  • Article: [MR mammography of fatty tissue necrosis].

    ABSTRACT: To evaluate MR-imaging for the differentiation of fatty necrosis (FN) of the breast. In 1016 MR mammographies FN was found in 13 patients aged 44 to 69 years. In all studies we used a proton-weighted fat suppressed sequence (STIR) and serial T1-weighted gradient echo sequences before and after bolus injection of Gd-DTPA, MRI was correlated to ultrasound and in 8 cases to mammographic findings. All 15 FN displayed fat-isointense signal on T1-weighted and on proton-weighted, fat-suppressed sequences. They were delineated by a more or less wide rim of low signal intensity with sharp border to the center. After intravenous injection of gadopentetate dimeglumine they showed no increase of signal intensity in the centre and no increase, or only a minor increase, of the rim. Ultrasound could not distinguish FN from recurrent tumour in 6 cases. 7 FN looked like atypical cysts. MR-mammography is very promising in the diagnosis of FN.

  • Article: [MR-mammography assessment of tumor response after neoadjuvant radiochemotherapy of locally advanced breast carcinoma].

    ABSTRACT: We investigated the role of magnetic resonance mammography in monitoring tumour response of locally advanced breast cancer (LABC) after neoadjuvant radiochemotherapy. 17 patients with LABS had a magnetic resonance mammography and ultrasonography before and after neoadjuvant radiochemotherapy. After neoadjuvant radiochemotherapy 14 patients showed in MR-mammography less pronounced and prolonged enhancement without washout. After treatment three patients had signal intensity-time curves still characteristic for tumour. Ultrasonography was true negative in two patients, true positive in 12 and false positive in three patients. Magnetic resonance mammography is suitable for monitoring tumour response after radiochemotherapy of LABC. However, a negative MRI does not exclude a residual tumour. Ultrasonography is of limited value in monitoring therapy of LABC.

  • Article: [Initial experiences with MR-mammography in after-care following surgical flap treatment of breast carcinoma].

    ABSTRACT: To demonstrate typical MR mammographic findings after plastic surgery of breast cancer. Postoperative (7-38 months, median 13) MR mammographic examinations of 25 patients operated for breast cancer (11 latissimus dorsi-flaps (LAT), 14 rectus abdominis myocutaneous-flaps (TRAM)), were reevaluated. The examinations were performed with a breast coil at 1 T. The sequences applied were a fat-suppressed 2-D turbo IR-sequence proton-weighted and a T1-weighted FLASH 3-D sequence as dynamic series. Scars between the myocutaneous flap and the remaining breast tissue always appeared in form of a ligament or septum. Signal and perfusion characteristics of scar tissue and muscle stalk in this investigation were clearly different from those of tumour. One tumour recurrence and one fat necrosis was found and rectified by biopsy. MR mammography is a very valuable diagnostic method for postoperative evaluation of myocutaneous flap-based therapy of breast cancer.

    ABSTRACT: Development of an empyema necessitatis under mechanical artificial respiration with high peak pressures is described on the basis of a case report on a patient in whom the phenomenon was the sequel of pneumonia with abscess formation. Conventional x-ray diagnosis can merely yield pointers towards possible differential diagnoses, whereas CT supplies an unequivocal diagnosis. X-ray image and differential diagnosis are discussed.

  • Article: ["Coral reef" arteriosclerosis: computed tomographic and angiographic findings].

    ABSTRACT: Patients suffering from "coral reef arteriosclerosis" are presented, the lesions being visualised via CT and angiography. This disease is characterised by an almost complete occlusion of the median ventricular aorta due to extreme, irregularly structured calcareous masses in the vascular lumen. The review of the literature points to the possible aetiology and pathogenesis of this disease pattern.

  • Article: [Functional angiography of the arteries near the knee joint: consequences for stent implantation?].

    ABSTRACT: Angiographic studies of the arteries adjoining the knee in 25 patients show extensive kinking and stenoses of the popliteal artery and less frequently of the distal femoral artery during flexion of the knee joint. This is due to the loss of elasticity with increasing age forcing the vessel into a tortuous course during shortening of the pathway of the popliteal artery with knee flexion. Independent of the principle of the different stents available they probably will not increase the contractility of the stented vessel in the longitudinal axis. It is to expect that after implantation of stents into the popliteal artery kinking will occur predominantly in the original segments of the vessel and at the transitions to the stented segments leading to intimal damage by shear forces thus propagating local progress of atherosclerosis.

    ABSTRACT: Rectal carcinomas are amongst the most common malignant tumours. The aim of this work was to determine whether high resolution CT with thin sections (1 mm) can provide satisfactory delineation of perirectal tumour infiltration. Correct determination of local tumour spread was possible in four out of nine patients and distinction between those tumours confined to the rectal wall and those infiltrating the perirectal tissues was possible in four out of eight patients.

  • Article: [Quantification of changes in the skeletal muscle blood circulation following revascularization using a 99m technetium-labeled tracer].

    ABSTRACT: A new 99mTc-labelled tracer (99mTc-Sestamibi) was used for the first time to demonstrate the perfusion of the skeletal muscle. In 16 patients with obstructive atherosclerosis of the lower limbs the change of perfusion of thigh and lower leg was studied with SPECT before and after vascular surgery (n = 11) or percutaneous transluminal angioplasty (n = 5). Comparative results of scintigraphic measurements and clinical observations (ankle-arm pressure, treadmill test) in 10 surgical patients (14 operated legs) showed correct positive or negative results in 86% (12/14).

  • Article: [The behavior of collaterals during percutaneous transluminal angioplasty].

    ABSTRACT: The fate of 106 collaterals was investigated following percutaneous transluminal dilatation of 113 stenoses and occlusions. Following dilatation, 78% of collaterals were unchanged and 22% could no longer be demonstrated. The success of the dilatation had no bearing on the fate of the collaterals. Collaterals beginning or ending in a dilated portion of vessel disappeared more frequently (30%) than the others (11%). Following five dilatations, some vessels were demonstrated in the area of stenosis, which had not been demonstrated previously. Evidently local events, such as displacement of arteriosclerotic plaques may lead to occlusion or reopening of vessels. Remaining collaterals may be of great importance in saving a limb if there is an acute occlusion complicating a dilatation.

  • Article: [Clinical experience with the Kensey catheter system--complications and results].

    ABSTRACT: Fifteen procedures have been carried out in 14 patients using the Kensey rotation angioplasty system. Sixteen occlusions of the superficial femoral artery, with an average length of 10.2 cm (3 to 24 cm) and one artery with multiple stenoses were treated. The following results were obtained: creation of a lumen, with subsequent balloon dilatation in eight out of fifteen procedures; in two patients the vessels were occluded shortly after the procedure, due to complications (one obstructive plaque and one haematoma); five perforations, three dissections and one peripheral embolus. The total complication rate was 11 out of 15 cases, clinical improvement was obtained in six out of 15 cases.

  • Article: Does velocity of dilatation influence the result of dilatation? A postmortem study.

    ABSTRACT: We evaluated the influence of velocity of dilatation on the success of and the vascular damage produced by dilatation. Nonatheroslerotic segments of distal superficial femoral arteries were dilated "fast" (n = 69) or "slow" (n = 45) under standard conditions with balloon catheters. The arterial wall was overstretched between 1% and more than 60%. The success of dilatation rose continuously with increasing overstretching during dilatation. The damage to the arterial wall by dilatation increased discontinuously with increasing overstretching. Dilatation success and damage patterns did not differ in the "slow" and "fast" groups.

  • Article: [Vascular recanalization using an electric thermal cauterizing catheter--in vitro experiences with a new catheter system].

    ABSTRACT: Recanalisation of arteriosclerotic occlusions by means of laser angioplasty, Kensey catheter and the "hot tip" catheter is undergoing early, successful clinical trials. The thermo-cautery catheter described in this paper uses the well known technique of cutting with high frequency electric currents in order to achieve vascular recanalisation. Seven attempts were made to recanalise occlusions at post mortem; four were successful, but perforation occurred in three. Histologically we found a narrow rim of carbonisation of tissues surrounded by a zone of homogenisation and a transitional zone.

    ABSTRACT: The prognosis and therapy of the fracture of the calcaneum largely depend on an involvement of the talo-calcaneonavicular joint. Incongruities and grated fractures in the region of the posterior joint facet are prognostically unfavourable but can be tackled by surgery. Standard X-ray films of the calcaneonavicular joint and of the tarsus do not offer sufficiently conclusive criteria. Oblique X-ray films with 60 degree internal and external rotation visualise the talo-calcaneonavicular joint almost free from overshadowing. The diagnostic value and technique of the standard and oblique X-ray films, as well as the topography of the joint facets under different projections, are demonstrated in this article by means of X-ray films of preparations and by case reports.

  • Article: [Diagnostic value of sonography and computed tomography in liver metastases].

    ABSTRACT: The diagnostic accuracy of sonography and computed tomography for detecting liver metastases was analysed in 526 patients with tumors and the findings were compared with those at laparotomy and at post mortem. The surgical protocols were analysed in respect of the primary tumour, the frequency of metastases and the number, localisation and size of metastases. A comparison of sonography and CT showed a higher sensitivity for CT (57.1%) compared with sonography (52.3%) although specificity was slightly higher for sonography. Liver metastases greater than 15 mm were detected by sonography (80.5%) and CT (92%), with much higher sensitivity. CT also proved superior, since the results depended less on the size, number and localisation of the deposits and on the primary tumor than did sonography.

    ABSTRACT: The standard radiographs in two planes of the ankle joint and of the foot do not allow an exact evaluation of the subtalar joint. Two additional views are proposed. With a dorsal flexion of the ankle joint one radiograph is obtained in 60 degrees internal rotation and the other in 60 degrees external rotation. The medial-oblique and lateral-oblique views considerably increase diagnostic information with a minor effort compared to tomography.

    ABSTRACT: Die Standardaufnahmen des Sprunggelenkes und des Fuβes in zwei Ebenen erlauben keine exakte Burteilung des unteren Sprunggelenkes. Vorgestellt werden zwei Spezialaufnahmen, die in 60° Innen-bzw. Auβenrotation und Dorsalextension im oberen Sprunggelenk angefertigt werden. Die medial-schräge und die lateral-schräge Einstellung ermöglichen bei im Vergleich zu Schichtuntersuchungen geringerem Aufwand Aufnahmen von hoher Aussagekraft.
