  • Article: Ultrasonographic Evaluation of Displaced Neurovascular Bundle in Dupuytren Disease.

    ABSTRACT: PURPOSE: Neurovascular injury is a serious complication after surgery for Dupuytren disease. The purpose of this study was to evaluate the relationship between the cord and the neurovascular bundle ultrasonographically. METHODS: We included 22 healthy volunteers and 14 Dupuytren disease patients (25 fingers) in this study. We evaluated the cord and the digital artery with high-resolution ultrasound. We first investigated the effect of the angle of metacarpophalangeal joint on the position of the radial and ulnar digital arteries in volunteers without evidence of Dupuytren disease. We compared 3 parameters of the radial and ulnar digital arteries, including differences in depth, differences in lateral shift, and the shape of the cross-section of the artery, between volunteers and patients with Dupuytren disease. RESULTS: None of these parameters changed with flexion of the metacarpophalangeal joint of 0°, 30°, and 60°. Digital arteries and cords could be identified ultrasonographically in all patients, and we confirmed ultrasonographic findings by operative findings in 13 fingers. We classified the fingers into 3 subgroups based on the ultrasonographic findings: type A (n = 13), in which the cord was above the artery; type B (n = 5), in which the cord was below the artery; and type C (n = 7), in which the cord was located between the radial and ulnar digital arteries. Types A, B, and C corresponded to natatory cord/abductor digiti minimi cord, spiral cord, and central cord, respectively. Comparisons among volunteers and patient subgroups showed that the difference in depth in type B patients was significantly larger than that of the other groups. When we set the cutoff point of the difference in depth to 3 mm, sensitivity and specificity to detect the spiral cord were 80% and 76%, respectively. CONCLUSIONS: The relationship between the neurovascular bundle and the type of Dupuytren disease cord can be evaluated by high-resolution ultrasound. TYPE OF STUDY/LEVEL OF EVIDENCE: Diagnostic III.

  • Article: Fungal arthritis of the wrist caused by Candida parapsilosis during infliximab therapy for rheumatoid arthritis.

    ABSTRACT: A 60-year-old woman with rheumatoid arthritis, who had been treated with infliximab, presented with uncontrollable wrist arthritis. Fungal arthritis caused by Candida parapsilosis was confirmed by examining her aspirated joint fluid. Her infliximab therapy was interrupted, and antifungal therapy with fluconazole was started. After the fungal infection had been ameliorated, surgical debridement and arthrodesis of the wrist joint were conducted, and her symptoms completely resolved. Although fungal arthritis is rare, it should be considered as a differential diagnosis of exacerbated monoarthritis in patients treated with biological agents.

    ABSTRACT: We report a case of necrotizing fasciitis in an upper extremity that started as pyogenic flexor tenosynovitis of the hand. The patient survived without severe loss of hand function by rapid limited debridement based on the pathoanatomy of a horseshoe abscess of the hand.

  • Article: Stiffness of the first annular pulley in normal and trigger fingers.

    ABSTRACT: The cause of snapping in trigger finger is not clearly understood. The purpose of this study was to investigate the influence of stiffness of the first annular pulley on snapping in trigger finger patients with sonoelastography, a new technique for quantitative assessment of the stiffness of soft tissues. We first studied 20 healthy volunteers (all women, aged 24-78 y) to define the change of stiffness with aging and with specific fingers. We then studied 10 digits of 9 patients (all women, aged 60-78 y) with trigger finger that involved middle or ring fingers. Thickness and stiffness were evaluated by sonoelastography. Stiffness was estimated by strain ratio of subcutaneous fat to the pulley (F/P strain ratio). The patients were treated with corticosteroid injection, and the measurement was repeated 3 weeks later. In the healthy volunteers, both thickness and F/P strain ratio showed a positive correlation with age. There was no difference between the middle and ring fingers. In trigger finger patients, the pulley thicknesses of the affected finger and the adjacent normal finger before corticosteroid injection were 0.99 ± 0.19 mm and 0.60 ± 0.14 mm (mean ± SD), respectively. The F/P strain ratios of the affected finger and the adjacent normal finger before the injection were 4.2 ± 1.3 and 2.4 ± 0.63, respectively. Three weeks after the injection, snapping disappeared in all patients, the pulley thickness decreased to 0.61 ± 0.15 mm, and the F/P strain ratio decreased to 2.5 ± 0.68. The cross-sectional area for flexor tendons within the pulley did not change after the injection. Increased stiffness and thickening of the A1 pulley are considered to be causes for snapping in trigger finger, and corticosteroid injection can alleviate snapping by changing these 2 features.

  • Article: Castleman's disease along the left tracheobronchial tree with a difficult preoperative diagnosis.

    ABSTRACT: A 39-year-old man was referred to our hospital because of an asymptomatic middle mediastinal tumor. A preliminary histological diagnosis of the tumor by bronchoscopy was difficult to obtain because the tumor was located along the left tracheobronchial tree, which is difficult to approach. The tumor was resected through a right anteroaxillary thoracotomy without any major complications, and histopathological examination revealed that the lesion was Castleman's disease, hyaline-vascular type. Radiological findings of the lesion were typical; however, the rarity of the tumor made the imaging diagnosis difficult. If a lesion is located along the tracheobronchial tree, Castleman's disease should be considered in the differential diagnosis.

    Article: The effects of sheet-type absorbable topical collagen hemostat used to prevent pulmonary fistula after lung surgery.

    ABSTRACT: Numerous reports have been published on the application of fibrin glues, biological adhesives used as sealants for air leaks after pulmonary resection; however, the use of blood products has been questioned from both safety and economic perspectives. Therefore we were prompted to attempt the use of Integran (method C), a sheet-type absorbable topical collagen hemostat that is neither expensive nor derived from blood. To compare the efficacy of method C with that of method G, a combined approach in which TachoComb or a polyglycolic acid (PGA) sheet was fixed with a fibrin glue in a randomized controlled trial to prevent pulmonary fistula formation after lung surgery. Of the patients who were scheduled to undergo pulmonary resection in 2006 at the Department of General Thoracic Surgery, Juntendo University, and who provided informed consent for the study before surgery, those who developed visible air leaks during lobectomy, segmentectomy, partial resection for lung tumor or pulmonary cyst, or intractable pneumothorax were included as the subjects of this study. The subjects were randomized for treatment with either of 2 procedures, namely, method C or method G. Pulmonary fistula was defined as an obvious air leak persisting until day 3 after surgery. A total of 38 patients were assigned to method C and 34 to method G. Three patients (7.9%) assigned to method C (including 1 who underwent lobectomy and 2 who underwent partial resection), and 6 (17.6%) patients assigned to method G, including 3 who each underwent a lobectomy and partial resection, developed postoperative pulmonary fistula. The incidence of pulmonary fistula was significantly lower in the group assigned to method C, with a statistically significant difference of p = 0.044. In a randomized controlled trial of sealing with a sheet-type collagen vs. a combined approach of fixing a collagen sponge, using fibrin glue for closure of air leaks, the use of Integran, a sheet-type absorbable topical collagen hemostat, is feasible to prevent pulmonary fistula after lung surgery. It is also affordable and safe because it is not a blood product.

  • Article: Formation age of the lunar crater Giordano Bruno

    ABSTRACT: Abstract— Using the Terrain Camera onboard the Japanese lunar explorer, SELENE (Kaguya), we obtained new high-resolution images of the 22-kilometer-diameter lunar crater Giordano Bruno. Based on crater size-frequency measurements of small craters (<200 m in diameter) superposed on its continuous ejecta, the formation age of Giordano Bruno is estimated to be 1 to 10 Ma. This is constructive evidence against the crater's medieval age formation hypothesis.

    Article: Global mapping of the degree of space weathering on asteroid 25143 Itokawa by Hayabusa/AMICA observations

    ABSTRACT: Abstract— We obtained color images of near-Earth asteroid 25143 Itokawa by the Hayabusa multiband imaging camera to characterize the regional color properties. Images were obtained for the whole disk from the gate position (GP) and home position (HP) at a spatial resolution of 0.8–3.7 m/pixel. Whole-disk spectra are adjusted to the telescopic data obtained by the University of Hawai'i's 88-inch telescope using the Eight Color Asteroid Survey (ECAS) system. The disk-resolved measurements show large variations in the three visible channels. We present a map of an index related to the degree of space weathering, which has been newly developed based on laboratory measurements. We find large variations in the degree of space weathering on Itokawa. Fresh materials are observed in regions of steep slopes and craters, whereas mature materials are ubiquitously distributed. This result suggests that pristine ordinary chondrite-like materials have been exposed through weathered layers by excavation. By also examining close-up images obtained during touchdown rehearsal, we find that most rocks in Itokawa's rough terrains are weathered. Instead of a regolith blanket, the surface of this small asteroid is covered with weathered rocks and gravels.

  • Article: A pleuro-peritoneal communication through the diaphragm affected with lymphangioleiomyomatosis.

    ABSTRACT: A 30-year-old Japanese woman with lymphangioleiomyomatosis (LAM) developed a left chylothorax and chylous ascites. A pleuro-peritoneal communication was confirmed by a scintigram with (99)mTc-labeled macroaggregated-albumin injected into the peritoneal cavity. Video-assisted thoracic surgery revealed a protruding papillary lesion on the left diaphragm. Chyle was oozing into the pleural cavity through this lesion. Histopathological analyses demonstrated that the protrusion was a diaphragmatic LAM lesion and that LAM-associated lymphangiogenesis enabled communication between the pleural and peritoneal cavities through lymphatic vessels. This case demonstrated a new mechanism for chylous pleural effusion in LAM and illustrates the significance of LAM-associated lymphangiogenesis.

  • Article: The global distribution of pure anorthosite on the Moon.

    ABSTRACT: It has been thought that the lunar highland crust was formed by the crystallization and floatation of plagioclase from a global magma ocean, although the actual generation mechanisms are still debated. The composition of the lunar highland crust is therefore important for understanding the formation of such a magma ocean and the subsequent evolution of the Moon. The Multiband Imager on the Selenological and Engineering Explorer (SELENE) has a high spatial resolution of optimized spectral coverage, which should allow a clear view of the composition of the lunar crust. Here we report the global distribution of rocks of high plagioclase abundance (approaching 100 vol.%), using an unambiguous plagioclase absorption band recorded by the SELENE Multiband Imager. If the upper crust indeed consists of nearly 100 vol.% plagioclase, this is significantly higher than previous estimates of 82-92 vol.% (refs 2, 6, 7), providing a valuable constraint on models of lunar magma ocean evolution.

  • Article: Usefulness of suplatast tosilate for chronic cough following lung cancer surgery.

    ABSTRACT: Chronic dry cough is reported to occur in about 25% of patients following lung cancer surgery. Experimental data suggest that it may be caused mainly by stimulation of C-fibers, which are widely distributed to the lower trachea and bronchi. We assessed the clinical usefulness of suplatast tosilate (IPD) for chronic dry cough after lung cancer surgery. The subjects were patients with stage I lung cancer who had undergone lobectomy combined with mediastinal lymph node dissection. IPD was administered orally at 400 mg daily, and its efficacy was evaluated by patient interview 1, 2, and 3 months after the start of treatment. The subjects were 19 patients, and the duration of cough before entering the study was 393.2 days. The response rate was 84.2% (16/19) 1 month after the start of treatment. It seems that IPD inhibits cough resulting from stimulation of the bifurcated trachea with a high content of C-fibers. The present study suggested the efficacy of IPD for controlling chronic dry cough after lung cancer surgery.

  • Article: Resuture using Shirodkar tape for sternal dehiscence after extended thymectomy via median sternotomy.

    ABSTRACT: Sternal dehiscence is one complication after median sternotomy. We followed a patient with sternal dehiscence for 6 months after extended thymectomy via median sternotomy. His diagnosis was myasthenia gravis without thymoma and with complicating diabetes mellitus. Sixteen days after the operation chest radiography revealed that one of six sternal wires was cut, although sternal dehiscence was not apparent. Six months after the operation, chest radiography revealed that five of six wires were cut. The patient experienced sternal dehiscence, could not cough, and felt pain at the median wound site. We implemented a resuture technique of the sternum using Shirodkar tape for postoperative sternal dehiscence. After the second operation, sternal dehiscence was not apparent. He was able to cough and had no respiratory deficiency. One year after the second operation, chest computed tomography revealed no sternal dehiscence. Shirodkar tape is extremely useful and is low in price.

  • Article: Long-lived volcanism on the lunar farside revealed by SELENE Terrain Camera.

    ABSTRACT: We determined model ages of mare deposits on the farside of the Moon on the basis of the crater frequency distributions in 10-meter-resolution images obtained by the Terrain Camera on SELENE (Selenological and Engineering Explorer) (Kaguya). Most mare volcanism that formed mare deposits on the lunar farside ceased at approximately 3.0 billion years ago, suggesting that mare volcanism on the Moon was markedly reduced globally during this period. However, several mare deposits at various locations on the lunar farside also show a much younger age, clustering at approximately 2.5 billion years ago. These young ages indicate that mare volcanism on the lunar farside lasted longer than was previously considered and may have occurred episodically.

  • Article: The impact of cigarette smoking on prognosis in small adenocarcinomas of the lung: the association between histologic subtype and smoking status.

    ABSTRACT: In this retrospective study, we clarified the impact of smoking on prognosis and the association of clinicopathological factors, particularly histologic subtype, in patients with small adenocarcinoma of the lung. Between 1996 and December 2006, 121 patients presenting with adenocarcinomas that had a diameter </=2 cm were analyzed. The clinicopathological records of the patients were examined for age, gender, nodal status (c-N and p-N), tumor size, serum carcinoembryonic antigen level, histologic subtype, and smoking history. A histologic subtype was defined using a modified World Health Organization classification. These subtypes are bronchioloalveolar carcinoma (BAC), adenocarcinoma with little or no BAC component (Non or min BAC), and mixed bronchioloalveolar carcinoma with other adenocarcinoma components. The overall 5-year survival rates were 94.4% for never-smokers (N = 55) and 79.2% for smokers (N = 66) (p = 0.05). Cancer-specific 5-year survival rates were 98.0% for never-smokers and 80.4% for smokers (p = 0.03). Gender, serum carcinoembryonic antigen level, and histologic subtype were significantly associated with smoking status. Histologic subtype (Non or min BAC) was the only significant prognostic factor in multivariate analyses. The prevalence of smoking by histologic subtype was 27.3% for BAC, 43.2% for mixed bronchioloalveolar carcinoma, and 74.6% for Non or min BAC. The prevalence was significantly higher in Non or min BAC than in the others. Furthermore, the smoking index (daily cigarette consumption times years of smoking) was significantly higher in Non or min BAC than in the other two subtypes. In addition, patients with a high smoking index showed a greater percentage of Non or min BAC subtypes. Finally, male gender was associated with Non or min BAC independent of smoking status (p = 0.03). When adenocarcinomas were small (diameter </=2 cm) cigarette smoking and male gender were associated with Non or min BAC histologic subtypes, which are thought to have more aggressive biologic features resulting in poorer outcome compared with other subtypes.

    Article: Lack of exposed ice inside lunar south pole Shackleton Crater.

    ABSTRACT: The inside of Shackleton Crater at the lunar south pole is permanently shadowed; it has been inferred to hold water-ice deposits. The Terrain Camera (TC), a 10-meter-resolution stereo camera onboard the Selenological and Engineering Explorer (SELENE) spacecraft, succeeded in imaging the inside of the crater, which was faintly lit by sunlight scattered from the upper inner wall near the rim. The estimated temperature of the crater floor, based on the crater shape model derived from the TC data, is less than approximately 90 kelvin, cold enough to hold water-ice. However, at the TC's spatial resolution, the derived albedo indicates that exposed relatively pure water-ice deposits are not on the crater floor. Water-ice may be disseminated and mixed with soil over a small percentage of the area or may not exist at all.

  • Article: Sensitive and specific new enzyme-linked immunosorbent assay for N-ERC/mesothelin increases its potential as a useful serum tumor marker for mesothelioma.

    ABSTRACT: Because mesothelioma initially progresses on the surface of the pleura and peritoneum without forming masses, it has been difficult to diagnose at an early stage. It would be very useful to identify a tumor marker that could be used for screening to enable more diagnoses to be made at an early, treatable stage. We had previously identified N-ERC/mesothelin as a potential biomarker for mesothelioma. In the current work, we used a newly developed ELISA system to gain data on N-ERC/mesothelin levels in various clinical settings. A total of 102 healthy volunteers were recruited. In addition, 39 patients were diagnosed with mesothelioma, 53 patients were diagnosed with diseases that should be distinguished from mesothelioma, and 201 subjects were diagnosed with asbestos-related nonmalignant diseases (including simple exposure to asbestosis) who were treated at any of the cooperating hospitals were enrolled. Serum N-ERC/mesothelin levels measured by a new ELISA system showed that the median values from patients with mesothelioma were extremely high compared with levels obtained from other patients. Analysis in terms of histologic type showed that serum levels of N-ERC/mesothelin were elevated in epithelioid type mesothelioma, especially. In four important models of clinical settings, the sensitivity and specificity of N-ERC/mesothelin were about 71% to 90% and 88% to 93%, respectively. N-ERC/mesothelin is a very promising tumor marker for mesothelioma, especially epithelioid mesothelioma.

  • Article: Successful resection of dermatomyositis associated with thymic carcinoma: report of a case.

    ABSTRACT: We report a case of thymic carcinoma associated with dermatomyositis (DM) in a 53-year-old man. The patient presented with the characteristic features of a skin rash with Gottron's papules, proximal muscle weakness, and increased serum levels of the muscle-associated enzymes. Comprehensive clinical examinations revealed an anterior mediastinal tumor. We resected the tumor and histological examination confirmed squamous cell carcinoma of the thymus. Thereafter, his clinical symptoms improved dramatically and his serum levels of muscle-associated enzymes dropped, indicating that the DM was a paraneoplastic phenomenon. Our search of the literature found only one other case report of DM accompanied by thymic carcinoma, and to our knowledge, this is the fi rst documented case of dramatic improvement of DM after resection of thymic carcinoma. We propose that thymic carcinoma should be added to the list of malignancies that can complicate DM as a paraneoplastic disease.

    ABSTRACT: To explore diaphragmatic lesions that are potentially associated with catamenial pneumothorax by using an advanced flexible laparoscope with wide optic angle. Case report. University hospital. A 30-year-old woman who was scheduled for thoracoscopic surgery because of recurrent right-sided catamenial pneumothorax. The peritoneal surface of the diaphragm was explored with an advanced flexible laparoscope that was equipped with a charge-coupled device on the tip, concurrently with thoracoscopic surgery. Lesions associated with endometriosis on the peritoneal surface of the posterior diaphragm abutting the posterior abdominal wall and liver. When the flexible laparoscope was inserted via an umbilical trocar and the peritoneal surface of the right-sided diaphragm was explored, guided by illumination from the thoracoscope, scattered specific lattice lesions and fenestrations were identified in the central tendon of the posterior diaphragm, a region that cannot be visualized with the conventional rigid laparoscope. By using the flexible laparoscope, endometriotic lesions potentially related to catamenial pneumothorax were identified on the posterior diaphragm.

  • Article: Clinicopathological factors associated with unexpected N3 in patients with mediastinal lymph node involvement.

    ABSTRACT: We have already shown that postoperative survival was poor in p-N3 patients from the experience of extended radical nodal dissection (ERD: cervical and bilateral mediastinal nodal dissection) for lung cancer. In this retrospective study, we aimed to clarify the clinicopathological factors associated with p-N3 in patients with mediastinal lymph node involvement (excluding c-N3) who underwent ERD, and we studied their impact on prognosis. Between 1996 and April 2006, in patients with lung cancer in the right upper lobe, we performed ERD after obtaining informed consent from the patients. The study comprised 8 females and 29 males (median age of 60 years), with 15/7/15 cases of c-N0/c-N1/c-N2, respectively. The clinicopathological records of each patient were examined for prognostic factors associated with p-N3, including age, gender, histology, c-N number, preoperative serum CEA level, number of metastatic stations, and distribution of metastatic nodes according to the system of Naruke et al. Because c-N3 cases were excluded from the study, we defined p-N3 as unexpected N3. Of the 37 study subjects, 19 (51.4%) had one or more metastases to the mediastinal lymph nodes. Of these 19 patients, 10 (52.6%) had metastases to cervical and/or contralateral mediastinal lymph nodes (unexpected N3; 5-year survival was 0%). C-N factor (c-N2), nonskip N2, multistation mediastinal lymph node metastasis, highest mediastinal nodal involvement, and pT status were significantly associated with unexpected N3. In particular, multistation mediastinal lymph node metastasis and highest mediastinal nodal involvement were significant prognostic factors in multivariate analyses. Because unexpected N3 patients showed a poor prognosis after ERD, treatment modalities other than surgery should be considered. On the other hand, because true N2 patients showed a good outcome after surgery, surgical resection may be considered an important therapeutic modality even for N2 patients, given that they show single-station mediastinal nodal involvement or c-N0-1.

    Article: Size-frequency statistics of boulders on global surface of asteroid 25143 Itokawa

    ABSTRACT: The surface of asteroid 25143 Itokawa is covered with numerous boulders although gravity is very small compared with that of other asteroids previously observed from spacecraft. Here we report the size-frequency statistics of boulders on the entire surface of Itokawa based on high-resolution images (1 pixel ∼0.4 m) obtained by the Hayabusa spacecraft. There are 373 boulders larger than 5 m in mean horizontal dimension on the entire surface—0.393 km 2 —and the number density is nearly 10 3 /km 2 . The cumulative boulder size distribution on the entire surface has a power-index of −3.1 ± 0.1. For the east and west sides and the head and body portions of Itokawa, the power-index of the size distributions and the number densities of boulders of these areas are thought to be similar from the statistical point of view. A global mapping of boulders shows that there is no apparent correlation in the locations of boulders and craters. The ratio of the total volume of the boulders to the total excavated volume of the craters on Itokawa is ∼25% when only craters larger than 50 m in mean diameter are considered, and this ratio is extremely larger than that on Eros and the Moon, respectively. The origin of boulders on the surface of Itokawa was examined quantitatively by calculating the number of boulders and the size of the largest boulder using a model based on impact cratering experiments. The result indicated that the boulders on the surface of Itokawa cannot solely be the product of craters. Our results suggest that the boulders originated from the disruption of the larger parent body of Itokawa, as has been described in previous papers (Fujiwata et al.
