• Comparison of cryotherapy and photodynamic therapy in treatment of oral leukoplakia.

  Oral leukoplakia is a pre-malignant lesion of the oral mucosa. The aim of this study is to compare the curative effects of photodynamic therapy and cryotherapy in the treatment of oral leukoplakia.Oral leukoplakia is a pre-malignant lesion of the oral mucosa. The aim of this study is to compare the curative effects of photodynamic therapy and cryotherapy in the treatment of oral leukoplakia. The first group, treated by photodynamic therapy (δ-aminolevulinic acid (ALA), 630-635nm wavelength), consisted of 48 patients suffering from leukoplakia. The second group consisted of 37 patients treated using cryotherapy. Analyses and comparisons of the complete responses, recurrences, numbers of procedures and adverse effects after both PDT and cryotherapy were obtained. In the first group, a complete response was obtained in 35 patients (72.9%), with thirteen recurrences observed (27.1%) over a six-month period. In the second group, a complete response was obtained in 33 patients (89.2%), and recurrence was observed in nine patients (24.3%). Photodynamic therapy and cryotherapy appear to be comparative methods of treatment that may both serve as alternatives for the traditional surgical treatment of oral leukoplakia. The advantages of PDT are connected with minimally invasive and localized character of the treatment and with not damage of collagenous tissue structures, therefore normal cells will repopulate these arrangements. PDT is more convenient for patients, less painful, and more esthetic.

  Oesophageal papilloma and Barrett's oesophagus are benign lesions known as risk factors of carcinoma in the oesophagus. Autofluorescence endoscopy is a fast and non-invasive method of imaging of tissues based on the natural fluorescence of endogenous fluorophores. The aim of this study was to prove the diagnostic utility of autofluorescence endoscopy with digital image processing in histological diagnosis of endoscopic findings in the upper digestive tract, primarily in the imaging of oesophageal papilloma. During the retrospective analysis of about 200 endoscopic procedures in the upper digestive tract, 67 cases of benign, precancerous or cancerous changes were found. White light endoscopy (WLE) image, single-channel (red or green) autofluorescence images, as well as green and red fluorescence intensities in two modal fluorescence image and red-to-green (R/G) ratio (Numerical Colour Value, NCV) were correlated with histopathologic results. The NCV analysis in autofluorescence imaging (AFI) showed increased R/G ratio in cancerous changes in 96% vs. 85% in WLE. Simultaneous analysis with digital image processing allowed us to diagnose suspicious tissue as cancerous in all of cases. Barrett's metaplasia was confirmed in 90% vs. 79% (AFI vs. WLE), and 98% in imaging with digital image processing. In benign lesions, WLE allowed us to exclude tissue as malignant in 85%. Using autofluorescence endoscopy R/G ratio was increased in only 10% of benign changes causing the picture to be interpreted as suspicious, but when both methods were used together, 97.5% were cases excluded as malignancies. Mean R/G ratios were estimated to be 2.5 in cancers, 1.25 in Barrett's metaplasia and 0.75 in benign changes and were statistically significant (p=0.04). Autofluorescence imaging is a sensitive method to diagnose precancerous and cancerous early stages of the diseases located in oesophagus. Especially in two-modal imaging including white light endoscopy, autofluorescence imaging with digital image processing seems to be a useful modality of early diagnostics. Also in observation of papilloma changes, it facilitates differentiation between neoplastic and benign lesions and more accurate estimation of the risk of potential malignancy.

  Chronic venous insufficiency, as well as crural vein ulceration, developing at its background, poses a serious clinical problem. Treatment is costly and lengthy. Intense development of physicalChronic venous insufficiency, as well as crural vein ulceration, developing at its background, poses a serious clinical problem. Treatment is costly and lengthy. Intense development of physical medicine allows to reduce the time of treatment, as well as to improve the life quality in numerous soft tissue ailments, including wounds that are hard to heal. This article presents the beneficial results of treatment of a 67-year-old woman patient with a diagnosis of crural vein ulceration in the right limb. The applied physiotherapeutic method, in the form of magnetostimulation, contributed to complete healing of the lesion, subsidence of symptoms, as well as improved life quality of the treated patient.

• The role of photosensitized macrophages in photodynamic therapy.

  Cancer stimulates macrophage infiltration and encourages angiogenesis, which is necessary for tumor growth and invasion. It seems, that the influence of photodynamic therapy (PDT) on immune cells andCancer stimulates macrophage infiltration and encourages angiogenesis, which is necessary for tumor growth and invasion. It seems, that the influence of photodynamic therapy (PDT) on immune cells and immune regulators plays a crucial role in this process. In order to study this effect, the influence of δ-aminolevulinic acid (ALA) PDT on the activity of the murine macrophage J-774A.1 cell line was assessed. J-774A.1 cells were incubated with different concentrations of ALA and irradiated with a VIS light source (400-750 nm) at 5, 10 and 30 J/cm2. The effects of ALA-PDT were evaluated on the basis of cell viability and the secretory activity of macrophages (nitric oxide, NO; reactive oxygen intermediates, ROI; tumor necrosis factor α, TNF-α; interleukin-1β, IL-1β; and nuclear factor κB, NF-κB; proteins, p50 and p65). Experiments showed that at the higher energy doses, there was a large increase in ROI and TNF-α release and decreased levels of NF-κB p50 and p65, IL-1β production and NO release. The increased levels of ROI and TNF-α release after PDT could be an additional factor for the complete eradication of tumors. The decrease in NF-κB p50 and p65 and IL-1β levels could inhibit tumor progression.

  Purpose: Thermography and contact thermometry were used to study the influence of body mass index (BMI) on the lowering of skin temperature caused by whole-body cryotherapy. Methods: The study wasPurpose: Thermography and contact thermometry were used to study the influence of body mass index (BMI) on the lowering of skin temperature caused by whole-body cryotherapy. Methods: The study was performed using the Thermovision Camera AGEMA Type 470 and A40. The thermograms of the chosen regions of interests were performed before and immediately after whole-body cryotherapy in a research room outside a cryogenic chamber where the temperature was stabilized. As an additional measurement technique during whole-body cryotherapy, contact thermometry was performed using thermocouples Ni-Cr-Ni-Al stacked to the skin surface. Results: The results obtained showed differences in the decrease of skin temperature of predetermined body parts. The largest temperature decrease was observed on the lower extremities. Some differences in the thermal response of similar body parts influenced by the extremely low temperature, with regard to the BMI of volunteers, were observed. This was also found in the results of contact thermometry studies. The statistical analysis confirmed the results of thermography and thermometry studies. Conclusion: The magnitude of skin temperature decrease due to the extremely low temperature used in whole-body cryotherapy may be connected to a patient's BMI.

  The developments in medicine have made it possible to effectively treat many diseases thanks to the application of modern methods of physical therapy. The therapeutic efficacy of physiotherapy has been proven in many clinical tests. Its analgesic action is listed as the most beneficial effect. Recent technological advancements have produced new appliances which enable the application of two synergic physical methods at the same time. This work presents a selection of physiotherapy applications used in the treatment of both acute and chronic pain in the form of associated therapy. The methodology, procedures and indications for application have been discussed.

  Lichen sclerosus (LS) is a chronic skin and mucosa inflammatory disease. It affects mainly the female anogenital area especially in postmenopausal period. The main symptoms include pruritus, burning,Lichen sclerosus (LS) is a chronic skin and mucosa inflammatory disease. It affects mainly the female anogenital area especially in postmenopausal period. The main symptoms include pruritus, burning, pain, sometimes urinary problems, or difficulties in defecation. Usually, porcelain-white plaques are seen in the skin and mucosa. The etiology and pathogenesis of LS are still uncertain. There are some research studies on possible genetic predisposition, yet autoimmune, hormonal, or infectious factors are not excluded. The typical treatment of LS is mainly pharmacological, although the alternative treatment method used in LS is photodynamic therapy (PDT), which is noninvasive technique based on selective destruction of lesions. Our study is focused on molecule markers of vascularisation (CD34), nervous cell function (myelin basic protein [MBP]), keratinocyte function (CD44), and proliferation index (Ki67) in cases treated with photodynamic method. A group of 100 patients treated in our department was included in the study. All 100 women had LS on the basis of clinical and histological criteria. All the subjects underwent PDT. In all cases, skin biopsies were taken before and after treatment, and samples were analyzed with CD34, CD44, MBP, and Ki67 antibodies using immunohistochemical staining. The study shows the high efficacy of PDT in LS treatment including beneficial changes to CD34, CD44, and MBP immunostained molecules. The Ki67 proliferation index did not change significantly. A significant increase of CD34 (microvessel density), MBP, and CD44 expression was confirmed in the histological images and in the partial or full remission of clinical objective and subjective symptoms. The PDT is a very effective therapeutic method in LS treatment.

Activities of antioxidant enzymes: superoxide dismutase (SOD) and its isoenzymes - (SOD-Mn) and (SOD-ZnCu), catalase (CAT), glutathione peroxidase (GPx), glutathione reductase (GR) and glutathione transferase (GST), as well as the total antioxidant status (TAS) and concentration of malondialdehyde (MDA) in plasma and/or erythrocytes, respectively were determined. In patients with AS, a statistically significant decrease in plasma activity of SOD, SOD-CuZn and TAS, significant drop in activity of SOD, GPx, GST and GR in erythrocytes, as well as increased concentration of MDA in comparison with control group of healthy volunteers was observed. Decrease in antioxidant status leading to generation of oxidative stress may play an important role in the pathogenesis of ankylosing spondylitis.

  The aim was to assess whether magnetic field influences defensin and CRP concentrations in patients with diabetic polyneuropathy and in healthy subjects. 61 diabetic patients were randomly dividedThe aim was to assess whether magnetic field influences defensin and CRP concentrations in patients with diabetic polyneuropathy and in healthy subjects. 61 diabetic patients were randomly divided into 2 groups: study group-32 patients exposed to low-frequency magnetic field; and control group-29 patients with sham exposure. Additionally, 20 healthy subjects exposed to low-frequency magnetic field. Exposures were performed during 3 weeks, 5 days in a week. Defensin and CRP concentrations were measured at baseline, after 3 weeks and at the end of the study. There were no significant changes in defensin concentration in patients with diabetes in both the real and sham exposure group. We observed increased concentration of defensin in healthy subjects in week 5 vs. baseline value (P<0.02). Magnetic field has no impact on defensin concentration in diabetic patients but has positive influence on this parameter in healthy subjects.

Myocardial infarction can step out both in persons with coronary disease diagnosis and at young person so far have not any symptoms from side circulation system. The study was applied 37 young patients (7 women and 30 men) from II Clinical and Cathedral of Cardiology in Zabrze of The Silesian Medical University in Katowice with myocardial infarction. In this work the authors used the questionnaire which including basic information about hospitalized patients as well as executed the retrospective opinion of chosen risk factors (dyslipidemia, diabetes mellitus, hypertension arterials, family interview and smoking). The authors were also executed qualification of frequency the type of myocardial infarction, diagnostic and therapeutic conduct with cardiologic interview in three aged groups (to 30 years of age, 31-35 years of age, 36-40 years of age). The five-year-old observation showed, that the smoking was the strongest risk factor, the raised level of cholesterol as well as the burdening family interview. It surprises however the lack of relationship with diabetes. In diagnostic ad therapeutic conduct the fundamental part play coronarography examination and PCI procedure. The creature of counteract in myocardial infarction in young people before 40 years old should be preventive action under propagating the healthy style of life aspect (the education, diet, physical activity) as well as the modification of reversible risk factors, which will permit to reduce or they will stop the intensive development of cardiac-vascular diseases.

  BACKGROUND: The aim of this prospective study was to verify whether magnetostimulation with weak variable magnetic fields with low value of induction could enhance the effects of pharmacologicalBACKGROUND: The aim of this prospective study was to verify whether magnetostimulation with weak variable magnetic fields with low value of induction could enhance the effects of pharmacological therapy in drug-resistant depression. MATERIALS AND METHODS: Thirty patients, 26 women and 4 men, with drug-resistant depression were enrolled in the study. The subjects from Group No. I (14 patients) were given fluvoxamine and treated with weak variable magnetic field using the VIOFOR JPS device; the subjects from Group No. II (16 patients) were also given fluvoxamine but they were treated with the VIOFOR JPS device in placebo mode. Changes in depressive symptoms were estimated with the 21-point Hamilton Depression Scale (HDRS), Montgomery-Asberg Depression Scale (MADRS) and Beck Depression Inventory (BDI) questionnaire. RESULTS: After 15days of treatment highly significant differences were revealed between the patients treated with magnetic field and the patients treated with placebo: the final HDRS score was 53% of the initial value for the group receiving combined treatment, and 86% in the placebo group (p<0.001); for MADRS score the values were 51% and 88% (p<0.001), respectively, and for BDI 60% and 87% (p<0.001). Thus, the average effect of placebo applied with fluvoxamine was a ca. 15% reduction of symptoms, while the concurrent application of magnetic field and SSRI treatment resulted in a 40-50% improvement. CONCLUSION: Our study indicates that adding a two-week low-induction variable magnetic field stimulation to a classical pharmacologic therapy reduces the intensity of symptoms in patients with drug-resistant depressive disorders.

The different compositions of the modification solution consist of corrole-SH and other thiol derivatives used as the "background compounds" such as 1-dodecanethiol (DDT), 6-mercapto-1-hexanol (HS(CH(2))(6)OH), or 11-mercapto-1-undecanol (HS(CH(2))(11)OH) were explored to find the best self-assembled monolayer (SAM) suitable for dopamine determination. Among them, the mixed SAM consisting of corroles with the -SH group and 6-mercapto-1-hexanol (HS(CH(2))(6)OH) in the molar ratio 1:10 was the most sensitive. The signals generated by the formation of a complex between the corrole host and the dopamine guest were measured by Osteryoung square-wave voltammetry (OSWV) and electrochemical impedance spectroscopy (EIS) with [Ru(NH(3))(6)](3+) as an electroactive marker. The developed sensor was free of interferences of components of human plasma such as ascorbic acid, creatinine, creatine, and uric acid. The detection limits observed by EIS in buffer solution and in the presence of centrifuged human plasma 80 times diluted with a 0.9% NaCl containing 0.01 M borate buffer solution of pH 7.0 were 3.3 x 10(-12) and 5.3 x 10(-12) M, respectively.

  BACKGROUND: Helicobacter pylori colonizes the human gastric mucosa, leading to chronic superficial gastritis and in some cases to peptic ulceration, gastric adenocarcinoma, or gastric lymphoma. It has been postulated that the clinical outcome depends on differences in H. pylori strain virulence as well as on individual factors of the host. Thus, we aimed to assess the relation between H. pylori cagA/vacA genotypes and the TNF-alpha gene expression in gastric mucosa specimens from patients with chronic gastritis. MATERIAL/METHODS: This study was conducted with gastric mucosa samples obtained during gastroendoscopy from 43 H. pylori-infected individuals with chronic gastritis. The presence of ureA and cagA genes and vacA allele combinations were analyzed in isolated DNA by the polymerase chain reaction method. Isolates of RNA were used for cDNA synthesis by reverse transcription. Synthesized cDNA was used to determine the TNF-alpha gene expression level by quantitative real-time polymerase chain reaction assay. RESULTS: The cagA gene was detected in 67.44% of H. pylori strains. Five vacA alleles in the tested H. pylori strains were detected: s1a/m1 (18.60%), s1a/m2 (23.26%), s1a/m3 (18.60%), s1b/m2 (6.98%), and s2m2 (32.56%). There were no significant differences in TNF-alpha gene expression in strains expressing cagA versus those not expressing this gene, and no significant differences among strains with different vacA alleles. CONCLUSIONS: TNF-alpha gene expression in the gastric mucosa of H. pylori-infected patients appears to be independent of H. pylori cagA/vacA genotype.

  This article is a review of laboratory and clinical research undertaken in Poland in PDD and PDT over the past 20 years. These are divided into two parallel research areas. The first is based on clinical trials where new modalities of photosensitizer synthesis, molecular mechanisms of PDT and other aspects are investigated. The second is concerned with clinical aspects of PDD and PDT in both pre-neoplastic and malignant disease. In Poland there were 2 National Congresses in 2006 and 2008 with 100 and 400 participants respectively. One of the oldest centers of Photodynamic Diagnostics and Therapy is located in Bytom. For about 10 years it has led clinical research in Poland with PDD and PDT in such medical disciplines as dermatology, gastroenterology, laryngology, pulmonology, gynecology, and orthopedics.

  BACKGROUND: Laser-induced fluorescence diagnostics (LIFE) can be used as an imaging system of precancerous and neoplasmatic lesions of oral mucosa. LIFE system utilizes healthy and neoplasmatically changed tissue in autofluorescence, without using any fluorescence substances. Neoplasmatic lesions are visible in pseudo colors, healthy tissue as a shade of green color and abnormal tissue as a shade of red color. All visible colors have different intensity. Color intensity is relevant to the grade of dysplasia, carcinoma progress and is called numerical color value (NCV). AIM: The aim of our study was to find correlation between autofluorescence diagnostics with NCV assessment and type of histopathological diagnostics of specimen biopsy. PATIENTS AND METHODS: Fourteen patients participated in our study. Lesions were located in different intraoral areas. The most common location was: buccal, gingival and mandibular mucosa. Patients were examined using laser induced fluorescence diagnostics (400-750 nm wavelength) with NCV using OncoLIFE system. Then the specimen biopsy from the lesion was taken and histopathological examination was performed. RESULTS: We have noted different NCV and dependence of NCV on histopathological grade. CONCLUSION: Diagnostics using white-light imaging with LIFE imaging is not only a significant faster method and a better diagnostics of preneoplasmatic and neoplasmatic lesions, but also there is a correlation between NCV and histopathological grade. The farther investigations are necessary to prove these preliminary findings.

Photodynamic therapy is a new treatment for a wide variety of malignancies and premalignant dysplasias. We wanted to examine the effectiveness of photodynamic therapy (PDT) on vulvar intraepithelial neoplasia (VIN). DESIGN: The aim of the study was to analyze the effectiveness of photodynamic therapy (PDT) on vulvar intraepithelial neoplasia (VIN). MATERIAL AND METHODS: We have analyzed 20 women with VIN, who were treated in our center - Clinic of Vulvar Diseases. All these women had photodynamic diagnosis (PDD), photodynamic therapy followed (PDT), with 5% ALA applied to the entire vulva. CONCLUSIONS: We have noted the reduction of subjective complaints, but the histopathological improvement was observed in fewer degree.

  Head and neck neoplasms are one of the most common carcinomas and their main etiological factor is tobacco smoking and alcohol abuse. There are about 7000 new cases noted in Poland yearly.Head and neck neoplasms are one of the most common carcinomas and their main etiological factor is tobacco smoking and alcohol abuse. There are about 7000 new cases noted in Poland yearly. Radiotherapy (radical or palliative), chemotherapy (neoadjuvant, adjuvant or combined with radiotherapy) and surgery aimed basically at tumor radical removal are methods used in the treatment of these neoplasms. In the case of a recurrence, surgery and radiotherapy are applied. The decision concerning the type of therapy is made on the grounds of a complete clinical picture and results of laboratory tests. The patient is offered the solution which gives the highest opportunity of achieving a optimal therapeutical effect at a possibly minimal risk. Literature analysis confirms that photodynamic diagnostics and therapy give a chance of early detection and effective treatment of preneoplastic and neoplastic states in the head and neck region. Photodynamic therapy is characterized by especially high efficacy in the treatment of changes in the early stages of clinical advancement. It can also play a significant role as an adjuvant intraoperative therapy in the case of advanced clinically tumors and also as a palliative treatment, being one of the elements of a complex therapy. The study is an overview of current scientific knowledge concerning the possibilities of photodynamic diagnostics and therapy application in the case of head and neck tumors, which was presented at the medical conference in Shanghai.

  Ocular infections are one of the most frequent causes of ailments among the patients coming to ophthalmologic offices. This article presents one of the physical medicine's methods--magnetotherapy--which uses the alternating low frequency magnetic fields in the therapy of viral conjunctivitis and keratitis in a 49-year-old female patient. Basing on the obtained results it was stated that this method broadens the treatment possibilities becoming a precious supplement and support treatment method in ophthalmology.

  The infections in orthopedic surgery and traumatology represents important medical problem. The results of treatment of motor-organs infections in high degree were improved by the introduction ofThe infections in orthopedic surgery and traumatology represents important medical problem. The results of treatment of motor-organs infections in high degree were improved by the introduction of antisepsis and asepsis in XIX century and the antibiotics' discovery in the beginning of XX century. However, widespread usage of antibiotics leaded to the rise of refractory tribes of bacteria on their activity, which caused higher percentage of fails in the therapy. Also the rapid civilization development, which flown on arising of a new invasive methods of operative trauma protection and stabilizations of fractures in motor-organs area increased the number of infectious complications during treatment. These facts gave a reason to look for a more effective therapeutic methods. It seems that photodynamic therapy gives us the new possibilities of infectious treatment, which avails oneself activity of therapeutic light laser with proper wave length on the human tissue with photosensitizer included. Indeed there are known until now splendid results of PDT in neoplasma treatment, however it seems, based on experimental investigations and publications in medical literature, that this method can be a chance of effectual and non-invasive treatment of the soft tissues and bones inflammations.
