ABSTRACT: The use of a short-stay observation unit (OBI) in a pediatric department has reduced the number of admissions. Significant cost savings and a better care for children and their families have been also achieved. During the year 2003 in our department 1759 children received OBI. 226 (12.8%) were then admitted. 1553 (87.2%) were discharged after a nine-hour mean stay. Descriptive statistics are used to outline the sample of patients and used treatments.

ABSTRACT: Most commercial applications have to cope with a large number of stored object instances and have data shared among many users and applications. For object-oriented as well as conventional application development RDBMS technology is currently being used in most cases. We describe a software module called Object Wrapper for storing and retrieving objects in an RDBMS. Having these capabilities in a separate component helps to isolate data management system dependencies and hence contributes to portable applications

ABSTRACT: Two patients with acute post-streptococcal polymyalgia are described with a review of the seven cases previously reported cases. The common features are sudden onset of muscular pain with fever usually after an acute upper respiratory tract infection. Antistreptolysin O titre and inflammatory indexes are increased and muscle enzymes are normal. CONCLUSION: Acute post-streptococcal polymyalgia should be considered as a possible diagnosis in every child complaining acute polymyalgia.

ABSTRACT: Thirty-one children examined for gastroesophageal (GE) reflux were studied with both barium meal X-ray and gastroesophageal scintigraphy. The study was performed with a double-blind method. The children with negative findings at both tests were submitted to intraluminal esophageal pH probe test. Barium X-ray detected GE reflux in 17/27 patients, whereas radionuclide scintigraphy was positive in 24/27 (positivity: 63% versus 89%; p less than 0.05). Their combined use provided positive findings in 26/27 cases. Scintigraphy appeared to be a safe and reliable screening test to detect GE reflux. Moreover, scintigraphy was superior to barium X-ray study for it had higher diagnostic accuracy, it could be easily performed on young patients, and delivered smaller radiation doses.

ABSTRACT: 272 cases of childhood epilepsy were studied with brain TC scan to correlate the type of seizure to the possible neurological damage. Children with generalized convulsions had a low incidence of abnormal TC findings if in absence of neurological signs (3/80) (Group A), but a more relevant incidence (13/34) in presence of neurological signs (Group B). 65 children had epileptic encephalopathy (Group C): 15/17 West syndromes and 33/48 Lennox syndromes presented TC abnormalities. The remnant 93 children had partial seizures. In Group D partial epilepsy was free of neurological signs; the incidence of abnormal TC findings was 17/76. An higher incidence (9/17) was found in Group E, where partial epilepsy was accompanied by neurological disturbances.

ABSTRACT: Iridocyclitis is a common complication of juvenile rheumatoid arthritis. It affects particularly female patients with mono-oligoarthritis. A positivity of ANA-test is usually observed, representing a marker for the disease. The course is chronic, leading potentially to scarring and blindness. The insidious onset, often not parallel to arthritis, requires careful ophthalmologic examination of all patients at risk, in order to achieve early diagnosis and treatment.

ABSTRACT: 56 children, aged between 0-13 month, affected by chronic vomiting were evaluated both with ultrasound and radiological methods, in a double blind study. 22/56 children resulted to be affected by gastro-oesophageal reflux (GER) with upper GI series versus 18/56 with ultrasound. Four cases, ultrasound negative and X-ray positive, represented a borderline situation in which a mild gastro-oesophageal reflux is to be considered paraphysiological phenomenon. The accuracy and handleness of ultrasound evaluation in GER are emphasized.

ABSTRACT: A very low birth weight female infant developed a systemic infection by Candida albicans. Her clinical conditions were very poor; involvement of CNS was demonstrated my multifocal hyperechogenic images, spreading to the periventricular and subcortical areas of brain, confirmed by CT as circumscribed hyperdense punctiform granulations suggesting initial cerebritis. The introduction of liposomal Amphotericin-B in the therapy resulted in a marked improvement of clinical conditions together with the disapparance of pathological brain images. Longitudinal examinations carried out every three months during the first year, and every six months during the second, have shown adequate psychomotor development. The results of late cerebral nuclear magnetic resonance (NMR) imaging were within the norm. The patient is now seven-years-old and shows a well-balanced neuropsychological development. Systemic Candida infections of VLBW infants with CNS involvement have usually a very poor prognosis in terms of death or residual brain damage. The use of liposomal Amphotericin-B allowed us to achieve the complete cure of our patient, suggesting an high efficacy together with at a lower toxicity in respect of traditional treatments.

ABSTRACT: A coconut phytobezoar was detected in the distal ileum of a 4-year-old boy, admitted for suspected appendicitis. While persimmon or citrus fruit ingestion has been previously reported as cause of phytobezoar intestinal occlusion, it is the first time that coconut pulp is described to cause ileal obstruction in children. No predisposing factor as previous gastric surgery or Meckel's diverticulum was present. The bezoar was successfully milked past the ileocecal valve into the right colon.

ABSTRACT: A twelve-years-old boy developed fever, pharyngitis and acute migrant polyarthralgia. An increasing ASO titre was observed, so that rheumatic fever was firstly diagnosed. Finally splenomegaly and positive IgG and IgM against Epstein Barr virus led to the diagnosis of acute EBV infection with polyarthritis. EBV infection should be considered into the differential diagnosis of migrant polyarthritis.

ABSTRACT: A 14-month-old girl developed chronic stridor and dyspnoea. Four months later she presented arthritis, anterior uveitis and positive ANA. Juvenile chronic arthritis (JCA) was diagnosed. Laryngoscopy demonstrated the presence of cricoarytenoid arthritis (CA). The left vocal cord was adducted and immobile, while the right vocal cord had decreased mobility. Erythema and swelling of the arytenoid cartilage on both sides was seen. Steroid treatment resulted in the resolution of these symptoms and made airway control unnecessary. This case demonstrates that CA may be the first sign of JCA, preceding peripheral arthritis. CA should be considered in every child with chronic stridor and laryngeal obstruction.

ABSTRACT: A follow up of 14 cases of tuberous sclerosis is presented. Cutaneous signs, epileptic seizures and developmental disabilities were recorded over a period ranging between 2 and 25 years from the onset of symptoms. A correlation can be established between the time of appearance of epilepsy and the degree of mental retardation. Antiepileptic treatment does not seem to prevent developmental disability.

ABSTRACT: Reflex sympathetic dystrophy is a well known syndrome in adults, but often misdiagnosed in children. We describe a case occurred in a 12 years old girl. Psychological factors played a major role in the mechanism of the disease. Early recognition and prompt management allow complete recovery without sequelae.

ABSTRACT: Immigration to Italy from Asian countries is sharply increasing, but little we know about the integration of children born in Italy from migrant families.
To evaluate which changes in feeding practices has occurred in respect of their original culture.
A cross-sectional study investigated the eating habits of 53 children (aged 2-14 years), born in Italy from parents who recently moved from Pakistan. Data were collected through a questionnaire and interviews.
Religious beliefs and poor economical conditions interact with the feeding habits of the families. Prolonged, but not exclusive breastfeeding is common. According the Pakistani uses, many beverages are introduced early in addition to maternal milk. Whereas protein intake is usually low in the first two years of life, it tends to becomes excessive during school-age. Besides the traditional diet, children widely use savoury and sweet convenience food.

ABSTRACT: 929 cases of acute enteritis are reviewed. They were admitted since 1972 to 1981 in the first Pediatric Department of Modena, and treated with dietetic therapy. The periods of stool normalisation according the treatment received are discussed (oral rehydrating therapy versus casein hydrolysate). The results are compared with the evolution of diarrhea in 322 children treated with antidiarrheal adsorbents. According to our experience the diet with casein hydrolysate results to be an effective therapy of acute diarrhea, affording a nearly complete reduction in the use of intravenous infusions.
