• [The disturbances of the eye position and motility in patients after scleral buckling surgery due to retinal detachment].

  The aim of the study was the evaluation of the frequency and intensity of the disturbances of the eye position and motility in patients after scleral buckling surgery due to retinal detachment,The aim of the study was the evaluation of the frequency and intensity of the disturbances of the eye position and motility in patients after scleral buckling surgery due to retinal detachment, treated in Ophthalmology Clinic in Poznar. Additionally the aim was to diagnose the reason for muscles damage by means of the Hess screen. Seventy patients were treated with this method between 2007-2009 and the study group was built by 40 patients, treated in average 15 months before the examination, at the age 12-79 years (mean age 55 years and 8 months). The correlation between the location of retinal holes treated with cryotherapy, the type of surgery and the changes in eye position and motility or diplopia in clinical evaluation and on the Hess screen, were evaluated. The disturbances of eye motility in clinical evaluation was diagnosed in 10 eyes (25% cases), and on the Hess screen in 16 eyes (40% cases). We noted, that the damaged muscle (overacting or weakened), was localized always on the site of cryotherapy or in the area at three o'clock from the retinal hole. (1) Patients during visual rehabilitation after scleral buckling surgery should be controlled for eye motility disturbances and diplopia. (2) The percentage of eye motility disturbances in our study group was comparable to this presented in literature, but higher in evaluation by means of Hess screen. (3) Accidental cryotherapy of the muscle, causing its weakening or overaction, seems to be a particular risk factor of extraocular muscles function disturbances.

  Cataract is a leading course of a treatable blindness and serious visual impairment in children. Treatment, especially in one-sided cataract is a challenge and it is connected with the risk ofCataract is a leading course of a treatable blindness and serious visual impairment in children. Treatment, especially in one-sided cataract is a challenge and it is connected with the risk of complications. The decision to perform a surgery in older children with already coexisting amblyopia is not easy to make. The aim of our study was to present the effects of the cataract treatment in children between 3 and 18 years old, in the Ophthalmology Clinic in Poznań. Between 2006 and 2008, the surgery was done in 36 children (42 eyes). The data of those patients, e.g. the type and location of the cataract, accompanying disorders and visual acuity reached, were analyzed. In this group of patient 60% of children reached visual acuity equal to 0.5, and 24% of them full visual acuity. The way to prepare the patient to the surgery, the way to calculate the power of the intraocular lens to be implanted and the surgery technique will be discussed.

The aim of the study is to present the case of Annette von Droste-Hülshoff syndrome. Fourteen years old girl was referred to the Outpatient Clinic of the Department of Ophthalmology of University of Medical Sciences in Poznan in order to attain qualification to surgical treatment of divergent strabismus of her left eye. Divergent strabismus of the left eye was diagnosed in early childhood. The girl was born in 32-nd week of twin pregnancy with a birth weight 1520 g. Her corrected vision was 1.0 for each eye with an apparent exotropia of both eyes and normal position of the head. The Hirschberg test revealed corneal reflex shifted to the nasal margin of the pupil. On eye fundus examination temporal pulling of vessels and temporal displacement of macula was noted in each eye. Kappa angle value was +8 degrees for each eye. Pseudoexotropia due to temporal macular ectopy was diagnosed (in the course of suffered ROP). The girl was not qualified to surgical treatment. This case emphasizes fundamental importance of correct diagnosis and qualification to surgical treatment of strabismus.

  The Lowe syndrome is a rare genetic disorder. It is inherited in an X-linked recessive manner. The clinical symptoms occur in eyes, central nervous system and kidneys. We describe the case of 16 years old boy with Lowe syndrome who underwent the bilateral congenital cataract surgery at the age of 14 months. The clinical symptoms of patient's mother are also described.

  We report case of acute retinal pigment epithelitis (ARPE), a very rare ophthalmic disorder, which has a very good final visual prognosis. The 18 years old woman was examined by ophthalmologist for sudden visual decrease of vision and dark spot in visual field of her left eye. The best corrected visual acuity (BCVA) of the left eye at the initial diagnosis was 0.1 and after 4 months improved to 0.9. The diagnosis or ARPE was made on the basis of fundoscopy, fluorescein angiography, electroretinography and optical coherence tomography (OCT) examination.

  Bilateral Adie's syndrome is characterized by the presence of a tonic pupil connected with accommodation problems. Very often, it coexists with latent hypermetropia, corneal hyposensivity. Typical signs are weakened tendons reflexes. We present the 8 years old girl with bilateral Adie's syndrome, which was the cause of school problems.

  PURPOSE: The aim of this study was to analyze risk factors influencing diode laser treatment of active phase of retinopathy of prematurity. MATERIALS AND METHODS: 106 premature infants were treatedPURPOSE: The aim of this study was to analyze risk factors influencing diode laser treatment of active phase of retinopathy of prematurity. MATERIALS AND METHODS: 106 premature infants were treated with the use of diode laser at Department of Neonatology since 1.01.2003-30.11.2004. The mean gestational age was 27.3, mean birth weight 1022 g. The earliest age of laser therapy was 37th day of life and the latest one 104th day (mean 64.3 days). 18 neonates were excluded from the study because they were from other departments, and 2 because they died. The results of treatment of the rest 86 children were evaluated. The occurence of retinal detachment or macular ectopy was classified as unfavorable outcome. RESULTS: The complete remission was obtained in 143 eyes (83.2%),the macular ectopy was diagnosed in 10 eyes (5.8%), the retinal detachment in 19 eyes (11%). Children with unfavorable outcomes were divided in two groups. The first one included 6 extremely immature infants, born between 24 to 25th week of gestation. The ROP in this group was diagnosed in I zone with plus disease. The second group included children with gestation age from 28 to 32 Hbd. 9 of them had severe infection (sepsis, congenital pneumonia, urinary tract infection), 1 serious maternal history (R type of diabetes). The procedure of laser therapy was performed on average at 72th day of life at the first group, and at 50th day at the second one. CONCLUSIONS: (1) Important risk factors of treatment failure of active phase of retinopathy of prematurity, is low gestational age and infections which take place in perinatal period. (2) In the second group of infants (gestational age from 28-32 Hbd), with treatment failure the retinopathy of prematurity occured much more earlier then in the group with successful treatment. (3) When making decision of the patient treatment besides estimation of the eye fundus and the progress of changes, we should also consider risk factors which have influence on retinopathy of prematurity, after treatment.

Authors describe indications for surgical procedures, methods of operations, results and complications of own material of 99 patients with TO treated in the Eye Clinic in Poznan and Plastic Surgery and Eye Clinic in Kobylniki from 1999 until 2003.

  PURPOSE: The assessment of the epidemiological data of acute-phase of retinopathy of prematurity (ROP). MATERIAL AND METHODS: Infants with birth weights less than 1500g and birth dates between AprilPURPOSE: The assessment of the epidemiological data of acute-phase of retinopathy of prematurity (ROP). MATERIAL AND METHODS: Infants with birth weights less than 1500g and birth dates between April 1, 2002, and May 30, 2003, admitted to special care baby unit in Department of Neonatology in Poznan, were examined. The incidence of retinopathy of prematurity, infants sex, multiple births, the onset of acute-phase ROP (considering the day of life and postmenstrual age) and also the day of life, at which the treatment was started, were analysed. RESULTS: Among 190 infants enrolled to the study 48.9% were female and 51.1% were male. There were 20.5% infants from multiple births. In the examined group in infants with a birth weight under 1250g the incidence of ROP was 32.6% and in infants with a birth weight above 1250g--7.4%. The ROP was diagnosed earliest in 4th week of life, the latest in 12th week of life (mean time of diagnosis the 8th week of life). The earliest time of performing the treatment with diode laser was 37th day of life, the latest--97th day of life (mean time of treatment the 67.5th day of life). CONCLUSIONS: Presented epidemiological data may be helpful in foreseeing the natural history of acute-phase of ROP in population of Polish premature infants with a birth weight under 1500g.

  PURPOSE: 1. The results of diode laser treatment of active phase of retinopathy of prematurity. 2. Determination of risk factors influencing results of treatment. 3. Determination of coexistencePURPOSE: 1. The results of diode laser treatment of active phase of retinopathy of prematurity. 2. Determination of risk factors influencing results of treatment. 3. Determination of coexistence frequency with other chronic diseases subsequent to ROP, like broncho-pulmonary dysplasia, hydrocephalus and periventricular leucomalacia. PATIENTS AND METHOD: 27 premature infants were treated with an aid of diode laser since June to December 2002. Birth weight varied from 550 to 1600 g (mean 941.85 g) and gestational age from 25 to 32 weeks (mean Hbd 27.6). In 6 premature infants (12 eyes) ROP was stated in the I zone. In 14 premature infants (28 eyes) threshold ROP was found in II zone. In 2 premature infants (3 eyes) threshold ROP diagnosed in III zone and in 5 premature infants (10 eyes) wide fibro-vascular proliferations were found in the area exceeding 12 hours. Treatment was performed with an aid of diode laser OcuLight SLx from the company IRIS Medical Instruments, Inc., emitting infrared radiation (810 nm). RESULTS: Good anatomical results were obtained in 17 cases (36 eyes), in 3 cases (5 eyes) macular ectopy, in 2 cases (3 eyes) partial retinal detachment and in 5 cases (10 eyes) total retinal detachment. Only in 4 children ROP occurred as a late complication of prematurity. CONCLUSIONS: 1. Obtained results of treatment with an aid of diode laser depend on localization of lesions, occurring in course of ROP and on the stage of its advancement. Treatment performed in premature infants in active phase, threshold ROP in the II-nd and III-rd zone causes good anatomical results. 2. The cause of treatment failure was localization of ROP in the I-st zone, accompanied by rapid course or wide fibro-vascular proliferations exceeding 12 hours. 3. Children with diagnosed retinopathy are at high risk of psycho-motoric disturbance because of other chronic disorders, that may often coexist.

• [Ocular complication in children with Stevens-Johnson syndrome]

  PURPOSE: Presentation of serious cases of Stevens-Johnson syndrome (SJS) with acute and chronic ocular manifestations and methods of their treatment. MATERIAL AND METHODS: 3 children aged between 6PURPOSE: Presentation of serious cases of Stevens-Johnson syndrome (SJS) with acute and chronic ocular manifestations and methods of their treatment. MATERIAL AND METHODS: 3 children aged between 6 to 12 years. Follow-up time was from 5, months up to 6 years. CONSERVATIVE TREATMENT WAS: Symblepharon massage, topical antibiotics, steroids, artificial tears. In chronic stage mitomycin C as eye drops was use in one case. Surgical treatment--intubation of lacrimal ducts and removal of eye lashes were performed in 2 cases. RESULTS: In 2 cases, in which ophthalmological therapy was introduced very early, the results of treatment were very good. In one case with corneal neovascularization and posterior lid margin keratinization, the results of treatment were not satisfactory. CONCLUSIONS: 1. The children with SJS should be treated by ophthalmologists as soon as possible. 2. Some pathological changes of conjunctiva, cornea and lacrimal ducts disappeared after few weeks of intensive topical therapy often. 3. The results of treatment of cicatrical changes of lids, conjunctiva and neovascularization of the cornea were not satisfactory.

We present a case of a boy, who underwent a plastic surgery of pupil at age of 3 year. Despite of relatively tardy onset of treatment and existing profound amblyopia (light perception with projection), a full vision recovery was achieved after 4 years of occlusion therapy.

  The survival rate of low and very low birth weight infants increases as a natural consequence of the improvement in perinatal care and introducing the new methods and techniques in neonatal intensiveThe survival rate of low and very low birth weight infants increases as a natural consequence of the improvement in perinatal care and introducing the new methods and techniques in neonatal intensive therapy. In the face of this progress the number of patients potentially threatened with hearing and vision impairment grows. In this paper we described the most essential risk factors of the occurrence of these complications especially in premature newborns. Also the methods of prevention, early detection of disturbances or treatments of hearing and vision impairment constituted the target of this work.
