ABSTRACT: Up to 30% of squamous cell carcinomas of the penis arise from a lichen sclerosus, but very few reports in which lichen planus was the preexisting lesion have been published. We report a male with verrucous carcinoma of penis that developed in an area of lichen planus.
A 53-year-old male presented with an exophytic tumor on the glans penis that had been present for about 6 months. The lesion developed in a previously biopsied area of lichen planus hypertrophicus. After excision, histological diagnosis was verrucous carcinoma.
The development of any subtype of squamous cell carcinoma of penis in a lichen planus can be coincidental, or a neoplastic transformation of lichen planus can take place.

ABSTRACT: Collecting duct renal cell carcinoma is an uncommon variant of renal carcinoma. Typically its behaviour is more aggressive than other forms of renal carcinoma and usually it is diagnosed at advanced stages. A 57-year-old man visited our hospital by right lumbar pain. Abdominal CT showed a enhanced mass on the right kidney. A right partial nephrectomy was done. Histological examination of the surgical specimen showed a low grade collecting duct carcinoma of the kidney. This is a new entity, with unknown behaviour but seems to be less aggressive than classical collecting duct carcinoma of the kidney. The clinical, radiological, pathological and immunohistochemical characteristics of this tumor are reported, and the literature is reviewed.

ABSTRACT: CDX1 and CDX2 are transcription factors involved in the development and maintenance of the intestinal epithelial cell. Expression of CDX2 has been reported in normal and metaplastic intestinal epithelium, and in those adenocarcinomas with that cellular origin. We have analyzed the expression of this marker in reactive and tumoral lesions arising in urinary bladder, urethra and urachus.
CDX2 was investigated through immunohistochemistry on paraffin-embedded tissue, using the labelled streptavidin-biotin method (LSAB2, Dako) with a monoclonal antibody (CDX2-88, BioGenex).
Expression of CDX2 was observed in intestinal-type cistitis glandularis, intestinal metaplasia of urinary bladder, bladder adenocarcinoma, mucinous urothelial-type carcinoma of prostatic urethra and urachal mucinous carcinoma. CDX2 was not detected in normal urothelium and prostatic glandular epithelium, Von Brunn nests, typical-type cistitis glandularis, glandular adenosis and transitional carcinoma.
Lesions, both benign and malignant, with enteric-cell morphological features show positivity for CDX2. Expression of this marker is not organ-specific but is just related to a cellular phenotype. Reactivity for CDX2 in an adenocarcinoma can be consistent with an origin in urinary tract or urachus.

ABSTRACT: We examined the presence of p53, Ki-67, bcl-2 and CK20, as detected by immunohistochemistry, and correlated with the classic variables (grade, stage and recurrence).
The authors evaluated 57 superficial transitional cell carcinomas. Biopsy specimens examined included non recurrent transitional cell carcinomas (n = 36) and recurrent transitional cell carcinomas (n = 21). Association of bcl-2, p53, Ki-67 y CK20 index immunoreactivity with tumor grade, clinical stage and tumor recurrence was examined.
Ki-67 and p53 expression were related to the degree of differentiation and recurrence of the disease. bcl-2 and CK20 were not correlated with grade, stage and recurrence of the disease.
Positivity for Ki-67 and p53 increase with grade of the disease. P53 and Ki-67 are predictors of tumor recurrence for patients with superficial transitional cell carcinoma.

ABSTRACT: Cytokeratins are intermediate filament proteins which are part of the framework of eucariotic cells. A characteristic pattern of expression of cytokeratins has been described for each type of epithelium. This pattern is maintained during the process of malignancy and metastasis. Immunohistochemical analysis of cytokeratins 7 and 20 (Ck 7 and Ck 20) has been considered particularly useful for identifying the origin of carcinomas.
A total of 122 cases of transitional cell carcinoma of urinary bladder were studied immunohistochemically with monoclonal antibodies against Ck 7 and Ck 20. The peroxidase labelled-streptavidin biotin technique was performed. Cases were considered positive when at least 1% of tumoral cells were stained.
All cases investigated expressed Ck 7 whereas Ck 20 was positive in 75% of them. Reactivity for Ck 20 was always focal. No tumor was negative for Ck 7.
Ck 7+/Ck 20+ immunophenotype is highly characteristic of transitional cell carcinoma of urinary bladder, whereas Ck 7+/Ck 20- is less frequent. Immunohistochemical determination of these cytokeratins is useful for the diagnosis when tumors are poorly differentiated and for identifying the primary site of metastatic carcinomas.

ABSTRACT: Bilateral adrenal metastases are rare, and the first manifestation from non-small cell lung cancer is also exceptional. We report a case of bilateral adrenal metastases from a poorly differentiated lung adenocarcinoma in a 50 year-old-man. The treatment was local excision of tumor and the diagnosis was histological. We comment the clinical presentation, diagnosis, treatment and evolution of this case, with special attention to the histopathological analysis of this lesion and literature review.

ABSTRACT: Unilateral and localized cystic disease of the kidney, is characterized by the substitution of either all or a portion of one of the kidneys, by no encapsulated, multiple simple cysts. We present a case--to our knowledge the first in the Spanish bibliography-, of this rare benign condition that needs to be recognized to be differentiated from more transcendent ones. Contrary to autosomal dominant polycystic kidney disease, it is unilateral, it is not familiar, and it is neither progressive nor associated with renal insufficiency or with cysts in other abdominal organs. Lack of encapsulation allows, in general, to distinguish it from multilocular cystic nephroma and cystic renal carcinoma.

Case report.
POVISA Medical Center.
A 45-year-old man with primary infertility for 13 years and with different partners; the patient has a family history of recurrent miscarriages and low fertility.
Lymphocytic karyotype and electron microscopy.
Physical examination and semen analysis.
The semen analysis revealed oligoasthenoteratospermia. The lymphocytic karyotype detected a translocation 3;22, and electron microscopy showed a lack of the central microtubule pair and peripheral doublet.
An association between translocation 3;22 and other abnormalities in infertile males has been reported, but no such association has ever been described in men whose only clinical manifestation is infertility.

ABSTRACT: The appearance of solitary late metastases of renal cell carcinoma has seldom been documented. A male patient, who 5 years ago underwent left radical nefrectomy for renal cell carcinoma, presented with a solitary pancreatic metastasis (an uncommon site of metastasis for renal cell carcinoma), which was successfully treated with partial pancreatectomy. The diagnostic, treatment and prognostic implications of solitary late metastases are discussed.

ABSTRACT: In spite of the numerous surgical techniques described, management of urethral stenosis continues to be an unresolved problem. Free graft urethroplasty is indicated in selected cases and several organic and synthetic materials have been described for this use. Our group reviews the synthetic alloplasts used for partial or total replacement of the male urethra. The search for an appropriate alloplast for urethral replacement has been rather frustrating. Complication rates are still too high to allow routine usage. The most encouraging results are those with new absorbable materials which cause minimal inflammatory reactions of a foreign body type. These are readily available and allow urethral replacement using tissues regenerated from removed ends.

ABSTRACT: Occurrence of prostatectomy sections with minimum or no tumour volume have been increasingly seen over the last few years in spite of a preoperative histology-based diagnosis of adenocarcinoma with a needle biopsy ("evanescent cancer phenomenon" ECP). Our group has reviewed 145 prostatectomies performed in our hospital between January 1988 and October 1997, and found 3 cases of ECP (2%). To explain this phenomenon patients must be divided into two groups. In those who receive preoperative hormonal blockade, treatment may result in a drastic reduction of tumoral volume as well as microscopic changes that make malignancy recognition more difficult. ECP occurs in less than 5% of these prostatectomies. In patients with no previous hormonal treatment, ECP is extremely rare and any explanation comes as a consequence of the extended use of screening methods that lead to the detection of very small tumours, probably of little clinical relevance, that are found in the needle biopsy. Subsequently, however, no residual neoplasia is seen in the section either due to technical limitations of the pathoanatomical methods or because of a true, complete ablation of the lesion through the biopsy.

ABSTRACT: In spite of the long-standing use of the appendage in urological reconstructive surgery, it wasn't until the 80's that became popular. Anatomical and histological normality is the major consideration to allow use of the appendage in autografting. Over a 9-year period, performance of incidental appendicectomies in abdominal major surgery in adult patients was included as a surgical routine practice in our service. This paper describes the pathoanatomical findings of incidentally removed appendages, discussing the relevance of such findings in the incorporation of the appendage to the urinary tract.

ABSTRACT: There is renewed interest on ureterosymoidostomy due to the application of gut detubulisation principles that provide a low pressure reservoir in continuity with the colon. We contribute our experience with 15 detubulised ureterosymoidostomies, 13 of the Mainz II type and 2 with dual detubulisation. Mean follow-up was 12.6 months. Continence was achieved in all cases immediately. Total continence rate was 92.8% with mean frequency of passing stools/urine of 4.4 times during daytime and 2.5 times at night. Detubulised ureterosymoidostomy is a means for an effective continent internal urinary bypass that provides patients with good quality of life.

ABSTRACT: Contribution of one case of prostate cancer presenting with a large abdominal mass associated to anaemia and weight loss. Serum PSA level was greater than 10,000 ng/mL. Imaging studies showed large retroperitoneal mass that led to other differential diagnoses. Biopsy revealed findings suggestive of adenocarcinoma with a distinct immunohistochemical pattern. No metastasis was evidenced in bone scan. Complete hormonal blockade resulted in significant decreased PSA levels and almost complete involution of adenopathies, as well as overall improvement.

ABSTRACT: We report the case of a 68-year-old woman with a cellular schwannoma in the retroperitoneum, which recurred 16 months after resection. This rare tumor is generally considered as benign in spite of its pseudosarcomatous histologic appearance. Immunohistochemical positivity for S-100 protein is very useful for the diagnosis. Recommended treatment is surgical excision but incomplete resections can be followed by recurrences.

ABSTRACT: To report a case of traumatic testicular dislocation, an unusual complication of pelvic trauma.
A case of traumatic testicular dislocation that had been diagnosed two months after the initial injury is presented. CT and color doppler US revealed a viable right testis displaced in the inguinal subcutaneous region. The testis was repositioned in the scrotum and orchidopexy was performed. Follow-up control evaluation at 6 months revealed a normal testis in the right hemiscrotum.
Traumatic testicular dislocation is a rare complication that may be undetected at the time of the initial injury. Surgical reduction and repositioning achieve good results.

ABSTRACT: An uncommon complication of prostatic transrectal biopsy is described.
A case of peritonitis arising from a recto-peritoneal fistula after transrectal biopsy of the prostate is reported. The patient presented symptoms of peritonitis 15 days after undergoing ambulatory transrectal prostatic biopsy elsewhere. The imaging studies showed intraprostatic hypodense collection and peritonitis due to a recto-peritoneal fistula. The diagnosis was confirmed by laparotomy. The patient died 7 days postoperatively due to acute renal failure.
Transrectal biopsy of the prostate is a simple procedure, but it is not free from serious complications.

ABSTRACT: The majority of ureteropelvic metastatic tumors are of non-urologic origin. A case of metastatic clear cell adenocarcinoma to the renal pelvis in a patient with a bilateral, asynchronous renal tumor is presented. Conservative surgical treatment involved open pyelotomy and local resection of the tumor. At the 12-month follow-up, the patient is alive, asymptomatic and not on dialysis. The hypothesis for the mechanisms of dissemination and the relevance of conservative treatment are discussed.
