ABSTRACT: The effects of gestodene in a triphasic combination with size of comedones. The investigators' and patients' global ethinylestradiol (Tri-Minulet®) on mild to moderate acne assessments of severity of acne both demonstrated improve- were studied in 25 healthy young women over six treatments after treatment cycles in an open-label, non-comparative, single- of sex hormone binding globulin were increased at all center study. A decrease in the mean total lesion count of 11.9 lesions (95% CI 5.4 to 18.5)from a mean baseline value of 26.1 lesions was observed at cycle 6, with the number of papules being the most reduced. The mean overall severity of acne grade was reduced by 0.8 points (95% CI 0.3 to 1.3). A trend towards reduction of sebum production was also observed. Cyanoacrylate skin surface strippings revealed a reduction in the number and size of comedones. The investigators' and patients' global assessments of severity of acne oboth demons demonstrated improvements after treatment (binomial test p < 0.001). Levels of sex homone binding globlin were increased at all visits, and levels of progesterone, androstenedione and androstanediol glucuronide were reduced after treatment. No patient was withdrawn from the study because of adverse events. It was concluded that treatment with gestodene and ethinylestradiol in a triphasic formulation for six treatment cycles was well tolerated and produced a trend to obvious improvement in mild to moderate acne.

ABSTRACT: Acne is a multifaceted disorder. Its clinical presentation differs according to the age and gender of the subjects. Acne of the adolescent is a frequent disorder. Some topical and oral antibiotics have proven their efficacy. However, the risk of bacterial resistance may be a concern for the clinicians and their patients. Hence, a therapeutic strategy without antibiotics merits to be considered. Retinoids have a place of choice in this therapeutic strategy. Benzoyl peroxide and miconazole are also active and valuable agents in this therapeutic algorithm.

ABSTRACT: A consultation motivated by an enlarged red leg is a common situation. The causes are multiple and the diagnosis primarily relies on case history and physical examination distinguishing inflammatory red legs from erythemato-violaceous presentations. Additional diagnostic search cannot be described as a single standardized procedure. Each disease calls for specific and different paraclinical investigations. The most frequent cause is erysipelas or a thrombophlebitis. Some vascular malformations and some functional circulatory disturbances are also possible, as well as sequelae of panniculitis or borreliosis.

ABSTRACT: Skin is subjected to many environmental threats, some of which altering the structure and function of the stratum corneum. Among them, surfactants are recognized factors that may influence irritant contact dermatitis. The present study was conducted to compare the variations in skin capacitance and corneosurfametry (CSM) reactivity before and after skin exposure to repeated subclinical injuries by 2 hand dishwashing liquids. A forearm immersion test was performed on 30 healthy volunteers. 2 daily soak sessions were performed for 5 days. At inclusion and the day following the last soak session, skin capacitance was measured and cyanoacrylate skin-surface strippings were harvested. The latter specimens were used for the ex vivo microwave CSM. Both types of assessments clearly differentiated the 2 hand dishwashing liquids. The forearm immersion test allowed the discriminant sensitivity of CSM to increase. Intact skin capacitance did not predict CSM data. By contrast, a significant correlation was found between the post-test conductance and the corresponding CSM data. In conclusion, a forearm immersion test under realistic conditions can discriminate the irritation potential between surfactant-based products by measuring skin conductance and performing CSM. In vivo skin preconditioning by surfactants increases CSM sensitivity to the same surfactants.

ABSTRACT: Acne is a multifactorial disorder affecting the vast majority of adolescents and young adults. Among the therapeutic armamentum, estroprogestative contraception can be offered to young women. The choice must, however, be carefully targeted because the estroprogestative associations do not show similar anti-acne efficacy. A new contraceptive associating 20 micrograms of ethinyl estradiol and 100 micrograms of levonorgestrel (Lowette) has proven its clinical efficacy in this indication.

ABSTRACT: Hair shedding and hair thinning have been reported to be affected by dandruff and seborrhoeic dermatitis. The present study was conducted in 150 men presenting with telogen effluvium related to androgenic alopecia associated with dandruff. They were randomly allocated to three groups receiving each one of the three shampoos in the market containing either 1% ketoconazole (KTZ), 1% piroctone olamine (PTO) or 1% zinc pyrithione (ZPT). Shampoos had to be used 2-3 times a week for 6 months. Hair shedding during shampoo was evaluated semiquantitatively. Hair density on the vertex was evaluated on photographs using a Dermaphot. Trichograms were used for determining the anagen hair percentage and the mean proximal hair shaft diameter using computerized image analysis. The sebum excretion rate (SER, mug cm(-2) h(-1)) was also measured using a Sebumeter. The three treatments cleared pruritus and dandruff rapidly. At end point, hair density was unchanged, although hair shedding was decreased (KTZ: -17.3%, PTO: -16.5%, ZPT: -10.1%) and the anagen hair percentage was increased (KTZ: 4.9%, PTO: 7.9%, ZPT: 6.8%). The effect on the mean hair shaft diameter was contrasted between the three groups of volunteers (KTZ: 5.4%, PTO: 7.7%, ZPT: -2.2%). In conclusion, telogen effluvium was controlled by KTZ, PTO and ZPT shampoos at 1% concentration. In addition, KTZ and PTO increased the mean hair shaft thickness while discretely decreasing the sebum output at the skin surface.

ABSTRACT: Aromas and fragrances are present in many cosmetics, some topical drugs, food and various hygiene, household and industrial products. They can be responsible for contact dermatitis. Multiple sensitizations can even involve in various combinations some fragrance compounds, a given degradation product or a contaminant. The diagnosis relies on clinical examination and oriented anamnesis. A histological examination is sometimes necessary. Specific path testing brings insight on the culprit chemical compounds.

ABSTRACT: There exists several clinical types of scleroderma. Most of them show cutaneous manifestations. Skin involvement may be isolated (morphea) or be part of a multi-organ systemic disease. Objective non-invasive functional methods are useful in order to better assess the severity and evolution of the disease and the effects of treatments. Measurements of the thickness and mechanical properties of the skin as well as the visualization of the cutaneous blood flow prove to be informative.

ABSTRACT: The density in the natural bacterial flora is quite controlled at the skin surface. The barrier to their penetration inside the body is a major function of the stratum corneum. Its more or less anfractuous structure, its hydration and acidic pH are important to maintain the microbiological equilibrium. The non-immune innate or inducible natural defenses rely on the production of epithelial peptide antibiotics (defensins, calprotectin) by keratinocytes. The phagocytic and immune defenses are superimposed to such mechanisms. This whole conundrum may be helped by, but it may also suffer from some skin care products and antiseptics.

ABSTRACT: Going in for sports is at increased risk for cutaneous infections that can be viral, bacterial, mycotic and parasitic in nature. The specific conditions of sports practice have a major influence on their occurrence. Other factors such as age, gender and genetic predisposition can also play a role. The cutaneous infections should be the target of preventive measures. To assume the worse, they should be recognized and treated without delay in order to avoid the more or less prolonged arrest of the sports activity and the disease transmission to partners.

ABSTRACT: Skin compatibility of detergent products is usually evaluated using predictive tests where products are applied on the back or the forearm of the volunteers, even if those products come more readily into contact with consumers' hands. The current study aimed at comparing the skin responsiveness of hands and volar and dorsal forearms to a surfactant solution.
Volunteers soaked simultaneously their forearms and hands in a solution of anionic surfactants, at 40 degrees C for 20 min, thrice a day and for 2 consecutive days. Skin condition on the hands, and the volar and dorsal forearms was evaluated by clinical examination and instrumental measurements.
Significant differences were observed at baseline for various skin parameters, especially those related to skin dryness. After repetitive contacts with the surfactant solution, a parallel evolution of most skin parameters was observed--i.e., an absence of change in instrumentally measured erythema and transepidermal water loss, a similar decrease in corneometry values, and a similar increase in staining of the stratum corneum strippings and in clinical dryness. However, an opposite effect of the soakings on the cohesiveness between corneocytes harvested by tape strippings from hand and forearm was observed.
For most parameters, skin responsiveness to surfactants appears to be quite similar on the hands and forearms. It is inferred that forearms can be conveniently used to predict interaction of surfactants with hands.

ABSTRACT: Latex devices release allergenic proteins which are responsible for immune IgE mediated reactions. Casual surgical gloves are most often responsible for contact urticaria. Other allergic manifestations include conjunctivitis, rhinitis, asthmatiform dyspnoea and diverse gastro-intestinal troubles. Cardiovascular collapse and anaphylactic shock are possible. Physicians and the paramedical staff are particularly affected by the allergy to latex proteins. Multi-operated patients and those under prolonged health care using latex-made medical probes also present a high-risk group for this type of allergy.

ABSTRACT: There is general concern about the possible cutaneous adverse effects of wearing garments treated with household laundry products, particularly on atopic skin. Our objective was to compare softened and non- softened fabrics in a forearm wet and dry test, under conditions simulating real-life conditions. Twenty atopic volunteers entered a single-blind 12-day (3 sessions per day) forearm wetting and drying test. Cotton fabrics were machine washed and liquid fabric conditioner was added or not to the final rinse. To simulate conditions of skin damage, a dilute solution of sodium lauryl sulphate was applied under occlusion to the forearm of each volunteer before the start of the study. Skin effects were evaluated by visual grading (redness, dryness and smoothness), squamometry and in vivo instrumental measurements (capacitance, transepidermal water loss and colorimetry). Rubbing of atopic skin with fabrics generally resulted in discrete to moderate alterations of the structure of the stratum corneum. Both for control and pre-irritated skin, all measured parameters indicated that softened fabric was less aggressive to the skin than unsoftened fabric. In the case of pre-irritated skin, the recovery of the skin was significantly faster when rubbed with softened than with unsoftened fabrics. In conclusion, softened fabrics help mitigate the skin condition in atopic patients.

ABSTRACT: Some mycotoxins are among the most toxic natural products. They are metabolites produced by food-borne and feed-borne moulds. Dirty and soiled nails, and contaminated or infected onyxis by these specific moulds can be the origin of oral contamination and mycotoxicosis. Hence, nail cleaning, correct hygiene and treatment of mould onychomycosis have a potentially great impact on health.

ABSTRACT: There are ethical concerns to conduct in vivo tests in the animal and human to provide evidence that cosmetics and other topical products are safe.
To compare two methods, namely the short-duration dansyl chloride extraction test and the microwave corneosurfametry bioassay, to test the irritation potential of concentrated surfactant systems.
Five surfactants and water were tested using the in vivo test and the bioassay.
A significant linear correlation was found between data yielded by the two procedures.
The short-duration dansyl chloride extraction test and the microwave corneosurfametry bioassay provide similar information. The latter has the advantage of being safe.
