents of calcium, magnesium and zinc ions were established by means of atomic absorption spectrometry in an oxygen acetylene flame. The content of phosphorus was established by means of spectrophotometry using a test basing on a reaction of colour phosphoric-molybdenic complex formation. Data normality was assessed with Shapiro-Wilk test. Student's t-test was used for all comparisons. A statistically significantly higher content of magnesium and a lower Ca/Mg ratio were found in worn teeth at the 5% level. The concentrations of the other minerals analysed were not significantly different between the worn and intact teeth at the 5% level. Higher magnesium content in the dentin of human worn teeth may constitute a defence reaction to dentin exposure.

  The endo-perio syndrome presenting with periodontitis is very difficult to manage due to its pathomechanism which until today has evaded clarification. Substantial loss of tooth-bearing tissuesThe endo-perio syndrome presenting with periodontitis is very difficult to manage due to its pathomechanism which until today has evaded clarification. Substantial loss of tooth-bearing tissues within the alveolar processes of the maxilla and the alveolar part of the mandible compounds the situation and complicates efforts to control the inflammatory process and restore the missing periodontal tissues, to minimize the costs for the patient, and to achieve a good cosmetic effect. The aim of this work was to assess therapeutic efforts undertaken in some cases of the endo-perio syndrome. The results of periodontic and implantologic treatment in three patients with the endo-perio syndrome caused by periodontitis are presented. Conservative treatment of the endo-perio syndrome leads to restoration of periodontal tissues, provides a good esthetic effect, and greatly minimizes the psychosocial costs due to temporary loss of a tooth in the visible part of dental arches.

• Prevalence and location of denticles on panoramic radiographs.

  Assessment of the prevalence of denticles and their location with respect to teeth groups and to dental cavities on panoramic radiographs in a randomly selected group of individuals. The materialAssessment of the prevalence of denticles and their location with respect to teeth groups and to dental cavities on panoramic radiographs in a randomly selected group of individuals. The material consisted of 165 panoramic radiographs of individuals aged 9-76 years (111 females and 54 males). Permanent teeth were studied using a negatoscope. The presence and location of denticles was determined with respect to groups of teeth and particular parts of dental cavities. Denticles were found in 51.5% of panoramic radiographs. They were present in both dental arches in 51.8% of radiographs. Altogether, 248 denticles were found, out of which 61.7% were located in the upper dental arch. Pulp stones were found in pulp chambers only, usually in filled molars, in 69.4% of radiographs. Over 50% of all denticles were found in the first molar teeth. (1) The prevalence of denticles exceeds 50%. (2) Denticles occurred in pulp chambers in 69.4% of the individuals. In 91.6% of cases, denticles were found in multirooted teeth, mainly in the first molars and usually under fillings. These findings are a confirmation that denticles appear during chronic inflammation as the consequence of pulp irritation caused by caries or fillings.

• Overall and anterior Bolton ratio in Class I, II, and III orthodontic patients.

  The aim of the investigation was to compare overall and anterior Bolton ratios in different malocclusion groups with Bolton's standards. The material comprised 600 pre-treatment study casts (262The aim of the investigation was to compare overall and anterior Bolton ratios in different malocclusion groups with Bolton's standards. The material comprised 600 pre-treatment study casts (262 males and 338 females, aged 12-25 years), selected from the models of 3088 patients who had applied for orthodontic treatment based on the following criteria: permanent dentition from the first right molar to the first left molar and no interproximal caries or restorations. There were 162 Class I, 144 Class II division 1, 155 Class II division 2, and 139 Class III patients. Statistical analysis of the data was undertaken using a Student's t-test. Statistically significant differences were found for the mean overall ratio when compared with the original Bolton norm for the whole study group, as well as for patients with Class I and III malocclusions when the mean anterior ratio was compared with the original Bolton norm. Significant differences were observed in all malocclusion groups for both genders. Discrepancies exceeding 2 SD were found in 31.2 per cent of the studied population for the anterior ratio when compared with Bolton's norm. The highest mean values for anterior ratio were in males with Class I (79.1) and Class III (80.1) malocclusions.

• Lobar holoprosencephaly with a median cleft: case report.

  Holoprosencephaly is a congenital abnormality of the prosencephalon associated with median facial defects. Its frequency is 1 in 250 pregnancies and 1 in 16,000 live births. The degree of facialHoloprosencephaly is a congenital abnormality of the prosencephalon associated with median facial defects. Its frequency is 1 in 250 pregnancies and 1 in 16,000 live births. The degree of facial deformity usually correlates with the severity of brain malformation. Early mortality is prevalent in severe forms. This report presents a child with lobar holoprosencephaly accompanied by median cleft lip and palate. The treatment and 9 months' follow-up are presented. This unique case shows that holoprosencephaly may present different manifestations of craniofacial malformations, which are not always parallel to the severity of brain abnormalities. Patients with mild to moderate brain abnormalities may survive into childhood and beyond.

SUBJECTS AND METHODS: Sixty two patients were subjected to magnetic stimulation with low frequency magnetic fields of mean induction 3 microT at the first intervention and 4 microT at the following fourteen ones. Saliva was sampled before magnetic stimulation and after the 5th, 10th and 15th interventions. The contents of calcium and magnesium ions were measured by means of atomic absorption spectrometry. The content of fluoride was determined using an ion-selective electrode. RESULTS: No statistically significant differences were found between the calcium concentrations before magnetic stimulation and after 5, 10 and 15 interventions. Statistically significant differences in the magnesium concentrations were observed only between 10th and 15th interventions. No statistically significant differences in fluoride concentrations were found. CONCLUSION: Low frequency magnetic fields have no or weak influences on the content of calcium, magnesium and fluoride in saliva.

• Spacing in deciduous dentition of Polish children in relation to tooth size and dental arch dimensions.

SUBJECTS AND METHODS: Dental casts of 50 preschool children free from malocclusion, were analysed. The width of each space was measured as the smallest distance between the proximal surfaces of the neighbouring teeth, parallel to the occlusal plane. Crown width was measured as the greatest distance between the mesial and distal surfaces of the crown, parallel to the occlusal plane. Intercanine widths were measured between cusp tips of upper and lower canines. Intermolar widths were measured between the central fossae of the second molars. Arch lengths were measured from the midpoint between the central incisors to the midpoint between the distal surfaces of the deciduous second molars. RESULTS: The primary spaces were present in the upper dental arch in 90% children, on the average 2.09mm in maxilla and 1.25mm in mandible. Their widths were dependent on upper intercanine width. Secondary spacing was present in 92% children in the upper dental arch (mean 2.86mm) and 90% in the lower (mean 3.08mm). Lower secondary spacing was correlated to lower intercanine width and lower arch length. Total absence of interdental spacing was found in 4% children. CONCLUSIONS: Interdental spacing of the deciduous dentition is present in 96% of Polish children. Children with wider intercanine widths have wider primary spaces in maxilla and wider secondary spaces in mandible.

• Orthodontic treatment of a patient with severe class II malocclusion, bony fibrous dysplasia, root resorptions, and missing four premolars.

  A case of distal occlusion is presented with root resorptions and missing premolars, treated at the end of the growth period with Herbst appliance. Orthodontic treatment in non-progressive fibrousA case of distal occlusion is presented with root resorptions and missing premolars, treated at the end of the growth period with Herbst appliance. Orthodontic treatment in non-progressive fibrous dysplasia is assessed. Anamnesis, results of extraoral and intraoral examinations, typodonts, panoramic view, hand-wrist radiogram, and cephalogram were obtained in a 14-year-old boy with severe distal occlusion, root resorptions resulting from previous orthodontic treatment, fibrous dysplasia of bone, and missing four premolars. Successful treatment was achieved with Herbst appliance followed by fixed appliances. Bands were placed on upper second molars and lower first premolars and the Herbst appliance was positioned forcing the mandible forward. After three months the patient was unable to position the mandible backwards. Canine and Angle Class I were achieved, overjet and overbite were reduced to 2 mm. Lower face height was reduced and the upper lip was retracted, producing lip competence. At this point the fixed appliances were removed, an upper removable retention plate was made, and a lower flexible spiral wire retainer was bonded. Active retreatment lasted 15 months. No hypermobility of any teeth nor progression or neoplastic transformation of fibrous dysplasia were noted at the end of orthodontic treatment. (1) It is possible to treat distal occlusion with Herbst appliance at the end of the growth period. (2) Root resorptions are not a contra-indication to treatment if potential benefits outweigh risks. (3) Orthodontic tooth movement appears possible and safe in patients with non-progressive fibrous dysplasia.

• The supernumerary lateral incisors--morphology and concomitant abnormalities.

  AIM: To assess the morphology of supernumerary lateral incisors and describe the concomitant malocclusion. MATERIAL AND METHODS: Intraoral examination and analysis of panoramic views as well as study casts has been proceeded in 18 patients with supernumerary laterals. The widths of the normal and supernumerary laterals were measured. RESULTS: 20 supernumerary incisors were found (in 2 patients - bilateral). In 1 patient - the width of the supernumerary tooth was the same as this of the adjacent normal teeth and in 12 patients - the widths were lower. In cases of unilateral hyperdontia, 4 of the laterals adjacent to supernumerary teeth were of the same, 9 were of lower and 1 was of higher widths than the contralateral ones. Supernumerary teeth were causing midline shift in 10 patients, significant space deficiency - in 4, ectopic eruption - in 1 and excessive overjet - in 4. In 3 patients the permanent supernumerary incisors were not yet erupted and in 1 patient the tooth was retained. CONCLUSIONS: The supernumerary lateral incisors were smaller than the adjacent ones, whereas the normal lateral incisors adjacent to supernumerary ones were smaller than the contralateral incisors. The concomitant occlusal anomalies were: excessive overjet, crowding or ectopic eruption and midline shift.

• [Treatment of crossbite with the quadhelix appliance and lower lingual arch to maintain constant lower intermolar width]

  The quadhelix appliance is often recommended for the treatment of children with crossbite. It is characterized by high effectiveness, low cost and short treatment time. Its modification with asymmetric arms enables asymmetric expansion of the upper arch. However, no clinical studies confirming such action are known. During maxillary expansion the lower arch tends to follow the maxillary teeth by tipping laterally. This phenomenon hinders crossbite treatment. The aim of the present study was to evaluate the effect of asymmetric arms of the quadhelix appliance during treatment of crossbite and to assess the effectiveness of the lower lingual arch in maintaining lower intermolar width. The study group included 20 children with crossbite. The treatment procedure comprised slow maxillary expansion with the quadhelix appliance. A lower lingual arch activated 1 mm inwards was simultaneously used in order to prevent increase in the lower intermolar width through occlusal contacts. The maxillary intermolar widths were measured between the central fossae of the right and left first permanent upper molars. Mandibular intermolar widths were measured between medial buccal cusps. The mean increase in upper intermolar width was 4.1 mm. The mean change in the lower intermolar width was -0.1 mm and was statistically insignificant. Posterior crossbite was eliminated in 16 children (80%). In 20% of children an improvement (e.g. reduction of the number of teeth in crossbite) was noted. The subtraction sum of the upper and lower intermolar widths was -5.7 mm before treatment and -1.4 mm after treatment. The upper dental arch before treatment had an asymmetric shape in all subjects. The width of the more deficient side was 19.65 mm and the wider side measured 22.3 mm. This difference was statistically highly significant (p < 0.0001). After treatment, the width of the side where the shorter arm was used was 22 mm. The width of the opposite side was 23.7 mm. The difference was statistically significant (p < 0.003). The width of the palate increased on the average by 3.5 mm. The change in overbite was statistically insignificant, enabling safe application of this method in patients with crossbite and open vertical skeletal configuration. The length of the upper arch increased by 0.2 mm (p < 0.01). The average treatment time was 17 weeks, depending on the number of teeth in crossbite before treatment (Rs = 0.58; p < 0.007). Longer treatment times (6-7 months) were reported in other studies describing patients of the same age. The quadhelix with asymmetric lateral arms is recommended for the treatment of crossbite with an asymmetric upper dental arch. The simultaneous application of lower lingual arch prevents lower intermolar expansion, thus shortening the time of treatment.
