• [Prevention of posterior capsule opacification in experimental studies].

Moreover, gene therapy with the use of apoptose genes was applied. None of the attempts to prevent PCO was useful for effective and safe clinical treatment.

  Even the best technical and organizational methods will not prevent the infection of the spot being operated on. The risks may be caused for example by the microbial air pollution of the operatingEven the best technical and organizational methods will not prevent the infection of the spot being operated on. The risks may be caused for example by the microbial air pollution of the operating theatre. Ways of air decontamination for the purposes of air conditioning and ventilating used in the operating theatre are both up-to-date and essential. Decontamination methods involve filtration by means of HEPA, ULPA, EU-13 filters with 99.99% effectiveness for < or = 0.1 microm particles. The most suitable sterile air flow is a displacing, vertical, ceiling system e.g. 3.2 x 3.2 m according with the requirements of the laminar air flow (LAF) at the speed of 0.3-0.5 m/sec. The above system should provide 1 cfu/m3 over the operating theatre and "safe bio conditions" at 30-100 cfu/m3 during at least 3-hour work. Decontamination by means of filtration is cost effective both in production and in use: the cost of filters exchanging, humidification, the problem of Legionella pneumofila or the air cooling system. MFI (Multifunction Ion Air Cleaning) does not show the abovementioned limitations. The method makes use of the combination of the ionising processes and electrostatic attraction of particles > 0.003 um which include cells of bacteria, fungi and some viruses. MFI technology used in Nanobio E 310 (Genano) working at 250 m3/h allows for eliminating microorganisms from the air at < or = 1 cfu/m3. These decontamination methods make it possible to achieve appropriate microbiological standards when the appliance is properly used.

• [Outcome of cataract surgery in patients with pigmentary retinal degeneration]

  PURPOSE: To evaluate the results of cataract surgery in patients with RP because retinitis pigmentosa is one of the disease entities that belongs to tapeto-retinal degenerations. The occurrence of RPPURPOSE: To evaluate the results of cataract surgery in patients with RP because retinitis pigmentosa is one of the disease entities that belongs to tapeto-retinal degenerations. The occurrence of RP appearance is 1:4000 to 1:3000. MATERIAL AND METHODS: Twenty patients with RP (7 women and 13 men, 33 eyes), who underwent cataract surgery were examined retrospectively. Average age in our group was 46.6 years. Visual acuity, intraocular pressure, slip lamp examination, fundus examination, cataract morphology, visual field were taken before surgery and on discharge, on the basis of medical documentation. Control examination was taken, on average, eighty one months after cataract surgery. Nine eyes were operated by phacoemulsification, 24 eyes by means of extracapsular cataract extraction. In the same way control group of 18 patients who underwent cataract surgery without RP (33 eyes) was examined. RESULTS: In RP group in 63.6% patients on discharge from the hospital and in 60.6% patients during the control examination, improvement of visual acuity was revealed. Deterioration was noted in 18.2% of patients on discharge from hospital and in 24.2% of patients during the control examination. In the control group improvement of visual acuity was revealed in 90.9% of patients on discharge and in 97% patients during the control examination, whereas deterioration of visual acuity occurred in 6.1% patients on discharge and in 3% patients during the check examination. CONCLUSIONS: In patients with retinitis pigmentosa cataract occurs earlier then in the control group. Cataract surgery for relatively minor opacities is beneficial in patients with RP, and causes improvement of visual acuity in most of eyes undergoing surgery.

  PURPOSE: To evaluate the results of cataract surgery in children with IOL implantation. MATERIALS AND METHODS: Our research involved 106 children (156 eyes) with congenital and traumatic cataract, inPURPOSE: To evaluate the results of cataract surgery in children with IOL implantation. MATERIALS AND METHODS: Our research involved 106 children (156 eyes) with congenital and traumatic cataract, in age from 18 months to 18 years, who underwent cataract surgery with intraocular lens implantation. We evaluated visual acuity for far vision and binocular vision after surgery. RESULTS: Visual acuity over 0.5 in unilateral congenital cataract was found in 27.25%, in traumatic cataract in 73%, whereas in bilateral congenital cataract visual acuity over 0.5 we received in 75% of operated children. Full binocular vision was obtained in children with good visual acuity (0.5 and more), with appropriate alignment of the eyes: 13.1% in unilateral congenital cataract, 45.4% in traumatic cataract and 55% in congenital bilateral cataract. CONCLUSIONS: Cataract surgery with intraocular lens implantation results in increase of visual acuity and in achievement binocular vision, if surgery is carried out in the early period, after it had been diagnosed.

  PURPOSE: To describe the technique of microincision cataract surgery and to present own initial results of this procedure. MATEDAL AND METHODS: 73 eyes with senile uncomplicated cataract were operated. In 35 eyes traditional phacoemulsification was performed, in another 35 eyes phacoemulsification through microincision was done with implantation of Acri. Smart 46S and in 3 eyes with implantation of AcrySof SA30AL. Results: In first part the differences between traditional phacoemulsification and through microincision, were described in detail. In our material we noted that microincision cataract surgery gives better visual acuity and less astigmatism in early postoperative period. CONCLUSIONS: Phacoemulsification of cataract through microincision seems to be a promising technique, with some advantages over traditional phacoemulsification.

  PURPOSE: 1) To evaluate the microbacterial flora of the conjunctival sac and the sensitivity of the isolated microorganisms to 0.3% gentamicin solution and 5.0% PVP-Jodi solution, which is used toPURPOSE: 1) To evaluate the microbacterial flora of the conjunctival sac and the sensitivity of the isolated microorganisms to 0.3% gentamicin solution and 5.0% PVP-Jodi solution, which is used to disinfect conjunctival sac. 2) To evaluate the role of the adhesive foil in prevention of the infection of the operative field. MATERIAL AND METHODS: 170 patients with senile cataract were examined. Before admission to Ophthalmology Department each patient received 0.3% gentamicin solution three times a day. All patients had their conjunctival sac disinfected with the use of 5% PVP-Jodi before surgery. Patients were divided into two groups. In the first group (120 patients), first samples were taken on admission to the Ophthalmology Department. Next in the day of the procedure, in the operation room--first before and second after disinfection of conjunctival sac. Adhesive foil was not used in this group. In the second group (50 patients), we used adhesive foil before surgery and samples were taken before and after disinfection of conjunctival sac. RESULTS: On admission, positive cultures were obtained in 40.0% cases. Most of the bacterial flora were Gram-positive bacteria. The number of the positive cultures before disinfection with 5.0% PVP-Jodi solution diminished to 5.8%, compared with positive results obtained on admission to the Ophthalmology Department. Secondary infection of the conjunctival sac has been revealed in 10% after disinfection with the use of the 5.0% PVP Jodi. It was due to bacterial flora coming from the skin and limbs of the lids (in the group without adhesive foil), whereas positive samples were obtained in 2.0% in the group where adhesive foil was used. CONCLUSIONS: 1) Gentamicin significantly reduces the amount of bacterial flora in conjunctival sac in patients before cataract surgery. 2) Adhesive foil is efficient to protect operative field from infection.

  PURPOSE: Evaluation of conjunctival microflora in patients with cataract, during the early post-operative period. MATERIAL AND METHODS: One hundred twenty patients scheduled for cataract extractionPURPOSE: Evaluation of conjunctival microflora in patients with cataract, during the early post-operative period. MATERIAL AND METHODS: One hundred twenty patients scheduled for cataract extraction were evaluated. In each patient, conjunctival swab was taken directly after cataract surgery and two to three days postoperatively. Following surgical procedure all patients received topical antibiotics alone or along with steroids. The agents were placed into conjunctival sac of the operated eye. Ophthalmic ointments were administered HS and eye drops QID. RESULTS: 9.2% (11 patients) of the cultures of conjunctival swabs collected right after surgical procedure were positive for growth with predominantly Gram-positive organisms, mainly Staphyloccocus spp. These, were compared with 24.2% (29 patients) of positive cultures from conjunctival swabs collected two to three days later. 4.2% of the latter were also positive when taken in the operating room, immediately after a surgical procedure. The remaining 20.0% of positive cultures swabbed two to three days after surgery were resulted from the contamination of conjunctival sac with germs during the postoperative hospitalisation. CONCLUSIONS: There is a risk of early post-operative infection in patients undergoing cataract surgery. It is caused by the appearance of microflora in the conjunctival sac.

MATERIAL AND METHODS: 104 patients (123 eyes) operated between January 1987 and December 1997 with congenital or traumatic cataract. Serial measurements were made of axial length and corneal curvature. RESULTS: The follow up time ranged from 2 to 10 years. The ranges of axial length changes were from 0,16 to 0,93 mm. Eyes in the first 6 years of life (group I) experienced more elongation than eyes in the older years (group II and III). The corneal curvature changes ranged from 0,02 to 0,05 mm. There was no significant difference between preoperative and postoperative corneal curvature. CONCLUSIONS: The pattern of axial elongation and corneal flattening was similar in the congenital and traumatic cataract groups to that observed in normal eyes. No significant retardation or acceleration of axial growth was found in the eyes implanted with IOLs, compared with normal eyes.

MATERIAL AND METHODS: 104 patients (123 eyes) operated between January 1987 and December 1997 with congenital or traumatic cataract. Refractive power in both operated and non operated eyes, were measured. RESULTS: The operated eyes tended to become myopic. Children, who underwent surgery in the first 6 years of life had a greater myopic shift than older children. The mean myopic shift ranged from -2.33 D to -0.51 D in operated eyes and from -1.05 D to -0.49 D in non operated eyes. CONCLUSIONS: Pseudophakia in children is predicted to result in a large quantity of myopic shift, particularly in very young children. This natural myopic shift is magnified by the optical effect of a high-power IOL.

In this prospective, multicenter, double-masked trial, 37 patients were treated twice daily with timolol for 4 weeks. Then, they were randomized to one of the treatment medications for 6 weeks, after which they were treated with timolol again for 2 weeks before placed on the opposite treatment medication for 6 weeks. A total of 36 patients completed the trial. The mean baseline intraocular pressure (IOP) was 22.3 +/- 3.7 mmHg. Following 6 weeks of treatment, the mean decrease of IOP was 18.0 +/- 2.2 mmHg for timolol/dorzolamide combination and 17.4 +/- 2.0 for timolol/pilocarpine combination. The mean diurnal curve IOP was 18.1 +/- 2.2 mmHg for timolol/dorzolamide group and 16.7 +/- 1.9 mmHg for timolol/pilocarpine group. These differences were statistically significant. Statistically more patients reported ocular pain and diminished vision during use of the timolol/pilocarpine combination. This study suggest that timolol 0.5%/pilocarpine 2% fixed combination can provide at least similar efficacious reduction in IOP as timolol 0.5 %/dorzolamide 2% fixed combination in patients with primary open-angle glaucoma and ocular hypertension.

The mean diurnal curve IOP was 18.1 +/- 2.2 mmHg for TDFC and 16.7 +/- 1.9 mmHg for TPFC (p = 0.0007). At the remaining time-points (10.00, 18.00 and 20.00 hours), TPFC IOPs were statistically lower than TDFC IOPs (p < 0.03). There were statistically more unsolicited reports of vision change and ocular pain associated with TPFC (p = 0.04). Six patients were discontinued early from TPFC therapy (17%) versus two from TDFC (6%) (p = 0.13). CONCLUSIONS: This study suggests that TPFC can provide at least a similar efficacious reduction in IOP as TDFC in patients with primary open-angle glaucoma or ocular hypertension.

• [Results of cataract extraction with IOL implantation in children aged 17 months to 8 years old]

  PURPOSE: To evaluate anatomical and functional results of cataract extraction and IOL implantation, together with posterior capsulectomy and anterior vitrectomy in children. MATERIALS AND METHODS:PURPOSE: To evaluate anatomical and functional results of cataract extraction and IOL implantation, together with posterior capsulectomy and anterior vitrectomy in children. MATERIALS AND METHODS: Cataract extraction and IOL implantation with posterior capsulectomy and anterior vitrectomy was performed in 32 eyes of children between 17 months and 8 years old. Patients were followed up for a mean period 36 months (from 6 months to 5.5 years). RESULTS: Good anatomical results were achieved in all, except 2 cases. One eye required removal of opacifications located posterior to the IOL In the second eye membrane from the anterior surface of the lens were excised. Final postoperative visual acuity in eyes with bilateral cataract was 0.59, whereas in unilateral cataract 0.25. Half of the children with bilateral cataract has significantly worse BCVA in one eye. CONCLUSIONS: Cataract extraction and IOL implantation with posterior capsulectomy and anterior vitrectomy in young children gives good anatomical results. Functional results are better in bilateral congenital cataract, comparing to unilateral cases. To achieve good functional results, visual rehabilitation is necessary.

• [Astigmatism after cataract surgery]

  PURPOSE: To evaluate postoperative astigmatism after cataract surgery. MATERIAL AND METHODS: The studied group consist of 135 patients subjected to cataract surgery; 75 patients (75 eyes) hadPURPOSE: To evaluate postoperative astigmatism after cataract surgery. MATERIAL AND METHODS: The studied group consist of 135 patients subjected to cataract surgery; 75 patients (75 eyes) had extracapsular cataract extraction with 12 mm corneoscleral incision (group I); 30 patients (30 eyes) had phacoemulsification with 3.5 mm corneal incision (group II); 30 patients (30 eyes) had phacoemulsification with 3.5 mm tunnel incision (group III). Astigmatism was measured on the 6th day and 2 years after surgery. RESULTS: Mean astigmatism on the 6th postoperative day was 4, 19 D (SD = 1.20) for group I, 2.50 D (SD = 0.47) for group II and 1.79 D (SD = 0.61) for group III. The differences between all the groups were statistically significant. The astigmatism assessed 2 years after surgery returned to preoperative values and there was statistic difference only between group I and III. CONCLUSION: Two years after cataract surgery astigmatism is comparable with the preoperative values irrespective of the operation technique.
